ABSTRACT

PURPOSE: An up-to-date overview of epidemiology, etiology and pathophysiological mechanisms, diagnostic and evaluation methods, current treatment status and future directions of subjective tinnitus in adults. METHODS: Review of current evidence-based literature on subjective tinnitus in adults. RESULTS: The prevalence of subjective tinnitus in the adult population is estimated to be around 14%, and it tends to increase with age. Subjective tinnitus is a complex condition with multiple factors contributing to its origin. However, the exact causes and underlying mechanisms remain unknown. Potential causes may include hearing loss, dysfunction in the somatosensory system, and auditory cortical dysfunction, although severe underlying pathology is rare. Currently, diagnosis primarily relies on patient self-reported medical history and physician-based clinical assessment due to the lack of objective testing. Various treatment and management options have been proposed, but their effectiveness varies, and there is no universally agreed-upon treatment option. CONCLUSIONS: Tinnitus is a complex and heterogeneous disease with a high incidence rate and a tendency to increase with age. A holistic perspective is needed to understand the generation, perception, and emotional responses to tinnitus. Diagnosis requires a comprehensive assessment based on medical history and relevant examinations, identification of concurrent psychosomatic comorbidities, and active pursuit of objective diagnostic methods. At the same time, on the basis of existing treatment plans and combining emerging technologies, we will develop new personalized, precise, and combined treatment plans.

ABSTRACT

In order to determine the polarizability and hyperpolarizability of a molecule, several key parameters need to be known, including the excitation energy of the ground and excited states, the transition dipole moment, and the difference of dipole moment between the ground and excited states. In this study, a machine-learning model was developed and trained to predict the molecular polarizability and second-order hyperpolarizability on a subset of QM9 data set. The density of states was employed as input to the model. The results demonstrated that the machine-learning model effectively estimated both polarizability and the order of magnitude of second-order hyperpolarizability. However, the model was unable to predict the dipole moment and first-order hyperpolarizability, suggesting limitations in its ability to predict the difference of dipole moment between the ground and excited states. The computational efficiency of machine-learning models compared to traditional quantum mechanical calculations enables the possibility of large-scale screening of molecules that satisfy specific requirements using existing databases. This work presents a potential solution for the efficient exploration and analysis of molecules on a larger scale.

ABSTRACT

In this study, with the assistance of quaternary ammonium salts we have successfully prepared a new family of salicylate-stabilized heterobimetallic Pb-Ti-oxo clusters, including H(TEA)[Pb2Ti6(µ2-O)2(µ3-O)2(OiPr)4(PA)2(Sal)6(NO3)2] (PTC-321; TEA = tetraethylammonium; HOiPr = isopropanol; H2PA = phenylphosphonic acid; H2Sal = salicylic acid), {PbTi3(µ2-O)(µ3-O)(OiPr)2(PA)(Sal)3(DMF)·CH3CN}n (PTC-322; DMF = dimethylformamide), {PbTi5(µ3-O)6(Sal)3(CH3COO)2(DMF)(OiPr)2}n (PTC-323), [Pb2Ti4(Sal)6(EtO)2(OiPr)6(HOiPr)2]·CH3NH2 (PTC-324; EtOH = CH3CH2OH), H[Pb4Ti9(µ2-O)2(µ3-O)(µ4-O)6(Sal)7(OiPr)13] (PTC-325), and Pb2Ti12(µ2-O)3(µ3-O)3(µ4-O)4(Sal)4(OEt)24 (PTC-326). Single-crystal X-ray diffraction studies demonstrate that the {Ti3Pb(Sal)3} unit acts as the building block to constitute the diverse assembly of PTC-321-PTC-323. Thereinto, the clusters in PTC-322 and PTC-323 are connected into infinite one-dimensional chains. Furthermore, the solvent effects have facilitated the heterobimetallic Pb-Ti-oxo clusters into various configurations in PTC-323-PTC-326. Solid-state ultraviolet-visible spectroscopy analysis indicates that the optical absorption bands of these compounds shift effectively toward the visible-light region, and they were also employed as electrode precursors to investigate their visible-light-driven photocurrent response.

ABSTRACT

Because of the complex components, simple content determination can hardly reflect the overall quality of Guizhi Fuling Capsules. Therefore, it is necessary to carry out a multi-component dissolution test. The variability of quality among different batches of products from different manufacturers is a common problem of Chinese medicine solid preparations. To comprehensively control the quality of Guizhi Fuling Capsules, we studied the dissolution behaviors of 7 index components in the capsules under different conditions, and investigated the consistency of dissolution behaviors among different batches of products from the same manufacturer. The basket method of general rule 0931 in Chinese Pharmacopoeia was adopted, and the rotating speeds were set at 50, 75, and 100 r·min~(-1), respectively. The hydrochloric acid solution(pH 1.2), acetate buffer solution(pH 4.0), pure water, and phosphate buffer solution(pH 6.8) were used as the dissolution media. Automatic sampling was carried out at the time points of 5, 10, 20, 30, 45, and 60 min, respectively. The cumulative dissolution of 7 index components was measured through ultra-performance liquid chromatography(UPLC). The difference factor f_1 and similarity factor f_2 were calculated to comprehensively evaluate the similarity of the dissolution curves among 8 batches of Guizhi Fuling Capsules, and a variety of dissolution and release equations were fitted. The results showed that multiple components had faster dissolution rates at higher rotating speed and in hydrochloric acid medium. The 8 batches of Guizhi Fuling capsules showed the average f_1 value lower than 15 and the average f_2 value higher than 50, which indicated that different batches of products had similar dissolution behaviors. Most components had synchronous dissolution behaviors and similar release cha-racteristics. This study provides a reference for the quality consistency evaluation among batches, processing optimization, and dosage form improvement of Guizhi Fuling Capsules.

ABSTRACT

Tumor hypoxia has proven to be the major bottleneck of photodynamic therapy (PDT) to clinical transformation. Different from traditional O2 delivery approaches, here we describe an innovative binary photodynamic O2-economizer (PDOE) tactic to reverse hypoxia-driven resistance by designing a superoxide radical (O2•-) generator targeting mitochondria respiration, termed SORgenTAM. This PDOE system is able to block intracellular O2 consumption and down-regulate HIF-1α expression, which successfully rescues cancer cells from becoming hypoxic and relieves the intrinsic hypoxia burden of tumors in vivo, thereby sparing sufficient endogenous O2 for the PDT process. Photosensitization mechanism studies demonstrate that SORgenTAM has an ideal intersystem crossing rate and triplet excited state lifetime for generating O2•- through type-I photochemistry, and the generated O2•- can further trigger a biocascade to reduce the PDT's demand for O2 in an O2-recycble manner. Furthermore, SORgenTAM also serves to activate the AMPK metabolism signaling pathway to inhibit cell repair and promote cell death. Consequently, using this two-step O2-economical strategy, under relatively low light dose irradiation, excellent therapeutic responses toward hypoxic tumors are achieved. This study offers a conceptual while practical paradigm for overcoming the pitfalls of phototherapeutics.

ABSTRACT

The current study aimed to explore the effect of self-acupoint massage (SEAM) on blood glucose level and quality of life in community-dwelling older adults with type 2 diabetes mellitus (T2DM). Sixty-six older adults with T2DM were enrolled and randomly divided into observation and control groups. Participants in the control group received routine nursing interventions, whereas participants in the observation group received a SEAM intervention in addition to routine nursing interventions. After 12 weeks of SEAM, glycosylated hemoglobin (HbA1c) levels in the observation group decreased from 8.35% (SD = 1.84%) at baseline to 7.29% (SD = 1.38%) (p < 0.01). Total score of the Diabetes-Specific Quality of Life Scale (DSQLS) in the observation group improved from 45.96 (SD = 4.29) at baseline to 41.3 (SD = 3.89) (p < 0.01). The physiological dimension of the DSQLS in the observation group improved from 49.65 (SD = 7.33) at baseline to 38.54 (SD = 4.68) (p < 0.01). As SEAM effectively decreased older adults' HbA1c level and improved their quality of life, it can be used as a complementary approach to routine nursing interventions for community-dwelling older adults with T2DM. ABSTRACT

Structure-inherent targeting (SIT) agents are of particular importance for clinical precision medicine; however, there still exists a great lack of SIT phototheranostics for simultaneous cancer diagnosis and targeted photodynamic therapy (PDT). Herein, for the first time, we propose a "one-for-all" strategy by using the Förster resonance energy transfer (FRET) mechanism to construct such omnipotent SIT phototheranostics. Of note, this novel tactic can not only endow conventional sensitizers with highly effective native tumor-targeting potency but also simultaneously improve their photosensitization activities, resulting in dramatically boosted therapeutic index. After intravenous injection of the prepared SIT theranostic, the neoplastic sites are distinctly "lighted up" and distinguished from neighboring tissues, showing a near-infrared signal-to-background ratio value as high as 12.5. More importantly, benefiting from the FRET effect, markedly amplified light-harvesting ability and 1O2 production are demonstrated. Better still, other favorable features are also simultaneously achieved, including specific mitochondria anchoring, augmented cellular uptake (>13-fold), as well as ideal biocompatibility, all of which allow orders-of-magnitude promotion in anticancer efficiency both in vitro and in vivo. We believe this one-for-all SIT platform will provide a new idea for future cancer precision therapy.

ABSTRACT

Osteosarcoma is a primary malignant bone tumor that has a high potential to metastasize to lungs. Little is known about the mechanisms underlying the dissemination of OS cancer cells to lungs. We performed whole exome sequencing of 13 OS primary tumors, with matched lung metastases and normal tissues. Phylogenetic analyses revealed that lung metastatic tumors often harbor clones that are nonexistent or rare in the matched primary OS tumors. Spatially and temporally separated lung metastases were from parallel seeding events with a polyphyletic pattern. Loss of TP53 or RB1 is among the early events during OS tumorigenesis, while loss of PTEN is involved at the later stages associated with lung metastases. Finally, KEAP1 was identified as a novel biomarker for increased metastatic risk. Patients whose primary tumors harbored KEAP1 amplification have significantly poorer lung-metastasis free survival. This finding was validated in two independent datasets. Further, in vitro experiments exhibited that KEAP1 depletion suppressed the invasion of OS cells. Our findings uncover the patterns of clonal evolution during OS progression and highlight KEAP1 as a novel candidate associated with the risk of lung metastasis in OS patients.

BACKGROUND: A few reports had revealed the clinical associations between hypopituitarism and non-alcoholic fatty liver disease (NAFLD). Further evidences were needed. OBJECTIVES: To report 5 cases of young patients with rapidly progressive NAFLD in conjunction with hypopituitarism, aiming to detect the associations. METHODS: Clinical data of the 5 patients were analyzed retrospectively. All had decompensated liver cirrhosis that was finally attributed to NAFLD. Hypopituitarism was a result of pituitary stalk interruption syndrome in 3 patients and craniopharyngioma in 2. RESULTS: 4 patients were overweight (BMI, 24.2 to 28.4kg/m2). All had insulin resistance (HOMA-IR, 4.8 to 7.2). All suffered from at least one metabolic disorder. All had decompensated liver cirrhosis. The average time duration was 6.9 years between the onset of abnormal liver function and decompensated liver cirrhosis. Fatty liver could be detected in all patients. All had anterior hypopituitarism, and 2 also had posterior pituitary dysfunction. The hormone supplements were insufficient. CONCLUSION: Hypopituitarism may be a rare cause of rapidly progressive NAFLD. Insulin resistance and metabolic disorders caused by multiple hormonal deficiencies may contribute to it. Hormone supplement therapy, especially the growth hormone supplement, should be given at the early age to prevent the severe liver disease.

ABSTRACT

This study reports an oriented and homogenous cobalt-metalloporphyrin network (PIZA-1) thin film prepared by liquid phase epitaxial (LPE) method. The thickness of the obtained thin films can be well controlled, and their photocurrent properties can also be tuned by LPE cycles or the introduction of conductive guest molecules (tetracyanoquinodimethane and C60 ) into the PIZA-1 pores. The study of quartz crystal microbalance adsorption confirms that the PIZA-1 thin film with [110]-orientation presents much higher selectivity of benzene over toluene and p-xylene than that of the PIZA-1 powder with mixed orientations. These results reveal that the selective adsorption of volatile organic compounds highly depends on the growth orientations of porphyrin-based metal-organic framework thin films. Furthermore, the work will provide a new perspective for developing important semiconductive sensing materials with improved selectivity of guest compounds.

ABSTRACT

Two copper(I) halide-based coordination polymers, namely, [Cu3Cl2(Tipa)2]Cl (1, (Tipa = tris(4-(1H-imidazol-1-yl)phenyl)amine) and [Cu3I3(CuI)2(Tipa)3] (2), were obatined under the solvothermal reactions, respectively. Compound 1 with the chain structure shows unique Cu···Cu interactions, C-H···Cl halogen hydrogen bonds, and π···π stacking interactions. It also exhibits unusual mechanochromic and photocatalytic properties on degradation of methylene blue (MB).

ABSTRACT

Arrays of ultrasmall and uniform carbon nanodots (CDs) are of pronounced interest for applications in optical devices. Herein, we describe a low-temperature calcination approach with rather inexpensive reactants. After glucose molecules had been loaded into the pores of metal-organic frameworks (MOFs), well-defined CD arrays were produced by heating to 200 °C. The size and spacing of the CDs could be controlled by the choice of templating MOF: HKUST-1, ZIF-8, or MIL-101. The sizes of the obtained CDs were approximately 1.5, 2.0, and 3.2 nm, which are close to the corresponding MOF pores sizes. The CD arrays exhibited interesting photophysical properties, including photoluminescence with tunable emission and pronounced nonlinear optical (NLO) effects. The NLO properties of the obtained CD arrays were significantly different from those of a CD suspension, thus indicating the existence of collective phenomena.

Six polyoxo-titanium clusters (PTCs) with varying nuclearities containing Ti-N bonds and heteronuclearity, namely, [Ti6(µ3-O)2(µ2-O)2(O3P-Phen)2(OiPr)10(1-hbta)2] (PTC-37), Ti8(µ3-O)2(µ2-O)2(O3P-Phen)2(OiPr)16(adn)2(NO3)2] (PTC-38), [Ti4(µ3-O)(µ2-O)(µ2-OiPr)2(OiPr)4(O3P-Phen)3(1,10-phn)](HOiPr) (PTC-39), [Ti4(µ3-O)(µ2-OiPr)3(OiPr)5(O3P-Phen)3(Im)] (PTC-40), [Ti4(µ3-O)(µ2-OiPr)3(OiPr)5(O3P-Phen)3(Im)][Ti3M(µ3-O)(µ2-OiPr)3(OiPr)3(O3P-Phen)3(Im)] (M = Co for PTC-41 and M = Zn for PTC-42; O3P-Phen = phenyl phosphonate, 1-hbta = 1-hydroxy benzotriazolate, adn = adenine, 1,10-phn = 1,10-phenanthroline, Im = imidazolate, and OiPr = isopropoxide) were prepared as crystalline samples and structurally characterized. Simultaneous doping of nitrogen and transition metal heteroatoms into the Ti-O clusters created complex chemical environments in the resulting hybrid materials. Thus, photocatalytic methylene blue degradation studies were performed to understand structure-property relationships in these Ti cluster-based materials. The complex chemical environment created in these novel molecular clusters had proved to exhibit ligand-dependent photocatalytic activities under normal sunlight. Adenine-functionalized PTC-38 presented moderate activities, while other PTCs all show rapid dye degradation.

ABSTRACT

Homochiral metal-organic frameworks (HMOFs) are efficient materials for enantioselective adsorption. However, the combination of size selectivity and enantioselectivity is still a major challenge in the field of HMOFs. Herein, two enantiomorphic HMOFs built from predesigned proline-derived ligands are presented. Both of them show multiple homochiral features: they contain four different helical chains and three types of helical channels. Due to the size effect of the helical channels, each HMOF can enantioselectively adsorb methyl lactate with high ee. The results reveal a new approach toward size-dependent enantioselective separation of racemic compounds by using HMOFs built from inexpensive proline derivatives.

By employing a tris(4-(1H-imidazol-1-yl)phenyl)amine (Tipa) ligand, three new metal-organic frameworks, [Zn2(Tipa)(4,4'-bpdc)1.5(H2O)(NO3)]·2(DMF)·H2O (1; 4,4'-bpdc = 4,4'-biphenyldicarboxylate, DMF = N,N-dimenthylformamide), [Cd(Tipa)Cl2]·2(DMF)·H2O (2), and [Co(Tipa)Cl2(H2O)]·DMF·H2O (3), have been synthesized solvothermally. Compound 1 features a three-dimensional (3D) pillared-layer structure with low band gap and interesting photocatalytic properties. Compound 2 is a 2-fold interpenetrating (3,6)-connected porous framework, and it shows highly selective adsorption of C2H2, CO2, and C2H4 over CH4. Compound 3 with honeycomb-like layers exhibits unusual 2D + 2D→ 3D polycatenation (2D = two-dimensional). The luminescent properties for these compounds were also investigated.

Two enantiomorphic metal-organic frameworks with zeotype SOD topology have been successfully synthesized from enantiopure L-alanine and D-alanine, respectively, which demonstrates the feasibility of fabricating MOFs that integrate the 4-connected zeotype topologies and homochiral nature by the employment of enantiopure amino acids.

A series of zinc(II) compounds, [Zn2(btca)2(im)2]·(DMA) (1; H2btca = benzotriazole-5-carboxylate acid, im = imidazole, and DMA = N,N-dimethylacetamide), [Zn(btca)(im)]·(DMF) (2; DMF = N,N-dimethylformamide), [Zn2(btca)2(tmdpy)]·2(DMF)·5(H2O) [3; tmdpy = 1,3-di(4-pyridyl)propane], and [H2N(CH3)2]2[Zn3(btca)4]·(DMF) (4), have been successfully synthesized via rational control of experimental conditions. Single-crystal X-ray diffraction analyses indicate that compounds 1 and 2 are isomers, and both of them exhibit two-dimensional structures with the same uninodal 3-connected fes topology. Additionally, the three-dimensional (3D) structure of 3 was obtained by using tmdpy instead of an im ligand under synthesis conditions similar to those of compound 2. Interestingly, compound 3 presents a pillared-layer structure with a (3,4)-connected (4.8(2).10(3))(4.8(2)) topology. When 4,4'-bipyridine was used to replace tmdpy, the assembly between Zn(2+) ions and H2btca ligands produced a chiral 3D framework of 4. Furthermore, compound 4 showed a new (3,4)-connected topology with a vertex symbol of (4.6.8(4))2(4.6.8)(4.8(2))(6.8(2))2(6.8(5)). A comparison of all compounds suggested that the structural diversity of the compounds could be tuned by altering the auxiliary ligand. In addition, the photoluminescent properties of compounds 1-4 were measured.

ABSTRACT

The TBS is a new method for clinicians to assess the bone quality. It is directly related to the mechanical strength of bone and helps predict fracture risk. The present analysis aimed to investigate the associations between serum PTH levels and TBS by analyzing data from the National Health and Nutrition Examination Survey (NHANES). A total of 3516 participants from the NHANES 2005-2006 were included in this cross-sectional study. The independent variable was serum PTH, and the outcome variable was TBS. The associations of serum PTH levels with TBS were examined using multivariable linear regression models. After adjusting for covariates, there was a negative association between serum PTH level and TBS (ß = - 0.0034; 95% confidence interval, - 0.0050 to - 0.0017). However, in the subgroup analysis stratified by gender, race, and age, this association became negative only in Non-Hispanic White (ß = - 0.0047, 95% CI: -  0.0071 to -  0.0048) and young people (age < 60) (ß = - 0.0036, 95% CI: - 0.0057, - 0.0016), regardless of gender. In addition, the association of serum PTH with TBS was an U-shaped curve, with a point of inflection at 6.71 pmol/L. This study showed that serum PTH level was negatively associated with TBS. Maintaining PTH levels in a lower reasonable clinical range may be beneficial to bone health, especially for young non-Hispanic white.

ABSTRACT

Purpose: Percutaneous endoscopic transforaminal lumbar interbody fusion (PE-TLIF) has become one of the most popular minimally invasive surgeries today. However, the issue of hidden blood loss (HBL) in this surgery has received little attention. This study aims to examine the HBL in PE-TLIF surgery and the effect of tranexamic acid (TXA) on blood loss. Methods: In our research, We conducted a retrospective analysis of 300 patients who underwent PE-TLIF from September 2019 to August 2023. They were divided into 2 groups based on whether they received intravenous TXA injection before surgery. The variables compared included: demographic data, pre-and postoperative hemoglobin (HB), hematocrit (HCT), platelets (PLT), red blood cells (RBC), total blood loss (TBL), visible blood loss (VBL), HBL, operation time, postoperative hospital stay, inflammatory markers, coagulation parameters, and adverse events. Results: Regarding demographic characteristics, besides the operation time, no significant differences were observed between the two groups. Compared with the control group, the TXA group showed a significant reduction trend in TBL, HBL, and VBL (P < 0.05). On the first day after surgery, there were significant differences in prothrombin (PT), activated partial thromboplastin time (APTT), and D-dimer (D-D) levels between the two groups. Similarly, HCT also found similar results on the third day after surgery. No adverse events occurred in either group. Conclusion: Research has found that there is a significant amount of HBL in patients undergoing PE-TLIF. Intravenous injection of TXA can safely and effectively reduce perioperative HBL and VBL. Additionally, compared to the control group, the TXA group shows a significant reduction in operation time.

Over the past 10 years, hip arthroscopy has been increasingly employed to effectively diagnose and safely treat a range of hip pathologies. With research related to hip arthroscopy continually expanding, the number of articles connected with hip arthroscopy has also consistently grown. We aimed to investigate trends and hotspots in hip arthroscopy-related research, and analyze the top 100 most-cited articles on hip arthroscopy. We searched for ("hip arthroscopy") AND ("article" OR "review") AND "English" in the Web of Science database from 1900 to 2022, which was used to obtain all publications relating to hip arthroscopy. Distribution of country, affiliated institution, journal, authors, citation frequency and keywords were analyzed using VOSviewer. A total of 1094 articles were selected from the Web of Science Core Collection (WoSCC) from 1900 to 2022. The number of publications concerning hip arthroscopy displayed an ascending trend over time. Among the countries, the United States emerged as the largest contributor to the number of articles. The highest prolific institution was American Hip Institute. Among the journals, the highest-ranking journal was "Arthroscopy-the Journal of Arthroscopic and Related Surgery," with 8316 citation counts and 262 articles. The area of greatest research interest was diagnosis and therapy in the field. The scientific articles on the subject of hip arthroscopy have risen continuously in recent years. The United States was the most influential country and made the most significant contributions to this field globally. We identified the research direction and trend for the first time and provided the most recent bibliometric analysis on hip arthroscopy, which may assist researchers in conducting studies on hip arthroscopy.
