Despite compelling evidence that some patients with a psychiatric diagnosis could benefit from genetic assessment, genetic testing for psychiatric patients is underutilized. Few studies have reported psychiatric genetics training for mental health specialists, and such research is especially lacking in Spain. We aimed to gather the opinions of Spanish mental health residents, including resident intern nurses (RINs), doctors (RIDs) and psychologists (RIPs). A short survey was prepared by an expert team and distributed to all mental health residency centers in Spain during the first semester of 2021. Of the 2028 residents, 18% responded. Participants were mainly females (71%), in their first year of residency (37%) and within the 27-31-year age range. While participants received little theoretical (13.4%) and practical (4.6%) training on average, RIDs had the most affirmative responses. Notably, RINs and RIDs were interested in genetics during residency (>40%) and strongly believed (85.0%) that genetics training using both theoretical and practical methodologies should be incorporated into residency training. However, RIPs were less interested (20%), and only 60% believed that genetics training should be incorporated. Spanish mental health residents, although interested in genetics in psychiatry, receive little training on this topic. They strongly believe that genetics training using theoretical and practical methodologies should be incorporated.

In recent years, the toxicity of graphene-related materials (GRMs) has been evaluated in diverse models to guarantee their safety. In most applications, sublethal doses of GRMs contact human barriers such as skin in a subchronic way. Herein, the subchronic effect (30 day exposure) of three GRMs (GO 1, GO 2, and FLG) with different oxidation degrees and sizes was studied. The effects of these materials on human skin cells, HaCaTs, were assayed through high-throughput metabolic-based readout and other cell-based assays. A differential effect was found between the different GRMs. GO 2 induced a metabolic remodeling in epithelial cells, increasing the level of tricarboxylic acid components, mirrored by increased cell proliferation and changes in cell phenotype. The oxidation degree, size, and method of manufacture of GRMs dictated harmful effects on cell metabolism and behavior generated by nontoxic exposures. Therefore, a "safe by design" procedure is necessary when working with these nanomaterials.

BACKGROUND AND OBJECTIVE: Antifibrotic drugs are the standard treatments for patients with idiopathic pulmonary fibrosis (IPF). This study aims to assess the safety of antifibrotic treatment in IPF patients undergoing lung transplantation. METHODS: Patients with a diagnosis of IPF who received a lung transplant between January 2015 and June 2019 at four Spanish hospitals specialized in lung transplantation were retrospectively recruited. Cases were defined as patients receiving antifibrotic treatments at time of transplant. Each case was matched with a control who did not receive antifibrotic treatment. RESULTS: A total of 164 patients were included in the study cohort (103 cases and 61 controls). There were no statistically significant differences between the cases and controls in any of the items studied related to transplantation except the time until the appearance of chest wall dehiscence: although there were no differences in the incidence of wall dehiscence in either group (12.3% vs. 13.7%; p = 0.318), the patients on antifibrotic drugs experienced it earlier (21 days [IQR = 12.5-41.5] vs. 63 days [IQR = 46.75-152.25]; p = 0.012). There were no differences in overall post-transplant survival between the two groups (p = 0.698) or in conditional survival at 30 days, 90 days, 3 years or 5 years. However, 1 year survival was significantly greater among controls (80.6% vs. 93.3%; p = 0.028). CONCLUSION: There was evidence that chest wall dehiscences appeared earlier post-transplant in patients using antifibrotics, even though this factor did not significantly impact survival.

BACKGROUND: Traditional clinical trials are conducted at investigator sites. Participants must visit healthcare facilities several times for the trial procedures. Decentralized clinical trials offer an interesting alternative. They use telemedicine and other technological solutions (apps, monitoring devices or web platforms) to decrease the number of visits to study sites, minimise the impact on daily routine, and decrease geographical barriers for participants. Not much information is available on the use of decentralization in randomized clinical trials with vaccines. METHODS: A hybrid clinical trial may be assisted by parental recording of symptoms using electronic log diaries in combination with home collected nasal swabs. During two influenza seasons, children aged 12 to 35 months with a history of recurrent acute respiratory infections were recruited in 12 primary health centers of the Valencia Region in Spain. Parents completed a symptom diary through an ad hoc mobile app that subsequently assessed whether it was an acute respiratory infection and requested collection of a nasal swab. Feasibility was measured using the percentage of returned electronic diaries and the validity of nasal swabs collected during the influenza season. Respiratory viruses were detected by real-time PCR. RESULTS: Ninety-nine toddlers were enrolled. Parents completed 10,476 electronic diaries out of the 10,804 requested (97%). The mobile app detected 188 potential acute respiratory infections (ARIs) and requested a nasal swab. In 173 (92%) ARI episodes a swab was taken. 165 (95.4%) of these swabs were collected at home and 144 (87.3%) of them were considered valid for laboratory testing. Overall, 152 (81%) of the ARIs detected in the study had its corresponding valid sample collected. CONCLUSIONS: Hybrid procedures used in this clinical trial with the influenza vaccine in toddlers were considered adequate, as we diagnosed most of the ARI cases on time, and had a valid swab in 81% of the cases. Hybrid clinical trials improve participant adherence to the study procedures and could improve recruitment and quality of life of the participants and the research team by decreasing the number of visits to the investigator site. This report emphasises that the conduct of hybrid CTs is a valid alternative to traditional CTs with vaccines. This hybrid CT achieved high adherence of participant to the study procedures. BACKGROUND: Hypoglycemia is common in very low birth weight neonates and may have adverse effects. Material and Method: Sixty preterm infants were monitored using continuous glucose monitoring (CGMS) and capillary techniques during the first week of life. Hypoglycemia was defined as glucose ≤47 mg/dL (≤2.6 mmol/L). Results: Hypoglycemic episodes were detected in 41.66% (95% CI: 29.07-55.12). In 69.64% the duration was greater than thirty minutes, in 26.78% (95% CI: 15.83-40.3) hypoglycemia exceeded two hours. Hypoglycemia was observed most frequently during the first 48 hours. In 35.7%, hypoglycemia was not detected with capillary tests. The agreement between the two techniques was good (r = 0.77, p < 0.001), Hypoglycemia was associated with a lower birth weight (OR: 0.99, p = 0.06). Conclusions: Hypoglycemia is frequent with significant duration in very low birth weight neonates. CGMS could be considered for use in these neonates to improve their glycemic control and prevent the associated morbidity.

BACKGROUND: During the last two decades, over 100 proteomics studies have identified a variety of potential biomarkers in CSF of Alzheimer's (AD) patients. Although several reviews have proposed specific biomarkers, to date, the statistical relevance of these proteins has not been investigated and no peptidomic analyses have been generated on the basis of specific up- or down- regulation. Herein, we perform an analysis of all unbiased explorative proteomics studies of CSF biomarkers in AD to critically evaluate whether proteins and peptides identified in each study are consistent in distribution; direction change; and significance, which would strengthen their potential use in studies of AD pathology and progression. METHODS: We generated a database containing all CSF proteins whose levels are known to be significantly altered in human AD from 47 independent, validated, proteomics studies. Using this database, which contains 2022 AD and 2562 control human samples, we examined whether each protein is consistently present on the basis of reliable statistical studies; and if so, whether it is over- or under-represented in AD. Additionally, we performed a direct analysis of available mass spectrometric data of these proteins to generate an AD CSF peptide database with 3221 peptides for further analysis. RESULTS: Of the 162 proteins that were identified in 2 or more studies, we investigated their enrichment or depletion in AD CSF. This allowed us to identify 23 proteins which were increased and 50 proteins which were decreased in AD, some of which have never been revealed as consistent AD biomarkers (i.e. SPRC or MUC18). Regarding the analysis of the tryptic peptide database, we identified 87 peptides corresponding to 13 proteins as the most highly consistently altered peptides in AD. Analysis of tryptic peptide fingerprinting revealed specific peptides encoded by CH3L1, VGF, SCG2, PCSK1N, FBLN3 and APOC2 with the highest probability of detection in AD. CONCLUSIONS: Our study reveals a panel of 27 proteins and 21 peptides highly altered in AD with consistent statistical significance; this panel constitutes a potent tool for the classification and diagnosis of AD.

PURPOSE: In recent decades, the life expectancy of HIV-infected patients has increased considerably, to the extent that the disease can now be considered chronic. In this context of progressive aging, HIV-infected persons have a greater prevalence of comorbid conditions. Consequently, they usually take more non-antiretroviral drugs, and their drug therapy are more complex. This supposes a greater risk of drug interactions, of hospitalization, falls, and death. In the last years, deprescribing has gained attention as a means to rationalize medication use. METHODS: Review of the different therapeutic approach that includes optimization of polypharmacy and control and reduction of potentially inappropriate prescription. RESULTS: There are several protocols for systematizing the deprescribing process. The most widely used tool is the Medication Regimen Complexity Index, an index validated in HIV-infected persons. Anticholinergic medications are the agents that have been most associated with major adverse effects so, various scales have been employed to measure it. Other tools should be employed to detect and prevent the use of potentially inappropriate drugs. Prioritization of candidates should be based, among others, on drugs that should always be avoided and drugs with no justified indication. CONCLUSIONS: The deprescribing process shared by professionals and patients definitively would improve management of treatment in this population. Because polypharmacy in HIV-infected patients show that a considerable percentage of patients could be candidates for deprescribing, we must understand the importance of deprescribing and that HIV-infected persons should be a priority group. This process would be highly feasible and effective in HIV-infected persons.

Lower-limb dynamics of muscle oxygen saturation during the back-squat exercise: effects of training load and effort level. J Strength Cond Res 34(5): 1227-1236, 2020-The aim of this study was to analyze the effect of strength training on lower limb muscle oxygenation. The sample consisted of 12 male subjects (22.4 ± 1.73 years; 1.81 ± 0.08 cm height and 77.76 ± 8.77 kg body mass). Six different strength training stimuli were analyzed, based on the training variables: load (60-75% 1 repetition maximum [1RM]) and level of effort (LE) (E1: 4 × 8 [20RM], E2: 4 × 12 [20RM], E3: 4 × 16 [20RM], E4: 4 × 4 [10RM], E5: 4 × 6 [10RM], and E6: 4 × 8 [10RM]) in the squat exercise up to 90° with a 2-second stop between repetitions to avoid the myotatic reflex. Oxygen saturation at the beginning of the series (SmO2start), oxygen saturation at the end of the series (SmO2stop), percentage of oxygen saturation loss (▽%SmO2), and reoxygenation time (SmO2recT) were assessed using a near-infrared spectroscopy device. In addition, the percentage of mean propulsive velocity loss (%MPVL) was recorded using a linear transducer. The results suggested an influence of LE and training load on muscle oxygenation. A greater LE was directly associated with SmO2recT (r = 0.864), ▽%SmO2 (r = 0.873), and %MPVL (r = 0.883) and inversely with SmO2stop (r = -0.871). When the same LE was used (E1 vs. E4, E2 vs. E5, and E3 vs. E6), it was found that the stimuli with a higher load had a lower SmO2recT, ▽%SmO2, and %MPVL and a higher SmO2stop. Muscle oxygen saturation was found to be minimal (%SmO2 = 0) in stimuli with a LE greater than 60% (E3 and E6). The SmO2 variables studied in the present research could be considered as an easier and more useful method for understanding skeletal muscle fatigue during resistance training.

OBJECTIVES: (1) To describe the fast Fourier transform (FFT) multijoint as monopodal postural stability measurement in well-trained athletes, (2) to compare the within-subject FFT between laterality, joints, and body segments, and (3) to establish the within- and between-subject relationship between joints. METHODS: Twelve national-level basketball players participated voluntarily in this investigation. The participants performed two 60-second repetitions of a monopodal stability test (1 repetition with each lower limb), separated by 3 minutes of active recovery. All tests were recorded by 4 WIMU PRO™ inertial devices located on the ankle, knee, lumbar spine, and thoracic spine. The main variable was total acceleration, where the FFT was applied. RESULTS: The higher instability results were found in the ankle and in the nondominant lower limb (dominant = 1.131 [0.122] a.u. (arbitrary units); nondominant = 1.141 [0.172] a.u). In the body segment analysis, the greater percentage of differences (%diff) were shown between lumbar spine and knee in the dominant (%diff = -2.989%; d = 0.87) and nondominant (%diff = -3.243%; d = 0.90) lower limb. Finally, very large between-subjects variability was found in all joints and body segments. CONCLUSIONS: The described protocol is proposed for monopodal postural stability assessment, being useful to provide information about the stability of joints and the body segment between joints. Besides, a within-subject analysis is recommended, and the FFT calculation will enable a linear analysis of each test.

Structural and functional abnormalities in the cerebral microvasculature have been observed in Alzheimer's disease (AD) patients and animal models. One cause of hypoperfusion is the thickening of the cerebrovascular basement membrane (CVBM) due to increased collagen-IV deposition around capillaries. This study investigated whether these and other alterations in the cerebrovascular system associated with AD can be prevented by long-term dietary supplementation with the antioxidant ubiquinol (Ub) stabilized with Kaneka QH P30 powder containing ascorbic acid (ASC) in a mouse model of advanced AD (3 × Tg-AD mice, 12 months old). Animals were treated from prodromal stages of disease (3 months of age) with standard chow without or with Ub + ASC or ASC-containing vehicle and compared to wild-type (WT) mice. The number of ß-amyloid (Aß) plaques in the hippocampus and entorhinal cortex was higher in female than in male 3 × Tg-AD mice. Extensive regions of hypoxia were characterized by a higher plaque burden in females only. This was abolished by Ub + ASC and, to a lesser extent, by ASC treatment. Irrespective of Aß burden, increased collagen-IV deposition in the CVBM was observed in both male and female 3 × Tg-AD mice relative to WT animals; this was also abrogated in Ub + ASC- and ASC-treated mice. The chronic inflammation in the hippocampus and oxidative stress in peripheral leukocytes of 3 × Tg-AD mice were likewise reversed by antioxidant treatment. These results provide strong evidence that long-term antioxidant treatment can mitigate plasma oxidative stress, amyloid burden, and hypoxia in the AD brain parenchyma.

High values of ambient temperature and relative humidity negatively affect livestock because they induce heat stress. In heat-stressed ewes, the reproduction is affected, although there apparently is a different effect depending on their physiological status. Therefore, we conducted a systematic review and meta-analyses to estimate the effect of heat stress on the reproductive performance and physiology of cycling or pregnant ewes. We included studies in which pregnant or cycling ewes were exposed to heat stress and included a comparison group. We assessed the risk of bias and used meta-analyses that included the period of exposure to calculate pooled estimates. We included 36 studies comprising 20,626 ewes, and the meta-analyses demonstrated that heat stress decreased 7.09 h (95% CI, - 10.75, - 3.43) the duration of estrus in cycling ewes but increased 0.57 days (95% CI, 0.23, 0.92) the length of the cycle, although differentially affecting the expression of sexual behavior. Heat-stressed cycling ewes had greater odds of embryo mortality (OR = 12.36; 95% CI, 7.10, 21.54) and a decreased chance of impregnation (OR = 0.41; 95% CI, 0.23, 0.75). In heat-stressed pregnant ewes, placental and fetal weights were reduced, respectively in 183.62 g (95%CI, - 252.25, - 114.99) and 1665.18 g (95% CI, - 2136.82, - 1193.55). Heat stress increased ewes' body temperature 1.13 °C (95% CI, 0.85, 1.42) and respiratory rate 93.53 breaths/min (95% CI, 63.5, 123.5) without affecting heart rate. In a sub-group analyses, the period of exposure was shown to cause a significant effect. In conclusion, our results revealed a differential effect of heat stress that depends on both the physiological status of ewes and the period of exposure, and identify susceptible reproductive stages during which the severe effects of heat stress can be prevented/ameliorated.

PURPOSE: The superior right colic vein (SRCV) has been proposed as the main cause of superior mesenteric vein bleeding by avulsion during laparoscopic right hemicolectomy. Our objective is to identify the main vessel causing transverse mesocolic tension during the extraction of the surgical specimen or extracorporeal anastomosis and to perform an anatomical description of the SRCV. METHODS: In this cadaveric study, we performed a simulation of right hemicolectomy and anatomical description of the surgical area of the gastrocolic trunk of Henle (SAGCTH), the gastrocolic trunk of Henle (GCTH), and SRCV. The length of the exteriorization of the anastomotic transverse colon (ATC) was measured before and after sectioning the vascular vessel causing the exteriorization tension. RESULTS: Five fresh cadavers and 12 formalin were dissected. In 100% of the specimens, the SRCV was present and drained in 95% into the GCTH and in 5% directly into the superior mesenteric vein (SMV). In 100% of the specimens, the SRCV caused the tension when extracting the ATC. The mean length of exteriorization of the ATC before and after SRCV section was 7.2 and 10.4 cm in formalin cadavers, meaning a 44% of increment in the length of exteriorization. In fresh cadavers, the mean length of exteriorization increased to 2.7 cm, meaning a 28% of the initial length of exteriorization. CONCLUSIONS: The SRCV is the main cause of tension in the extraction of the surgical specimen after right hemicolectomy. Its high tie increases the length of the ATC exteriorization, in about 3 cm, and could reduce the risk of SMV bleeding during laparoscopic right hemicolectomy and facilitate an extracorporeal anastomosis free of tension.

UNLABELLED: Aging is directly related with loss of physical independency. Composite Physical Function questionnaire (CPF) assess, throw 12 items, a range of daily life activities in order to determine dependency levels in elderly. However, there is not a Spanish version of this instrument. AIM: To translate and culturally adapt the CPF to Spanish for its use in Chilean elderly. METHOD: Standardized international methodology was used in this study, which consisted in double direct translation to Spanish, harmonization of versions and back-translation to English. Acceptability and familiarity of the obtained version was analyzed using probing and paraphrasing methods using a sample of 20 older adults aged from 65 to 80 years old. RESULTS: All items were clear and understandable, although minor adaptations needed to be done in order to improve the understandability of two items. These adaptations consisted in adding information in brackets at the end of the sentence. CONCLUSION: Spanish version of the CPF questionnaire was obtained to its use in Chile. This questionnaire has been proved to be understandable and adapted to its use in Chilean older adults. Its ease of use makes this questionnaire potentially useful in future researches and surveys.

The unprecedented COVID-19 health crisis severely disrupted global sports in 2020, prompting lengthy suspensions followed by resumed competitions under abnormal behind-closed-doors conditions without fans. These disruptions necessitated tactical adaptations by coaches and teams, attempting to still achieve successful outcomes. This study investigates the pandemic's impacts on performance metrics and indicators within Spanish professional soccer. Utilizing systematic notational analysis, 760 match cases from the 2019-2020 La Liga season were examined, comprising 27 matchdays from the pre-COVID context and 11 after resumption. Multivariate tests identified significant pre/post differences and interactions for various technical indicators including shots, cards, corners, and offside calls. The pandemic was associated with a reduction from 12 to just 5 identifiable playing styles, suggestive of increased conservatism featuring more passive play, limited attacking depth, and horizontal ball movement. Such tactical changes appear provoked by condensed fixture scheduling post-lockdown, the lack of supportive crowds, and compromised player fitness/recovery. By quantifying these COVID-precipitated changes, the analysis provides tangible evidence for coaches to make informed adjustments in training and preparation for functioning effectively in disrupted environments. The findings emphasize that versatility and flexibility will be vital to optimize performance during times of unprecedented uncertainty.

Mitochondrial dynamics and trafficking are essential to provide the energy required for neurotransmission and neural activity. We investigated how G protein-coupled receptors (GPCRs) and G proteins control mitochondrial dynamics and trafficking. The activation of Gαq inhibited mitochondrial trafficking in neurons through a mechanism that was independent of the canonical downstream PLCß pathway. Mitoproteome analysis revealed that Gαq interacted with the Eutherian-specific mitochondrial protein armadillo repeat-containing X-linked protein 3 (Alex3) and the Miro1/Trak2 complex, which acts as an adaptor for motor proteins involved in mitochondrial trafficking along dendrites and axons. By generating a CNS-specific Alex3 knockout mouse line, we demonstrated that Alex3 was required for the effects of Gαq on mitochondrial trafficking and dendritic growth in neurons. Alex3-deficient mice had altered amounts of ER stress response proteins, increased neuronal death, motor neuron loss, and severe motor deficits. These data revealed a mammalian-specific Alex3/Gαq mitochondrial complex, which enables control of mitochondrial trafficking and neuronal death by GPCRs.

RESUMEN

HLA-A, -B, -DQB1, and -DRB1 typing has been performed in a sample of Georgian population (South Caucasus). Allele frequencies, neighbour joining and correspondence relatedness analyses and extended HLA haplotypes have been obtained with comparison with other Middle East and Mediterranean populations. Our Georgian sample tends to be genetically related in these analyses with Eastern Mediterraneans and Middle East people. This is important for future regional transplant programs, and Georgian HLA and disease epidemiology and pharmacogenomics.

Amerindian Mapuche (Araucanians) are now living in Chile and Argentina at both sides of Andean Mountains. They are anthropologically and genetically different from southernmost South America Patagonian Amerindians. Most of the HLA alleles found in our Mapuche sample are frequent or very frequent in North and South America Amerindians: (1) Class I: A*02:01, A*03:01, A*68:01, B*39:09, B*51:01, (2) Class II: DRB1*03:01, DRB1*04:03, DRB1*07:01, DRB1*08:02, DRB1*14:02, DRB1*16:02. One of the nine most frequent extended haplotypes seems to be from European origin, suggesting the existence of a degree of admixture with Europeans in our Mapuche sample. It has been calculated of about 11 % admixture. Three of the extended haplotypes are also found in other Amerindians and five of them are newly found in Mapuche Amerindians: A*68:01-B*39:09-DRB1*08:02-DQB1*04:02; A*68:01-B*51:01-DRB1*04:03-DQB1*03:02; A*29:01-B*08:01-DRB1*03:01-DQB1*02:01; A*02:01-B*15:01-DRB1*04:03-DQB1*03:02; A*33:01-B*14:02-DRB1*07:01-DQB1*03:03. The medical importance of calculating HLA profile is discussed on the diagnostic (HLA and disease) and therapeutical bases of HLA pharmacogenomics and on the construction of a virtual transplantation HLA list profile. Also, anthropological conclusions are drawn.

Kazrin is a protein widely expressed in vertebrates whose depletion causes a myriad of developmental defects, in part derived from altered cell adhesion and migration, as well as failure to undergo epidermal to mesenchymal transition. However, the primary molecular role of kazrin, which might contribute to all these functions, has not been elucidated yet. We previously identified one of its isoforms, kazrin C, as a protein that potently inhibits clathrin-mediated endocytosis when overexpressed. We now generated kazrin knock-out mouse embryonic fibroblasts to investigate its endocytic function. We found that kazrin depletion delays juxtanuclear enrichment of internalized material, indicating a role in endocytic traffic from early to recycling endosomes. Consistently, we found that the C-terminal domain of kazrin C, predicted to be an intrinsically disordered region, directly interacts with several early endosome (EE) components, and that kazrin depletion impairs retrograde motility of these organelles. Further, we noticed that the N-terminus of kazrin C shares homology with dynein/dynactin adaptors and that it directly interacts with the dynactin complex and the dynein light intermediate chain 1. Altogether, the data indicate that one of the primary kazrin functions is to facilitate endocytic recycling by promoting dynein/dynactin-dependent transport of EEs or EE-derived transport intermediates to the recycling endosomes.
