BACKGROUND: Diabetes is reported to accelerate sarcopenia (age-related loss of muscle mass and function). We aimed to assess muscle mass and strength in elderly diabetics, elderly non-diabetics, younger diabetics and healthy subjects, and to define correlates of muscle mass and strength in these subjects. METHODS: Sixteen elderly diabetics, 16 younger diabetics, 16 elderly non-diabetics and 18 younger non-diabetics were included. Elderly and diabetic subjects were first evaluated with exercise testing. Isokinetic leg extension and flexion tests were performed using a Cybex 350 dynamometer. Muscle mass was calculated using bioelectric impedance analysis. RESULTS: Muscle mass was similar between all groups; however, muscle strength was significantly lower in diabetic and non-diabetic elderly subjects compared with younger diabetic subjects and non-diabetics. Muscle strength was positively correlated with albumin, metabolic equivalent and hemoglobin, and inversely correlated with age, HbA1c, functional capacity and CRP. Independent correlates of muscle strength were age and hemoglobin. There was no clinically significant correlate of muscle mass. Presence or duration of diabetes was not associated with muscle mass or strength. CONCLUSIONS: Uncomplicated diabetes does not seem to accelerate aging-related muscle mass or strength loss. Exercise test parameters may be useful markers in the screening of sarcopenia.

OBJECTIVE: To compare the results of different nutritional screening tools, including NRS-2002, PG-SGA, and NUTRISCORE for the detection of malnutrition in oncology outpatients. STUDY DESIGN: A descriptive study. Place and Duration of the Study: Daily Chemotherapy Unit, Umraniye Training and Research Hospital, Istanbul, Turkiye, between June and July 2021. METHODOLOGY: A total of 69 patients were included in the study, receiving cancer therapy in an outpatient setting. The NRS-2002, PG-SGA, and NUTRISCORE scores were calculated to determine the nutritional status. RESULTS: The mean age of the patients was 56.74 ± 13.48 years, and 59.4% were females. The mean BMI was 27.29 ± 5.27 kg/m2. Among the patients, 55.1% had insufficient nutritional intake or were at risk of malnutrition according to the NRS-2002, 40.6% according to NUTRISCORE, and 59.4% according to the PG-SGA. There was a significant agreement between the results of the NRS-2002 and PG-SGA in a McNemar test (Kappa: 0.320, p = 0.008). CONCLUSION: NRS-2002 and PG-SGA tools offered greater sensitivity in terms of capturing more patients in the precachectic state than NUTRISCORE. Among these, the NRS-2002 is a shorter test, and thus, would seem to be more practical than the PG-SGA. KEY WORDS: Oncology, Malnutrition, Screening tools, NRS-2002, PG-SGA, NUTRISCORE.

INTRODUCTION: The trivalent inactivated influenza vaccine was approved for use in Turkey during the 2018-2019 influenza season. We evaluated beliefs regarding the vaccine and vaccination outcomes in a Turkish population. METHODS: Individuals who were vaccinated with the trivalent inactivated influenza vaccine between November 1 and December 31, 2018, at the Sisli Hamidiye Training and Research Hospital in Istanbul, Turkey, were included in this study. A 15-item questionnaire was completed by a physician during a face-to-face interview with the participants. All participants were followed during the 2018-2019 influenza season through May 2019. The participants were instructed to consult the same physician in case of sudden illness. Participants' beliefs and outcomes were assessed by their vaccination status for the 2017-2018 influenza season. RESULTS: A total of 150 participants were recruited. Their median age was 66 (range, 22-88) years. During the 2017-2018 influenza season, 4.1% had been hospitalized, 53.5% had developed an upper respiratory disease (URD), and 16.2% had been diagnosed with pneumonia. There were no cases of influenza, pneumonia, or hospitalization in the 2019 season; 49.3% of the participants developed a URD (n = 74). Among participants who had been vaccinated during both influenza seasons, 47.5% had had and/or developed a URD, with a higher number of cases during the 2018-2019 season. CONCLUSIONS: After vaccination, no cases of influenza, hospitalization, and pneumonia were observed and the incidence of URD decreased compared with that of the previous season.

BACKGROUND: A 58-year-old schizophrenic male who had been drinking at least 4-5 l of pure water every day for 30 years was admitted to a hospital with complaints of nausea, fatigue, and irregular, fluctuating fevers (up to 39 degrees C). He had previously been prescribed fluoxetine at a dose of 20 mg/day and had been using the drug for 28 days. INVESTIGATIONS: Review of medical history, physical examination, blood analysis, urine analysis, chest radiography and abdominal ultrasound. DIAGNOSIS: Psychogenic polydipsia and fluoxetine-induced syndrome of inappropriate antidiuretic hormone secretion. MANAGEMENT: Fluid restriction and discontinuation of fluoxetine.

OBJECTIVE: It is known that depression is common in obese individuals. Besides the effects of obesity, pathogenic effects of increase in visceral and abdominal fat mass on depression are also being investigated. Our study aimed to show the relationship between visceral fat percentage detected with practical methods and the presence and severity of depression. MATERIALS AND METHODS: Our study included 104 obese patients and 50 healthy controls. In all individuals, the severity of depression was assessed using the Beck Depression Inventory (BDI). Anthropometric measurements, visceral fat percentage, and body fat percentage were measured using the bioelectric impedance method. RESULTS: The mean age was 51.5±12.3 years, and 65 participants (62.5%) were women. BDI scores were statistically higher in the obese group than in the control group (23.1±10.9 and 12.1±9.4, p<0.001). In the obese group, 63.5% of patients were depressed, and in the control group, this was 24%. Women were more depressed in the obese group, but there was no significant difference between men and women in the control group. Body fat percentage was the highest correlating parameter with depression severity. Positive correlation was found between depression severity and body mass index, waist circumference, hip circumference, and visceral fat percentage. In the logistic regression analysis, obesity was found as an independent risk factor for depression (OR: 4.84, 2.1-10.7, p<0.001). CONCLUSION: According to the results of our study, obesity is a significant and independent risk factor for depression. Obesity type and body composition are important factors that determine the severity of depression.

Objectives: Omentin-1, an adipocytokine that increases the insulin sensitivity, has been determined to be reduced in patients with insulin resistance, impaired glucose tolerance, and Type-2 diabetes mellitus. In this study, we have investigated the alterations in Omentin-1 levels with the blood glucose regulation in diabetic patients having poor glycemic control. By this way, we aimed to determine the role of Omentin-1 as a marker in follow-up and monitoring progression of diabetes. Methods: Totally 58 patients with type 2 diabetes mellitus, older than 18 years of age who were having poor glycemic control (HbA1c≥9) were included in this study. In the first visit, all clinical and biochemical parameters of patients were recorded. After baseline evaluation, the patients were advised life style changes, and their medical treatment was determined individually according to the recommendations of the American Diabetes Association guidelines. At the end of the third month patients were re-evaluated. Serum Omentin-1 levels were measured with ELISA. Results: In patients using only oral antidiabetic agents, after exchanging the treatment with insulin, on 3rd month of treatment, there was a significant decrease in serum C-peptide and Omentin-1 levels compared with the initial results (p=0.034, p=0.048, respectively). On the other hand, in patients using insulin treatment from the beginning of the study, there was not any significant alterations in serum C-peptide or Omentin-1 levels compared with the initial results (p>0.05). Conclusions: Serum Omentin-1 levels may change with insulin and metformin treatments in Type-2 diabetic patients. In patients with poor glycemic control, Omentin-1 levels do not change with the regulation of blood glucose levels. A decrease in Omentin-1 and C-peptide levels has been determined after the initiation of insulin therapy. This suggests that, Omentin-1 levels are closely associated with the endogenous insulin reserve and may be used in follow-up of patients.

OBJECTIVE: Arterial stiffness parameters including pulse wave velocity (PWV) and augmentation index (AIx) are associated with increased risk of cardiovascular disease. A close relationship has been demonstrated between vitamin D deficiency and cardiovascular disease. The aim of the present study was to investigate effects of vitamin D deficiency and supplementation on arterial stiffness parameters in patients with normal cardiac functions. METHODS: Study population consisted of 45 patients with vitamin D deficiency and normal cardiac functions. Median age (interquartile range) was 45.0 (12.00) years, and 33 patients were female. Patients were treated with oral administration of vitamin D3. Arterial stiffness parameters were evaluated using Mobil-O-Graph arteriograph system, which detected signals from the brachial artery before and after treatment. RESULTS: Vitamin D levels significantly increased after treatment (9.0 [6.00] nmol/L vs 29.0 [11.50] nmol/L, p<0.001). No significant difference was observed among conventional echocardiographic parameters before or after treatment. Post-treatment PVW and AIx were significantly lower than baseline measurements (6.8 [1.55] m/s vs 6.4 [1.30] m/s, p<0.001 and 23.0 [22.00]% vs 31.0 [14.50]%, p<0.001, respectively). Baseline vitamin D levels significantly correlated with PWV (r=-0.352, p=0.018). Post-treatment vitamin D levels also significantly correlated with post-treatment PWV (r=-0.442, p=0.002) and AIx (r=-0.419, p=0.004). Multivariate linear regression analysis revealed no independent predictor of baseline log-transformed PWV. CONCLUSION: Vitamin D supplementation has beneficial effects on arterial stiffness. Arterial stiffness parameters may aid in the assessment of cardiovascular risk in patients with vitamin D deficiency.

Vitamin D deficiency is associated with impaired myocardial deformation parameters and cardiovascular disease (CVD). Increased epicardial fat thickness (EFT) is also associated with increased risk of CVD. The aim of the study is to evaluate the effect of vitamin D deficiency and supplementation on myocardial deformation parameters and EFT. The study population consisted of 50 patients with vitamin D deficiency who were free of cardiovascular risk (mean age: 42.6 ± 8.9 years, 37 female). Patients were treated with oral administration of vitamin D3. Myocardial deformation parameters and EFT were evaluated before and after treatment of those patients. Vitamin D levels significantly increased after treatment (30.5 ± 10.5 vs. 9.9 ± 5.3 nmol/l, p < 0.001). There was no significant difference between conventional echocardiographic parameters before and after treatment. Baseline EFT was significantly higher than post-treatment measurements (35.2 ± 8.0 vs. 27.5 ± 5.6 mm, p < 0.001). Post-treatment myocardial deformation parameters were also significantly higher than baseline measurements. Baseline vitamin D levels correlated with baseline EFT and left ventricular global longitudinal strain (LV-GLS). Post-treatment vitamin D levels also correlated with post-treatment EFT, body mass index, and LV-GLS. Baseline vitamin D level was an independent predictor of baseline LV-GLS (p = 0.002). Patients with impaired LV-GLS had significantly lower vitamin D levels than patients with normal LV-GLS (6.6 ± 3.8 vs. 11.0 ± 5.3 nmol/l, p = 0.005). Baseline vitamin D level was also an independent predictor of baseline impaired LV-GLS (p = 0.010). Vitamin D supplementation has beneficial effects on myocardial deformation parameters and EFT. Moreover, baseline vitamin D levels are a predictor of impaired LV-GLS.
