RESUMEN

A 76-year-old woman presented to the medical oncology outpatient clinic with painful, burning, pruritic erythematous plaques involving both palms and axillae that had suddenly appeared five days before. Examination revealed no additional relevant findings and laboratory studies did not show any alteration. The patient had been recently diagnosed with a high-grade angiosarcoma of the breast (probably radiation induced) and after frequent local recurrences, was being treated with liposomal doxorubicin (three cycles were administered, the last of which was seven days before the appearance of the mentioned lesions). Oral corticosteroids were started, treatment with liposomal doxorubicin was stopped, and cutaneous biopsies performed that revealed features compatible with toxic erythema of chemotherapy induced by liposomal doxorubicin. Complete resolution of the cutaneous lesions was verified one month after. No signs of recurrence of angiosarcoma were documented at follow-up three months later.

RESUMEN

CONTEXT: Solid pseudopapillary tumor of the pancreas is a rare tumor more common in young girls and rare in males. CASE REPORT: We present a case of a solid pseudopapillary tumor of the pancreas in a 13-year-old boy, with typical imaging features. CONCLUSIONS: Our case report specifically illustrates the potential of diffusion-weighted imaging findings on solid pseudopapillary tumor in pediatric patients.

OBJECTIVE: Measure umbilical cord pulsatility time and evaluate correlation/association with maternal and neonatal characteristics. METHOD: Cross-sectional study, with 76 binomials, carried out in 2017, in a maternity hospital in Alagoas. Analysis with Pearson or Spearman correlation test and Mann-Whitney or Kruskal-Wallis test. RESULTS: Sixty-two parturients and their newborns participated of the research. The women had a gestational age ≥ 37 weeks, natural cephalic birth, without distortions. The newborns had average weight of 3326.29g, mostly male. The umbilical cord pulsatility time was 285.48s. There is a correlation between umbilical cord pulsatility time and placental delivery time (p<0.001). Other correlations/associations were not significant. CONCLUSION: It suggested using a correlation between pulsatility time and placental delivery time in clinical decision making for good practices in childbirth assistance.

BACKGROUND AND AIMS: Erythrina speciosa is a Neotropical tree that grows mainly in moist habitats. To characterize the physiological, morphological and growth responses to soil water saturation, young plants of E. speciosa were subjected experimentally to soil flooding. METHODS: Flooding was imposed from 2 to 4 cm above the soil surface in water-filled tanks for 60 d. Non-flooded (control) plants were well watered, but never flooded. The net CO(2) exchange (A(CO2)), stomatal conductance (g(s)) and intercellular CO(2) concentration (C(i)) were assessed for 60 d. Soluble sugar and free amino acid concentrations and the proportion of free amino acids were determined at 0, 7, 10, 21, 28 and 45 d of treatments. After 28, 45 and 60 d, dry masses of leaves, stems and roots were determined. Stem and root cross-sections were viewed using light microscopy. KEY RESULTS: The A(CO2) and g(s) were severely reduced by flooding treatment, but only for the first 10 d. The soluble sugars and free amino acids increased until the tenth day but decreased subsequently. The content of asparagine in the roots showed a drastic decrease while those of alanine and gamma-aminobutyric increased sharply throughout the first 10 d after flooding. From the 20th day on, the flooded plants reached A(CO2) and g(s) values similar to those observed for non-flooded plants. These events were coupled with the development of lenticels, adventitious roots and aerenchyma tissue of honeycomb type. Flooding reduced the growth rate and altered carbon allocation. The biomass allocated to the stem was higher and the root mass ratio was lower for flooded plants when compared with non-flooded plants. CONCLUSIONS: Erythrina speciosa showed 100 % survival until the 60th day of flooding and was able to recover its metabolism. The recovery during soil flooding seems to be associated with morphological alterations, such as development of hypertrophic lenticels, adventitious roots and aerenchyma tissue, and with the maintenance of neutral amino acids in roots under long-term exposure to root-zone O(2) deprivation.

Diabetes is a serious, chronic disease with a rising prevalence worldwide. Its complications are a major cause of morbidity and mortality and contribute substantially to health care costs. In this article the authors review the most common and sensitive skin manifestations that can be present on patients with diabetes and prediabetes. The prompt recognition of these frequently underestimated entities is extremely important as it may trigger not only an adequate metabolic evaluation but also a timely referral and appropriate treatment, minimizing the secondary effects of long-term diabetes and improving the prognosis of diabetic patients.


A diabetes mellitus é uma doença crónica, com uma prevalência crescente a nível mundial. As complicações da diabetes são uma causa major de morbilidade e mortalidade, condicionando custos importantes na área da saúde. Neste artigo é efetuada uma revisão das manifestações cutâneas mais frequentes presentes em doentes com diabetes e pré-diabetes. O reconhecimento atempado destas entidades é fulcral, levando não só a uma avaliação atempada do perfil metabólico como a uma referenciação e tratamento precoces. Desta forma, é possível minimizar os efeitos secundários da diabetes a longo prazo, melhorando significativamente o prognóstico dos doentes.

Cutaneous metastasis from a carcinoma is a relatively uncommon phenomenon. Prompt diagnosis is crucial, as it will have future implications, particularly regarding prognosis and treatment. Skin metastases can be suspected and recognized earlier through physical examination than metastases in other organs or systems. However, they can be a diagnostic challenge due to the variable clinical presentation. This case highlights the importance of having a high index of suspicion for cutaneous metastases, especially in patients with a previous history of cancer. LEARNING POINTS: Cutaneous metastases occur in approximately 0.7-10% of patients with invasive carcinomas and, when present, usually indicate an unfavourable outcome.Clinically, cutaneous metastasis can pose a diagnostic challenge and a high index of suspicion is mandatory for a prompt diagnosis.Skin biopsy of such lesions is important to confirm the diagnosis and, in the right clinical scenario, they can provide information on the primary origin of the tumour.

Langerhans cell histiocytosis (LCH) is a multisystemic disorder that results from the clonal proliferation of immunophenotypically and functionally immature Langerhans cells (LC). The detection of the V600E mutation in the BRAF oncogene in LCH biopsy specimens supports previous evidence that LCH is a neoplastic disorder. This mutation is present in other cutaneous lesions including malignant melanoma and benign nevi. Single case reports of a correlation between LCH and the appearance of eruptive nevi limited to the inguinal folds after chemotherapy have previously been described in the literature. This suggested that LCH could be an additional cause of eruptive melanocytic nevi, with a specific distribution mimicking that of LCH cutaneous lesions. We present the case of a 6-year-old boy, previously treated with chemotherapy for Langerhans cell histiocytosis, with disseminated junctional nevi. Although this co-occurrence may be coincidental, the skin involvement is distinct from other previously reported clinical cases. It would be interesting to evaluate whether the BRAF mutation described in LCH cells might in fact support a genetic background for the development of nevi in these patients. LEARNING POINTS: Langerhans cell histiocytosis (LCH) is a clonal neoplastic proliferation of immature Langerhans cells, with the V600E mutation in the BRAF oncogene present in approximately 60% of cases.The V600E mutation in the BRAF oncogene is also documented in other cutaneous lesions, namely malignant melanoma and benign nevi.There are case reports of a correlation between LCH and the appearance of eruptive nevi after chemotherapy, but it is not known whether the BRAF mutation described in LCH cells supports a genetic background for the development of nevi in these patients.

Extramammary Paget disease is a rare neoplastic condition that more commonly affects postmenopausal Caucasian women. Although the vulvar area is the most frequently affected location, it corresponds solely to 1 to 2% of all vulvar malignancies. A 72-year-old female patient was observed in our outpatient clinic with a 2-year history of an erythematous and pruritic plaque on the vulva. Histopathology and immunohistochemistry studies were compatible with extramammary Paget disease of the vulva. Associated neoplastic conditions were excluded. Due to multiple relapses, the patient was submitted to three surgical interventions, including a total vulvectomy, and to external radiotherapy. The present case illustrates the chronic and recurrent nature of extramammary Paget disease despite aggressive procedures as well as the challenge in obtaining tumor-free resection margins.

OBJECTIVE: to describe the health behaviors related to the sexual experiences of women in the female prison system from January to March, 2017. METHOD: descriptive study with a qualitative approach. A semi-structured questionnaire containing mixed questions and patient records was used. The data analysis was performed through the Bardin's content analysis and adopted as reference the Theory of Basic Human Needs. RESULTS: 18 women, young, single, brown, with a complete fundamental level participated in the study. After this characterization, two categories were evidenced: Health behaviors in sexual experiences and Health care in the face of sexual experiences. 33.3% reported on health behaviors such as Sexually Transmitted Infections (STIs), 27.7% associated with male condom use, 16.6% on prevention of unwanted pregnancies, 11.1% on health care and hygiene. FINAL CONSIDERATIONS: it was evidenced that the health behaviors reported by women in sexual experiences are associated with actions to prevent STIs, use of condoms, unwanted pregnancies, and health and hygiene care. However, it is possible to infer that they understand and possess superficial knowledge about health behaviors and that there is no adoption of regular practice in their sexual experiences.

OBJECTIVE: To find the most reliable screening method for Trypanosoma cruzi infection in blood banks. MATERIAL AND METHODS: Epidemiological data, lymphoproliferation assay, parasitological, conventional serological tests: immunofluorescence, haemagglutination, ELISA with epimastigote and trypomastigote antigens and reference serological tests: trypomastigote excreted-secreted antigens (TESA) blot and chemiluminescent ELISA assay with mucine from trypomastigote forms were applied to individuals with inconclusive serology, non-chagasic individuals and chronic chagasic patients. RESULTS: TESA blot had the best performance when used as a single test in all the groups. In the inconclusive group 20.5% of individuals were positive for TESA blot, 23.3% for either lymphoproliferation or TESA blot, and 17.8% for lymphoproliferation only. Positive lymphoproliferation without detectable antibodies was observed in 5.47% of all inconclusive serology cases. Analysis of six parameters (three serological assays, at least one parasitological test, one lymphoproliferation assay and epidemiological data) in the inconclusive group showed that diagnosis of Chagas' disease was probable in 15 patients who were positive by two or more serological tests or for whom three of those six parameters were positive. CONCLUSION: TESA blot is a good confirmatory test for Chagas' disease in the inconclusive group. Although lymphoproliferation suggests the diagnosis of Chagas' disease in the absence of antibodies when associated with a high epidemiological risk of acquiring Chagas' disease, the data from this study and the characteristics of the lymphoproliferation assay (which is both laborious and time-consuming) do not support its use as a confirmatory test in blood-bank screening. However, our findings underscore the need to develop alternative methods that are not based on antibody detection to improve the diagnosis when serological tests are inconclusive.

RESUMO Este artigo buscou discutir as relações entre psicopedagogia clínica e psicodrama, de modo a promover discussões que articulam os aspectos teóricos com uma prática. Surgiu, então, a seguinte questão norteadora: como a abordagem do psicodrama, enfocando a tríade escuta-afeto-aprendizagem, pode contribuir para o atendimento de estudantes com dificuldades de aprendizagem na psicopedagogia clínica? Com o objetivo geral de aprofundar a compreensão da interação entre a escuta ativa, o vínculo afetivo e o processo de aprendizagem, este estudo procurou responder a essa questão, destacando a importância da representação de papéis durante as sessões psicodramáticas. Dessa forma, este trabalho contribui para o avanço da psicopedagogia clínica, salientando a relevância do psicodrama como uma abordagem eficaz que visa não apenas à aprendizagem, mas também ao desenvolvimento emocional e à superação das dificuldades enfrentadas pelos aprendentes.


ABSTRACT This article aimed to discuss the relations between clinical psychopedagogy and psychodrama, intending to promote discussions that integrate theoretical aspects with practice. Thus, the following guiding question emerged: how can the psychodrama approach, focusing on the triad of listening, affect, and learning, contribute to the assistance of students with learning difficulties in clinical psychopedagogy? With the overall objective of deepening the understanding of the interaction between active listening, emotional bond, and the learning process, this study sought to answer this question, highlighting the importance of role-playing during psychodramatic sessions. Therefore, this work contributes to the advancement of clinical psychopedagogy, emphasizing the relevance of psychodrama as an effective approach that aims not only at learning, but also at emotional development and overcoming the difficulties faced by learners.


RESUMEN Este artículo busca discutir las relaciones entre Psicopedagogía Clínica y Psicodrama, con el objetivo de promover discusiones que articulen los aspectos teóricos con la práctica. Surge, entonces, la siguiente pregunta orientadora: ¿cómo puede el enfoque del Psicodrama, centrándose en la tríada escucha-afecto-aprendizaje, contribuir a la atención de estudiantes con dificultades de aprendizaje en Psicopedagogía Clínica? Con el objetivo general de profundizar la comprensión de la interacción entre la escucha activa, el vínculo afectivo y el proceso de aprendizaje, este estudio busca responder a esta pregunta, destacando la importancia de la representación de roles durante las sesiones psicodramáticas. De esta manera, este trabajo contribuye al avance de la Psicopedagogía Clínica, resaltando la relevancia del Psicodrama como un enfoque eficaz que busca no solo el aprendizaje, sino también el desarrollo emocional y la superación de las dificultades que enfrentan los aprendices.

Objetivo: Analisar o perfil dos profissionais atuantes na Atenção Primária em Saúde (APS) no cuidado a pacientes com depressão pós-parto (DPP) e as principais dificuldades na realização do diagnóstico precoce. Método: Por meio de uma pesquisa observacional, de caráter exploratório, abordagem quantitativa descritiva, com delineamento transversal, das informações colhidas através de questionário enviados por meio do aplicativo Google Forms, aplicada aos profissionais da APS, da região do Departamento Regional de Saúde de Araçatuba (DRSII). Resultado:Observa-se que 22,62% dos Agentes Comunitários de Saúde (ACS) permanecem na APS por 3 anos ou mais, enquanto 3,16% dos médicos, de 1 a 3 anos. Dos profissionais, 93,2% trabalham com protocolos de pré-natal, e destes, somente 33% são atualizados anualmente. Avaliando as consultas de pré-natal, 44,44% das gestantes com mais de 5 consultas retornam à Unidade Básica de Saúde (UBS) no puerpério. Dentre todas as categorias profissionais, 73,00% dos médicos se sentem aptos a realizarem orientações sobre a DPP. Cerca de 85% dos participantes da pesquisa identificaram os sintomas da doença e 18% o período do diagnóstico precoce corretamente. Atendimentos individuais e orientações às gestantes são realizados em 14,48% das práticas desenvolvidas pelos profissionais. Ademais, 91% dos profissionais nunca receberam capacitação a respeito do tema. Conclusão:Observou-se no estudo que há alta rotatividade dos profissionais da atenção primária de saúde, ausência do uso de protocolos de pré-natal atualizados e falta de conhecimento para a realização do diagnóstico precoce e preciso da DPP.


Objective: To analyze the profile of professionals working in Primary Health Care (PHC) in the care of patients with Postpartum Depression (PPD) and the main difficulties in carrying out an early diagnosis.Method:Through an observational, applied, exploratory research, descriptive quantitative approach, with a cross-sectional design, the information collected through a questionnaire sent through the Google Forms application, to PHC professionals, in the region of the Regional Department of Health of Araçatuba (DRSII), for the elaboration of effective strategies, aiming at the prevention, diagnosis and treatment of this pathology. Result: It is observed that 22.62% of Community Health Agents (ACS) remain in PHC for 3 years or more, while 3.16% of physicians, from 1 to 3 years. Of the professionals, 93.2% work with prenatal protocols, and of these, only 33% are updated annually. Evaluating prenatal consultations, 44.44% of pregnant women with more than 5 consultations return to the Basic Health Unit (UBS) in the puerperium. Among all professional categories, 73.00% of physicians feel able to provide guidance on PPD. About 85% of the survey participants correctly identified the symptoms of the disease and 18% the period of early diagnosis. Individual consultations and guidance for pregnant women are carried out in 14.48% of the practices developed by professionals. Furthermore, 91% of professionals have never received training on the subject. Conclusion: It was observed in the study that there is a high turnover of primary health care professionals, absence of the use of updated prenatal protocols and lack of knowledge to carry out an early and accurate diagnosis of PPD

The effects of immunodepression on several disease states have constituted an important area of research, leading to the identification of relevant associations between immunodepression and a vast set of comorbidities, including infectious diseases, cardiovascular diseases and cancer. Research on the effects of immunodepression has mostly been conducted in individuals under immunodepression by the human immunodeficiency virus and transplant recipients under pharmacological immunosuppression, due to the difficulties in obtaining relevant samples sizes in other contexts of immunodepression. Overall, immunosuppressed individuals tend to show increased incidence of malignancies, but only transplant recipients show significantly increased incidence of skin cancer; human immunodeficiency virus-infected individuals only show increased incidence of malignancies of infectious origin. This paper presents a literature review on the effects of immunodepression in the development of tumours in humans, with special emphasis on the development of the different types of skin cancer.


Os efeitos da imunodepressão no desenvolvimento de estados de doença têm constituído uma importante área de investigação, tendo sido verificadas associações entre a imunodepressão e o desenvolvimento de um vasto conjunto de comorbilidades, entre as quais se destacam as doenças infeciosas, doenças cardiovasculares e o cancro. A investigação dos efeitos da imunodepressão em humanos tem sido sobretudo realizada em indivíduos imunodeprimidos pelo vírus da imunodeficiência humana e indivíduos sob imunossupressão farmacológica após transplante, devido aos constrangimentos associados à obtenção de amostras de número relevante noutros contextos. De um modo geral, doentes imunodeprimidos tendem a apresentar maior incidência de neoplasias malignas. No que respeita aos doentes transplantados, o cancro cutâneo tem uma incidência significativamente aumentada, ao passo que, entre doentes portadores da infeção com vírus da imunodeficiência humana, apenas as neoplasias de origem infeciosa apresentam incidência aumentada. Este artigo apresenta uma revisão da literatura relativa aos efeitos da imunodepressão no desenvolvimento de tumores em humanos, com especial enfâse no desenvolvimento dos diferentes tipos de cancro cutâneo.

Objetivo: identificar quais os fatores de risco/condições clínicas frequentemente estão associados ao trabalho de parto prematuro em uma maternidade referência para alto risco. Método: trata-se de um estudo quantitativo, descritivo, documental. Compreenderam-se, na população estudada, as gestantes com diagnóstico de trabalho de parto prematuro internadas na maternidade de um hospital público no período de junho a setembro de 2018. Resultados: identificou-se que, das 40 gestantes, 21 (52,5%) tinham de 20 a 34 anos e, em relação à paridade, 24 (60%) eram multigestas; dos casos que apresentaram alguma condição clínica associada ao diagnóstico de trabalho de parto prematuro, a Infecção do Trato Urinário foi a intercorrência mais incidente, correspondendo a 27 (65%) dos casos. Conclusão: constata-se que os fatores de risco mais incidentes para o desenvolvimento do trabalho de parto prematuro foram: a idade materna, a paridade e intercorrências como a Infecção do Trato Urinário. Visualizou-se que cabe, aos profissionais envolvidos no atendimento do pré-natal, prover suporte adequado para as mães e familiares, baseado na solidariedade e acolhimento, aliando também os saberes técnicos e científicos de modo que se reduza a morbimortalidade.(AU)


Objective: to identify which risk factors/clinical conditions are often associated with premature delivery in a reference maternity for high-risk pregnancies. Method: quantitative, descriptive, and documentary study. The population studied comprised pregnant women diagnosed with premature delivery and admitted to a public maternity hospital from June to September 2018. Results: of the 40 pregnant women, 21 (52.5%) were aged between 20 and 34 years old and, concerning parity, 24 (60%) were multiparous; of the cases that presented some clinical condition associated with premature delivery, urinary tract infection was the most incident complication with 27 (65%) cases. Conclusion: the most incident risk factors for premature delivery were maternal age, parity, and complications such as urinary tract infection. It was seen that the professionals involved in prenatal care are charged with the responsibility of providing adequate support for mothers and family members, based on solidarity and welcome, also combining technical and scientific knowledge to reduce morbidity and mortality.(AU)


Objetivo: identificar qué factores de riesgo / condiciones clínicas se asocian frecuentemente con el parto prematuro en una maternidad de referencia de alto riesgo. Método: se trata de un estudio cuantitativo, descriptivo, documental. En la población estudiada se entendió que las gestantes diagnosticadas de parto prematuro ingresaron en la maternidad de un hospital público de junio a septiembre de 2018. Resultados: se identificó que, de las 40 gestantes, 21 (52,5% ) tenían entre 20 y 34 años y, en relación a la paridad, 24 (60%) eran multigrávidas; de los casos que presentaron alguna condición clínica asociada al diagnóstico de parto prematuro, la Infección del Tracto Urinario fue la complicación más incidentes, correspondiendo a 27 (65%) de los casos. Conclusión: parece que los factores de riesgo más incidentes para el desarrollo de parto prematuro fueron: edad materna, paridad y complicaciones como Infección del Tracto Urinario. Se vio que corresponde a los profesionales involucrados en la atención prenatal brindar un apoyo adecuado a las madres y familiares, basado en la solidaridad y la acogida, combinando también los conocimientos técnicos y científicos con el fin de reducir la morbilidad y la mortalidad.(AU)

Objetivo: analisar as complicações obstétricas de gestantes adolescentes por meio da Classificação de Robson. Método: trata-se de uma pesquisa quantitativa documental. Foram pesquisados 150 prontuários de gestantes adolescentes de alto risco. O estudo foi de setembro a novembro/2019 e iniciou após a aprovação do Comitê de ética em Pesquisa da Universidade Federal de Alagoas. Resultados: as taxas de cesáreas do grupo 1 foram o dobro do recomendado (18,92%), pela Classificação de Robson. No grupo 2, houve 100% de cesárea, enquanto recomendação é de 20 a 35%. No grupo 4 observou-se 100% de parto vaginal, enquanto os grupos 5, 8 e 10 excederam o número de cesáreas em cerca de 15,40 a 20%. Conclusão: evidenciou-se, por meio da Classificação de Robson, que o tipo de parto das gestantes adolescentes que foram afetadas por uma complicação clínico-obstétrica foi o parto cesáreo, com aumento nos grupos de gestante 1, 2, 5, 8 e 10.


Objective: to analyze obstetric complications in pregnant adolescents using the Robson Classification. Method: in this quantitative study, 150 medical records of high-risk adolescent pregnant women were searched between September and November 2019, after approval by the research ethics committee of Alagoas Federal University. Results: by the Robson Classification, cesarean section rates in group 1 were twice as high as recommended (18.92%). Group 2 returned 100% cesarean section, while the recommendation is 20 to 35%; group 4 showed 100% vaginal deliveries; and, in groups 5, 8 and 10, these exceeded the number of cesarean sections by about 15.40 to 20%. Conclusion: using the Robson Classification, it was shown that pregnant adolescents affected by a clinical obstetric complication were delivered by cesarean sections, which increased in groups 1, 2, 5, 8 and 10.


Objetivo: analizar las complicaciones obstétricas en adolescentes embarazadas mediante la Clasificación de Robson. Método: se trata de una investigación cuantitativa documental. Se investigaron 150 historias clínicas de adolescentes embarazadas de alto riesgo. El estudio fue de septiembre a noviembre/2019 y se inició después de la aprobación del Comité de Ética en Investigación de la Universidad Federal de Alagoas. Resultados: Las tasas de cesáreas en el grupo 1 fueron el doble de lo recomendado (18,92%) según la clasificación. En el grupo 2, hubo 100% de cesáreas, mientras que la recomendación es del 20 al 35%. En el grupo 4, el 100% tuvo parto vaginal, mientras que los grupos 5, 8 y 10 superaron el número de cesáreas en aproximadamente del 15,40 al 20%. Conclusión: queda claro, a través de la Clasificación de Robson, que el tipo de parto de las adolescentes embarazadas que se vio afectado por una complicación clínico-obstétrica fue el cesáreo, con incremento en los grupos de embarazadas 1, 2, 5, 8 y 10.
