RESUMEN

Infection by Helicobacter pylori affects over half of the world's population and is a public health issue. Its prevalence is significantly higher in developing countries such as Brazil. It has been suggested that infection by H pylori can impact calorie intake and homeostasis. AIM: Analyze the influence of infection by H pylori and of its eradication on nutritional status and dietary intake. MATERIAL AND METHODS: The study included 124 patients (48 of whom H pylori positive, 45 H pylori negative, and 31 eradicated). Weight and height were measured to calculate the body mass index (BMI). Dietary intake was assessed using a 24-hour diet recall (24hR) in two occasions. RESULTS: Pre-obesity was the highest among the eradicated patients (p = 0.011). Homogenicity was found in the dietary intake for the nutrients studied, except for saccharose, whose intake was the lowest among the eradicated group (p < 0.001). CONCLUSIONS: Patients eradicated for H pylori had a higher rate of pre-obesity. The dietary intake was similar among the groups for nearly all nutrients assessed, except for saccharose, whose intake by eradicated patients was lower. This entails discussions about possibly including the investigation of nutritional status and diet in the routine procedures towards those who seek a diagnosis of infection by H pylori.

OBJECTIVE: To evaluate combinations of larvicides and fish which are predators of larvae of Aedes aegypti, namely Betta splendens, Trichogaster trichopterus and Poecilia reticulata. METHODS: We used 15 water tanks with a capacity of 250 litres of water. In 10 tanks, larvicide - Temephos, Bti and Novaluron - was added, the other five contained only one specimen of fish and unchlorinated water. The fish were monitored for a week without changing the water, and their survival recorded on a form. An estimate of the Kaplan-Meier survival was performed to determine the significance of the tests. RESULTS: Betta splendens showed the lowest mortality range of larvicides tested. CONCLUSION: Combined use of larvivorous fish and larvicides in large water tanks is feasible.

The aim is to determine the prevalence of sarcopenia components and associations with socioeconomic variables in older persons living in rural areas in the state of Ceará, Brazil. We conducted a cross-sectional study with 274 older adults. Muscle mass was assessed using muscle mass index (MMI), measured by bioelectrical impedance analysis and calf circumference. Muscle strength (MS) was assessed using the handgrip test. Physical performance was measured using the walk test to calculate gait speed (GS) and the timed up-and-go test. Probable sarcopenia was defined as the presence of low MS, while confirmed sarcopenia was defined as the presence of low MS and low MMI. Severe sarcopenia was defined as the presence of the latter two criteria and slow GS. We tested for associations between sociodemographic characteristics and lifestyle habits and the components of sarcopenia. The logistic regression produced the following results: i) prevalence of low MS was higher among men, individuals aged >69 years, and in those not working at the time of the study; ii) the prevalence of low MMI was higher in individuals aged >69 years, those not living with a spouse, and those with an inadequate level of physical activity; iii) the prevalence of slow GS was higher in individuals aged >69 years, those who had lived in rural areas for less than 30 years, and those not working at the time of the study.


O objetivo é estimar prevalência de componentes da sarcopenia e associações com variáveis socioeconômicas em idosos de população rural do Ceará. Estudo transversal com 274 idosos. Massa muscular foi determinada por Índice de Massa Muscular (IMM) por bioimpedância elétrica e por circunferência da panturrilha; força muscular (FM) por teste de preensão palmar; e desempenho físico por testes da caminhada (VC) e Timed-up-and-go. Provável sarcopenia foi caracterizada por diminuição da FM, sarcopenia quando associada a diminuição do IMM, e sarcopenia grave quando estes dois foram acompanhados de baixa VC. Foram estimadas associações entre condições sociodemográficas, hábitos de vida e componentes da sarcopenia. Na regressão logística, foi observada frequência mais elevada: i) no sexo masculino, idade>69 anos, e nos que não trabalham, entre os idosos com FM diminuída; ii) com idade>69 anos, nos que não moram com o cônjuge, e nos que não praticam atividade física num nível adequado, entre os idosos com IMM diminuído; iii) com idade>69 anos, nos que residem na zona rural há menos que 30 anos, e nos que não trabalham, entre os idosos com VC diminuída.

This study was carried out to evaluate the residual effect of three larvicides under laboratory conditions for 100 days in Aedes aegypti. The larval mortality rate was measured without water renewal or with daily water renewal (80%). With temephos, there was 100% mortality in both groups until the 70th day. In the Bacillus thuringiensis israelensis (Bti)-WDG test, there was no difference during the first 20 days. With Bti-G, without water renewal, mortality was sustained above 90% for up to 35 days. The second experiment (with water renewal) reduced the mortality to below 90% after the first 20 days. When renewed water was provided, the residual effect was significantly lower for all larvicides.

The two fish species Betta splendens (Regan) and Poecilia reticulata (Peters) are known predators of Aedes aegypti (L., 1762) larvae. Both species have been used for biological control in northeastern Brazil. However, the feasibility of these fish for the control of Ae. aegypti larvae in domestic containers may be limited by their survival in chlorinated water, as supplied by the public water system. We exposed fish to three different concentrations of chlorine: 1, 1.5, and 2.0 mg/liter. All B. splendens survived at 1.0 mg/liter chlorine concentration; 72.5 and 39.3% of B. splendens survived chlorine concentrations of 1.5 and 2.0 mg/liter, respectively. In contrast, only 4.4% of P. reticulata survived at a chlorine concentration of 1.0 mg/liter. We conclude that B. splendens may be an appropriate species for biological control of Ae. aegypti in domestic water tanks.

Objetivo: Estimar a prevalência, a incidência e os fatores associados a quedas entre idosos atendidos em uma clínica escola. Método: O estudo foi dividido em dois componentes, sendo o seguimento transversal de agosto de 2016 a novembro de 2018 (n=129), e o de coorte de agosto de 2018 a novembro de 2018 (n=66). Realizaram-se análises estatísticas, a partir da Regressão Múltipla de Poisson, entre o desfecho e as ca- racterísticas sociodemográficas e de saúde. Resultados: A prevalência e a incidência de quedas foram de 44,2% e 36,4%, respectivamente. Na análise de regressão múltipla de Poisson do estudo transversal, as variáveis sintomas dispépticos, baixos valores no teste Time Up and Go e ter hipertensão foram associadas com a variável queda. No estudo de coorte, a análise reforçou associação entre não ser ex-etilista e ter constipação. Conclu- são: Considera-se elevada a prevalência e incidência de quedas entre os idosos avaliados, e ressalta-se a necessidade de identificação de grupos mais susceptíveis a esse desfecho. Objective: To estimate the prevalence, incidence and factors associated with falls among elderly people attended at a teaching clinic. Method: The study was divided into two components, the cross-sectional follow-up from August 2016 to November 2018 (n=129), and the cohort from August 2018 to November 2018 (n=66). Statistical analyzes were carried out, based on Multiple Poisson Regression, between the outcome and sociodemographic and health characteristics. Results: The prevalence and incidence of falls were 44.2% and 36.4%, respectively. In Poisson's multiple regression analysis of the cross-sectional study, the variables dyspeptic symptoms, low values in the Time Up and Go test and having hypertension were associated with the variable fall. In the cohort study, the analysis reinforced the association between not being an ex-alcoholic and having constipation. Conclusion: The prevalence and incidence of falls among the elderly evaluated is considered high, and the need to identify groups more susceptible to this outcome is emphasized.

En el análisis de regresión múltiple de Poisson del estudio transversal, las variables síntomas dispépticos, valores bajos en el test Time Up and Go y padecer hipertensión se asociaron a la variable caída. En el estudio de cohortes, el análisis reforzó la asociación entre no ser ex-alcohólico y tener estreñimiento. Conclusiones: La prevalencia e incidencia de caídas entre los ancianos evaluados se considera elevada, destacándose la necesidad de identificar grupos más susceptibles a este desenlace.

OBJECTIVE: To evaluate the efficacy of fish as predators of the Aedes aegypti larvae in laboratory conditions. METHODS: The male and female of five different fish were included in the experiment. The tests to measure their consumption ability lasted five weeks for each species. Each trial involved four test tanks and four control tanks. Two control tanks contained just one fish, and the other two just larvae. Each of the test tanks contained one fish and the larvae. During the first week, 100 larvae were placed in the tank, with an additional 100 added every week, up to a maximum daily amount of 500 larvae. The length and weight of the fish were measured at the beginning and end of every week. RESULTS: A total of 369,000 larvae were used. The Trichogaster trichopteros was the only species in which both sexes ate 100% of the available larvae. The Betta splendens failed to eat only 15 larvae. The male Poecilia reticulate showed a strong capacity for larvae eating, compared with the female of the same species. In terms of weight and size, the Betta splendens proved capable of eating 523 larvae per gram of weight per day. CONCLUSIONS: The female and male Trichogaster trichopteros and Astyanax fasciatus, and the female Betta splendens and Poecilia sphenops proved to be the most effective predators of the Aedes aegypti larvae. And although the male Poecilia sphenops and female Poecilia reticulata were less effective, they were also capable of eradicating the total number of Aedes aegypti larvae that could appear over 24 hours in a breeding site under natural conditions. The male Poecilia reticulata, however, proved incapable of doing so.

The identification of the prevalence of cognitive impairment and associated factors among older adults is important in countries facing rapid demographic transition, given the significant implications for public policy and health planning. OBJECTIVE: To determine the prevalence of functional cognitive impairment (FCI) and associated factors in Brazilian community-dwelling older adults. METHODS: A cross-sectional study involving 461 elderly subjects residing in Fortaleza city, Ceará was conducted. Cognitive assessment was performed using three tests: the MMSE (Mini-Mental State Examination), VF (Verbal Fluency) and CT (Clock Test). The functional capacity evaluation was based on a survey of 21 basic and instrumental activities of daily living (ADLs). Cognitive impairment was defined by MMSE cut-off points adjusted for literacy. Functional impairment was defined as dependency to carry out more than four ADLs. RESULTS: The prevalence of FCI was 13.64% (95% CI: 10.33 to 16.64%). FCI was proportionally associated with age with OR=2.24 (95% CI: 1.04 to 4.79) for individuals aged 70 to 79 years and OR=8.27 (95 % CI: 4.27 to 16.4) for those aged 80 to 100 years. FCI was associated with self-reported diseases including hypertension OR=2.06 (95% CI: 1.17 to 3.65), stroke OR=2.88 (95% CI: 1.66 to 5.00) and acute myocardial infarction OR=2.94 (95% CI: 1.59 to 5.42). The occurrence of FCI was proportionally correlated with the number of drugs used. CONCLUSION: Functional cognitive impairment is a prevalent condition in Brazilian community-dwelling older adults and its occurrence is associated with age, number of drugs used, and vascular morbidities.


A identificação da prevalência de déficit cognitivo, e fatores associados, é importante, dadas as implicações significativas para as políticas públicas e planejamento em saúde. OBJETIVO: Determinar a prevalência de CCF e fatores associados em idosos de uma área urbana brasileira. MÉTODOS: Estudo transversal envolvendo 461 idosos residentes em uma área urbana de Fortaleza, Ceará. Foi aplicado um questionário socioeconômico e de saúde e realizou-se avaliação cognitiva através de três testes de rastreio: Miniexame do Estado Mental (MEEM), FV (Fluência Verbal) e TR (Teste do Relógio). A avaliação da capacidade funcional foi feita por um questionário de 21 atividades da vida diária. Comprometimento cognitivo foi definido pelo comprometimento no MEEM ajustado para escolaridade. Comprometimento funcional foi definido pela incapacidade para mais de 4 AVDs. RESULTADOS: A prevalência de CCF foi de 13,64% (IC 95%: 10,33-16,64%). CCF foi proporcionalmente associado à idade: razão de chances (RC) 2,24 (IC95%: 1,04-4,79) para 70 a 79 anos e RC 8,27 (IC 95%: 4,27-16,4) para 80 a 100 anos. CCF apresentou associação significante com doenças autorreferidas: hipertensão arterial sistêmica apresentou RC 2,06 (IC 95%: 1,17-3,65), acidente vascular cerebral de 2,88 (IC 95%: 1,66-5,00) e infarto agudo do miocárdio de 2,94(IC 95%: 1,59-5,42). A ocorrência de CCF correlacionou-se com maior número de medicamentos usados. CONCLUSÃO: O CCF é condição comum entre os idosos no Brasil e sua ocorrência está associada à idade, ao número de fármacos utilizados e às morbidades vasculares.

BACKGROUND: This study intended to implement a novel intervention strategy, in Brazil, using an ecohealth approach and analyse its effectiveness and costs in reducing Aedes aegypti vector density as well as its acceptance, feasibility and sustainability. The intervention was conducted from 2012 to 2013 in the municipality of Fortaleza, northeast Brazil. METHODOLOGY: A cluster randomized controlled trial was designed by comparing ten intervention clusters with ten control clusters where routine vector control activities were conducted. The intervention included: community workshops; community involvement in clean-up campaigns; covering the elevated containers and in-house rubbish disposal without larviciding; mobilization of schoolchildren and senior inhabitants; and distribution of information, education and communication (IEC) materials in the community. RESULTS: Differences in terms of social participation, commitment and leadership were present in the clusters. The results showed the effectiveness of the intervention package in comparison with the routine control programme. Differences regarding the costs of the intervention were reasonable and could be adopted by public health services. CONCLUSIONS: Embedding social participation and environmental management for improved dengue vector control was feasible and significantly reduced vector densities. Such a participatory ecohealth approach offers a promising alternative to routine vector control measures.

OBJECTIVE: To evaluate the pulmonary function in high-risk pregnant women. METHODS: This was a prospective cross-sectional study on 60 pregnant women, of whom 30 were high-risk and 30 were low-risk cases, with gestational age ≥28 weeks. For the high-risk group, preeclampsia and/or gestational diabetes were the conditions taken into consideration. To evaluate pulmonary function, the following parameters were assessed: forced vital capacity (FVC), forced expiratory volume in the first second (FEV1), Tiffeneau index (FVC/FEV1) and forced expiratory flow (FEF25-75%). Fisher's exact test or the chi-square test was used to analyze the variables. RESULTS: There were no statistical differences in the pulmonary function parameters according to gestational age (p > 0.05). Similarly, there were no statistical differences in the pulmonary function tests for the variables of smoking habit, birth weight, Apgar index, duration of gestation, childbirth type and need for the newborn to go to the intensive care unit (p > 0.05). For the pregnant women with preeclampsia and gestational diabetes, only FEF25-75% presented statistical significance (p = 0.01 and 0.034, respectively). CONCLUSION: In high-risk pregnant women, pulmonary function suffered alterations characterized by limitations regarding airflow through the airways, although without repercussions on gestational outcome.

Canindé has a population of 71,235 inhabitants. In April 2001, the city started using larvivorous fish in cement water tanks, as a means of biological control of Aedes aegypti larvae. During house-to-house visits by health agents, instead of treating the water tank with larvicide, a Betta splendens fish was introduced into each tank. The number of houses and the number of tanks was estimated by monthly surveys. Then, the number of tanks per house was determined. Taking into account this estimated number and the number of houses visited, the infestation level for each kind of deposit was analyzed. In January 2001, 70.4% of the water tanks presented mosquitoes. Following the intervention, in January 2002 only 7.4% were positive and by December 2002 the rate had dropped to 0.2%. The efficacy of Betta splendens as a biological control agent in cement water tanks was clearly demonstrated, by achieving a 320 times reduction in the infestation level.

Knowledge, attitudes, and behaviors related to high blood pressure among adults in a low-income community in Caucaia, Ceará, Brazil, as well as factors influencing their attitudes toward behavioral risk factors (smoking, alcohol consumption, salt and fat consumption, and lack of physical exercise) were studied. A total of 228 individuals were interviewed using a structured interview protocol. Thirteen focus groups provided a more in-depth analysis of attitudes. According to the findings, the reasons behind behaviors and attitudes are linked to emotions and socioeconomic issues. Behavioral changes involve giving up certain pleasures, for people who are struggling to survive in living conditions imposed by poverty. Living in poverty interferes with the possibility of behavioral change.

OBJECTIVE: The prevalence of hepatitis C virus (HCV) is higher in patients on hemodialysis than in the general population, probably due to greater exposure to risk situations. The purposes of the study were to determine anti-HCV antibodies prevalence among hemodialysis patients and dialysis clinics and patients factors associated with HCV transmission. METHODS: A cross-sectional study was conducted in 752 hemodialysis patients in all 12 dialysis clinics of Fortaleza, Brazil, and were screened using third generation ELISA. Sociodemographic, clinical, and epidemiological data of 663 patients were collected through interviews. Nosocomial factors were assessed using a specific questionnaire tool. Statistical analysis was conducted using Student's t test, odds ratio and multivariate analysis. RESULTS: The prevalence of anti-HCV was 52% (390/746; ranged from 6% to 72%). The anti-HCV positivity was higher in patients who had previous peritoneal dialysis (OR=1.76; 95% CI 1.12-2.76) and blood transfusion (OR=2.75; 95% CI 1.25-6.03). Dialysis age has been associated with anti-HCV positivity (OR=1.47; 95% CI 1.35-1.61). Clinics practices associated with anti-HCV positivity were: previous preparing of heparin (OR=2.92; 95% CI 1.23-6.92), failure in gloves use or change (OR=5.73; 95% CI 1.75-18.72), unsatisfactory dialysis machine disinfection (OR=2.79; 95% CI 1.57-4.96), and patient isolation in dialysis room (OR=0.18; 95% CI 0.05-0.61). CONCLUSIONS: The results show high anti-HCV prevalence among hemodialysis patients and the association of nosocomial factors with new HCV infection cases.

PURPOSE: To evaluate the maternal risk factors that require newborn assistance in neonatal Intensive Care Units (ICU). METHODS: A prospective observational case-control study was conducted on 222 pregnant women (1:1 case-control ratio) attended at a public maternity. The following variables were analyzed in the puerperae: age at menarche, age at first sexual intercourse, history of chronic diseases, habits, prenatal care, obstetric history, clinical complications during pregnancy and childbirth, and sociodemographic variables. The variables of the newborns were: Apgar scores, gestational age, birth weight, presence or absence of malformation, need for resuscitation, and complications during the first 24 hours. Proportions were compared using the Fisher exact test or the Person γ2 test. Multivariable models were developed by logistic regression analysis using adjusted Odds Ratio with a 95% confidence interval (CI). RESULTS: Regarding reproductive history, ≥3 pregnancies and 2 or 3 previous cesareans were sytatistically significant (p=0.0 and 0.0, respectively). Among the complications that required assistance in the neonatal ICU, prematurity was responsible for 61 cases (55.5%), followed by risk of intrapartum infection in 46 cases (41.8%). Regarding the maternal history, the presence of hypertensive disease showed statistical significance (p=0.0). Premature rupture of membranes was strongly associated with the need for the neonatal ICU (Odds Ratio - OR=6.1, 95%CI 2.6-14.4). CONCLUSIONS: Premature rupture of membranes and hypertensive disease should receive special attention in prenatal care due to their strong association with newborns requiring assistance in the neonatal ICU.

INTRODUCTION: The Brazilian National Health System may reduce inequalities in access to health services through strategies that can reach those most in need with no access to care services. OBJECTIVE: To identify factors associated with the use of health service by children aged 5 to 9 years in the city of Sobral, Ceará, northeastern Brazil. RESULTS: Only 558 (17.0%) children used health care services in the 30 days preceding this survey. Children with any health condition (OR = 3.90) who were frequent attenders of primary care strategy of organization (the Family Health Strategy, FHS) (OR = 1.81) and living in the city's urban area (OR = 1.51) were more likely to use health services. Almost 80% of children used FHS as their referral care service. Children from poorer families and with easier access to services were more likely to be FHS users. CONCLUSION: The study showed that access to health services has been relatively equitable through the FHS, a point of entry to the local health system.

ABSTRACT Context Complementary and Alternative Medicine (CAM) are unconventional care practices that, according to the World Health Organization, should be implemented in the Health System, particularly in poor regions of the country. In Brazil, they have been adopted by Care Programs and introduced into undergraduate medical education. In this study we were interested in evaluating the teaching-learning process of Integrative Practices in Brazilian medical schools. Methodology A cross-sectional study was carried out at Brazilian medical schools with a self-administered questionnaire for teachers and a secondary data survey was obtained from medical school websites and government database institutions. For the presentation of the variables, frequency distribution and the Pearson coefficient (X2) -Chi-square tests were used. The proportions were compared using the Chi-square test or the Fisher's Exact Test. When the expected value of a contingency table was equal to or greater than 5, the Chi-square test was used; in all other situations, Fisher's Exact Test was used. The difference between proportions was estimated by the Odds Ratio, calculated through simple logistic regression (95% CI). Results 57 of the 272 medical schools in Brazil address CAM, with it being proportionally higher in the South and Mid-West regions. The medical schools are highly concentrated in state capitals, and the Northeast region presents a significant concentration of medical schools with CAM in the capitals. The number of schools with active and traditional methodologies in CAM is equivalent. Homeopathy, Acupuncture and Integrative Medicine predominate, with a minority using Indigenous Practices, Chronotherapy and Anthroposophic Medicine. The new educational guidelines have not affected the number of schools with CAM. Growth in CAM has been insignificant (p <0.05) in the last ten years. Conclusion There has been no growth in teaching Complementary and Alternative Medicine in undergraduate medical training in Brazil since the introduction of the new curricular guidelines, even in view of the needs of the health system.


RESUMO Contexto Medicinas Complementares e Alternativas (MAC) são práticas de cuidados não convencionais que, segundo a Organização Mundial da Saúde, devem ser implementadas no sistema de saúde, particularmente nas regiões pobres do País. No Brasil, têm sido inseridas na saúde como Práticas Integrativas por meio de programas assistenciais e no ensino de graduação. Objetivou-se avaliar o processo de ensino-aprendizagem em Práticas Integrativas nas escolas médicas brasileiras. Metodologia Um estudo transversal foi realizado em escolas médicas brasileiras com um questionário autoaplicável para professores, e um levantamento de dados secundários foi obtido em websites de escolas médicas e instituições de bancos de dados governamentais. Na apresentação das variáveis utilizou-se a distribuição de frequências e os testes do coeficiente de Pearson (x2) - Qui-Quadrado. As proporções foram comparadas por meio do teste Qui-Quadrado ou do Teste Exato de Fisher. Quando o valor esperado de uma tabela de contingência foi igual ou superior a 5, utilizou-se o teste Qui-Quadrado; em todas as outras situações, utilizou-se o Teste Exato de Fisher. A diferença entre as proporções foi estimada pela Odds Ratio, calculada por meio de regressão logística simples (95% IC). Resultados Das 272 escolas médicas do Brasil, 57 abordam MAC, sendo essa proporção maior nas regiões Sul e Centro-Oeste. Há uma concentração de escolas médicas em capitais, e a Região Nordeste apresenta uma concentração significativa de escolas médicas com MAC nas capitais. O número de escolas com metodologias ativas e tradicionais no ensino de MAC é equivalente. A homeopatia, a acupuntura e a medicina integrativa predominam, com uma minoria usando práticas indígenas, cromoterapia e medicina antroposófica. As novas diretrizes educacionais não causaram impacto no número de escolas com MAC. O crescimento em MAC foi insignificante (p < 0, 5) nos últimos dez anos. Conclusão Não houve crescimento no ensino em Práticas Integrativas e Complementares no Brasil na graduação médica após as novas diretrizes curriculares, mesmo diante das necessidades do sistema de saúde.

Introdução: Embora os primeiros casos de dengue no estado do Ceará tenham ocorrido em 1986, há registros da presença do mosquito Aedes aegypti desde os anos de 1851/1852. Mesmo após 30 anos, a dengue permanece como um problema grave de saúde pública com epidemias cada vez mais frequentes. Objetivo: Resgatar, reunir e sintetizar a evidência científica produzida nos primeiros 30 anos de dengue no Ceará, contribuindo para melhorar sua compreensão e as intervenções de vigilância e controle. Métodos: Realizou-se uma revisão de literatura com busca de artigos (inglês, português e espanhol) nas bases de dados Pubmed, Scielo, Clinicalkey, Lilacs, Google Acadêmico e banco virtual de teses e dissertações da CAPES, além de livros. O período de 1986 a 2016 foi usado como limite de busca e a mesma foi realizada entre os meses de agosto a novembro de 2016. Utilizaram-se os descritores controlados: dengue, Ceará, Fortaleza e Aedes; com interposição do operador boleano "AND". Resultados: Foram identificadas 574 publicações potencialmente elegíveis, sendo 461 artigos e 113 dissertações ou teses. Foram retirados 272 artigos duplicados ou que não atenderam aos critérios de inclusão. Foram lidos 114 artigos publicados em 60 revistas diferentes, sendo 72,8% artigos completos, 75,2% em inglês, 42,2% experimentais e 81,5% com abordagem quantitativa. Foi publicado apenas um artigo na década de 1980, já na década seguinte foram sete, número que cresceu bastante nos anos 2000, com 38 artigos. No entanto, já há 69 artigos publicados apenas na primeira metade da década de 2010. No período, o Ceará confirmou quase um milhão de casos de dengue, com a circulação dos quatro sorotipos (DENV1 - 1986, DENV2 - 1994, DENV3 - 2002 e DENV4 - 2011) e registrou pelo menos 14 epidemias. A partir de 2015, o Ceará passou a apresentar um cenário diferenciado de tripla epidemia, com a cocirculação autóctone de dois outros arbovírus: Chikungunya e Zika. Conclusão: Nesse período de 30 anos (1986-2016), o Ceará vivenciou várias epidemias de dengue, e, de certa forma, isto vem impulsionando a busca por respostas para o controle desta doença. Há claramente um crescimento em número de artigos publicados a cada ano, revelando a pujança dos grupos locais, que contribuiu de forma importante para a produção científica em diversos fatores relacionados à compreensão da epidemiologia e controle da dengue. Introduction: Although the first cases of dengue in the state of Ceará occurred in 1986, there are records of the presence of the Aedes aegypti mosquito since the years 1851/1852. Even after 30 years, dengue remains a serious public health problem with frequent epidemics. Objective: To rescue, gather and synthesize scientific evidence in the first 30 years of dengue in Ceará, contributing to improve its understanding, surveillance and control interventions. Methods: An integrative review was carried out with the search of articles (English, Portuguese and Spanish) in the databases Pubmed, Scielo, Clinicalkey, Lilacs, Google Scholar and virtual bank of thesis and dissertations of CAPES, as well as books. The period from 1986 to 2016 was used as a search limit and it was carried out between August and November 2016. The following descriptors were used: dengue, Ceará, Fortaleza and Aedes; with interposition of the Boolean operator "AND". Results: 574 potentially eligible publications were identified, 461 articles and 113 dissertations or thesis. 272 duplicate articles were removed or did not meet the inclusion criteria. We have read 114 articles published in 60 different journals, 72.8% were full articles, 75.2% were in English, 42.2% were experimental and 81.5% were quantitative. Only one article was published in the 1980s, and in the next decade there were seven, this number grew up considerably in the 2000s with 38 articles. However, there are already 69 articles published only in the first half of the decade of 2010. In this period, Ceará confirmed almost 1 million cases of dengue with the circulation of the four serotypes (DENV1 - 1986, DENV2 - 1994, DENV3 - 2002 and DENV4 - 2011) and recorded at least 14 epidemics. From 2015 on, Ceará started to present a differentiated scenario of a triple epidemic, with the autochthonous co-circulation of two other arboviruses: chikungunya and Zika. Conclusion: In this period of 30 years (1986-2016), Ceará experienced. (AU)

Introdução: Uma alimentação incluindo frutas e hortaliças sugere um efeito protetor ao desenvolvimento e progressão do câncer, pois estas são ricas em compostos que possuem ação antioxidante. Objetivo: Avaliar os fatores associados ao consumo de frutas e verduras em idosos portadores de câncer de próstata no Ceará. Método: Estudo transversal com 211 idosos apresentando diagnóstico prévio de câncer de próstata, atendidos em instituição de referência para diagnóstico e tratamento da doença. Aplicou-se questionário contendo dados sociodemográficos e de estilo de vida. Foram aferidos peso e altura, para cálculo do índice de massa corporal e circunferência da cintura. O consumo de frutas e hortaliças foi avaliado por Questionário de Frequência Alimentar, obtendo-se quantidade diária em gramas e número de porções. Foram consideradas adequadas: frequência diária, quantidade em gramas ≥ 400 g/dia e três ou mais porções/dia. Foram utilizados os testes de Shapiro Wilk e Qui-Quadrado, adotando-se 5% como nível de significância. Resultados: A maioria dos idosos apresentou frequência e quantidade (gramas e porções) ingeridas inadequadas para frutas e hortaliças e essa inadequação se associou aos seguintes fatores: ser preto ou pardo (p=0,002), não ter o ensino fundamental completo (p=0,030) e ser sedentário (p=0,041). Conclusão: A inadequação no consumo de frutas e hortaliças reforça a necessidade de medidas de promoção da alimentação saudável nesse público.(AU)


Introduction: A diet including fruits and vegetables suggests a protective effect on the development and progression of cancer, as these are rich in compounds that have antioxidant properties. Objective: To evaluate the factors associated with consumption of fruits and vegetables in elderly patients with prostate cancer in Ceará. Methods: Cross-sectional study of 211 elderlies presenting a previous diagnosis of prostate cancer, seen at a reference institution for diagnosis and treatment of this disease. It was applied a questionnaire containing demographic and lifestyle data. They were measured weight and height, to calculate body mass index, and waist circumference. The consumption of fruits and vegetables was evaluated by Food Frequency Questionnaire, obtaining daily amount in grams and number of servings. They were considered adequate: daily frequency, amount in grams ≥ 400 g / day and three or more servings / day. Shapiro Wilk and Chi-square tests were used, adopting a 5% of significance level. Results: Most of the elderly presented frequency and amount (grams and portions) taken unsuitable for fruits and vegetables and this inadequacy was associated with the following factors: being black or brown (p=0.002) having not complete elementary education (p=0.030) and being sedentary (p=0.041). Conclusion: Inadequate consumption of fruits and vegetables reinforces the need for measures to promote healthy eating in this group.(AU)

ABSTRACT The identification of the prevalence of cognitive impairment and associated factors among older adults is important in countries facing rapid demographic transition, given the significant implications for public policy and health planning. Objective: To determine the prevalence of functional cognitive impairment (FCI) and associated factors in Brazilian community-dwelling older adults. Methods: A cross-sectional study involving 461 elderly subjects residing in Fortaleza city, Ceará was conducted. Cognitive assessment was performed using three tests: the MMSE (Mini-Mental State Examination), VF (Verbal Fluency) and CT (Clock Test). The functional capacity evaluation was based on a survey of 21 basic and instrumental activities of daily living (ADLs). Cognitive impairment was defined by MMSE cut-off points adjusted for literacy. Functional impairment was defined as dependency to carry out more than four ADLs. Results: The prevalence of FCI was 13.64% (95% CI: 10.33 to 16.64%). FCI was proportionally associated with age with OR=2.24 (95% CI: 1.04 to 4.79) for individuals aged 70 to 79 years and OR=8.27 (95 % CI: 4.27 to 16.4) for those aged 80 to 100 years. FCI was associated with self-reported diseases including hypertension OR=2.06 (95% CI: 1.17 to 3.65), stroke OR=2.88 (95% CI: 1.66 to 5.00) and acute myocardial infarction OR=2.94 (95% CI: 1.59 to 5.42). The occurrence of FCI was proportionally correlated with the number of drugs used. Conclusion: Functional cognitive impairment is a prevalent condition in Brazilian community-dwelling older adults and its occurrence is associated with age, number of drugs used, and vascular morbidities.


RESUMO A identificação da prevalência de déficit cognitivo, e fatores associados, é importante, dadas as implicações significativas para as políticas públicas e planejamento em saúde. Objetivo: Determinar a prevalência de CCF e fatores associados em idosos de uma área urbana brasileira. Métodos: Estudo transversal envolvendo 461 idosos residentes em uma área urbana de Fortaleza, Ceará. Foi aplicado um questionário socioeconômico e de saúde e realizou-se avaliação cognitiva através de três testes de rastreio: Miniexame do Estado Mental (MEEM), FV (Fluência Verbal) e TR (Teste do Relógio). A avaliação da capacidade funcional foi feita por um questionário de 21 atividades da vida diária. Comprometimento cognitivo foi definido pelo comprometimento no MEEM ajustado para escolaridade. Comprometimento funcional foi definido pela incapacidade para mais de 4 AVDs. Resultados: A prevalência de CCF foi de 13,64% (IC 95%: 10,33-16,64%). CCF foi proporcionalmente associado à idade: razão de chances (RC) 2,24 (IC95%: 1,04-4,79) para 70 a 79 anos e RC 8,27 (IC 95%: 4,27-16,4) para 80 a 100 anos. CCF apresentou associação significante com doenças autorreferidas: hipertensão arterial sistêmica apresentou RC 2,06 (IC 95%: 1,17-3,65), acidente vascular cerebral de 2,88 (IC 95%: 1,66-5,00) e infarto agudo do miocárdio de 2,94(IC 95%: 1,59-5,42). A ocorrência de CCF correlacionou-se com maior número de medicamentos usados. Conclusão: O CCF é condição comum entre os idosos no Brasil e sua ocorrência está associada à idade, ao número de fármacos utilizados e às morbidades vasculares.
