RESUMEN

Intravenous glucose tolerance test was done and concentrations of lactic acid and pyruvic acid were determined together with the lactic acid/pyruvic acid index and lactic dehydrogenase activity in fasting venous blood and 35 and 90 min. after glucose load. The investigations were carried out in 30 patients with cerebral concussion and 20 patients with cerebral contusion on the 1st, 3rd and 7th days after trauma. Thirty healthy subjects served as controls. Presence of hyperglycaemia in fasting state and impairment of glucose tolerance were demonstrated in the first week of the disease. These disturbances were accompanied by significant decrease of the activity of lactic and pyruvic acid metabolism during the first three days after trauma. The values of lactic acid/pyruvic acid index and LDH activity were not changed significantly. Disturbances of carbohydrate metabolism persisted during the first week after trauma and were more frequent, more intense and persisted longer in patients with brain contusion than in those with brain concussion but showed no significant differences related to the degree of trauma.

RESUMEN

The authors determined uropepsin activity in 24-hour urine in 30 cases of mild craniocerebral trauma. The determinations were done on the 1st, 3d and 7th days after trauma. The control group consisted of 15 healthy subjects. The activity of the enzyme was raised only on the 1st day after trauma and it may be explained as a result of stress.

RESUMEN

The authors determined the level of neuraminic acid in the serum of 50 patients with encephalomalacia and 25 patients with cerebral haemorrhages on the 1st, 3d and 7th days of the disease. Twenty healthy subjects served as controls. A statistically significant rise in the concentration of this substance was observed in both groups of patients throughout the whole period of observations.

RESUMEN

The authors determined the activity of lipoprotein lipase in the blood and cerebrospinal fluid in 25 patients with exacerbation of multiple sclerosis and in 20 controls. A significant decrease in the activity of the enzyme was found in blood of the patients, while only trace activity was detected in the cerebrospinal fluid in both groups. The significant decrease of lipoprotein lipase activity in blood of patients the with multiple sclerosis requires further investigations.

RESUMEN

Gasometric determinations of pO2, pCO2 and pH were carried out in capillary blood obtained from the upper extremities before and after 10 minutes of kinesitherapy in 30 patients with hemiplegia following stroke. The determinations were done with an acid-base equilibrium analyser Radiometer Copenhagen pH M 71 MKZ. Ten minutes of kinesitherapy caused significant changes in the pH of the blood towards alkalinization, less pronounced in the paralytic extremities. However, differences in the determinations in paralytic and non-paralytic extremities were statistically not significant. These results confirm earlier observations of the authors and point out that 10 minutes of kinesitherapy seems to be the shortest times of exercise giving significant biochemical and clinical effect.

RESUMEN

The authors determined the leukergic reaction in the blood of multiple sclerosis patients treated with dexamethasone and sodium salicylate. The investigations were done in two groups of 25 patients each. Dexamethasone was used in a daily dose of 4 mg, and sodium salicylate in a dose of 2.0 g. A significant effect of dexamethasone on reduction of the leukergic forms of leucocyte in the peripheral blood of these patients was observed. There was no significant effect of sodium salicylate on leukergy.

RESUMEN

Clinical course has been studied and tomographic examination has been performed in 50 patients with diabetes mellitus and cerebral stroke. 46 patients were thought to have a clinically ischaemic stroke, 2 ones were thought to have a haemorrhagic stroke and two were TIA cases in the first diagnosis. Among 50 patients with clinically focal neurologic deficits cortico-subcortical ischaemic focus was the most frequent CT finding (28%) with or without concomitant cortico-subcortical atrophy. There were 2 haemorrhagic pictures (4%). 13 CT scans (26%) were without any focal changes. CT cerebral atrophy was found in 34 patients (68%).

RESUMEN

There are varied causes of lesions of the facial nerve: 75% of them is idiopathic paralysis or Bell paralysis (1), then there are injuries, diseases of the ears (otitis media and externa, otic zoster) and carcinomas of neuron VII, cerebellopontine angle tumors, tumors of the pons, of the petrosal bone, the middle ear, leukemias, diabetes, tumors of the salivary gland (2, 3, 4). Especially careful diagnosing is required for the cases of the lesions of the peripheral neuron n. VII, where, despite conventional treatment, there is no distinct improvement or where it is necessary to determine the right etiopatogenic factor among several possible ones. The patient under observation merits particular attention because it is necessary to determine the underlying cause of the lesion of the peripheral neuron of the facial nerve.
