RESUMO

There is a growing body of scientific understanding surrounding the topic of functional chronic pain. Numerous studies have highlighted a persistent low-grade inflammation within the fascial and central nervous systems, coupled with detrimental neuroplasticity involving substantial remodeling of pain pathways in the spinal cord and brain. These findings provide a more concrete basis for explaining functional chronic pain to adolescents, simplifying the establishment of trust and the justification of proposed treatments in the process.


Il y a de plus en plus de compréhension scientifique autour du thème de la douleur chronique fonctionnelle. Plusieurs études mettent en évidence une inflammation de bas grade, persistante, au niveau des systèmes fascial et nerveux central, ainsi qu'une neuroplasticité délétère avec un remodelage majeur des voies douloureuses de la moëlle épinière et du cerveau. Tous ces éléments permettent une meilleure compréhension des mécanismes qui sous-tendent la douleur chronique fonctionnelle et donnent ainsi la possibilité de l'expliquer plus concrètement aux adolescents comme partie intégrante de la prise en charge. Créer le lien de confiance devient plus simple, de même que la justification des traitements proposés.

RESUMO

The period between the ages of 0 and 25 is a crucial phase in human development, during which many aspects of physical, psychological, personality, behavioral and health development take place. Important neurobiological changes during adolescence play a key role in their natural propensity to seek reward, putting them at greater risk of different kind of addictive behaviours. In addition, childhood adversity can foster the emergence of vulnerabilities which, for some, increases the risk of addiction by the time they reach adolescence. This article describes the normal development of the brain from childhood to adulthood, and its influence on addictive behaviours.


La période entre 0 et 25 ans est une phase cruciale du développement humain, au cours de laquelle de nombreux aspects physiques, psychologiques, de la personnalité, du comportement et de la santé se forment. D'importants changements neurobiologiques durant l'adolescence jouent un rôle essentiel dans la propension naturelle des jeunes à rechercher la récompense, ce qui les rend plus à risque de différents types de comportements addictifs. De plus, l'adversité rencontrée pendant l'enfance peut favoriser l'émergence d'une vulnérabilité augmentant, pour certains, le risque d'addictions lorsqu'ils atteignent l'adolescence. Cet article décrit le développement normal du cerveau de l'enfance à l'âge adulte ainsi que l'influence de ce dernier sur les comportements en lien avec les addictions.

Adolescence is a developmental phase that exposes young people to exploratory behaviors, including substance abuse. In Switzerland, recreational ketamine use among young people is on the rise and is likely to lead to irreversible somatic complications. Primary care physicians play an essential role in identifying ketamine users, and should offer brief interventions, risk-reduction advice and a psychosocial assessment of the situation, involving close relatives. In the event of secondary damage, particularly urological damage, coordinated management by specialists is essential to achieve symptom regression. Primary and secondary prevention measures are also essential in the fight against addiction among young people.


L'adolescence est une phase développementale exposant les jeunes à des comportements exploratoires, dont la prise de substances. En Suisse, la consommation récréative de kétamine chez les jeunes est en augmentation et est susceptible d'entraîner des complications somatiques irréversibles. Le médecin de premier recours joue un rôle essentiel dans l'identification des usager-e-s de kétamine et doit proposer des interventions brèves, des conseils de réduction des risques et une évaluation globale de la situation (anamnèse psychosociale), en impliquant les proches. En cas d'atteinte secondaire, en particulier urologique, une prise en charge coordonnée entre spécialistes est primordiale pour viser une régression des symptômes. Des mesures de prévention primaires et secondaires sont également essentielles dans la lutte contre les addictions chez les jeunes.

RESUMO

This article outlines the management of transgender and non-binary adolescents at CHUV. The current rise in visibility of this topic is associated with an increased demand for intervention and at the same time with an increased generation of continuously expanding medical evidence to guide interventions. The close collaboration among various specialized adolescent health services enables an interdisciplinary evaluation of diagnostic elements and indications for potential psychological, social or medical interventions. This article underscores the significance of an interdisciplinary and individualized approach to transgender and non-binary adolescents, respecting the diversity of the human being.


Cet article présente la prise en charge des adolescent-e-s transgenres et non binaires (TNB) au Centre hospitalier universitaire vaudois (CHUV). L'actuel gain en visibilité de la thématique conduit à une augmentation des demandes de prise en charge et, parallèlement, à une augmentation de la production d'évidence médicale pour orienter les prises en charge. La collaboration étroite entre les différents services spécialisés en santé des adolescents permet une évaluation interdisciplinaire des éléments diagnostiques et des indications aux éventuelles interventions psychologiques, sociales ou médicales. Cet article souligne l'importance d'une approche interdisciplinaire, basée sur l'évidence médicale et individualisée des adolescent-e-s TNB, dans le respect de la diversité de l'être humain.

The adolescent's process of brain maturation has important consequences for their ability to reason in decision-making processes. The notion of evolving capacity, for which age is not a sufficient indicator, implies a progressive vision of the nature and degree of participation according to the level of maturity. Health professionals are at the forefront of actively promoting young people's participation, supporting their decision-making about their own health, and seeing them as true partners and agents of change. The active participation of young people in all decisions that concern them is essential to make real progress in the field of adolescent health. This step implies a paradigm shift from "thinking for them" to "thinking with them".


Le processus de maturation cérébrale des adolescents a des conséquences importantes sur leur capacité de raisonnement dans les processus de décision. La notion de capacité évolutive, dont l'âge n'est pas un indicateur suffisant, implique une vision progressive de la nature et du degré de participation en fonction du niveau de maturité. Les professionnel-le-s de la santé sont aux premières loges pour favoriser activement la participation des jeunes, en soutenant leurs prises de décision concernant leur propre santé et en les considérant comme de véritables partenaires et acteurs de changement. La participation active des jeunes dans toutes les décisions qui les concernent est indispensable pour réellement progresser dans le domaine de la santé des adolescents. Cette étape implique un changement de paradigme de penser « avec eux ¼ plutôt que « pour eux ¼.

RESUMO

This article summarizes the conclusion of a two-days interprofessional online workshop held under the auspices of the Brocher Foundation. The objectives were to review the impact of COVID on adolescent health and development and to generate some key responses to the situation. The pandemic has severely affected the mental health of a large proportion of adolescents and has significantly reduced access to health care. It has as well disturbed the school education of vulnerable youngsters and decreased social contacts with adults and peers. In the future, authorities should better consider the rights of young people and request their opinion and participation in decision making. Physicians should systematically explore their young patients' opinions and queries regarding the COVID and address problematic situations such as family conflicts, misuse of internet or risky behaviour.


Cet article résume le contenu d'un colloque participatif interprofessionnel en ligne de 2 jours visant à faire un état des lieux et à élaborer des réponses à la situation des adolescent·e·s à l'ère du Covid. La pandémie affecte leur santé en réduisant l'accès aux soins (confinement) et en induisant des problèmes psychologiques chez certains d'entre eux. Elle affecte la formation et la scolarité des plus démunis, réduit les contacts sociaux avec les adultes et les pairs. À l'avenir, les autorités devraient mieux respecter les droits des jeunes en sollicitant leur avis et leur participation et en adaptant leurs messages. Les médecins doivent, lors des consultations avec eux, offrir systématiquement un dialogue et des informations, et repérer des problématiques telles que conflits familiaux, abus d'internet ou prise de risque sanitaire.

RESUMO

The objective of this article is to review how primary care providers - paediatricians, family practitioners, nurse practitioners - could address the use of substances by adolescents and respond to problematic behaviours and situations. Given the highly addictive properties of nicotine, adolescents should be supported in avoiding any consumption of tobacco or quitting if they already do so. Screening and investigation of substance use is usually not difficult, provided that it is processed in a neutral and empathetic atmosphere and securing confidentiality if the adolescent asks for it. Any type of substance use is potentially linked with adverse events such as injuries, violence, unplanned and unsafe sexual experience, alcoholic coma, 'bad trips' or unpleasant psychological experiences. As such, substance use should be identified and openly discussed with the adolescent. Moderate intermittent recreational use of legal and illegal substances (mainly alcohol and cannabis) should be carefully monitored over time, as it can lead to problematic use. Problematic use and substance use disorder require various interventions, including motivational interviewing, family counselling and psychotherapy, especially when substance use is linked with mental health disorders. Despite the importance of confidential care, parents or caregivers should be involved in problematic situations as much as possible. What is known: • While problematic use of substances is transient amongst many adolescents, it may be a threat to health and school/professional functioning • Primary care providers (PCPs) can address queries or offer preventive interventions to adolescents who use substances What is new: • Some interview tools can assist PCPs efficiently to identify problematic users of substances • There are effective approaches that PCPs can use to address problematic use by adolescent patients.

OBJECTIVE: Nutrition restoration in inpatients with anorexia nervosa (AN) is a core element in treatment, enabling recovery of cognitive functions essential for psychological care. This study aims to identify factors associated with inpatient weight gain. METHODS: Medical records from 107 inpatients aged 13-55 years with AN, hospitalized for more than 7 days at a specialized unit, were examined in a retrospective study. Weight evolution graphs were created for each patient and graded independently as optimal, moderate, and inadequate weight gain after 2 weeks and increasing, flat or decreasing weight in the first 2 weeks by expert clinicians. Driven by explicit hypotheses, bivariable analyses were carried out to detect relevant factors associated with weight gain during and after the first 2 weeks of admission. RESULTS: Initial weight gain in the first 2 weeks of hospitalization and the introduction of a protocol harmonizing treatment procedures around rapid refeeding were strong factors associated with optimal weight gain after 2 weeks of hospitalization, whereas prior hospitalization in a psychiatric unit, diagnosis with binge-eating/purging subtype and age over 18 years were significantly associated with inadequate weight gain (p < 0.001-0.05). CONCLUSION: To promote weight gain during hospitalization, clinicians should consider the following therapeutic measures: rapid refeeding strategies, renutrition protocols, and controlling purging behaviors. LEVEL OF EVIDENCE: Level III, retrospective cohort study.

Digital media are central in our modern society and, as such, have an impact not only on teenagers but on most of us. Adolescents are among the most observed and stigmatized in terms of screens use. The encounter between the crucial developmental phase they cross and the neurobiological changes in their brains can be relatively «â€…noisy ¼. Because of the pervasive nature of screens in our daily lives and the potential effect on adolescent health, it is essential for health professionals to offer a systematic assessment of media use when consulting with an adolescent. This article provides practical tools for screening the use of media in consultation with the -adolescent and illustrates with a clinical story what lies behind the screens.


Les écrans occupent une place centrale dans la société et ont un impact non seulement sur les adolescents mais sur la plupart d'entre nous. Les adolescents figurent toutefois parmi les plus observés et montrés du doigt en termes d'utilisation d'écrans. La résonnance entre les modifications neurobiologiques de leur cerveau durant cette période de vie et l'utilisation d'écrans peut se manifester de façon relativement «â€…bruyante ¼. Étant donné le caractère ­omniprésent de ces derniers dans notre vie quotidienne et du potentiel effet sur la santé des jeunes, il est indispensable pour les professionnels de la santé d'intégrer l'évaluation ­systématique de l'utilisation des écrans en consultation avec un adolescent. Cet article propose des outils pratiques pour en ­investiguer l'usage et illustre au moyen d'une histoire clinique ce qui se cache derrière les écrans.

Transgender, non-binary and questioning teenagers are increasingly visible. However, they face barriers in accessing appropriate care that meet their needs, both specific and regarding their general health. Primary care physicians increasingly see them in consultations but often lack elements of communication and recent knowledge that is needed to accompany them and their close ones in their -individual trajectories. This article aims to answer this need and provides a synthesis about recent evidence and suggested communication approaches for primary care physicians, who play a central role for the health of all patients.


Les adolescents transgenres, non binaires ou en questionnement sont de plus en plus visibles et font face à de nombreux obstacles pour accéder à des soins appropriés, tant pour leurs besoins de santé spécifiques que généraux. Les médecins de premier recours les rencontrent au cabinet et peuvent manquer d'éléments de communication et de connaissances spécifiques récentes pour mener la consultation et accompagner ces personnes et leurs proches dans leurs trajectoires individuelles. Cet article propose une synthèse des éléments et des ressources utiles pour le médecin de premier recours, dont le rôle est ­central pour la santé de tous les patients.

RED-S is a clinical concept introduced in 2014 that broadens the understanding of the athlete's triad. It includes nine areas affected by energy deficiency relative to the body's needs for optimal functioning and it is known that both adolescents and athletes are at higher risk of suffering from a RED-S than the general population. We highlight that sports coaches in French-speaking Switzerland do not know of RED-S nor of its health consequences. They desire to learn more but mention several barriers to this training. Since they have a central role in the lives and health of adolescent athletes, they are the key persons to train in order to improve the prevention and detection of adolescent athletes in need of medical attention.


Le RED-S (Relative Energy Deficiency in Sports) est un concept clinique qui élargit la compréhension de la triade de l'athlète. Il comprend neuf domaines affectés par un déficit énergétique et survient lorsque les apports sont insuffisants pour les besoins de fonctionnement optimal. Les adolescents et les athlètes sont à risque de souffrir d'un RED-S en raison de leurs besoins énergétiques augmentés. Notre étude met en évidence un besoin de formation des entraîneurs sportifs, car ils ne connaissent ni le RED-S ni ses conséquences sur la santé. Ils sont en revanche désireux de se former, mais mentionnent des barrières d'accès à la formation. Leur rôle central dans la vie et la santé des athlètes adolescents en fait les acteurs-clés pour améliorer la prévention et la détection du RED-S et l'orientation vers une prise en charge médicale.

Youth sports participation is often perceived as a positive contributor to physical and psychological development. However, we must acknowledge that sometimes the sports environment can be plagued by events of abuse and non-accidental violence of different types. A global and systemic approach will allow the limitation of these catastrophic events in young athletes' lives. Healthcare professionals must be able and knowledgeable to identify abuse correctly and in a timely fashion, to report it and to manage applying interdisciplinary principles. Their role is paramount in the education of all stakeholders, helping to raise awareness around abuse in adolescent athletes. Sporting organizations must apply existing recommendations from health and sporting authorities to guarantee the welfare of the adolescents they welcome in their world.


L'activité sportive contribue au bon développement physique et psychologique des adolescents. Nous devons néanmoins constater que les pratiques y sont parfois néfastes et recèlent des éléments de maltraitance ou de violence non accidentelle. Seule une approche globale et systémique pourra permettre de limiter la survenue de ces événements extrêmement délétères pour la santé des jeunes athlètes. Les professionnels de la santé doivent identifier la maltraitance, la signaler et la prendre en charge en équipe interdisciplinaire. L'éducation et la sensibilisation de tous les acteurs sont les mesures indispensables de départ, puis les organisations sportives doivent de leur côté appliquer les recommandations existantes pour garantir le bien-être des adolescent·e·s qu'ils accueillent.

The adolescent's painful functional disorders deserve management by a specialized interdisciplinary team. In recent years, intensive interdisciplinary physical therapy (IIPT) programs have developed in a hospital environment in Europe and around the world. While the short-term objective is often to restore trust between the patient and the therapists by relieving parents of the need to coordinate a network of multidisciplinary practitioners. These programs have proven to be effective in the medium and long term in improving the patient's clinical performance and for the benefits associated with restoring normal life for the family. Finally, these programs can have a positive impact in a cost-benefit approach when considering these often long and complex care arrangements.


Les troubles fonctionnels douloureux chroniques de l'adolescent méritent une prise en charge interdisciplinaire spécialisée. Depuis quelques années, des programmes de thérapie physique intensive interdisciplinaire (intensive interdisciplinary physical therapy, IIPT) se sont développés dans un environnement hospitalier en Europe et dans le monde. Si l'objectif à court terme est souvent de remettre les patients en mouvement, un autre objectif est de restaurer une confiance entre le patient et les professionnels de la santé en soulageant notamment les parents de la coordination d'un réseau de praticiens multidisciplinaires. Ces programmes ont montré leur efficacité à moyen et long termes sur l'amélioration clinique du patient et sur les bénéfices associés à la reprise d'une vie normale pour la famille. Enfin, ces programmes peuvent avoir un impact positif en termes de coût/bénéfice dans ces prises en charge souvent longues et complexes.

In many European countries, paediatric junior staff has no formal training in adolescent medicine and is ill-equipped to deal with issues and health problems such as substance use, unprotected sex, eating disorders and transition to adult care. This position paper of the European Academy of Paediatrics proposes a set of competency-based training goals and objectives as well as pedagogic approaches that are expected to improve the capacity of paediatricians to meet the needs of this important segment of the paediatric population. The content has been developed from available publications and training programmes and mostly covers the generic aspects of adolescent healthcare, such as how to communicate effectively, how to review and address lifestyles, how to perform a respectful and relevant physical examination, how to address common problems of adolescents and how to support adolescents in coping with a chronic condition. CONCLUSION: The European Academy of Paediatrics urges national bodies, paediatric associations and paediatric teaching departments to adopt these training objectives and put them into practice, so that paediatricians will be better prepared in the future to meet the challenge of delivering appropriate and effective healthcare to adolescents.

Offering an interdisciplinary approach to the athletic adolescent helps to expand the evaluation framework when investigating a medical complaint. It is important to use the opportunity of each consultation to take not only a detailed history of the sporting practice, but also to investigate the social and familial context as well as healthy lifestyle of the young athlete. Considering young athletes within the larger picture of their developmental context of adolescence offers an opportunity to better respect the psychological and physical needs of this life phase. These needs are strongly related to the promotion of a good health among young athletes.


L'approche interdisciplinaire de l'adolescent qui pratique du sport permet d'élargir le cadre d'évaluation lors de l'investigation d'une plainte médicale. Il est important de profiter de chaque opportunité de consultation pour faire non seulement une anamnèse détaillée de la pratique sportive, mais également du contexte social et familial ainsi que de l'hygiène de vie du jeune sportif. Replacer le jeune sportif dans le contexte développemental de l'adolescence offre une possibilité de mieux respecter les besoins fondamentaux psychiques et physiques liés à cette phase de vie, éléments qui sont fortement liés à la promotion d'une bonne santé.

A new paradigm in the management of adolescents with eating disorders promotes different treatment orientations aiming at increasing therapeutic effectiveness as well as offering a more integrated and coherent care for families. Particular attention is given to the process of refeeding to avoid the severe somatic sequelae of eating disorder and the risk of a chronic course for a disorder which is well known for its « addictive ¼ power and influence. The parents and the need for a strong alliance with the healthcare team stand at the centre of this new paradigm. This type of management requires professionals who are trained in these specific approaches and in multidisciplinary work.


Les paradigmes de prise en charge des troubles alimentaires chez l'adolescent ont changé et entraînent de ce fait des traitements différents, visant une meilleure efficacité thérapeutique, ainsi qu'une prise en charge plus intégrée et cohérente des familles. Une attention particulière est accordée à la renutrition, pour éviter tant les séquelles somatiques du trouble alimentaire que l'installation dans un trouble dont on connaît maintenant le pouvoir « addictif ¼ et l'emprise. Une alliance solide avec les parents ­ placés au centre de ce processus, soutenus par les équipes de soins ­ est nécessaire. La prise en charge par des équipes formées et travaillant en collaboration multidisciplinaire s'avère essentielle.

RESUMO

Functional disorder during adolescence is a very complex problem too often limited to a psychological origin. Its pathogenesis remains unclear but it definitively associates individual and environmental factors. Recent data show anatomo-functional evidence of neural networks of the brain that is involved in pain and its psychological representation as well as the representation of pain in the body. We describe a holistic approach to manage functional disorders that include the family and promote adolescent centered care in order to reach a rapid and optimal rehabilitation.


Le trouble fonctionnel de l'adolescent est une pathologie complexe trop souvent réduite à un abord exclusivement psychologique. Sa genèse est encore insuffisamment comprise, mais elle associe certainement des facteurs individuels et environnementaux. Les données récentes montrent qu'il existe un support anatomo-fonctionnel au sein des réseaux neuronaux cérébraux impliqués dans la douleur et sa représentation psychique et corporelle. A l'instar des réseaux neuronaux, il s'agit souvent aussi d'une difficulté de travailler en réseau interdisciplinaire. Nous proposons ici une approche holistique de ces troubles, incluant les familles et plaçant l'adolescent au centre d'une prise en charge qui vise une réhabilitation rapide.
