RESUMO

We recently developed a one-dimensional imager of neutrons on the Z facility. The instrument is designed for Magnetized Liner Inertial Fusion (MagLIF) experiments, which produce D-D neutrons yields of ∼3 × 1012. X-ray imaging indicates that the MagLIF stagnation region is a 10-mm long, ∼100-µm diameter column. The small radial extents and present yields precluded useful radial resolution, so a one-dimensional imager was developed. The imaging component is a 100-mm thick tungsten slit; a rolled-edge slit limits variations in the acceptance angle along the source. CR39 was chosen as a detector due to its negligible sensitivity to the bright x-ray environment in Z. A layer of high density poly-ethylene is used to enhance the sensitivity of CR39. We present data from fielding the instrument on Z, demonstrating reliable imaging and track densities consistent with diagnosed yields. For yields ∼3 × 1012, we obtain resolutions of ∼500 µm.

RESUMO

Comamonas testosteroni has rarely been implicated as a human pathogen. We report a case of infectious endocarditis due to this organism, occurring in a 49-year-old man. The posterior leaflet of the mitral valve contained a 1 x 1 cm vegetation and showed myxoid degeneration and acute inflammation. The patient had no evidence of reinfection after 32 months. The infection was almost certainly community acquired, as is usually true for this organism.

A 56-year-old man was admitted to our hospital with a diagnosis of suspected constrictive pericarditis. After the diagnosis was confirmed by cardiac catheterization, an elective pericardiectomy was performed without complication. Four days after surgery dyspnea developed in the patient, and he was found to have an acute decrease in left ventricular ejection fraction (LVEF) by echocardiography. The patient's symptoms and the LVEF improved over time and returned to normal 4 weeks after surgery. Transient hemodynamic dysfunction of the left ventricle has previously been reported after pericardiectomy or pericardiocentesis; however, we know of no reports in the literature that confirm an acute reduction in LVEF by echocardiography after pericardiectomy for constrictive pericarditis.

RESUMO

The large-scale production of vast numbers of suitable salt nuclei and their upward launch is one of the main technological barriers to the experimental testing of marine cloud brightening (MCB). Very promising, though not definitive, results have been obtained using an adapted version of effervescent spray atomization. The process is simple, robust and inexpensive. This form of effervescent spraying uses only pressurized water and air sprayed from small nozzles to obtain very fine distributions. While it is far from optimized, and may not be the best method if full deployment is ever desired, we believe that even in its present form the process would lend itself well to preliminary field test investigations of MCB. Measurements obtained using standard aerosol instrumentation show approximately lognormal distributions of salt nuclei with median diameters of approximately 65 nm and geometric standard deviations slightly less than 2. However, these measurements are not in agreement with those based on scanning electron microscopy imaging of collected particles, an observation that has not yet been explained. Assuming the above distribution, 10(15) particles per second could be made with 21 kW of spray power, using approximately 200 nozzles. It is envisioned that existing snow making equipment can be adapted to launch the nuclei 60-100 m into the air, requiring approximately 20 kW of additional power.

RESUMO

The idea behind the marine cloud-brightening (MCB) geoengineering technique is that seeding marine stratocumulus clouds with copious quantities of roughly monodisperse sub-micrometre sea water particles might significantly enhance the cloud droplet number concentration, and thereby the cloud albedo and possibly longevity. This would produce a cooling, which general circulation model (GCM) computations suggest could-subject to satisfactory resolution of technical and scientific problems identified herein-have the capacity to balance global warming up to the carbon dioxide-doubling point. We describe herein an account of our recent research on a number of critical issues associated with MCB. This involves (i) GCM studies, which are our primary tools for evaluating globally the effectiveness of MCB, and assessing its climate impacts on rainfall amounts and distribution, and also polar sea-ice cover and thickness; (ii) high-resolution modelling of the effects of seeding on marine stratocumulus, which are required to understand the complex array of interacting processes involved in cloud brightening; (iii) microphysical modelling sensitivity studies, examining the influence of seeding amount, seed-particle salt-mass, air-mass characteristics, updraught speed and other parameters on cloud-albedo change; (iv) sea water spray-production techniques; (v) computational fluid dynamics studies of possible large-scale periodicities in Flettner rotors; and (vi) the planning of a three-stage limited-area field research experiment, with the primary objectives of technology testing and determining to what extent, if any, cloud albedo might be enhanced by seeding marine stratocumulus clouds on a spatial scale of around 100×100 km. We stress that there would be no justification for deployment of MCB unless it was clearly established that no significant adverse consequences would result. There would also need to be an international agreement firmly in favour of such action.

RESUMO

Paragangliomas, also known as extra-adrenal pheochromocytomas, arise from chromaffin cells within the autonomonic ganglia. Catecholamine secreting paragangliomas have been described in the mediastinum, but rarely invade the heart. This case describes the use of a novel bioabsorbable matrix that was used to replace a tumor invading the posterior left atrium.

RESUMO

A multiinstitution collaboration is developing a neutron imaging system for the Sandia Z facility. The initial system design is for slit aperture imaging system capable of obtaining a one-dimensional image of a 2.45 MeV source producing 5x10(12) neutrons with a resolution of 320 microm along the axial dimension of the plasma, but the design being developed can be modified for two-dimensional imaging and imaging of DT neutrons with other resolutions. This system will allow us to understand the spatial production of neutrons in the plasmas produced at the Z facility.

RESUMO

Synovial sarcoma comprises approximately 10% of all soft tissue sarcoma diagnoses; a primary synovial sarcoma of the myocardium is exceedingly rare. There have been very few cases reported in the literature thus far. With the identification of the characteristic and diagnostic chromosomal abnormality t(X;18), this may become an increasingly recognized entity. Our report adds to the limited published cases of primary cardiac synovial sarcoma with the characteristic t(X;18). Further elucidation of the effects of this translocation on the cell cycle may lead to directed therapies in the future.

OBJECTIVES: The purpose of this study was to investigate the possibility that some patients with coronary artery disease (CAD) but negative exercise or chemical stress test results might have mental stress-induced ischemia. The study population consisted solely of those with negative test results. BACKGROUND: Mental stress-induced ischemia has been reported in 20% to 70% of CAD subjects with exercise-induced ischemia. Because mechanisms of exercise and mental stress-induced ischemia may differ, we studied whether mental stress would produce ischemia in a proportion of subjects with CAD who have no inducible ischemia with exercise or pharmacologic tests. METHODS: Twenty-one subjects (14 men, 7 women) with a mean age of 67 years and with a documented history of CAD were studied. All subjects had a recent negative nuclear stress test result (exercise or chemical). Subjects completed a speaking task involving role playing a difficult interpersonal situation. A total of 30 mCi 99mTc-sestamibi was injected at one minute into the speech, and imaging was started 40 min later. A resting image obtained within one week was compared with the stress image. Images were analyzed for number and severity of perfusion defects. The summed difference score based on the difference between summed stress and rest scores was calculated. Severity was assessed using a semiquantitative scoring method from zero to four. RESULTS: Six of 21 (29%) subjects demonstrated reversible ischemia (summed difference score > or =3) with mental stress. No subject had chest pain or electrocardiographic changes during the stressor. Mean systolic and diastolic blood pressure and heart rate all increased between resting and times of peak stress. CONCLUSIONS: Mental stress may produce ischemia in some subjects with CAD and negative exercise or chemical nuclear stress test results.

Independently emerging fluorescence profiles of unknown, photochemically induced degradation products of several naturally non-fluorescent pesticides were monitored using single exposure excitation-emission fluorescence spectroscopy. Three-way parallel factor analysis (PARAFAC) was employed to uniquely resolve the pure fluorescent spectra of the overlapping photolysis products. The quantitative utility of EEM photolysis-based determinations was demonstrated by employing four-way PARAFAC models built from EEM time cubes of multiple fenvalerate samples. The 4-way PARAFAC models were then used to predict original pesticide concentrations resulting in conservative limit of detection and root mean square errors of calibration (RMSEC) of 3 microM each.
