RESUMO

PURPOSE OF REVIEW: To briefly review epidemiology and pathophysiology of SICM and provide a more extensive review of the data on diagnostic and management strategies. RECENT FINDINGS: SICM is likely underdiagnosed and that has mortality implications. Current evidence supports speckle tracking echocardiography to identify decreased contractility irrespective of left ventricular ejection fraction for the diagnosis of SICM. There continues to be a dearth of large clinical trials evaluating the treatment of SICM and current consensus focuses on supportive measures such as vasopressors and inotropes. Sepsis is a significant cause of mortality, and sepsis-induced cardiomyopathy has both prognostic and management implications for these patients. Individualized work-up and management of these patients is crucial to improving outcomes.

RESUMO

Angiotensin-converting enzyme (ACE) inhibitors are viewed by many as important treatment options for both the management of hypertension and the provision of end-organ protection. ACE inhibitors have not been looked on as having any clearly recognizable direct pulmonary benefits; rather, their pulmonary effects have been felt to be a nuisance by virtue of their association with cough. ACE inhibitor-related cough is well characterized. What is less well appreciated is the relationship between sleep apnea, pneumonia, and ACE inhibitor-related cough. These new pulmonary observations in patients afflicted with an ACE inhibitor-related cough should revive interest in this bothersome side effect.

RESUMO

Inspection bronchoscopy during percutaneous dilational tracheostomy has become the standard of care to reduce complications of the procedure. During bronchoscopy, anatomic defects can be visualized before performing the procedure. We describe a case of discovering herniated tracheal rings in preprocedure bronchoscopy and subsequent treatment of this rare finding.
