OBJECTIVE: Stroke is the most common cause of epilepsy in older age. Subclinical cerebrovascular disease is believed to underlie some of the 30%-50% of late-onset epilepsy without a known cause (Li et al. Epilepsia. 1997;38:1216; Cleary et al. Lancet. 2004;363:1184). We studied the role of modifiable vascular risk factors in predicting subsequent epilepsy among participants ages 45 or older in the Framingham Heart Study (FHS), a longitudinal, community-based study. METHODS: Participants of the Offspring Cohort who attended FHS exam 5 (1991-1995) were included who were at least 45-years-old at that time, had available vascular risk factor data, and epilepsy follow-up (n = 2986, mean age 58, 48% male). Adjudication of epilepsy cases included review of medical charts to exclude seizure mimics and acute symptomatic seizures. The vascular risk factors studied included hypertension, diabetes mellitus, smoking, and hyperlipidemia. The role of the Framingham Stroke Risk Profile score was also investigated. Cox proportional hazards regression models were used for the analyses. RESULTS: Fifty-five incident epilepsy cases were identified during a mean of 19 years of follow-up. Hypertension was associated with a near 2-fold risk (hazard ratio [HR]: 1.93, 95% confidence interval [CI]: 1.10-3.37, p = .022) of developing epilepsy, even after adjustment for prevalent and interim stroke. In secondary analysis, excluding patients with normal blood pressure who were receiving anti-HTN (anti-hypertensive) treatment (n = 2613, 50 incident epilepsy cases) the association was (HR: 2.44, 95% CI: 1.36-4.35, p = .003). SIGNIFICANCE: Our results offer further evidence that hypertension, a potentially modifiable and highly prevalent vascular risk factor in the general population, increases 2- to 2.5-fold the risk of developing late-onset epilepsy.

BACKGROUND: Patients' social support has been shown to impact outcomes after bariatric surgery. We have previously shown that a Facebook group administered by bariatric providers offers an effective alternative social support mechanism to in-person support groups, with higher patient participation. Our aim was to determine whether participation in this Facebook group could improve patient outcomes after bariatric surgery. METHODS: After institutional board approval, our center's Facebook group members were electronically surveyed about their perceived value of group participation and their Facebook group usage frequency. We also collected patient age, sex, insurance, preoperative weight, type of procedure, hospital stay, postoperative complications, and weight loss from the electronic medical record. To assess the impact of Facebook group participation we compared patient outcomes between "frequent users" (those checking the Facebook group's activity at least once a week), "infrequent users", and a control group of all patients operated on during the year prior to the establishment of the Facebook group. The groups were compared after adjusting for potential confounding factors. RESULTS: 250 out of 1400 Facebook group patients responded to the survey (18%). 195 patients were frequent and 55 were infrequent users. Outcomes were compared with 211 control patients. The groups did not differ in their baseline characteristics apart from their sex. Frequent users had a higher weight loss compared to the other groups up to 2 years postoperatively but no difference in the overall complications. On multivariable analysis, frequency of Facebook use was the main factor associated with 0.5-, 1-, and 2-year weight loss. CONCLUSION: Frequent participation in a Facebook support group after bariatric surgery was associated with improved early weight loss outcomes. If additional longer-term studies confirm our findings, offering similar social support groups may become essential after bariatric surgery, especially during times of social isolation when in-person social support meetings may not be feasible.

BACKGROUND: Bone mineral density (BMD) is a potential surrogate marker of lifetime estrogen exposure previously linked to increased risk of Alzheimer dementia among elderly women. We examine the association between BMD in the "young old" with imaging biomarkers of brain aging and cognitive performance. METHODS: Offspring participants (N=1905, mean age 66) of a population-based cohort who had BMD, brain imaging and detailed cognitive assessment were included in the study. Sex-stratified, linear, and logistic regression models were used for analysis. RESULTS: Higher femoral neck BMD was associated with lower white matter hyperintensity burden and better performance on Trails B-A in both sexes, even after adjustment for cerebrovascular risk factors. Among women, the positive association with Trails B-A performance was seen only in APOE4 allele carriers. Higher BMD measurements were linked to better visual reproductions test performance in men. Finally, among women, higher femoral trochanter BMD was associated with better logical memory and Hooper visual organization test performance. CONCLUSION: Among the "young old," higher BMD is associated with less white matter hyperintensity burden and better, domain-specific, cognitive performance. This suggests that lifetime estrogen exposure may modulate the degree of cumulative vascular brain injury independent of cerebrovascular risk factors.

Human remains have been displayed in museums in Europe since many centuries for historical, cultural, and educational reasons. Of particular interest are skeletal remains and body parts that have suffered violent deaths and such remains often feature in Criminology Museums. Despite the well-acknowledged value of human remains in medical education, bioarchaeology, and research, the display of human remains still raises ethical considerations ranging in severity depending on the cultural substrate and legal framework of each country. Recent developments in medical imaging and visualisation are offering an alternative way. Taking into account the emerging issues regarding exhibition and handling of human remains, this research project aims to use virtual methods to reconstruct the circumstances of the death of a nineteenth-century outlaw comprising one of the human exhibits at the Criminology Museum of Athens in Greece.For the purpose of the project, the severed head of the outlaw Stavrou was CT scanned, and the data were used to reconstruct and to evaluate the ballistic trauma sustained at the time of his death. From the scans, it was possible to determine the minimum number of shots, entrance and exit wounds, approximate calibre of the bullets, approximate distance from which they were fired, and general velocity of the round.Shots are fired from the lateral left and backside of the head and bullets exited from the right frontal and temporal area causing extensive damage to the right craniofacial region. The direction of the shots coming from the back and left suggests that Stavrou was more likely ambushed by gendarmerie squads and not shot in a fair fight which would have caused entry wounds in the anterior surface of the body. This is in agreement with historical evidence that placed him in gunfire with his fellow gang member Karapanos against a government squad. On the other hand, the possibility of being captured alive and executed cannot be rejected based only on wound ballistics.This work would be excellent supplementary material to the actual human exhibit for the accurate presentation of Stavrou's history at the Criminology Museum. In addition, it would allow the virtual exhibition of the material for historical and teaching purposes to museums and universities anywhere in Greece and along the globe, thus overcoming the obstacles of moving the actual remains.

Zinc (Zn) is one of the most important essential nutrients of great public health significance. It is involved in numerous biological functions and it is considered as a multipurpose trace element, due to its capacity to bind to more than 300 enzymes and more than 2000 transcriptional factors. Its role in biochemical pathways and cellular functions, such as the response to oxidative stress, homeostasis, immune responses, DNA replication, DNA damage repair, cell cycle progression, apoptosis and aging is significant. Zn is required for the synthesis of protein and collagen, thus contributing to wound healing and a healthy skin. Metallothioneins are metal-binding proteins and they are potent scavengers of heavy metals, including Zn, and protect the organism against stress. Zn deficiency is observed almost in 17% of the global population and affects many organ systems, leading to dysfunction of both humoral and cell-mediated immunity, thus increasing the susceptibility to infection. This review gives a thorough insight into the most recent evidence on the association between Zn biochemistry and human pathologies, epigenetic processes, gut microbial composition, drug targets and nanomedicine.

Tetrahymena thermophila (T. thermophila) is a ciliated protozoon that can detect freshwater pollution by heavy metals ("whole-cell biosensor"). This work employed a systematic bioinformatics approach to predict and analyze the metalloproteome of T. thermophila for the essential Zn, Cu and the non-essential Cd. 3784 metal-binding proteins were identified compared to the 456 annotated so far in UniProt. The localization, functional classification, and the functionally enriched network of the newly identified metalloproteome are presented. Cd toxicity could be explained in terms of the metal replacing Cu and especially Zn in MAPKs, transporters and antioxidant enzymes. The predicted results for Cd toxicity and responses reflect those observed experimentally in different organisms after their exposure to Cd.

Respiratory distress syndrome (RDS) has a major contribution to neonatal mortality worldwide. Multiple factors associated with increased risk for RDS have been documented to effectively understand the emergence and progression of this disorder. A portion of these parameters has been broadly examined whereas the role of others, despite being clinically described, has not been fully evaluated. In this report, we analyze a forensic RDS case of a late preterm infant. Taking the maternal medical history into account, we focused on 2 not widely established risk factors, oligohydramnios and maternal age, discussing their possible pathophysiological relation to the development of RDS. Simultaneously, the fundamental role of the histopathological examination as a diagnostic tool resurfaces. Following a multidisciplinary approach derived from the collaboration of clinicians and researchers, the identification of factors that precipitate or contribute to this syndrome can be enhanced, leading to novel prognostic and therapeutic strategies against RDS.

INTRODUCTION: Soft tissue infections with Mycobacterium avium complex are more likely to appear in immunocompromised patients. However, they may rarely arise in non-immunosuppressed individuals. AIM: To present the case of an ear infection with Mycobacterium avium in the absence of demonstrable immunosuppression. CASE REPORT: Clinical case description, blood tests, routine histology and tissue cultures. DISCUSSION: A female, apparent immunocompetent patient presented with a 6-month reddish, oedematous and painless lesion with fine scaling in the right ear. Histology showed numerous granulomas, composed of epithelioid histiocytes without central necrosis. Cultures grew Mycobacterium avium. An unusual accidental ear injury was the portal of microbial entry. The patient's lesion fully regressed after a 9-month course of antibiotics. CONCLUSION: Although M. avium infections are rare in immunocompetent patients, the possibility of such infections should be considered even in these subjects, when relevant clinical features and exposure to risk factors are present.

Substituted phenethylamines are a class of designer drugs that have recently emerged in the drug abuse market. Such substances remain legal to use, possess, and supply until these compounds become classified as scheduled. 2C-I-NBOMe or 25I-NBOMe is the N-benzyl-derivative of the iodo-substituted dimethoxy-phenethylamine (2C-I) that appeared recently in the drug market under the street name "N-Bomb". Due to its high potency, intoxications and fatal cases related to 2C-I-NBOMe use are increased worldwide. The use and trafficking of this substituted phenethylamine is banned only in some countries. A comprehensive review was performed using PubMed and Medline databases, together with additional non-peer reviewed information sources, including books and publications of state authorities in different countries, regarding chemistry, availability, pharmacology, and toxicology of 2C-I-NBOMe. Intoxications or lethal cases, published or reported, as well as the current legislation on this newly introduced drug are also reviewed.

In the last decades, nano-silica (NS), nano-alumina (NA), and nano-calcium oxide (NC) particles have been incorporated into cementitious materials, and it seems that each one of them contributes uniquely to the materials' properties. This research explores the influence of each nanomaterial on the fresh properties of cement pastes and their compressive strength evolution over one year. Low proportions (1.5% by weight) of nanomaterials were added to cement pastes, and their fresh properties, such as heat of hydration and X-ray diffraction patterns in the first hours, were analyzed. The compressive strength and open porosity were also measured long-term. The acceleration of hydration heat in NA-cement pastes is linked to enhanced hydration product formation at early ages. Among the tested nanomaterials, NA increased compressive strength by 10% at later ages. Although the fresh properties of NC-cement pastes remained unaffected, their open porosity decreased by 54% at 28 days. In contrast, the increase in heat of hydration in NS-cement pastes did not result in significant strength improvement. Based on these findings, NA was selected for ultra-high-performance cement (UHPC)-based material use. Its incorporation not only preserved the ultra-high-performance (UHP) properties but also provided additional benefits such as an increase in compressive strength under a CO2 atmosphere. Through detailed analysis, this research establishes that nano-alumina incorporation optimizes the microstructural development and compressive strength of ultra-high-performance cement-based systems, presenting a novel advancement in enhancing the mechanical properties and durability of these materials under various environmental conditions.

Additive manufacturing (AM) technologies, including 3D mortar printing (3DMP), 3D concrete printing (3DCP), and Liquid Deposition Modeling (LDM), offer significant advantages in construction. They reduce project time, costs, and resource requirements while enabling free design possibilities and automating construction processes, thereby reducing workplace accidents. However, AM faces challenges in achieving superior mechanical performance compared to traditional methods due to poor interlayer bonding and material anisotropies. This study aims to enhance structural properties in AM constructions by embedding 3D-printed polymeric meshes in clay-based mortars. Clay-based materials are chosen for their environmental benefits. The study uses meshes with optimal geometry from the literature, printed with three widely used polymeric materials in 3D printing applications (PLA, ABS, and PETG). To reinforce the mechanical properties of the printed specimens, the meshes were strategically placed in the interlayer direction during the 3D printing process. The results show that the 3D-printed specimens with meshes have improved flexural strength, validating the successful integration of these reinforcements.

The study of carcinogenic potential of a variety of chemical agents such as food additives and drugs of abuse via the application of various in vitro methodologies constitutes the first step for the evaluation of their toxicogenomic profile. Considering the chromosomal theories of carcinogenesis, where it is stated that aneuploidy and chromosomal imbalance (instability) are among the main causes of carcinogenesis, chemicals capable to induce such changes in the cells could be considered as potential carcinogens. Chromosomal imbalance and aneuploidy directly affect the overall DNA content of the exposed cell as well as other cellular morpho- and densitometric features. These features can be measured by means of computerized DNA image analysis technologies and include DNA content (DNA Index), Proliferation Index, Ploidy Balance, Degree of Aneuploidy, Skewness and Kurtosis. Considering the enormous number of untested chemicals and drugs of abuse that follow non-genotoxic mechanisms of carcinogenesis, the establishment of a reliable technology for the estimation of chemically induced chromosomal imbalance is of particular importance in toxicogenomic studies. In the present article and based on our previously published work, we highlight the advantages of the applications of DNA image analysis technology in an easy-to-use experimental model for the evaluation of the potential risk of various chemicals. The use of this technology for the detection of chemically induced chromosomal instability will contribute to the development of safer regulatory directives concerning the use of chemicals in food and pharmaceutical industry, as well as in the clarification of mechanisms of action of drugs of abuse.

The growing need to preserve natural resources and minimize landfill waste has led to an increased consideration of incorporating waste materials in road construction and maintenance. This study focuses specifically on utilizing waste glass as part of the aggregates in hot asphalt, particularly in Asphalt Concrete (AC) for surface layers, known as "Glassphalt". Glass, due to its poor adhesion to bitumen, presents challenges when used in asphalt mixtures. Two types of waste glass, monolithic and tempered, were incorporated at two distinct contents, 10% and 15%, into the AC. Several properties such as stiffness, resistance to permanent deformation (evaluated through cyclic compression tests), indirect tensile strength, and the indirect tensile strength ratio (ITSR) were assessed for all Glassphalt mixtures, as well as the conventional mixture. Additionally, the Solar Reflectance Index (SRI) was measured to evaluate the reflectivity of the resulting Glassphalts. The findings indicate that the incorporation of both types of waste glass resulted in reduced stiffness and resistance to permanent deformation. Regarding water sensitivity (ITSR), the Glassphalts containing 15% waste glass, regardless of the glass type, exhibited ITSR values below the accepted threshold of 80%. The addition of waste glass did not yield significant changes in SRI measurements.

Zinc is a multipurpose trace element for the human body, as it plays a crucial part in various physiological processes, such as cell growth and development, metabolism, cognitive, reproductive, and immune system function. Its significance in human health is widely acknowledged, and this has led the scientific community towards more research that aims to uncover all of its beneficial properties, especially when compared to other essential metal ions. One notable area where zinc has shown beneficial effects is in the prevention and treatment of various diseases, including cancer. This review aims to explain the involvement of zinc in specific health conditions such as cancer, coronavirus disease 2019 (COVID-19) and neurological disorders like Alzheimer's disease, as well as its impact on the gut microbiome.

The importance of micronutrients in health and nutrition is undisputable, and among them, zinc is an essential element whose significance to health is increasingly appreciated and whose deficiency may play an important role in the appearance of diseases. Zinc is one of the most important trace elements in the organism, with three major biological roles, as catalyst, structural, and regulatory ion. Zinc-binding motifs are found in many proteins encoded by the human genome physiologically, and free zinc is mainly regulated at the single-cell level. Zinc has critical effect in homeostasis, in immune function, in oxidative stress, in apoptosis, and in aging, and significant disorders of great public health interest are associated with zinc deficiency. In many chronic diseases, including atherosclerosis, several malignancies, neurological disorders, autoimmune diseases, aging, age-related degenerative diseases, and Wilson's disease, the concurrent zinc deficiency may complicate the clinical features, affect adversely immunological status, increase oxidative stress, and lead to the generation of inflammatory cytokines. In these diseases, oxidative stress and chronic inflammation may play important causative roles. It is therefore important that status of zinc is assessed in any case and zinc deficiency is corrected, since the unique properties of zinc may have significant therapeutic benefits in these diseases. In the present paper, we review the zinc as a multipurpose trace element, its biological role in homeostasis, proliferation and apoptosis and its role in immunity and in chronic diseases, such as cancer, diabetes, depression, Wilson's disease, Alzheimer's disease, and other age-related diseases.

BACKGROUND: A large number of plants, seeds, and berries have been used for medicinal, psychotropic, or aphrodisiac purposes for a thousand years. Mandragora officinarum belongs to the family of Solanaceae and is traditionally known as an aphrodisiac and is closely associated with witchcraft. OBJECTIVES: In this study we report a case of an accidental poisoning after ingestion of some "aphrodisiac" berries and the contribution of the toxicological analysis in the case investigation. CASE REPORT: A 35-year-old man was admitted to the hospital with clinical signs and symptoms of an anticholinergic syndrome. The diagnosis of the poisoning was made by the toxicological analysis of the patient's urine. The cause of the poisoning was revealed by his girlfriend's disclosure that the patient had intentionally consumed some "aphrodisiac" berries to enhance his sexual performance. Subsequently, berries similar to the ones consumed were sent to the laboratory. The analysis of the urine and the berries revealed the presence of hyoscyamine and scopolamine; the berries were identified as Mandragora officinarum berries. Decontamination and symptomatic treatment were proven effective for the control of this poisoning. The patient recovered completely after hospitalization for 4 days. CONCLUSION: This case report indicates the importance of analytical toxicology in diagnosis of intoxications after the consumption of unknown plants or plant products and presents the clinical aspects of Mandragora intoxication.
