Some features of molecular mechanisms of adenylate-cyclase system of native human erythrocytes were studied by the UHF-dielectrometry method. The results obtained show great changes in cell structure hydration when there is some influence on the membrane protein components which are involved in transmembrane signal transformation. The observed effects permit definite conclusions to be drawn concerning some molecular mechanisms of cell regulation. The obtained data demonstrated the following: the key role of N-protein, spatial separation of PGE2 regulatory centres and hormones on the beta-receptor, system modulation by the change of Ca2+ ions concentration in the presence of PGE2 and the connection of the alpha-receptor with the calcium slow channel.

The examination of 194 patients with coronary heart disease concurrent with clinical manifestations of angina pectoris established that the development of coronary atherosclerosis was accompanied by suppressed prostacyclin (PGI2) synthesis and increased thromboxane A2 (TxA2) leukotriene (LT) synthesis. The activation of synthesis of the vasoconstrictor and proaggregate TxA2 depended on the severity of clinical signs of angina pectoris. The imbalance of PGI2 to- TxA2 metabolite ratios was associated with dyslipidemias and the extent of coronary atherosclerosis. Bicycle ergometry testing resulted in prostanoid imbalance. The determination of the dynamics of prostanoids was used to check the efficiency of antianginal therapy. The concomitant application of inhibitors of thromboxane synthetase and LT synthesis was recommended for secondary prevention of thrombotic complications in patients with coronary atherosclerosis.

A moderate elevation of the daily excretion of free noradrenaline and adrenalin is observed in chronic circulatory insufficiency, beginning with Stage IIA. The catecholamines metabolism is elevated, as shown by the daily excretion of normethanpherine and methanpherine and of vanillyl-mandelic acid. The activity of renin and angiotensinases was growing along with the progressing cardiac insufficiency. The blood level of angiotensinogen was decreasing, especially in patients with Stage IIB and III of decompensation. The daily excretion of aldosterone was growing along with the development of cardiac insufficiency. The functional state of the glucocorticoid function of the adrenal cortex was of a phased nature in cases of circulatory insufficiency. The study of the functional state of the epiphysis was conducted by way of determining the blood level of melatonine and of its daily excretion. In Stages I and IIA the level of this hormone was clearly elevated, in Stages IIB and III -- decreased as compared with the initial and normal levels. The plasma level of the antidiuretic hormone was distinctly growing, beginning with Stage IIB, reaching its maximal values in Stage III.

Alpha-atrial natriuretic peptide (ANP) levels, plasma renin activity, and plasma aldosterone levels were measured in 103 patients with coronary heart disease (CHD) having signs of various circulatory failures. The parameters were studied in relation to central hemodynamic values, and their dependence on the stage of chronic circulatory failure. Alpha-ANP was found to be one of the factors of the humoral systems implicated in the pathogenesis of chronic circulatory failure in CHD patients. The plasma level of alpha-ANP was found to be related to the state of the renin-angiotensin-aldosterone system both in healthy subjects and CHD patients.

A study of platelet aggregation and platelet sensitivity to prostacyclin in 82 patients with acute myocardial infarction demonstrated a relationship between clinical pattern of the disease and platelet hemostasis. Platelet functional activity was the highest by the end of week 2 in uncomplicated myocardial infarction. In cases where myocardial infarction was complicated by acute left-ventricular insufficiency, platelet function differed depending on the severity and outcome of the complication. Some patients with pulmonary edema showed protracted low-grade, yet irreversible, two-wave platelet aggregation. Platelet sensitivity was significantly lower in pulmonary edema, as compared to cardiac asthma, while the lowest sensitivity was associated with terminal pulmonary edema.

The effect of chalone-containing aortic extracts on proliferative activity of smooth muscle cells was examined in conditions of experimental hypercholesterolemia. The tissue-specific cell proliferation inhibitor present in the extract is shown to depress mitotic activity of smooth muscle cells in postnatal rats. The area under aortic atherosclerotic lesion diminished twofold as compared to that of the controls. Morphologically, atherosclerotic changes are seen as lipid plaques.

Using the fluorimetric method, the activity of dipeptidyl-carboxypeptidase (DCP) was studied in 85 patients suffering from essential hypertension. It was ascertained that the DCP activity in the blood serum of such patients was significantly higher than in normal subjects. The highest activity of the enzyme was observed in cases where essential hypertension was aggravated by chronic heart failure or an acute impairment of the cerebral circulation. Inflammatory processes in hypertensive patients were associated with a considerable fall in the serum enzyme activity.

Seventy-six patients with chronic coronary heart disease of the atherosclerotic genesis were examined using clinical laboratory and instrumental research methods. The blood serum levels of total cholesterol, triglycerides, lipoproteins and bile acids were measured throughout the course of treatment. When hyperlipoproteinemias were divided according to phenotypes, type II hyperlipoproteinemia proved to be most commonly occurring (65.8%). The patients exhibited lower blood serum levels of bile acids as compared to control.

The values of depressive humoral factors drastically decrease in healthy subjects and patients with hypertensive disease running with crisis in the areas of hot climate in summer. After arresting hypertensive crises, the levels of kallikreinogen, kallikrein, kininogen increase. In the crises, there is a significant inverse correlation between the blood pressure and blood kallikrein and kininogen concentrations. The values of kallikrein-kinin system components clearly characterize the clinical status of patients with hypertensive disease running with crisis and are of predictive value for clarification of the body's protective reserves.

Clinical and functional examination of the miners who suffered from coronary heart disease showed a higher ectopic activity of the myocardium during their work under underground conditions and during the subsequent 4 hours after its termination. It also indicated a significantly higher fatal cases of sudden coronary death in the miners in the aforementioned periods. To prevent the development of complications occurred in miners, the status of the cardiovascular system was proposed to be dynamically controlled.

The authors have developed methods for diagnosing and forecasting myocardial infarction outcomes with the use of current mathematical approaches. Studied the problems of simulating appropriate conditions and processes. Developed some effective algorithms of predicting myocardial infarction outcomes on the basis of the models obtained. Using the approaches (algorithms) suggested the authors have reviewed the methods for finding optimal correction of myocardial infarction treatment, based on the idea of maximization of the survival probability as one of the outcomes of the disease in question. The computer-aided methods for diagnosing, forecasting outcomes and choice of optimal treatment tactics for myocardial infarction during stay at a hospital can be very instrumental in raising the efficacy of the treatment and diagnosis of the disease under consideration.

The authors studied the time-course of lipid peroxidation with special reference to malonic dialdehyde in blood serum comparatively to the time-course of uricemia in 189 patients with acute large-focal myocardial infarction. It was established that the time-course of malonic aldehyde and uric acid is characterized by a consistent increase which reaches the maximal values at the height of the destructive phase of myocardial infarction as well as by the differences in uncomplicated and complicated healing. The joint index of destruction was developed, permitting one to compare the concentrations of malonic dialdehyde and uric acid in blood serum and to exercise an efficacious biochemical "tracing" of the healing.

A study is presented of hemodynamic effects and reactions of prostacyclin--thromboxane system in patients with stable (63 pts) and unstable angina pectoris (57 pts) who took aldizem 240 mg daily during 3 week. It was found that the aldizem improved clinical and hemodynamic indices and increased plasma concentration of 6-keto-PGF1 alpha and FPE2, but reduced TxB2 and PGF2 alpha. Also these changes depended on time of therapy: long-term therapy resulted in improvement of the prostacyclin-thromboxane balance.

ELISA using the test systems and a complex of equipment manufactured by Flow Company was employed to study over time the content of fibronectin, fibrinogen, products of its degradation and myoglobin in 178 patients suffering from coronary heart disease (stable and progressive angina pectoris, acute myocardial infarction). The concentration of myoglobin, fibronectin, fibrinogen and products of its degradation was established to depend on the gravity of coronary heart disease and the tame elapsed since the disease onset. In patients with progressive disease, there was an increased consumption of fibronectin which may be due both to its expenditure during blood coagulation and fulfillment of angioprotective function because of exacerbation of systemic atherosclerosis.

The paper provides the results of studying the clinical disease manifestations, the plasma content of fibronectin and fibrin and fibrinogen degradation products in patients with chronic hepatitis (CH). It is shown that they are determined not only by the morphological form of CH but also by the structure of inflammatory infiltrates in liver tissue.

A study of the effect of the main risk factors of atherosclerosis in coronary patients on the prevalence of lipoidosis and prelipid changes in the aortal intima showed a week interrelationship between the studied risk factors and prevalence of lipoidosis. A noticeable significant interrelationship of the prevalence of prelipid changes in the aortal intima, patient's age, the IRI level and alpha-XC concentration in the blood plasma was established. An equation was described. A nomogram was presented permitting determination of the prevalence of prelipid changes in the aortal intima and, consequently, the phase of a course of atherosclerosis during the patient's lifetime on the basis of the above risk factors.

Experiments carried out on 91 dogs have shown that changes in the inflammatory process course result in complicated postinfarction repair of myocardium, disagreement in the development, i.e. in desynchronization of necrotic repair processes in the myocardial infarction zone. It is supposed that regulation of the myocardial infarction healing by inflammation can be one of the efficient methods of its therapy.

Changes in plasma fibronectin in acute and chronic viral hepatitides have been investigated in 125 patients. In acute and chronic viral hepatitis B plasma fibronectin level lowered in parallel with changes in the disease severity.

Serum levels of eicosanoids in patients suffering from chronic hepatic diseases were analysed in correlation with morphological characteristics of the hepatic lesion, body reactivity, manifestations of the alternative and immune phases of the inflammation. The results could be contributing to diagnosing and treating chronic inflammation in the liver.
