RESUMO

PURPOSE: Echocardiography is a difficult tool to master. Competency requires the supervised interpretation of hundreds of exams. Perceptual learning modules (PLMs) are novel learning tools that aim to speed up this learning process by enabling learners to go online and interpret numerous clinical images, followed systematically by expert feedback. We developed and tested a PLM aimed at improving novices' ability to quickly visually estimate left ventricular ejection fraction (LVEF) on transesophageal echocardiography images, a critical skill in acute care. We hypothesized that using the PLM would improve the accuracy and the speed of learners' estimations. METHODS: Learners without echocardiography experience were randomly assigned to a group that used the 96-case PLM (n = 26) or a control group (n = 26) that did not. Both groups took a pre-test and an immediate post-test that measured the accuracy of their visual estimations during a first session. At six months, participants also completed a delayed post-test. RESULTS: In the immediate post-test, the PLM group showed significantly better accuracy than the control group (median absolute estimation error 6.1 vs 9.0; difference 95% CI, 1.0 to 4.6; P < 0.001). Nevertheless, at six months, estimation errors were similar in both groups (median absolute estimation error 10.0 vs 10.0; difference 95% CI, -1.3 to 2.1; P = 0.27). CONCLUSIONS: Participation in a short online PLM significantly improved novices' short-term ability to accurately estimate LVEF visually, compared with controls. RéSUMé: OBJECTIF: L'échocardiographie est un outil difficile à maîtriser. Afin d'acquérir cette compétence, l'interprétation supervisée de centaines d'examens est nécessaire. Les modules d'apprentissage perceptuel (MAP) sont des outils d'apprentissage innovants qui visent à accélérer ce processus d'apprentissage en permettant aux apprenants d'aller en ligne et d'interpréter de nombreuses images cliniques, lesquelles sont systématiquement suivies par des rétroactions d'experts. Nous avons mis au point et testé un MAP visant à améliorer la capacité des nouveaux apprenants à rapidement estimer visuellement la fraction d'éjection ventriculaire gauche (FEVG) sur des images d'échocardiographie transœsophagienne, une compétence critique dans les soins aigus. Nous avons émis l'hypothèse que l'utilisation du MAP améliorerait la précision et la rapidité des estimations des apprenants. MéTHODE: Les apprenants sans expérience de lecture d'échocardiographie ont été aléatoirement alloués à un groupe qui a utilisé le MAP de 96 cas (n = 26) ou à un groupe témoin (n = 26) qui ne l'a pas utilisé. Les deux groupes ont passé un prétest et un post-test immédiat qui ont mesuré l'exactitude de leurs estimations visuelles au cours d'une première séance. Six mois plus tard, les participants ont également passé un autre post-test retardé. RéSULTATS: Dans le post-test immédiat, le groupe MAP a démontré une précision significativement meilleure que le groupe témoin (erreur d'estimation absolue médiane, 6,1 vs 9,0; différence de l'IC 95 %, 1,0 à 4,6; P < 0,001). Néanmoins, à six mois, les erreurs d'estimation étaient similaires dans les deux groupes (erreur d'estimation absolue médiane, 10,0 vs 10,0; différence de l'IC 95 %, -1,3 à 2,1; P = 0,27). CONCLUSION: La participation à un bref MAP en ligne a considérablement amélioré la capacité à court terme des nouveaux apprenants à estimer visuellement et avec précision la FEVG, par rapport à un groupe témoin. A series of boron-modified polyorganosilazanes was synthesized from a poly(vinylmethyl-co-methyl)silazane and controlled amounts of borane dimethyl sulfide. The role of the chemistry behind their synthesis has been studied in detail by using solid-state NMR spectroscopy, FTIR spectroscopy, and elemental analysis. The intimate relationship between the chemistry and the processability of these polymers is discussed. Polymers with low boron contents displayed appropriate requirements for facile processing in solution, such as impregnation of host carbon materials, which resulted in the design of mesoporous monoliths with a high specific surface area after pyrolysis. Polymers with high boron content are more appropriate for solid-state processing to design mechanically robust monolith-type macroporous and dense structures after pyrolysis. Boron acts as a crosslinking element, which offers the possibility to extend the processability of polyorganosilazanes and suppress the distillation of oligomeric fragments in the low-temperature region of their thermal decomposition (i.e., pyrolysis) at 1000 °C under nitrogen. Polymers with controlled and high ceramic yields were generated. We provide a comprehensive mechanistic study of the two-step thermal decomposition based on a combination of thermogravimetric experiments coupled with elemental analysis, solid-state NMR spectroscopy, and FTIR spectroscopy. Selected characterization tools allowed the investigation of specific properties of the monolith-type SiBCN materials.

BACKGROUND: Total ankle arthroplasty (TAA) is a preferred surgical option for end-stage ankle osteoarthritis; however, it is a demanding procedure with a higher historical rate of revision compared with ankle fusion. Patient-specific instrumentation (PSI) has been introduced to optimize prosthesis alignment and theoretically overall improve TAA outcomes. The goal of this study is to report on the experience and surgical outcomes of one implant with specific evaluation of the accuracy and reproducibility of the system with respect to prosthesis alignment and prediction of implant size. METHODS: A retrospective, multicentered study involving 4 foot and ankle fellowship-trained orthopaedic surgeon's patients undergoing TAA between January 1, 2015, and December 31, 2018, using the PROPHECY PSI system. RESULTS: 80 TAA procedures were performed. On average the postoperative tibial component alignment was 89.9 (range, 86.1-96.5) degrees in the coronal plane, with a mean sagittal alignment of 88.1 (range, 81.3-96.7) degrees. The mean deviation from neutral sagittal alignment improved from 4.9 ± 3.9 degrees preoperatively to 2.7 ± 1.7 degrees postoperatively, whereas the mean coronal alignment improved from 3.3 ± 2.5 degrees to 1.3 ± 1.1 degrees. The PSI software correctly determined the tibial implant size in 70 patients (89%). Prediction of talar implant sizing was less accurate than the tibial component, with 56 patients (71%) using the predicted sized implant. The overall implant survival at a mean follow-up of 45 months (range, 27-76) was 97.5%. CONCLUSION: We found that this PSI system accurately and reliably assisted in implant total ankle prosthesis positioning within a clinically acceptable margin and without significant outliers. Prediction of implant size was not as accurate as component orientation. LEVEL OF EVIDENCE: Level III, retrospective study.

Cardiovascular disease (CVD) is the leading killer of Americans. CVD is understudied among Latinos, who have high levels of CVD risk factors. This study aimed to determine whether access to health care (ie, insurance status and having a usual source of care) is associated with 4 CVD prevention factors (ie, health care utilization, CVD screening, information received from health care providers, and lifestyle factors) among Latino adults and to evaluate whether the associations depended on CVD clinical risk/disease.Data were collected as part of a community-engaged food environment intervention study in East Los Angeles and Boyle Heights, CA. Logistic regressions were fitted with insurance status and usual source of care as predictors of the 4 CVD prevention factors while controlling for demographics. Analyses were repeated with interactions between self-reported CVD clinical risk/disease and access to care measures.Access to health care significantly increased the odds of CVD prevention. Having a usual source of care was associated with all factors of prevention, whereas being insured was only associated with some factors of prevention. CVD clinical risk/disease did not moderate any associations.Although efforts to reduce CVD risk among Latinos through the Affordable Care Act could be impactful, they might have limited impact in curbing CVD among Latinos, via the law's expansion of insurance coverage. CVD prevention efforts must expand beyond the provision of insurance to effectively lower CVD rates.

ZnO nanorods were grown on microfibers of Polyethylene terephthalate (PET) fabric by seeding method to develop hierarchical roughness structure. XRD and XPS analysis show the presence of crystalline ZnO and chemical Zn species at the fiber surface at each stage of the process. Five series of samples with different seed concentrations have been realized, and their surface morphology and topography were characterized by AFM and SEM. Increasing seed concentrations lead to samples with superhydrophilic properties. Not only the water contact angle at fabric surface tends to zero but also the water capillary diffusion inside fabric is faster. Nanostructuration affects the structure inside the fabric, and further experiments with decane liquid have been made to get a better understanding of this effect. To study the superhydrophobicity, nanorods treated samples were modified with octadecyltrimethoxysilane (ODS) by two method; solution deposition and vapor deposition. The superhydrophobicity was characterized by measuring the water contact angle and water sliding angle with 5 µl water droplet. The samples modified with ODS by vapor deposition showed higher water contact angles and low water sliding angle than the ones modified with solution method. The lotus effect has been well correlated with the surface morphology of the nanorods structured fibers. The application of the Cassie-Baxter equation is discussed.
