RESUMEN

Currently lacking research to explore the correlation between inflammatory markers and the efficacy of immune checkpoint inhibitors (ICIs) combined with chemotherapy in the treatment of advanced gastric cancer. This study is a retrospective study and included patients with advanced gastric cancer who receiving ICIs combined with chemotherapy from January 2020 to December 2022. We analysed the relationship between systemic inflammatory markers and the efficacy of ICIs combined chemotherapy and constructed a clinical prediction model. A nomogram was constructed based on the results of the bidirectional stepwise regression model. A total of 197 patients were enrolled in the training group, with a median follow-up period of time 26 months. Kaplan Meier analysis showed that the median OS of patients with low systemic immune-inflammatory index (SII) and low platelet to lymphocyte ratio (PLR) was superior to those with high SII and PLR. Univariate and multivariate Cox regression analysis showed that SII, NLR, PLR, and N stage as independent prognostic factors for OS. Adding SII to the conventional model improved the predictive ability of the 12-month OS. A total of 95 patients were included in the validation group, and external validation of the SII-based nomogram showed favourable predictive performance. Baseline SII, PLR, and N stage may serve as independent predictive factors for survival outcomes in advanced gastric cancer patients undergoing ICIs combined with chemotherapy. The SII-based nomogram can provide intuitive and accurate prognosis prediction of individual patients.

BACKGROUND: Triple-negative breast cancer (TNBC) is a life-threatening subtype of breast cancer with limited treatment options. Therefore, this network meta-analysis (NMA) aimed to evaluate and compare the effect of various neoadjuvant chemotherapy (NCT) options on the long-term survival of patients with TNBC. METHODS: PubMed, Embase, Medline, Cochrane Library, Web of Science, and major international conference databases were systematically searched for randomized controlled trials (RCTs) on the efficacy of various NCT options in patients with TNBC. Searches were performed from January 2000 to June 2023. Study heterogeneity was assessed using the I2 statistic. Hazard ratios (HRs) and 95% confidence intervals (CIs) were used to evaluate disease-free survival (DFS) and overall survival (OS). Odds ratios (ORs) and 95% CIs were used to evaluate the pathologic complete response (pCR). The primary outcome was DFS. RESULTS: We conducted an NMA of 21 RCTs involving 8873 patients with TNBC. Our study defined the combination of anthracyclines and taxanes as the preferred treatment option. On this basis, the addition of any of the following new drugs is considered a new treatment option: bevacizumab (B), platinum (P), poly-ADP-ribose polymerase inhibitors (PARPi), and immune checkpoint inhibitor (ICI). Based on the surface under the cumulative ranking curve (SUCRA) values, the top three SUCRA area values of DFS were taxanes, anthracycline, and cyclophosphamide (TAC; 89.23%); CT (84.53%); and B (81.06%). The top three SUCRA area values of OS were CT (83.70%), TAC (62.02%), and B-containing regimens (60.06%). The top three SUCRA area values of pCR were B + P-containing regimens (82.7%), ICI + P-containing regimens (80.2%), and ICI-containing regimens (61.8%). CONCLUSIONS: This NMA showed that standard chemotherapy is a good choice with respect to long-term survival. Moreover, B associated with P-containing regimens is likely to be the optimal treatment option for neoadjuvant TNBC in terms of pCR.

Chemotherapy-induced fatigue reduces not only the quality of life of patients but also effect their recurrence-free survival rate. Although electroacupuncture can relieve fatigue, it has limited affect on some patients. Therefore, appropriate biomarkers are needed to help screen patients who can benefit from electroacupuncture treatment of fatigue. We conducted this study to explore the predictive ability of SNPs on the efficacy of electroacupuncture in the treatment of fatigue in patients with breast cancer after adjuvant chemotherapy. Our study included breast cancer patients with fatigue after receiving paclitaxel and/or anthracycline based adjuvant chemotherapy. The patients were divided into the electroacupuncture group and the control group. The electroacupuncture treatment group received adjuvant chemotherapy and electroacupuncture treatment, while the control group only received adjuvant chemotherapy, and then compared the fatigue relief degree of two groups. In addition, we used NCBI dbSNP and PharmGKB databases to select fatigue related genes and their SNPs. We collected peripheral blood from the included patients for SNPs typing, and recorded the efficacy of electroacupuncture to analyzed the correlation between different SNPs and therapeutic efficacy. The side effects of electroacupuncture treatment were also recorded. 76 patients in the electroacupuncture group and 48 patients in the control group were enrolled. In the electroacupuncture group, 63 patients (82.9%) experienced moderate to severe fatigue (BFI score > 3). After electroacupuncture treatment, the number of patients with a BFI score of > 3 was 46 (60.5%). Therefore, the fatigue symptoms of 26.9% patients were significantly improved (P < 0.05). In the control group, which did not receive electroacupuncture treatment, 40 of 48 patients had a BFI score of > 3. Following the same observation time used in the electroacupuncture group, 36 patients had a BFI score of > 3 points. Thus, fatigue was not significantly relieved in the control group (83.3% vs. 75.0%, P > 0.05). We included 56 patients in our analysis of the correlation between SNPs and electroacupuncture treatment effects. We divided the patients into an effective group and ineffective group according to therapeutic effects. Our results indicated that the effective rate of electroacupuncture treatment with IL1A rs3783550 AC and CC genotypes was higher than that with other genotypes (AC: 84.6%, CC: 81.8%, AA: 33.0%, P < 0.05). Similarly, the effective rate of electroacupuncture treatment with HTR1A rs6295 GG and CC genotypes was higher than that with other genotypes (GG: 63.0%, CC: 55.6%, GC: 18.2%, P < 0.05). However, no other genotypes were related to the effect of electroacupuncture treatment on fatigue. Our result showed that electroacupuncture has therapeutic effect on fatigue after adjuvant chemotherapy for breast cancer and the side effects are tolerable. In addition, IL1A rs3763550 and HTR1A rss6295 can predict the therapeutic effect of electroacupuncture on fatigue after adjuvant chemotherapy in breast cancer, which helps to better screen patients who can benefit from electroacupuncture treatment.

Two-dimensional (2D) mesoporous polymers, combining the advantages of organic polymers, porous materials, and 2D materials, have received great attention in adsorption, catalysis, and energy storage. However, the synthesis of 2D mesoporous polymers is not only challenged by the complex 2D structure construction, but also by the low yield and difficulty in controlling the dynamics of the assembly during the generation of mesopores. Herein, a facile multi-dimensional molecular self-assembly strategy is reported for the preparation of 2D mesoporous polydiaminopyridines (MPDAPs), which features tunable pore sizes (17-35 nm) and abundant N content up to 18.0 at%. Benefitting from the abundant N sites, 2D nanostructure, and uniform-large mesopores, the 2D MPDAPs exhibit excellent catalytic performance for the Knoevenagel condensation reaction. After calcination under N2 atmosphere, the obtained 2D N-doped mesoporous carbon (NMCs) with large and uniform pore sizes, high surface areas, abundant N content (up to 23.1%), and a high ratio of basic N species (57.0% pyridinic N and 35.9% pyrrolic N) can show an excellent CO2 uptake density (11.7 µmol m-2 at 273 K), higher than previously reported porous materials.

Synthetic polymer-derived hollow carbon spheres have great utilitarian value in many fields for which the synthesis of proper polymer precursors is a key process. The exploration of new suitable polymer precursors and the construction of refined hollow structures in emerging polymers are both of great significance for synthetic methodology and novel carbon materials. Here, for the first time Schiff base polymer (SBP) colloid spheres with refined hollow structures were synthesized by tandem gradient growth and confined polymerization processes. The Hill equation was employed as a mathematical model to explain the gradient growth of SBP spheres. The size-dependent inner structure of SBP spheres can be adjusted from hollow to multichamber-surrounded hollow, and then to a multichamber structure. SBP-derived carbon spheres having similar surface area and chemical composition but different inner structures provide an effective way to investigate the relationship between inner structure and performance.

Optical absorption and scattering result in quality degradation of underwater images, which hampers the performance of underwater vision tasks. In practice, a well-posed underwater image recovery requires a combination of scene specificity and adaptability. To this end, this paper breaks down the overall recovery process into in-situ enhancement and data-driven correction modules, and proposes a Multi-stage Underwater Image Enhancement (MUIE) method to cascade the modules. In the in-situ enhancement module, a channel compensation with scene-relevant supervision is designed to address different degrees of unbalanced attenuation, and then the duality-based computation inverts the result of running a enhancement on inverted intensities to recover the degraded textures. In response to different scenarios, a data-driven correction, encoding corrected color-constancy information under data supervision, is performed to correct the improper color appearance of in-situ enhanced results. Further, under the collaboration between scene and data information, the recovery of MUIE avoids ill-posed response and reduces the prior dependence of specific scenes, resulting in a robust performance in different underwater scenes. Recovery comparison results confirm that the recovery of MUIE shows the superiority of scene clarity, realistic color appearance and evaluation scores. With the recovery of MUIE, the Underwater Image Quality Measurement (UIQM) scores of recovery-challenging images in the UIEB dataset were improved from 1.59 to 3.92.

In order to understand the molecular mechanism of cotton's response to drought during the flowering and boll stage, transcriptomics and metabolomics were carried out for two introgression lines (drought-tolerant line: T307; drought-sensitive line: S48) which were screened from Gossypium hirsutum cv. 'Emian22' with some gene fragments imported from Gossypium barbadense acc. 3-79, under drought stress by withdrawing water at flowering and boll stage. Results showed that the basic drought response in cotton included a series of broad-spectrum responses, such as amino acid synthesis, hormone (abscisic acid, ABA) signal transduction, and mitogen-activated protein kinases signal transduction pathway, which activated in both drought-tolerant and drought-sensitive lines. However, the difference of their imported fragments and diminished sequences triggers endoplasmic reticulum (ER) protein processing, photosynthetic-related pathways (in leaves), and membrane solute transport (in roots) in drought-tolerant line T307, while these are missed or not activated in drought-sensitive line S48, reflecting the different drought tolerance of the two genotypes. Virus-induced gene silencing assay of drought-tolerant differentially expressed heat shock protein (HSP) genes (mainly in leaf) and ATP-binding cassette (ABC) transporter genes (mainly in roots) indicated that those genes play important role in cotton drought tolerant. Combined analysis of transcriptomics and metabolomics highlighted the important roles of ER-stress-related HSP genes and root-specific ABC transporter genes in plants drought tolerance. These results provide new insights into the molecular mechanisms underlying the drought stress adaptation in cotton.

It was aimed at the therapeutic value of loop electrosurgical excision procedure (LEEP) under propofol intravenous anesthesia combined with nano-silver gel in chronic cervicitis. 100 patients with chronic cervicitis were selected and randomly divided into the control group with 50 cases (LEEP under intravenous anesthesia with fentanyl) and the experimental group with the other 50 cases (LEEP under propofol intravenous anesthesia combined with the nano-silver gel). It was suggested from the results that the mean arterial pressure (MAP) and heart rate (HR) (76.88±5.65mmHg, 75.45±5.06beats/min) of patients in the experimental group at T1 time were better than those of the control group (83.76±5.43mmHg, 68.31±5.28beats/min). Visual analogue scale (VAS) (1.85±0.73 points), onset time of anesthesia (0.56±0.21min), time to open eyes (2.45±1.38min) and time of consciousness recovery (5.22±1.42min) were all lower than those of the control group (2.83±0.79 points, 0.93±0.25min, 4.33±1.45min, and 7.15±1.34min, respectively). The incidence of adverse reactions, the effective rate of treatment, the time of wound healing, and the incidence of complications (6%, 94%, 23.83±2.05 days, and 6%, respectively) were all better than those of the control group (24%, 70%, 29.25±2.16 days, and 6%) (P<0.05). All in all, intravenous anesthesia with propofol was beneficial to perform LEEP better, and the combination of LEEP combined with nano-silver gel had an important value in the treatment of chronic cervicitis and was worthy of clinical promotion.

BACKGROUND: Radial artery cannulation is a crucial investigative procedure for measuring patients' blood pressure invasively and serial blood gases. However, radial artery cannulation can be challenging for medical residents, and it is necessary to establish a facile and straightforward teaching strategy. This study aimed to evaluate the efficiency of acoustic shadowing-facilitated ultrasound guidance on radial artery cannulation teaching for medical residents. METHODS: A total of 116 medical postgraduates who underwent standardized residency training programs in the Department of Anesthesiology were randomly divided into a new ultrasound-guided teaching group and a traditional ultrasound-guided teaching group. In the new ultrasound-guided teaching group, radial artery puncture technique was taught by acoustic shadowing-facilitated ultrasound guidance. The training included both theoretical and practical components. After the training, the success rate of the first puncture attempt, the success rate of the catheterization, the ultrasonic positioning time, and the catheterization time of the two groups were compared in a unified manner. A questionnaire on the subjective evaluation of the various aspects of the program by participants was conducted at the end of the training period. RESULTS: The study included 101 medical residents. The success rate for radial artery puncture at the first attempt in the new ultrasound-guided teaching group was 78.43%. It was significantly higher than that of the traditional ultrasound-guided group (58.00%, odds ratio = 0.380; 95% CI = 0.159 to 0.908; p = 0.027). The success rate for the first arterial catheterization in the new ultrasound-guided teaching group was significantly higher than that of the traditional ultrasound-guided group (74.51% vs. 52.00%, odds ratio = 0.371; 95% CI = 0.160 to 0.858; p = 0.019). The ultrasonic positioning time and catheterization time in minutes in the new ultrasound-guided teaching group were significantly shorter than that of the traditional ultrasound-guided group (14.36 ± 3.31 vs. 18.02 ± 4.95, p < 0.001; 10.43 ± 2.38 vs. 14.78 ± 8.02, p = 0.012). However, no significant differences were observed in the incidence of local hematomas and teaching satisfaction scores between the two groups. CONCLUSION: Acoustic shadowing facilitates ultrasound-guided radial artery puncture and catheterization is beneficial in the standardized training and teaching of residents. It improves the success rate of the first attempt at radial artery puncture and catheterization and shortens the time of ultrasound location and catheterization. JQ1, a specific inhibitor of bromodomain-containing protein 4 (BRD4), could have great potential in the treatment of cervical cancer. However, its clinical application is limited by its short plasma half-life and limited antitumor efficacy. In this work, cisplatin (CDDP) was first utilized as the stabilizer and cooperator in the nanosystem (mPEG113-b-P(Glu10-co-Phe10)-CDDP/JQ1, called PGP-CDDP/JQ1) to break through the efficiency limitation of JQ1. The PGP-CDDP/JQ1 had a combination index (CI) of 0.21, exerting a strong cytotoxic synergistic effect. In vivo experiments revealed that PGP-CDDP/JQ1 had a significantly higher tumor inhibition effect (tumor inhibition rate: 85% vs 14%) and plasma stability of JQ1 (area under the curve (AUC0-∞): 335.97 vs 16.88 µg × h/mL) than free JQ1. The mechanism underling the synergism of JQ1 with CDDP in PGP-CDDP/JQ1 was uncovered to be inhibiting Plk1-mutant Trp53 axis. Thus, this study provides an optional method for improving the clinical application of JQ1 in cervical cancer.

The COVID-19 global pandemic has posed unprecedented challenges to nations and cities worldwide. Governments have adopted Information and Communication Technologies (ICTs) to rapidly control the spread of a novel coronavirus. As an innovative but controversial ICT-based tool, health QR code plays a vital role by assisting rapid contact tracing. Yet, whether and how citizens accept this policy tool remains an unknown theoretical and empirical question. In this paper, we study the sources that determine citizens' acceptance of health QR code in city governance. Based on a nation-wide online survey covering 28 major provincial-capital cities in China, we find that individual experiences and political identities affect citizens' acceptance of QR code. Even though public opinion regarding this issue is diverse, the government's responses to citizens' requests play a critical role in enhancing their acceptance of using QR code both in the current and future stages. Specifically, as the citizens perceive a higher level of city government responsiveness, they are less worried about privacy leaks and more likely to perceive the effectiveness of health QR code in improving public health, thus resulting in a higher acceptance. The results offer broad policy implications for smart cities and urban governance.

A visible-light-driven, photocatalyst-free route starting from easily accessed ortho-hydroxycinnamic esters and O-perfluoropyridin-4-yl oximes has been successfully developed to rapidly assemble a wide range of 3-cyanoalkyl coumarins. This process does not require addition of external photocatalysts, exhibiting beneficial features including mild reaction conditions, synthetic simplicity, and excellent substrate compatibility. Extensive mechanistic investigations revealed that the in situ generated phenolate anions served as photosensitizers to drive this photoinduced transformation.

The COVID-19 pandemic is a governance challenge for nations and cities across the world. While early observations have primarily focused on government actions, neighborhoods are at the frontline for coordinating grassroots level joint actions to fight against the pandemic. We draw from the collaborative governance theory and develop a theoretical framework for understanding the horizontal and hierarchical dynamics of collaborative neighborhood governance during crisis responses in urban China. Using a large-scale questionnaire survey of frontline community workers operated in six Chinese cities in February 2020, we conduct statistical analyses and find that the effectiveness of neighborhood collaboration in the pandemic control is predicted by both neighborhood social capital (i.e. civic engagement and citizen participation) and hierarchical steering by the government through setting policy priorities and providing support. Our research contributes to the international literature on neighborhood governance dynamics and provides policy lessons for improving neighborhood governance capacity in crisis response situations.

Triazine-based materials with porous structure have recently received numerous attentions as a fascinating new class because of their superior potential for various applications. However, it is still a formidable challenge to obtain triazine-based materials with precise adjustable meso-scaled pore sizes and controllable pore structures by reported synthesis approaches. Herein, we develop a solvent polarity induced interface self-assembly strategy to construct mesoporous triazine-based carbon materials. In this method, we employ a mixed solvent system within a suitable range of polarity (0.223≤Lippert-Mataga parameter (Δf) ≤0.295) to induce valid self-assembly of skeleton precursor and surfactant. The as-prepared mesoporous triazine-based carbon materials possess uniform tunable pore sizes (8.2-14.0 nm), high surface areas and ultrahigh nitrogen content (up to 18 %). Owing to these intriguing advantages, the fabricated mesoporous triazine-based carbon materials as functionalized porous solid absorbents exhibit predominant CO2 adsorption performance and exceptional selectivity for the capture of CO2 over N2 .

Na3 V2 (PO4 )3 (NVP) is regarded as a potential cathode material for sodium-ion batteries, whereas, its performance is usually limited by inherent low electronic conductivity and dense bulk structure. Herein, we develop a facile polymer stabilized droplet template strategy to synthesize porous single crystal structured NVP. The pore structures (macrostructures, hierarchically meso/macrostructures, and mesostructures), pore sizes (5-2000 nm), and specific surface areas (26-158 m2 g-1 ) of the samples can be readily controlled by tuning the sizes of droplet templates. The resultant hierarchically meso/macropores NVP demonstrates superior sodium storage performances, because its porous single crystal structure owns solid-liquid Na+ transmission mode, shortens ion diffusion distance and provides large electrode-electrolyte contact area, greatly facilitating fast ionic transport. We believe the presented method will supply a novel avenue to prepare porous single crystal structured materials for anticipative applications.

Lycoris radiata is a main source of Amaryllidaceae alkaloids; however, the low content of these alkaloids in planta remains a limit to their pharmaceutical development and utilization. The accumulation of secondary metabolites can be enhanced in plants inoculated with fungal endophytes. In this study, we analysed the diversity of culturable fungal endophytes in different organs of L. radiata. Then, by analysing the correlation between the detectable rate of each fungal species and the content of each tested alkaloid, we proposed several fungal candidates implicated in the increase of alkaloid accumulation. This was verified by inoculating these candidates to L. radiata plants. Based on the results of two independent experiments conducted in May 2018 and October 2019, the individual inoculation of nine fungal endophytes significantly increased the total content of the tested alkaloids in the entire L. radiata plants. This is the first study in L. radiata to show that fungal endophytes are able to improve the accumulation of various alkaloids. Therefore, our results provide insights into a better understanding of interactions between plants and fungal endophytes and suggest an effective strategy for enhancing the alkaloid content in the cultivation of L. radiata.

The designed synthesis of nanotwin architectures and thus-induced phase junctions expresses huge significance for semiconductor photocatalysts. However, current methods of producing nanotwins mainly involve high-temperature thermal treatment and tedious reaction steps, generally resulting in large bulk structure with ill-defined morphology and low specific surface area. Here, we propose a mild ligand-assisted coordinative self-assembly method to synthesize uniform mesoporous ZnxCd1-xS nanospheres with ultrahigh surface areas (148-312 m2 g-1) and controllable diameter (90-370 nm). Moreover, the sample possesses abundant phase junctions induced by nanotwins containing both hexagonal and cubic segments. With the synergy of the twin-induced phase junctions and high surface area, the as-prepared mesoporous Zn0.82Cd0.18S nanospheres exhibit a remarkable photocatalytic H2 evolution rate of 13.46 mmol h-1 g-1 with free noble metal. The mechanism of photocarrier dynamics was studied by transient photovoltage spectroscopy, manifesting that the photocarrier lifetime of Zn0.82Cd0.18S is largely prolonged and therefore improves the charge separation efficiency and photocatalytic activity.

Taper-free InP twinning superlattice (TSL) nanowires with an average twin spacing of ∼13 nm were grown along the zinc-blende close-packed [111] direction using metalorganic vapor phase epitaxy. The mechanical properties and fracture mechanisms of individual InP TSL nanowires in tension were ascertained by means of in situ uniaxial tensile tests in a transmission electron microscope. The elastic modulus, failure strain, and tensile strength along the [111] direction were determined. No evidence of inelastic deformation mechanisms was found before fracture, which took place in a brittle manner along the twin boundary. The experimental results were supported by molecular dynamics simulations of the tensile deformation of the nanowires that also showed that the fracture of twinned nanowires occurred in the absence of inelastic deformation mechanisms by the propagation of a crack from the nanowire surface along the twin boundary.

Mesoporous metal oxides (MMOs) have been demonstrated great potential in various applications. Up to now, the direct synthesis of MMOs is still limited to the solvent induced inorganic-organic self-assembly process. Here, we develop a facile, general, and high throughput solvent-free self-assembly strategy to synthesize a series of MMOs including single-component MMOs and multi-component MMOs (e.g., doped MMOs, composite MMOs, and polymetallic oxide) with high crystallinity and remarkable porous properties by grinding and heating raw materials. Compared with the traditional solution self-assembly process, the avoidance of solvents in this method not only greatly increases the yield of target products and synthesis efficiency, but also reduces the environmental pollution and the consumption of cost and energy. We believe the presented approach will pave a new avenue for scalable production of advanced mesoporous materials for various applications.

The objective of this study was to explore the effects of myocardial bridge compression on blood flow, normal stress, circumferential stress and shear stress in mural coronary artery. An original mural coronary artery simulative device has been greatly improved and its measured hemodynamic parameters have been expanded from a single stress (normal stress) to multiple stresses to more fully and accurately simulate the true hemodynamic environment under normal stress, circumferential stress and shear stress. This device was used to more fully explore the relationship between hemodynamics and mural coronary atherosclerosis under the combined effects of multiple stresses. Results obtained from the mural coronary artery simulator showed stress abnormality to be mainly located in the proximal mural coronary artery where myocardial bridge compression was intensified and average and fluctuation values (maximum minus minimum) of proximal stress were significantly increased by 27.8% and 139%, respectively. It is concluded that myocardial bridge compression causes abnormalities in the proximal hemodynamics of the mural coronary artery. This is of great significance for understanding the hemodynamic mechanism of coronary atherosclerosis and has potential clinical value for the pathological effect and treatment of myocardial bridge.
