RESUMEN

Foam rolling and eccentric exercise interventions have been demonstrated to improve range of motion (ROM). However, these two modalities have not been directly compared. Twenty-three academy soccer players (age: 18 ± 1; height: 1.74 ± 0.08 m; body mass: 69.3 ± 7.5 kg) were randomly allocated to either a foam rolling (FR) or eccentric exercise intervention designed to improve dorsiflexion ROM. Participants performed the intervention daily for a duration of four weeks. Measurements of dorsiflexion ROM, isometric plantar flexion torque and drop jump reactive strength index were taken at baseline (pre-intervention) and at three subsequent time-points (30-min post, 24-hours post and 4-weeks post). A significant time x group interaction effect was observed for dorsiflexion (P = 0.036), but not for torque or reactive strength index. For dorsiflexion, there was a significant increase in both acute (30-min; P < 0.001) and chronic (4-week; P < 0.001) ROM for the eccentric group, whilst FR exhibited only an acute improvement (P < 0.001). Eccentric training would appear a more efficacious modality than foam rolling for improving dorsiflexion ROM in elite academy soccer players.

My background is in nursing, midwifery, and acupuncture. In November of 2021, I came across a blog post about volunteering as an acupuncturist in a rehabilitation clinic for migrant refugees and asylum seekers on the island of Lesvos. With experience in the National Acupuncture Detoxification Association (NADA) protocol and trauma training provided by Acupuncturists Without Borders, I decided to apply to the nongovernmental organization Earth Medicine rehabilitation clinic. I stayed for 2 weeks in January 2022, and went again in September 2022 for 2 weeks, but that time, I was based inside the camp. On returning home from my first trip, and while giving a talk about Lesvos to our regional group, a fellow acupuncturist suggested that we could do something closer to home. Thus, we set up a project in our city, Exeter, in the United Kingdom. Thanks to a willing team of volunteers, asylum seekers and refugees are offered free acupuncture treatments weekly on Saturday mornings. This takes place in a community center in a group setting, using the NADA ear protocol, as well as other acupuncture points and occasional full-body treatments when the circumstances allow this. Although the work on Lesvos was hard, it was also very rewarding. Working there has had a profound effect on me and I plan to go back. At our local project, we receive very positive feedback from the people who come to us. Using acupuncture to address post-traumatic stress disorder is very worthwhile, and I encourage others to consider doing the same.

RESUMEN

Within the acupuncture literature there is debate on the safety of using specific acupuncture points during pregnancy. Termed 'forbidden' or contraindicated, they refer to acupuncture points that can be used to induce labour but may also include points with no known inducing or labour-enhancing effects. Recommendations range from avoiding these acupuncture points at any time in pregnancy to statements that despite the warnings in the literature, these points are not contraindicated during a normal pregnancy. This discussion paper examines the historical use of contraindicated points, the physiology of the pregnant body and the effect of these points during research trials. It is hoped that this will encourage further discussion and provide a background for practitioners to make informed choices about how they use these points in clinical practice.
