RESUMEN

The use of IV methylprednisolone has been shown to be associated with some adverse effects. The most feared side effect is acute gastrointestinal perforation and accelerated hypertension particularly during pulse therapy. Hiccups occur less frequently but can cause high levels of discomfort to the patient. In intractable cases, respiratory arrest and death can occur. This article reports the occurrence of hiccups in a patient managed for pseudo Foster-Kennedy syndrome. The hiccups were observed shortly after IV methylprednisolone was administered to the patient and abetted over a period of one week after it was discontinued. Hiccups occur through the neuronal pathway of the hiccup reflex arc, comprising the vagus nerve, phrenic nerve, parts of the sympathetic nervous system (T6-T12), and efferent fibers from the phrenic nerve that supply the glottis and the accessory muscles of respiration. The hiccups resolved with the use of gabapentin. This case report aims to add to the existing body of knowledge of the efficacy of gabapentin in the management of hiccups.

RESUMEN

The surgical management of persistent fetal vasculature (PFV) is challenging and the visual outcome can be compromised by coexisting ocular pathologies and amblyopia. It can be considered for relief of retinal traction and improved cosmetic appeal when a squint or a posterior capsular cataract is present. In the case presented in this report, the intermittent exotropia improved from 45 to 30 degrees in one year, which suggests an improvement in binocular single vision. There was also a resolution of the leukocoria and retinal traction. The patient underwent the following examinations: visual activity, slit-lamp biomicroscopy, intraocular pressure (Goldmann), fundus photography (OCT TOPCON, 3D OCT-1 Maestro, Topcon, Tokyo, Japan), B-scan Doppler ultrasonography (Mindray DC-N3, Mindray, Shenzhen, China), Keratometry (Topcon KR 800, Topcon, Tokyo, Japan), and axial length (Sonomed 300AP+A Scan/Pachymeter, Sonomed Escalon, Lake Success, NY). Intraocular lens (IOL) power was calculated with the Sanders-Retzlaff-Kraff (SRK) II formula. During surgery, a rigid polymethyl methacrylate (PMMA) IOL was inserted into the sulcus after excision of the lesion and anterior vitrectomy. The first day's postoperative evaluation included visual acuity, corneal transparency, depth of anterior chamber, pupil size, shape, pupillary reaction to light, and position of the IOL. Intraocular pressure was normal within the follow-up period. Fundus photography and B-scan examinations were performed at one month and one year.

RESUMEN

Myelination is an uncommon condition that rarely causes visual impairment. We report a case of a young man with bilateral peripapillary nerve fiber layer myelination and visual impairment. The patient's complaints, clinical history, and systemic review were noted. Visual acuity test, objective and subjective refraction, ocular motility, fundus examination, optical coherence tomography, and low vision test, were performed. We compared our findings with published articles. We suggest that optic neuropathy was the cause of visual impairment in the patient.
