RESUMO

The synthesis and characterisation of two mononuclear aluminium alkyl complexes with the general composition [Al(Me)2{Ph2P(E)N(CH2)2N(CH2CH2)2O}] (E = Se (2a); S (2b)), and two binuclear aluminium complexes, [Al(Me)2{Ph2P-(E)N(CH2)2N(CH2CH2)2O}(AlMe3)] (E = Se (3a) and S (3b)), are described. The binuclear aluminium alkyl complex 3a proved to be a proficient catalyst for the addition of simple nucleophiles to heterocumulenes, leading to the synthesis of a variety of products such as urea, biuret, isourea, isothiourea, phosphorylguanidine, and quinazolinone derivatives, in contrast to its mononuclear analogues. Complex 3a is the first example of a single competent catalyst, which is also low-cost and eco-friendly and derived from a main-group metal, under solvent-free conditions either at room temperature or mild temperatures. Complex 3a possessed a wide functional group tolerance including heteroatoms, yielding the corresponding insertion products in good quantities and with high selectivity.

Hemophagocytic lymphohistiocytosis (HLH) is a rare, potentially fatal, and systemic hyperinflammatory syndrome with exacerbated and uncontrolled activation of histiocytes and lymphocytes against mature cells. Secondary HLH can occur in association with a myriad of underlying infections or malignancies. Our patient is a 38-year-old male prisoner with poorly controlled diabetes and no known other medical conditions. He was referred to our emergency department with three-week history of worsening malaise, weight loss, fever, bruising, and shortness of breath; imaging showed pneumomediastinum, lung nodule, and adrenal mass. Biopsy of the lung nodule revealed acid-fast bacilli. Furthermore, bone marrow biopsy showed foci of necrosis with associated acid-fast bacilli and hemophagocytosis highlighted by CD163 stain; consequently, secondary HLH was suggested. Hence, lab results were reviewed and found to satisfy five of the eight secondary HLH criteria. Moreover, ferritin was >10,000 ng/ml, which has been suggested to be highly suspicious for HLH. The patient was started on anti-MAC therapy. Unfortunately, the patient's status declined rapidly; he developed multi-organ failure and succumbed to disease. Later, his culture confirmed Mycobacterium tuberculosis. In conclusion, we presented a rare and challenging case of secondary HLH associated with disseminated Mycobacterium tuberculosis. A high index of suspicion is required for early diagnosis and treatment, and pathologists should be aware of Mycobacterium tuberculosis' association with secondary HLH.

We report a series of titanium and zirconium complexes supported by dianionic amidophosphine-borane ligands, synthesized by amine elimination and salt metathesis reactions. The TiIV complex [{Ph2P(BH3)N}2C6H4Ti(NMe2)2] (1) was obtained by the reaction between tetrakis-(dimethylamido)titanium(IV) and the protic aminophosphine-borane ligand [{Ph2P(BH3)NH}2C6H4] (LH2) at ambient temperature. Both the heteroleptic zirconium complexes-[η5-(C5H5)2Zr{Ph2P(BH3)N}2C6H4] (2) and [[{Ph2P(BH3)N}2C6H4]ZrCl2] (3)-and the homoleptic zirconium complex [[{Ph2P(BH3)N}2C6H4]2Zr] (4) were obtained in good yield by the salt metathesis reaction of either zirconocene dichloride [η5-(C5H5)2ZrCl2] or zirconium tetrachloride with the dilithium salt of the ligand [{Ph2P(BH3)NLi}2C6H4] (LLi2), which was prepared in situ. The molecular structures of the complexes 1, 2, and 4 in their solid states were confirmed by single-crystal X-ray diffraction analysis. Of these complexes, only titanium complex 1 acts as an effective catalyst for the facile hydroboration of terminal alkynes, yielding exclusive E-isomers. The hydroboration of organic nitriles yielded diborylamines with a broad substrate scope, including broad functional group compatibility. The mechanism of hydroboration occurs through the formation of titanium hydride as an active species.

RESUMO

Chimeric antigen receptor (CAR)-T-cell therapy has sparked a wave of optimism in hematological malignancies, reflected by the successful results of early clinical trials involving patients with pre-B-cell acute lymphoblastic leukemia, B-cell lymphomas and multiple myeloma. CAR-T-cell therapy is considered to be a novel immunotherapy treatment that has the potential for curing certain hematological cancers. However, as use of CAR-T-cell therapy has grown, new challenges have surfaced. These challenges include the process of manufacturing the CAR-T cells, the mechanisms of resistance that underlie disease relapse, adverse effects and cost. This review describes the published results of clinical trials and expected developments to overcome CAR-T resistance.
