RESUMEN

The application of a model based on the three time dimensions of past, present and future can be used to generate an alternative taxonomy for the classification of depression, ill-being and quality of life. In relation to time, depression can be defined as the passage from a healthy past to a painful present, ill-being as the painful present of the individual and the quality of life (QOL) as the degree to which the subject's present life is commensurate with his aspirations. Such an approach opens up conceptual and methodological areas of research which will allow the construction of a new type of scale. Depression scales should be constructed with two time dimensions: past and present; ill-being scales only refer to the present and QOL scales should have two time dimensions: present and future. It would be possible to combine these three scales into a single scale with three dimensions. Such a scale would enable depression and ill-being to be quantified, accurately diagnosed and monitored. The relative importance of principal symptoms could be assessed, facilitating the choice of therapy and follow-up methods.

From the theory of Three-Time-dimensions based on the disturbed temporal horizon of the depressed individual: Past, Present, therapeutic-Future, new concepts of phenomenological-depression and the intrinsic-quality of life have been defined, in which both are considered as the perception of a discrepancy between, on one hand, the healthy Past and Present ill-being, and on the other, the intensity of demand for recovery or a therapeutic-Future. In order to determine the symmetry of these two concepts, the Three-Time-dimensions Synoptic Scale (3TSS) was presented to 110 subjects who had been diagnosed as suffering from major primary depression. The demonstration was carried out in three stages, the first statistical, the second mathematical, and finally, a geometrical one; it showed the importance of the temporal horizon of the depressed person in diagnosis.

The aim of this study is to evaluate the validity of the Dutch version of the Quality of Life in Depression Scale (QLDS) in a population of depressive patients aged 60 years and over, who were seen on an outpatient basis by psychiatrists in the Flemish region of Belgium. A sample of severely depressive patients was followed during an eight-week medical treatment. During this period, each patient was evaluated four times with the Hamilton Rating Scale for Depression and with the QLDS. We observed that the QLDS was very sensitive to the failing of the quality of life in depressive states. The QLDS was also a very good measure of change during the improvement of depressives states. Furthermore, the scale appears to be very homogeneous across the depressive states range. The results are very encouraging, and constitute strong corroboration of the construct validity of the scale.
