BACKGROUND: Congenital Anomalies of the Upper Limb (CAUL) are a group of structural or functional abnormalities that develop during intrauterine life and can lead to limb dysfunction. OBJECTIVES: To analyze the prevalence of congenital anomalies of the upper limbs in Brazil and assess maternal and neonatal variables. DESIGN AND SETTING: A cross-sectional, descriptive study was conducted on congenital upper limb malformations among live births across Brazil. METHODS: The study spanned from 2010 to 2019. Data were sourced from the Department of Informatics of the Unified Health System (DATASUS) and the Live Birth Information System (SINASC) portal. Analyses focused on the information reported in field 41 of the Live Birth Declaration Form entered into the computerized system. RESULTS: The most common anomaly in Brazil was supernumerary fingers, classified as ICD-Q69.0, affecting 11,708 children, with a prevalence of 4.02 per 10,000 live births. Mothers aged over 40 years had a 36% higher prevalence of having children with CAUL than mothers under 40 years old (OR = 1.36; 95% CI 1.19-1.56). Newborns weighing ≥ 2,499 g were 2.64 times more likely to have CAUL compared to those weighing ≥ 2,500 g (OR = 2.64; 95% CI 2.55-2.73). CONCLUSION: There was an observed increase in the reporting of CAUL cases over the decade studied. This trend serves as an alert for health agencies, as understanding the prevalence of CAUL and its associated factors is crucial for preventive medicine.

The main purpose of this research was to do an intraindividual comparison of outcomes between the open ulnar incision (OUI) and the Paine retinaculotome with palmar incision (PRWPI) techniques in patients with bilateral carpal tunnel syndrome (CTS). The patients underwent OUI surgery on one hand and PRWPI surgery on the contralateral hand. The patients were evaluated with the Boston carpal tunnel questionnaire, visual analogue scale for pain, palmar grip strength, and fingertip, key, and tripod pinch strengths. Both hands were examined in the preoperative and postoperative periods after 2 weeks, 1 month, and 3 and 6 months. Eighteen patients (36 hands) were evaluated. The symptoms severity scale (SSS) scores were higher, in the preoperative period, in the hands that underwent surgery with PRWPI ( p -value = 0,023), but lower in the 3rd month postoperative ( p -value = 0.030). The functional status scale (FSS) scores were lower in the periods of 2 weeks, 3 months, and 6 months ( p -value = 0,016) on the hands that underwent surgery with PRWPI. In a different two-group module study, the PRWPI group presents the SSS scores average on the 2nd week and 1st month, and the FSS scores average on the 2nd week, less 0.8 and 1.2 points respectively comported to open group. The hands that underwent surgery with PRWPI presented significantly lower SSS scores at 3 months postoperative, and lower FSS scores at 2 weeks, and 3 and 6 months postoperative, compared to open surgery group.

Objective The objective of the study was to determine the prevalence of absence of palmaris longus tendon in a population sample from a multiethnic Brazilian city. Methods A cross-sectional observational study was carried out between October 2017 and April 2018. We included male and female volunteers aged 18 years or older. The absence of palmaris longus tendon was determined by asking the volunteers to perform the Schaeffer test bilaterally. Results We prospectively collected data on 1,008 volunteers, 531 male and 477 female, with age between 18 and 74 years (mean 38.4 years). The absence of palmaris longus tendon was observed in 264 (26.2%) volunteers. Bilateral absence was detected in 123 volunteers (12.2%), 60 female (48.8%) and 63 male (51.2%). Unilateral absence was found in 141 patients (14.0%), 54 female (38.2%) and 87 male (61.8%) ( p < 0.05). Conclusion The prevalence of absence of palmaris longus tendon in our study was 26.2%. There was no statistically significant difference between gender and bilaterally. The absence of palmaris longus tendon was predominant on unilateral right side in males.

Physical activity level (PAL) and sedentary behavior (SB) are independent predictors of mortality. It is unclear how these predictors interact with each other and health variables. Investigate the bidirectional relationship between PAL and SB, and their impact and health variables of women aged 60 to 70 years. One hundred forty-two older adults women (66.3 ± 2.9 years) considered insufficiently active were submitted to 14 weeks of multicomponent training (MT), multicomponent training with flexibility (TMF), or the control group (CG). PAL variables were analyzed by accelerometry and QBMI questionnaire, physical activity (PA) light, moderate, vigorous and CS by accelerometry, 6 min walk (CAM), SBP, BMI, LDL, HDL, uric acid, triglycerides, glucose and cholesterol total. In linear regressions, CS was associated with glucose (B:12.80; CI:9.31/20.50; p < 0.001; R2:0.45), light PA (B:3.10; CI:2, 41/4.76; p < 0.001; R2:0.57), NAF by accelerometer (B:8.21; CI:6.74/10.02; p < 0.001; R2:0.62), vigorous PA (B:794.03; CI:682.11/908.2; p < 0.001; R2:0.70), LDL (B:13.28; CI:7.45/16.75; p < 0.002; R2:0.71) and 6 min walk (B:3.39; CI:2.96/8.75; p < 0.004; R2:0.73). NAF was associated with mild PA (B:0.246; CI:0.130/0.275; p < 0.001; R2:0.624), moderate PA (B:0.763; CI:0.567/0.924; p < 0.001; R2:0.745), glucose (B:-0.437; CI:-0.789/-0.124; p < 0.001; R2:0.782), CAM (B:2.223; CI:1.872/4.985; p < 0.002; R2:0.989) and CS (B:0.253; CI: 0.189/0.512; p < 0.001; R2:1.94). The NAF can enhance CS. Build a new look at how these variables are independent but dependent simultaneously, being able to influence the quality of health when this dependence is denied.

Integrated agricultural systems have been greatly expanding in Brazil and currently cover over 30 million hectares. Although these land use systems help soil and environmental quality, little is known of their impacts on earthworm communities. In the present study we evaluated earthworm abundance, biomass and species richness in various integrated and conventional agricultural production systems (pasture, crops), as well as native forest and Eucalyptus plantations. The research was carried out at Fazenda Canchim (Embrapa Livestock Southeast) and at the neighboring campus of the University de São Carlos (UFSCar), in São Carlos municipality in São Paulo state, Brazil. Two earthworm sampling methods, i.e., quantitative (25 x 25 x 20 cm deep monoliths) and qualitative (holes of similar size and niche sampling), were used in three integrated production systems, three conventional systems and two forest systems, totaling 232 quantitative samples and over 160 qualitative samples. Earthworms were counted and identified using available keys. Eight earthworm species of five families (Rhinodrilidae, Ocnerodrilidae, Megascolecidae, Benhamiidae and Glossoscolecidae) were found, with most of the 2,145 individuals being exotic or peregrine species, particularly Pontoscolex corethrurus (88% of total) and various Dichogaster spp. (8% of total). Two native species were encountered in low abundance (55 individuals), both of which are new to science (Fimoscolex n.sp.7 and Glossoscolex n.sp.9) and must still be formally described. The new species were found in the agricultural systems, but not in the native forest and the Eucalyptus plantation, where P. corethrurus and Megascolecids predominated. One unidentified Ocnerodrilid sp. was found only in the intensive pasture. Species richness was highest in the intensive pasture (7 spp.), followed by the agrosilvopastoral and agropastoral systems, highlighting the importance of integrated and pastoral systems in preserving earthworm richness and native species.

RESUMEN

The relationship between viral infection in pregnancy and congenital anomalies is reported in the medical literature. The risks for the SARS-CoV-2 virus interfering with implantation, growth, and fetal development are not yet known. Many drugs with potential teratogenic risk are being used for treatment. The studies with the vaccine excluded pregnant women from clinical trials, currently preventing high-quality evidence. We present a review of the most common anomalies in the upper extremities caused by congenital viral infection and the risks of anti-COVID-19 therapy and vaccine during pregnancy. We aim to alert orthopedic and hand surgeons to the possibility of these conditions in the future. Level of evidence V; Narrative review.


A relação entre infecção viral na gravidez e anomalias congênitas é relatada na literatura médica. Os riscos de o vírus SARS-CoV-2 interferir na implantação, crescimento e desenvolvimento fetal ainda não são totalmente conhecidos. Muitos medicamentos com potencial de risco teratogênico estão sendo usados para o tratamento. Os estudos clínicos com a vacina excluíram gestantes, o que tem impedido obter evidências de alta qualidade atualmente. Apresentamos uma revisão das anomalias mais comuns nos membros superiores causadas por infecção viral congênita e os riscos da terapia anti-COVID-19 e da vacina durante a gravidez. Nosso objetivo é alertar os cirurgiões ortopédicos e de mão para a possibilidade desses problemas no futuro. Nível de evidência V; Revisão narrativa.

Despite being a procedure widely used all over the world with high rates of symptom remission, surgical treatment of carpal tunnel syndrome may present unsatisfactory outcomes. Such outcomes may be manifested clinically by non-remission of symptoms, remission of symptoms with recurrence a time after surgery or appearance of different symptoms after surgery. Different factors are related to this unsuccessful surgical treatment of carpal tunnel syndrome. Prevention can be achieved through a thorough preoperative clinical evaluation of the patient. As such, the surgeon will be able to make differential or concomitant diagnoses, as well as determine factors related to patient dissatisfaction. Perioperative factors include the correct identification of anatomical structures for complete median nerve decompression. Numerous procedures have been described for managing postoperative factors. Among them, the most common is adhesion around the median nerve, which has been treated with relative success using different vascularized flaps or autologous or homologous tissue coverage. The approach to cases with unsuccessful surgical treatment of carpal tunnel syndrome is discussed in more detail in the text.

PURPOSE: Various postoperative protocols have been proposed to improve outcomes and accelerate nerve regeneration. Recently, the use of physical exercise in a post-surgical neurorraphy procedure has shown good results when started early. We experimentally investigated the hypothesis that post-operative exercise speeds up results and improves clinical and morphologic parameters. METHODS: Isogenic rats were randomly divided into four groups: 1 SHAM; 2 SHAM submitted to the exercise protocol (EP); 3 Grafting of the sciatic nerve; and 4 Grafting of the sciatic nerve associated with the EP. The EP was based on aerobic activities with a treadmill, with a progressive increase in time and intensity during 6 weeks. The results were evaluated by the sciatic functional index (SFI), morphometric and morphologic analysis of nerve distal to the lesion, and the number of spinal cord motor neurons, positive to the marker Fluoro-Gold (FG), captured retrogradely through neurorraphy. RESULTS: Functional analysis (SFI) did not show a statistical difference between the group grafted with (-50.94) and without exercise (-65.79) after 90 days. The motoneurons count (Spinal cord histology) also showed no diference between these groups (834.5 × 833 respectively). Although functionally there is no difference between these groups, morphometric study showed a greater density (53.62) and larger fibers (7.762) in GRAFT group. When comparing both operated groups with both SHAM groups, all values were much lower. CONCLUSIONS: The experimental model that this aerobic treadmill exercises protocol did not modify nerve regeneration after sciatic nerve injury and repair with nerve graft.

Objective To perform the translation and cultural adaptation to Brazilian Portuguese of the Children's Physical Activity Questionnaire Methods The process involved the stages of translation, synthesis, back-translation, and revision by the translation group. A prefinal version of the questionnaire was then created, and, subsequently, the final version. Results Due to the lifestyle of the population, eight activities described in the questionnaire were changed to adapt it to Brazilian culture: six sports activities and two leisure activities. Eight parents/legal guardians answered the questionnaire during the pretest, quickly, without inconsistencies, and without reporting difficulties in understanding when the final interview was conducted. Conclusion The translation and cultural adaptation of the Children's Physical Activity Questionnaire to Brazilian Portuguese resulted in the version called C-PAQ.PT.

OBJECTIVE: To evaluate the cure rate for dorsal synovial cysts of the wrist with aspiration and percutaneous sclerotherapy using 75% hypertonic glucose. METHODS: We enrolled 45 patients older than 18 years with untreated dorsal synovial cysts of the wrist. They underwent percutaneous aspiration and sclerotherapy with 75% hypertonic glucose 75%. A maximum of two procedures were conducted for each cyst at a 4-week interval. We analyzed age, sex, affected side, history of previous trauma, wrist goniometry, grip strength, wrist pain by the visual analogue scale, complications, and hand function questionnaire scores. RESULTS: We evaluated 45 patients (30 female, 15 male, mean age 38.2 years with 47 cysts). Four weeks after the first procedure, 72.3% cysts were palpable and visible. At 24 weeks after the first procedure, 57.4% cysts evolved to cure and 42.6% persisted. CONCLUSION: Treatment of dorsal synovial cyst of the wrist with aspiration and percutaneous sclerotherapy using 75% hypertonic glucose achieved a 57.4% cure rate after 24 weeks. OBJETIVO: Avaliar o índice de cura do cisto sinovial dorsal do punho, com aspiração e escleroterapia percutânea utilizando glicose hipertônica 75%. MÉTODOS: Foram selecionados pacientes maiores de 18 anos, portadores de cisto sinovial dorsal do punho, sem tratamento prévio. Foram submetidos a aspiração e escleroterapia percutânea com glicose hipertônica 75%. Foi realizado no máximo dois procedimentos em cada cisto, em um intervalo de 4 semanas. Foram analisados idade, gênero, lado acometido, história de trauma prévio, goniometria, força de preensão, dor no punho pela escala visual analógica, complicações e os escores dos questionários funcionais. RESULTADOS: Foram avaliados 45 pacientes com 47 cistos, houve perda de seguimento de 3 pacientes (3 cistos). A média de idade foi de 38,2 anos. Na quarta semana após o primeiro procedimento, tivemos cura em 23,4% cistos, 4,3% cistos apresentavam-se palpáveis porém não visíveis e 72,3% cistos se apresentavam palpáveis e visíveis. Os cistos visíveis e palpáveis foram submetidos ao segundo procedimento. Após 24 semanas do primeiro procedimento, 57,4% cistos foram curados e 42,6% cistos apresentaram persistência. CONCLUSÃO: O tratamento do cisto sinovial dorsal do punho com escleroterapia percutânea utilizando Glicose Hipertônica 75%, proporcionou cura de 57,4% após 24 semanas. BACKGROUND: Osteoarthritis is the most common form of hand arthritis and arthritis of the carpometacarpal joint of the thumb is a potentially limiting disease. There is no homogeneity in the evaluation of outcomes for the rhizarthrosis treatment. In an attempt to standardize the evaluation of results, some subjective questionnaires, non-specific, were used to evaluate rhizarthrosis. Trapeziometacarpal Arthrosis Symptoms and Disability (TASD) was described by Becker et al.with the purpose of evaluating symptom intensity and degree of disability, as to compare results after treatment. Our objective is to translate, validate and do the cultural adaptation of the questionnaire TASD into the Brazilian Portuguese. METHODS: The questionnaire was translated, with reverse translation. The translations were evaluated and synthesized by a committee, arriving at TASD-BR. Thirty-one patients with a diagnosis of rhizarthrosis answered the questionnaire. We evaluated, the internal consistency, reliability, agreement and ceiling and floor effect for validation. RESULTS: The questionnaires were translated and adapted according to defined protocols. The internal consistency, through Cronbach's α coefficient for TASD-BR, was 0.927. The questionnaire's reliability, through the Intraclass Correlation Coefficient, was also shown to be quite high, with κ = 0.961 (0.954-0.967). The agreement, measured through the Standard Error Measurement, remained with standardized values below 5%. There was no ceiling and floor effect. CONCLUSION: Through specific methodology we consider TASD-BR translated and valid for the Brazilian Portuguese.

Objective To perform the translation, cultural adaptation and validation of the Thumb Disability Exam (TDX) questionnaire into Brazilian Portuguese. Method The questionnaire was translated, with reverse translation. The translations were evaluated and synthesized by a committee, resulting in the THUMB DISABILITY EXAM - BRASIL (TDX-BR). A total of 31 patients diagnosed with rhizarthrosis were selected and answered the questionnaire. Quality parameters were evaluated, such as internal consistency, reproducibility and ceiling and floor effect. Results The questionnaires were translated and adapted according to defined protocols. Internal consistency, through the Cronbach α coefficient for the TDX-BR, was 0.962. The reliability of the questionnaire, through intraclass correlation coefficient (ICC )also proved to be quite high, with κ = 0.953 (0.947-0.959). Agreement, measured using the standard error of measurement, remained with standard values < 5%. There was no ceiling and floor effect. Conclusion Through specific methodology, we consider TDX-BR as translated and valid for the Portuguese language of Brazil.

Objectives To evaluate the prevalence of absence of flexor digitorum superficialis muscle tendons function of the fourth and fifth fingers of the hand in the Brazilian population Methods Prospective study performed at the orthopedics and traumatology department of a university hospital. The study was conducted from October 2017 to April 2018. The sample consisted of volunteers with no history of upper limb trauma, surgery or any other condition. This study included both male and female individuals over 18 years old. The flexor digitorum superficialis muscle tendons of the fourth and fifth fingers were evaluated through clinical tests by three independent examiners. The data were analyzed using GraphPad Prism 5.0 (GraphPad Software, San Diego, CA, USA). Results A total of 2,016 hands from 1,008 volunteers were analyzed. The prevalence of absence of flexor digitorum superficialis muscle tendon function of the fourth finger was 0.56% at the right hand and 0.99% at the left hand. At the right hand, this prevalence was higher in females compared to males ( p = 0.0328). No difference between genders was noted in the left hand ( p = 0.7562). The prevalence of absence of flexor digitorum superficialis muscle tendon function of the fifth finger was 34.53% in the right hand and 30.06% in the left hand. This prevalence was higher in females compared to males, both at the right hand ( p = 0.0001) and the left hand ( p = 0.0003). Conclusion Even though there are studies performed separately in different ethnic groups, there were no data in the literature regarding the prevalence of absence of flexor digitorum superficialis muscle tendons function of the fourth and fifth fingers of the hand in a multiethnic population such as the Brazilian one.

Objective To examine the prevalence of carpal tunnel syndrome in powerlifting athletes with disabilities. Methods The present study evaluated the presence and intensity of pain (numerical scale), nocturnal paresthesia (self-report), and nerve compression (Tinel and Phalen signs) in wheelchair- and non-wheelchair-bound powerlifting athletes with disabilities. The clinical diagnosis of carpal tunnel syndrome was confirmed by the presence of two or more signs/symptoms. Results In total, 29 powerlifting athletes with disabilities were evaluated. None of the athletes reported the presence of pain or nocturnal paresthesia. The Tinel sign was present in 1 (3.45%) wheelchair-bound athlete. A positive Phalen test was present in 3 (10.35%) athletes (1 wheelchair-bound and 2 non-wheelchair-bound). Concurrent positive Tinel sign and Phalen sign tests were found in 2 (6.89%) athletes (1 wheelchair-bound and 1 non-wheelchair-bound). Conclusion Carpal tunnel syndrome was clinically diagnosed in 2 (6.89%) out of 29 powerlifting athletes with disabilities.

BACKGROUND: Electrical studies are often used to diagnose carpal tunnel syndrome (CTS). We noticed differences in results when we considered the clinical tests. OBJECTIVES: What's the prevalence of CTS in adapted sports athletes based on clinical diagnostic? Is there any relationship between the presence of symptoms and the use of wheelchairs, the type of sport practiced, dominance and how long the sport had been practiced? HYPOTHESIS: The prevalence of CTS in adapted sports athletes is close to that of the general population. STUDY DESIGN: Cross-sectional study. METHODS: We evaluated adapted athletes. Data collection included the dominant side, type of sports, sports practice time, wheelchair or non-wheelchairs users. To evaluate intensity of pain in the hands, we used the numerical pain rating scale (NPRS). For the evaluation of nocturnal paresthesia, we asked athletes about this symptom's presence. We evaluated the presence of paresthesia through the Tinel signal and Phalen test. For this study's purpose, we characterized CTS by the presence of two or more signs or symptoms. RESULTS: We evaluated 72 athletes, 144 hands. No athlete reported pain in the hands or nocturnal paresthesia. Fifteen athletes had at least one sign or symptom in a total of 21 hands. The presence of two concomitant clinical symptoms occurred in 4 (6%) right hands and 3 (4%) left hands. The presence of more than two concomitant clinical symptoms did not occur in any hand. We did not find a relation between the presence of signs and/or symptoms with the type of adapted sport, wheelchair use, the dominant side and the time of sports practice. DISCUSSION: Reviewing the literature we observed that the most researchers used electrodiagnostic studies to determine the diagnosis of CTS on persons with disabilities, without considering physical examination. CONCLUSION: The prevalence of CTS in the disabled athletes was 6 in 72 (8%). LEVEL OF EVIDENCE: III.

RESUMEN

Objective To evaluate the effects of swimming on nerve regeneration after sciatic nerve injury in Wistar rats. Methods A total of 30 Wistar rats was divided into 3 groups: Sham + Nat group animals that were not submitted to graft surgery and were submitted to swimming ( n = 10); Graft group: animals submitted to autologous sciatic nerve graft ( n = 10); and Graft + Nat group: animals submitted to autologous sciatic nerve graft surgery and to swimming ( n = 10). The results were analyzed on the software (GraphPad Software, San Diego, CA, USA). Results In the first evaluation, all sciatic functional index (SFI) values were similar ( p = 0.609). Thirty days after the surgical procedure, we observed differences between all the comparisons: Sham + Nat (-34.64 ± 13.89) versus Graft (-145.9 ± 26.06); Sham + Nat versus Graft + Nat (-89.40 ± 7.501); Graft (-145.9 ± 26.06) versus Graft + Nat (-89.40 ± 7.501). In the measurements (60 and 90 days), there was no statistical difference between the Graft and Graft + Nat groups, with significantly lower values in relation to the control group ( p < 0.001). The number of motor neurons presented differences in the comparisons between the Sham + Nat and Graft groups (647.1 ± 16.42 versus 563.4 ± 8.07; p < 0.05), and between the Sham + Nat and Graft + Nat groups (647.1 ± 16.42 versus 558.8 ± 14.79; p < 0.05). There was no difference between the Graft and Graft + Nat groups. Conclusion Animals submitted to the swimming protocol after the sciatic nerve grafting procedure did not present differences in the SFI values and motor neuron numbers when compared to the control group. Therefore, this type of protocol is not efficient for the rehabilitation of peripheral nerve lesions that require grafting. Therefore, further studies are needed.

Objective To determine the prevalence of the Linburg-Comstock anomaly in a Brazilian population sample. Methods A cross-sectional observational study was carried out between October 2017 and April 2018. We included male and female volunteers aged 18 years or older. The presence of the Linburg-Comstock anomaly was determined by performing the clinical tests described by Linburg and Comstock. The data were analyzed using the GraphPad Prism software, and we considered differences with p < 0.05. Results The study analyzed 1,008 volunteers (2,016 hands) with a mean age of 38.3 years, 531 (52.67%) of which were male, and 477 (47.33%) were female. The Linburg-Comstock anomaly was diagnosed in 564 (55.95%) individuals, and it was bilateral in 300 (53.2%) of them, right-sided in 162 (28.72%), and left-sided in 102 (18.08%). No significant differences were found when comparing the prevalence between genders. However, a the prevalence of the right-sided anomaly in the male population (n = 99; 70.21%) was higher than in the female one (n = 63; 51.21%), with p = 0.0016. In addition, the presence of pain by the maneuver described by Linburg and Comstock was more prevalent in women (n = 150; 54.94%) than in men (n = 105; 36.08%), with p = 0.0001. These results show the importance of epidemiological studies on the Linburg-Comstock anomaly, mainly in order to investigate the presence of associated conditions. Conclusion The prevalence of the Linburg-Comstock anomaly in the studied population was of 55.95%, and it was bilateral in 53.2% of the volunteers. The presence of the connection was observed more frequently in the right side and among men, but the pain symptom was more frequent among women.

Objective This study evaluates the conditions for microvascular procedures found by hand surgeons in Brazilian clinical practices. Methodology A prospective, observational, and analytical primary clinical research conducted during the 37 th Brazilian Congress of Hand Surgery, from March 30 th to April 1 st , 2017, in Belo Horizonte, in which physicians answered 12 closed, objective, multiple-choice questions regarding their geographic region, type of institution (public or private), microsurgical training, time of experience, technical conditions, the availability of a standby team for emergencies and compensation. Results The study analyzed 143 hand surgeons; among them, 65.7% participants were based at the Southeast region, 13.3% in the Northeast region, 11.9% in the South region, 6.3% in the Central-West region and 2.8% in the North region. Regarding the time of experience, 43.4% of the hand surgeons had less than 5 years, 16.8% had 5 to 10 years, 23.8% 10 to 20 years, and 23% had more than 20 years of practice in microvascular surgery. Seven percent of the surgeons had no training in microvascular surgery; for 63.6%, training occurred during medical residency, whereas 30.8% were trained in another institution, and 7.7% in another country. Among these surgeons, 76.9% worked at both private and public hospitals, 14.7% at private hospitals and 5.6% at public hospitals. Regarding compensation, 1.8% of the surgeons considered it adequate, and 98.2%, inadequate in public hospitals, whereas 5.0% considered it adequate, and 95.0%, inadequate in private hospitals. Conclusion This research shows that most surgeons were trained in microsurgery, had never performed reattachments, and considered that compensation is inadequate; moreover, standby teams were not available. There are few, unevenly distributed hand surgeons with microsurgical ability in emergency settings, and their compensation is low.

OBJECTIVES: Machinery injuries account for a substantial share of traumatic upper limb injuries (TULIs) affecting young active individuals. This study is based on the hypothesis that there is an important relationship between the improper use of power saws and TULIs. The aim of the study is to assess the prevalence and epidemiology of TULIs caused by power saws and determine the risks related to power saw use. METHODS: A cross-sectional evaluation of medical records from a two-year period was performed. Patients sustaining TULIs related to power saws were analyzed. Data on the epidemiology, site of injury, mechanism of trauma, technical specifications of the tool, cutting material, personal protective equipment, time lost and return to work were obtained. RESULTS: A database search retrieved 193 TULI records, of which 104 were related to power saws. The majority of patients were male (102/104; 98.1%), right-handed (97/104; 93.3%), and manual workers (46/104; 44.2%), with an average age of 46.8 years. The thumb was the most frequently injured site (32/93; 34.4%). Most of the injuries were caused by manual saws (85/104; 81.7%), and masonry saws accounted for 68.2% (58/85) of the cases. Masonry saws improperly used for woodwork resulted in 86.2% (50/58) of the injuries. TULI caused by masonry saws was 5 times higher in manual workers than in other patients. In addition, masonry saws had a risk of kickback 15 times higher than that of other saws, and the risk of injury increased by 5.25 times when the saws were used improperly for wood cutting. CONCLUSIONS: The profile of TULIs related to power saws was demonstrated and was mainly associated with manual saws operated by manual workers that inappropriately used masonry saws for woodworking.
