RESUMO

The authors describe the planning and implementation of a survivor-informed medical home for child trafficking survivors. Key partnerships necessary for establishing clinical infrastructure are highlighted. The trauma-informed clinical practices are described in detail. An overview of next steps for evaluation of this clinical program is provided.

With an increase in human trafficking in the United States, there is a need for nursing curricula to provide education and engagement in recognizing and appropriately caring for victims. The purpose of this article is to describe an undergraduate nursing simulation involving a human trafficking victim with a discussion of how the American Association of Colleges of Nursing's (AACN) Essentials were addressed by the simulation. The results of the course evaluation indicated that providing an opportunity for baccalaureate nursing students to engage in a simulation involving a victim of human trafficking allowed them to solidify classroom theory and education. Students reported increased confidence in recognizing victims after participating in the education and simulation. In addition, the simulation met many of the American Association of Colleges of Nursing's new Essentials further highlighting the importance of this clinical activity within the nursing curriculum. Nursing education is obligated to empower students to identify social determinants of health and advocate for social justice related to vulnerable populations. Because nurses are the largest group of healthcare providers, they are likely to encounter human trafficking victims and should be educated to identify victims in practice.

Exploitation and labor and sex trafficking of children and adolescents is a major public health problem in the United States and throughout the world. Significant numbers of US and non-US-born children and adolescents (including unaccompanied immigrant minors) are affected by this growing concern and may experience a range of serious physical and mental health problems associated with human trafficking and exploitation (T/E). Despite these considerations, there is limited information available for health care providers regarding the nature and scope of T/E and how providers may help recognize and protect children and adolescents. Knowledge of risk factors, recruitment practices, possible indicators of T/E, and common medical, mental, and emotional health problems experienced by affected individuals will assist health care providers in recognizing vulnerable children and adolescents and responding appropriately. A trauma-informed, rights-based, culturally sensitive approach helps providers identify and treat patients who have experienced or are at risk for T/E. As health care providers, educators, and leaders in child advocacy and development, pediatricians play an important role in addressing the public health issues faced by children and adolescents who experience exploitation and trafficking. Working across disciplines with professionals in the community, health care providers can offer evidence-based medical screening, treatment, and holistic services to individuals who have experienced T/E and assist vulnerable patients and families in recognizing signs of T/E.

Human trafficking is a lucrative crime and violation of human rights that exploits women, children and men. It includes trafficking for forced labour, sexual exploitation, forced begging and military conscription. The health effects of trafficking and the health needs of trafficked individuals and trafficking survivors are well documented and urgent. WHO European Region Member States recognize their responsibility to prevent and respond to human trafficking and to mitigate its health effects. As frontline health professionals may be the sole public servants to meet trafficked individuals while they are trafficked, the health system also has both a responsibility and an opportunity to promote and protect the health and other rights of trafficked people. No country or sector alone can address the ongoing challenges of trafficking, nor of emergent challenges such as the COVID-19 pandemic and the war in Ukraine. This scoping review examines 237 articles, reports and grey literature containing research from Canada, the United States of America and the WHO European Region. It summarizes and synthesizes this evidence-informed research to help Member States in addressing the intersection of health care and trafficking and in attempts to prevent and respond to human trafficking. While considerable research covers the epidemiology of trafficking and the service delivery response, there is limited research on prevention efforts. The identified research shapes the considerations for policies to promote a comprehensive health systems approach to human trafficking.

Objetivamos apresentar uma proposta de atendimento psicossocial grupal oferecida para mulheres adultas que cometeram ofensa sexual, cuidadoras e mães. A experiência está sendo desenvolvida no Distrito Federal, Brasil, com pessoas do gênero feminino provenientes de encaminhamento judicial. Carece que os profissionais das áreas da justiça, saúde, serviço social e psicologia avancem no estudo e na compreensão desta temática, de modo a pensarem a atuação e o apoio terapêutico a essas mulheres. O modo de atendimento é focal e breve, com ênfase na criação de um ambiente lúdico como facilitador das interações grupais e da discussão sobre os temas: identidade; confiança nas relações afetivas e sociais; vivência pessoal com violência física e sexual; configuração de gênero; e expressão da sexualidade e futuro. A abordagem individual também se baseia no enfoque dos temas mencionados. O oferecimento de ajuda à mulher cuidadora ou à mãe tem participação ativa na interrupção do circuito abusivo sexual, pois essa violência é extremamente ocultada, ocasionando uma prolongada vulnerabilidade para as vítimas. Ressalta-se o valor do texto indicando a descrição de ação voltada para uma população permanentemente não estudada e evitada em seu reconhecimento. Os limites desta proposta encontram-se na falta de outras iniciativas que possibilitem uma discussão sobre essa experiência.(AU)


We aim to present a proposal of a group psychosocial intervention offered for adult female sexual offenders, caregivers, and mothers. The intervention is being developed at Federal District, Brazil, with female people coming from judicial referrals. Professionals in the areas of justice, health, social work, and psychology need to advance in the study and understanding this theme to think about action and therapeutic support for these women. The intervention is a focal and brief approach, with emphasis on the creation of a ludic environment as a facilitator of group interactions and discussion about the themes: identity; trust in affective and social relationships; personal experience with physical and sexual violence; gender configuration; and sexuality expression and future. The individual approach is also based on focusing on these themes. The offering of help to the female caregiver or the mother has an active participation in the interruption of the sexual offense circuit, since this violence is extremely hidden, bringing a prolonged condition of vulnerability to the victims. The value of this text is highlighted indicating the description of an action directed to a population that is permanently not studied and whose recognition is avoided. The limits of this proposal are found in the absence of other initiatives that would allow a discussion about this experience.(AU)


Este texto presenta una propuesta de atención psicosocial grupal destinada a mujeres adultas que han cometido delito sexual, a cuidadoras y madres. La intervención se está desarrollando en el Distrito Federal (Brasil), con personas del género femenino provenientes de remisiones judiciales. Es necesario que los profesionales de las áreas de justicia, salud, trabajo social y psicología avancen en el estudio y comprensión de esta temática para pensar en el desempeño y apoyo terapéutico de estas mujeres. El servicio es enfocado y breve, con énfasis en la creación de un ambiente lúdico como facilitador de interacciones grupales y discusión sobre los temas: identidad; confianza en las relaciones afectivas y sociales; experiencia personal con violencia física y sexual; configuración de género; y expresión de la sexualidad y el futuro. El enfoque individual también se centra en estos temas. La oferta de ayuda a la mujer cuidadora o a la madre es importante para la interrupción del circuito de abuso sexual, ya que esta violencia es extremadamente oculta y provoca una vulnerabilidad prolongada a las víctimas. Se destaca el valor del texto con la descripción de la acción dirigida a una población que no es objeto de estudios ni reconocida. Los límites de esta propuesta se encuentran en la ausencia de otras iniciativas que permitan un debate sobre esta experiencia.(AU)

A violência sexual e o aborto legal são temas tabus em nossa sociedade. No campo da saúde, a(o) psicóloga(o) atua em fases distintas, seja na avaliação psicológica do pedido pelo aborto legal, que culminará ou não em sua aquiescência; seja no momento posterior à solicitação, no atendimento em enfermarias ou ambulatorial. Partindo de relato de experiência, este artigo tem como objetivo refletir sobre as possibilidades e desafios da atuação psicológica no atendimento em saúde para pessoas em situação de gestação decorrente de violência sexual e que buscam pelo aborto legal. Para tanto, dividimos o artigo em três momentos. No primeiro deles, será possível encontrar dados conceituais, estatísticos e históricos sobre ambos os temas, trazendo recortes nacionais e internacionais. No segundo, trazemos apontamentos sobre o que chamamos de "eixos norteadores", ou seja, dialogamos com aspectos fundamentais para o trabalho nesta seara, sendo eles gênero, família, sexualidade e trauma. Por fim, no terceiro, aprofundamos a reflexão sobre o atendimento psicológico atrelado aos conceitos já discutidos, analisando de forma crítica principalmente um dos pontos mais espinhosos da atuação: a avaliação para aprovação (ou recusa) do pedido pelo aborto. Apoiamo-nos no referencial psicanalítico e defendemos que esta atuação psicológica é primordialmente uma oferta de cuidado, comprometido com as demandas das pessoas atendidas e com a promoção de saúde mental, e consideramos que o papel da psicologia é essencial para o reconhecimento do sofrimento e dos efeitos do abandono socioinstitucional na vida do público atendido.(AU)


Sexual abuse and legal abortion are taboo subjects in our society. On the health area, the psychologist works on different fields, such as psychological evaluation from the request of legal abortion, that will end or not on its approval, and also in a further moment, either the care on wards or ambulatorial treatment. Relying on a case report, this article aims to contemplate the possibilities and challenges from psychological work on healthcare to pregnant women from sexual violence and seek legal abortion. For this purpose, we divide this article in three moments. On the first, it will find definitions, statistics, and historical data about both issues, including national and international information. On the second, we bring notes called 'guiding pillar,' that is, we interact with fundamental aspects from this area, such as gender, family, sexuality, and trauma. On the third one, in-depth discussions we dwell on psychological care tied to the concepts previously addressed, critically analyzing one of the hardest moments of working in this area: the evaluation to approve (or refuse) the request for abortion. We lean over psychoanalytic thoughts and argue that this psychological work is primarily an offer of care, committed to the needs from those who seek us and to promoting good mental health and, also, we consider that psychology is essential to acknowledge the suffering and the effects of social and institutional neglect on the lives of the people seen.(AU)


La violencia sexual y el aborto son temas tabús en nuestra sociedad. En el campo de la salud, el(la) psicólogo(a) actúa en diferentes fases: en la evaluación psicológica de la solicitud del aborto legal, que culminará o no en su obtención, y/o en el momento posterior a la solicitud en la atención en enfermería o ambulatorio. Desde un reporte de experiencia, este artículo pretende reflexionar sobre las posibilidades y los desafíos de la Psicología en la atención en salud para personas en estado de embarazo producto de violencia sexual y que buscan un aborto legal. Para ello, este artículo está dividido en tres momentos. En el primer, presenta datos conceptuales, estadísticos e históricos sobre los dos temas, trayendo recortes nacionales e internacionales. En el segundo, comenta los llamados "ejes temáticos", es decir, se establece un diálogo con aspectos fundamentales para el trabajo en este ámbito, como género, familia, sexualidad y trauma. Por último, en el tercer, profundiza en la reflexión sobre la atención psicológica asociada a los conceptos discutidos, analizando de forma crítica uno de los puntos más espinosos de la actuación: la evaluación para la aprobación (o negativa) de la solicitud de aborto. Se utilizó el referencial psicoanalítico y se argumenta que esta atención psicológica es sobre todo una forma de cuidado, comprometida con las demandas de las personas atendidas y la promoción de la salud mental, y el papel de la Psicología es esencial para reconocer el sufrimiento y los efectos del abandono socioinstitucional en la vida del público atendido.(AU)

O presente texto tem o objetivo de explanar ações desempenhadas por psicólogas(os) trabalhadoras(es) dos Centros de Referência Especializados de Assistência Social (Creas), em situações de violência intrafamiliar, identificadas a partir do estude empírico realizado por mim, psicóloga pesquisadora, também trabalhadora de um Creas. Participaram da pesquisa doze psicólogas(os), trabalhadoras(es) destes centros, em sete municípios do interior do Rio Grande do Sul, onde foram realizadas, presencialmente, as entrevistas. A análise dos dados apontou para uma compreensão metodológica a partir de três dimensões já apontadas na bibliografia, sendo elas: a) Acolhida Inicial, que demonstra que esses profissionais geralmente iniciam suas práticas com uma família ou indivíduo indo ao encontro destes, buscando a vinculação dos mesmos com o serviço; b) Acompanhamento Especializado, onde essas(es) trabalhadoras(es) desenvolvem suas práticas com diversidade e criatividade, a partir de visitas domiciliares, trabalhos com grupos, indivíduos ou famílias, geralmente em conjunto com outros profissionais, principalmente assistentes sociais; c) Articulação com a Rede, onde se identificou um importante movimento para o trabalho em conjunto com outros serviços disponíveis no território. Por fim, as considerações finais indicam que ainda há um caminho a ser trilhado com relação à definição das práticas dos psicólogos no Creas. Porém, há muito que se falar a respeito de práticas que já estão ocorrendo. Assim, tornam-se relevantes as pesquisas acadêmicas nesse contexto, pois ao inserir os profissionais psicólogos trabalhadores da política, eles podem promover uma articulação entre a produção do fazer cotidiano e a reflexão teórica e acadêmica sustentada pelas pesquisas.(AU)


This study aims to explore the activities developed by psychologists from the Centros de Referência de Atenção Especializada (CREAS - Brazilian Specialized Social Assistance Reference Centers), regarding situations of intrafamily violence identified by me, the author, a research psychologist and, CREAS worker. Participants include 12 psychologists who work on such centers from 7 cities in the countryside of Rio Grande do Sul, Brazil, where the interviews were conducted in person. The data analysis pointed toward a methodological comprehension based on 3 dimensions that have already been mentioned in the literature: the Initial Approach, which shows that these workers usually initiate their practices with a family or an individual by going after them, seeking bond development with the service. The Specialized Follow-up, in which these workers develop their practices with diversity and creativity, through home visits and activities with group, family or individual, often with other workers, such as social assistance workers. And the Network Articulation, in which a significant movement toward working with the public services available on each territory is identified. Finally, there is still a path to be taken regarding the definition of psychological practices on CREAS, however, there is much to be noticed of what has already been occurring. Thus, academic research on such context is relevant since the inclusion of psychologists who work in this policy may promote an articulation between daily activity and the theoretical and academic reflection, supported by the research.(AU)


Este texto tuvo por objetivo explicar las acciones desarrolladas por psicólogas/os trabajadoras/es en los "Centros de Referência Especializados de Assistência Social" (CREAS) (Centros de Referencia Especializados en Asistencia Social), respecto a las situaciones de violencia intrafamiliar, identificadas por un estudio empírico hecho por una psicóloga-investigadora que actúa en un CREAS. Participaron 12 psicólogas/os que trabajan en estos centros, en siete ciudades del interior de Rio Grande do Sul (Brasil), donde se llevó a cabo las entrevistas en persona. El análisis de datos apuntó a una comprensión metodológica de tres dimensiones ya destacadas en la bibliografía: la Acogida Inicial, que enseña que estos profesionales generalmente empiezan sus prácticas con una familia o individuo buscando el encuentro para la promoción de la vinculación con el servicio; El Seguimiento Especializado, en el que desarrollan sus prácticas con diversidad y creatividad desde visitas domiciliarias, trabajos con grupos, individuos o familias, generalmente junto a otros profesionales, sobre todo con trabajadores sociales; y Articulación con la Red de Servicios, en la cual se identificó un importante movimiento para el trabajo con otros servicios disponibles en el territorio. Por fin, se observa que todavía hay un camino por recorrer en relación a la definición de las prácticas de psicólogos en CREAS, aunque hay mucho que decir respecto a las prácticas que ya están ocurriendo. Así, se vuelven relevantes las investigaciones académicas en ese contexto por introducir a los profesionales psicólogos trabajadores de la política, las cuales pueden promover una articulación entre la producción del hacer cotidiano y la reflexión teórica y académica sustentada por las investigaciones.(AU)

Human trafficking is increasingly recognized as a significant global public health issue. Human trafficking (HT) exists in all nations, including Canada, and is estimated to impact 25 million people globally. HT is a crime that is not always visible. It is associated with a myriad of deleterious health outcomes arising from adverse living and working conditions, and the physical, sexual, and/or psychological violence often accompanying it. Human trafficking also disproportionately affects people living under vulnerable circumstances, particularly those with intersecting vulnerabilities. Public health can advance health equity for trafficked persons and add value to existing anti-trafficking (AT) efforts. Among its many contributions, public health can bring its expertise in health promotion and surveillance. While efforts to incorporate a public health perspective are already underway in at least the United States and the United Kingdom, the nexus of public health and AT is still nascent and requires further development. A public health approach to trafficking focused on intervening on the upstream drivers of well-being can add value to the extant counter-trafficking paradigm. This commentary is intended to catalyze discussion in Canada and elsewhere as to what public health can contribute to this emergent field.


RéSUMé: Le trafic d'êtres humains (TEH) est de plus en plus reconnu comme un grave problème de santé publique dans le monde. Il est présent dans tous les pays, y compris au Canada, et toucherait 25 millions de personnes selon les estimations. Le TEH est un crime qui n'est pas toujours visible. Il est associé à d'innombrables résultats cliniques délétères découlant de mauvaises conditions de vie et de travail et de la violence physique, sexuelle et/ou psychologique qui l'accompagne souvent. Le trafic d'êtres humains touche aussi démesurément les personnes en situation de vulnérabilité, surtout celles ayant des vulnérabilités croisées. La santé publique peut favoriser l'équité en santé chez les victimes et ajouter de la valeur aux efforts existants de lutte contre ce fléau. Elle peut, entre autres, contribuer son savoir-faire en promotion et en surveillance de la santé. Une démarche d'intégration d'une perspective de santé publique est déjà entamée, du moins aux États-Unis et au Royaume-Uni, mais la jonction entre la santé publique et la lutte contre la traite est encore naissante et doit être développée. Une démarche de santé publique à l'égard du trafic d'êtres humains qui intervienne sur les facteurs favorisant le bien-être en amont pourrait ajouter de la valeur au paradigme existant de la lutte contre la traite. Notre commentaire se veut un moyen de susciter un débat au Canada et ailleurs dans le monde sur ce que la santé publique peut apporter à ce domaine en émergence.

The convergence of multiple social determinants is thought to increase an individual's vulnerability to exploitation by forcing reliance on precarious opportunities and dependence on potentially harmful individuals and groups. Determining which individual, interpersonal, and systemic factors contribute to an individual's vulnerabilities can be key to preventing the person from experiencing human trafficking. In this article, the authors closely examine the social determinants of health to better understand how they can contribute to a person becoming trafficked. The authors also highlight an integrated public health care approach to addressing human trafficking based on understanding the impact of social determinants on vulnerable populations, establishing therapeutic relationships with patients who have experienced trafficking, and the use of interdisciplinary teams to address patient vulnerabilities. The authors contend that human trafficking is a violation of one's right to health and should be viewed as such when developing programs, rendering services, and treating this patient population.

BACKGROUND: Little is understood about child welfare involvement (CWI) in cases where the birth mother has experienced human trafficking. OBJECTIVES: The aim of this study was to explore provider perceptions of the impact of CWI for the trafficked mother. METHODS: Participants were selected among providers caring for trafficked birth mothers. Semi-structured interviews were conducted with providers and qualitative content analysis was conducted. RESULTS: Interviewees reported reasons for CWI, positive and negative impacts of CWI and provided recommendations for systems improvement. CONCLUSION FOR PRACTICE: Recommendations from this exploratory study include mechanisms to support trafficked mothers, train hospital social workers, and systems change. During the prenatal period, strategies to support the trafficked mother may include addressing gaps in social determinants of health, ensuring appropriate medical and mental health care, early screening and referral to substance use treatment services, enhancing community support, and working to develop safety plans for survivors and their families. Enhanced engagement of social workers and all providers to improve understanding of the unique complexity of trafficked mothers is needed. Education should include an understanding that judgement of a caretaker's ability to parent should be current and holistic and not reflexive based on history in the electronic medical record. An exploration of the child welfare system itself should also be undertaken to identify and modify discriminatory laws and policies. Finally, efforts to address social determinants of health in the community and enhance the trauma-informed nature of child welfare referrals could improve the lives of trafficked mothers.

OBJECTIVES: Human trafficking is a public health issue that has substantial health consequences. However, research on the preparedness of health facilities to serve people who have experienced or are experiencing human trafficking is limited. We examined health facility preparedness related to human trafficking in Michigan. METHODS: We used a cross-sectional design to explore health facility preparedness related to human trafficking in Michigan. We sampled 10 prosperity regions according to health facility type (Federally Qualified Health Centers, health departments, and hospitals). An email invitation was sent to identified health facility personnel (eg, administrators, physicians, nurses) from May to June 2019 with a link to a 26-item survey with items about current policies, protocols, and areas for improvement related to human trafficking. RESULTS: Forty-two Federally Qualified Health Centers, health departments, and hospitals responded to the survey. Most health facilities had screening policies and response protocols related to child maltreatment (screening: 32/41 [78.0%]; response: 38/40 [95.0%]) and intimate partner violence (screening: 33/41 [80.5%]; response: 37/40 [92.5%]). However, fewer facilities had a screening policy (16/41 [39.0%]) and response protocol (25/40 [62.5%]) for human trafficking; more of these facilities focused on sex trafficking than on labor trafficking. The top needs related to human trafficking were (1) individual-level health care provider training, (2) health facility-level screening policies and response protocols, (3) community-level resources, and (4) societal-level awareness, funding allocation, and data. CONCLUSIONS: Facility-level resources are needed to ensure that health care providers have adequate training and support to address human trafficking in the health care system.

The objectives of this study were (1) to assess the knowledge and perceptions of human trafficking (HT) among leaders and staff from 11 community-based organizations (CBOs) and faith-based organizations (FBOs) in South Los Angeles, and (2) to identify gaps in knowledge of HT and inform community organizations regarding possible best practices in health promotion for addressing this emerging public health problem. A self-administered survey was conducted during the period from 4 December 2015 to 28 January 2016. Descriptive statistics were generated and a logistic regression model was constructed using SAS 9.3. A total of 277 CBO and FBO leaders and staff completed the survey. Participants demonstrated high levels of knowledge of HT but their knowledge was not comprehensive, as gaps exist in recognizing the context in which HT usually takes place; understanding the local laws that govern this activity; and ways to follow related policies/procedures when the problem is suspected. A majority (a) believed there were not enough services in Los Angeles County to help survivors of HT, (b) could not recognize the signs of HT, and (c) did not know what steps to take if they suspected this criminal activity. A statistically significant association was found between education and participants' knowledge of HT, and with their beliefs and attitudes toward this violation of human rights. Study findings suggest that, generally, CBO/FBO leaders and staff in South Los Angeles have good knowledge about HT. However, notable gaps in knowledge and misperceptions remain, suggesting opportunities for Public Health to further educate and intervene.

American Indian and Alaska Native (AI/AN) land rights, sovereignty conflicts, and health outcomes have been significantly influenced by settler colonialism. This principle has driven the numerous relocations and forced assimilation of AI/AN children as well as the claiming of AI/AN lands across the United States. As tribes across the country begin to reclaim these lands and others continue to struggle for sovereignty, it is imperative to recognize that land rights are a determinant of health in AI/AN children. Aside from the demonstrated biological risks of environmental health injustices including exposure to air pollution, heavy metals, and lack of running water, AI/AN children must also face the challenges of historical trauma, the Missing and Murdered Indigenous Peoples crisis, and health care inequity based on land allocation. Although there is an undeniable relationship between land rights and the health of AI/AN children, there is a need for extensive research into the impacts of land rights and recognition of sovereignty on the health of AI/AN children. In this article we aim to summarize existing evidence describing the impact of these factors on the health of AI/AN children and provide strateg ies that can help pediatricians care and advocate for this population.

ABSTRACT: Climate change is a threat to the public health with wide-reaching impacts that are becoming more studied and recognized. An aspect of climate change that has not yet gained adequate scholarly attention is its potential impact on human trafficking. We review the potential impact of climate change on risk factors to human trafficking including poverty, gender inequality, political instability, migration or forced displacement, and weather disasters. We conclude that climate change is a crucially important consideration in understanding the complex and multifactorial risks for human trafficking. These findings add to the priority for health professionals to embrace efforts to prevent and to mitigate the effects of climate change and to take account of these risk factors in screening and identifying trafficked persons.

American Indian and Alaska Native (AI/AN) populations have substantial health inequities, and most of their disease entities begin in childhood. In addition, AI/AN children and adolescents have excessive disease rates compared with the general pediatric population. Because of this, providers of pediatric care are in a unique position not only to attenuate disease incidence during childhood but also to improve the health status of this special population as a whole. This policy statement examines the inequitable disease burden observed in AI/AN youth, with a focus on toxic stress, mental health, and issues related to suicide and substance use disorder, risk of and exposure to injury and violence in childhood, obesity and obesity-related cardiovascular risk factors and disease, foster care, and the intersection of lesbian, gay, bisexual, transgender, queer, and Two-Spirit and AI/AN youth. Opportunities for advocacy in policy making also are presented.

Trafficking of adults and children for both sex and labor is a human rights violation occurring with alarming frequency throughout the world, and resulting in profound harm to close-knit communities and severe health consequences for victims. Certain areas, such as the country of Paraguay, are at a higher risk for trafficking due to unique economic, cultural, and geographic factors. Thousands of people, especially children, are trafficked within Paraguay's borders, and many eventually are transported to neighboring countries and sometimes to Europe and elsewhere. Using case study methodology and "city" as the unit of study, researchers interviewed 18 key anti-trafficking stakeholders from government and nongovernmental organizations in two major metropolitan centers for trafficking in Paraguay, Asunción, and Encarnación. Through semistructured interviews, this qualitative study examines risk factors for trafficking, health outcomes, interventions needed within the health care sector, and programs needed to combat trafficking. We identified risk factors including poverty, marginalization of indigenous people, gender inequality, domestic servitude of children (criadazgo), and political hesitance to enact protective legislation. Victims of trafficking were reported to suffer from physical injuries, unintended pregnancies, sexually transmitted infections, and mental health issues such as depression and posttraumatic stress disorder. These predispose victims to difficulties reintegrating into their communities and ultimately to retrafficking. A major gap was identified in the lack of sufficient lodging and rehabilitation services for rescued victims, affordable access to trauma-sensitive health care for victims, and scarce mental health services. Many of the findings are applicable across the world and may be of use to guide future anti-trafficking efforts in Paraguay and beyond.

Human trafficking is associated with a profound burden of physical and psychological trauma. Survivors of trafficking interact with the health care system during and after their experiences of trafficking. Socioeconomic isolation, stigma, shame, guilt, fear of judgment, fear of retribution by traffickers, fear of law enforcement authorities, and other factors known to inhibit disclosure can exert a formative influence on survivors' health care experiences, health care access, and health services engagement. Using a mixed qualitative-quantitative social science research method, known as by-person factor analysis (or Q-methodology), the current analysis systematically examines the scope of trafficking survivors' health care experiences and perceptions of medical care, health care access behaviors, and degree of engagement with health services. Among 33 survivors of human trafficking surveyed, 21 met inclusion criteria for this analysis. Three distinct profiles of survivor health care experiences and health services engagement-Avoidant, Distrustful, and Constrained-are identified from the aggregate of survivors' perceptions of medical care. Although there are salient differences across the three survivor profiles, a feeling of disenfranchisement is a common thread and directly related to health care access behaviors and health services engagement. Understanding that the feeling of disenfranchisement functions as a filter through which trafficking survivors perceive and experience medical care can help health care professionals take appropriate countermeasures. Implications for improving health care access and engagement include the implementation of trauma-responsive, culturally sensitive, and survivor-centered care practices.
