PURPOSE: To report the relative efficacy of combining phacoemulsification with a Schlemm's canal microstent (Phaco/Hydrus) or dual blade trabecular excision (Phaco/KDB). DESIGN: Retrospective study. METHODS: One hundred and thirty-one eyes of 131 patients who underwent Phaco/Hydrus or Phaco/KDB procedures from January 2016 to July 2021 at a tertiary care center were included and assessed for up to 36months postoperatively. Primary outcomes were intraocular pressure (IOP) and number of glaucoma medications, evaluated by generalized estimating equations (GEE). Two Kaplan-Meier estimates (KM) assessed survival without additional intervention or pressure lowering medication while maintaining: (1) IOP≤21mmHg and≥20% IOP reduction or (2) IOP≤preoperatively designated goal. RESULTS: Mean preoperative IOP was 17.70±4.91 (SD) mmHg on 0.28±0.86 medications in the Phaco/Hydrus cohort (n=69) and 15.92±4.34mmHg on 0.19±0.70 medications in the Phaco/KDB cohort (n=62). At 12months, mean IOP was reduced to 14.98±2.77mmHg on 0.12±0.60 medications after Phaco/Hydrus and 13.52±4.13mmHg on 0.04±0.19 medications after Phaco/KDB. GEE models of IOP (P<0.001) and medication burden (P<0.05) showed significant patterns of reduction across all timepoints in both cohorts. There were no differences in IOP reduction (P=0.94), number of medications (P=0.95) or survival (P=0.72 by KM1, P=0.11 by KM2) between procedures. CONCLUSIONS: Both Phaco/Hydrus and Phaco/KDB resulted in significantly reduced IOP and medication burden for over 12months. Phaco/Hydrus and Phaco/KDB confer similar outcomes in terms of IOP, medication burden, survival, and procedural time in a population with predominantly mild and moderate open-angle glaucoma.

The radiative forcing before and after rain events was studied between 12 February 2016 and 14 March 2017 in León, Spain. For this purpose, the radiative forcing fluxes were calculated using the Radiative Transfer Model Global Atmospheric ModEl (RTM GAME). After the application of a set of selection criteria (based on the availability of AERONET data, rain characteristics and lightning maps), 16 stratiform rain events were identified, concentrated in spring and winter, and 15 convective rain events were found concentrated in spring and summer. Rainfall events were grouped according to the atmospheric forcing (ΔFATM) before rain: "low" or "high" (lower or higher than 30 W m-2). The threshold has been set at this value because it is the mean ΔFATM of all the selected events before rain. There were significant statistical differences between stratiform and convective events in rain duration, mean raindrop diameter and parameters a and b of radar reflectivity Z and rainfall intensity R relationship (Z = a Rb). When comparing "low" and "high" groups, raindrop diameter was similar in stratiform (0.51 ± 0.08 vs 0.48 ± 0.12 mm) and convective events (0.96 ± 0.98 vs 0.83 ± 0.63 mm), registering higher values for the latter. In stratiform events, the rain scavenging effect on aerosol particles is clearly observed in the "high" group with a decrease of radiative forcing of -27.0 ± 25.3%, and to a lesser extent, in the "low" group, probably because of a lower aerosol load in the atmosphere. In stratiform events, the mode of the raindrop size gamma distribution presented statistical differences between "low" (0.25 ± 0.13 mm) and "high" (0.35 ± 0.05 mm) groups. We claim that this points towards a relationship between radiative forcing before rain and the specific characteristics of rainfall measured at ground level. This study increases our knowledge on the important role of rainwater as a clean agent of the atmosphere and its impact on climate (through radiative forcing).

The objective of this work was to assess the prevalence of human immunodeficiency virus-related oral lesions (HIV-ROL) in HIV-positive/acquired immunodeficiency syndrome (AIDS) patients receiving highly active antiretroviral therapy (HAART) including HIV-protease inhibitors. One hundred fifty-five (154) AIDS patients (69 intravenous drug users [IDU], 53 heterosexuals, 29 males who have sex with males, 1 transfused, and 2 of unknown contagious source) receiving HAART, were examined. We found the following prevalences: HIV-ROL 53.2%; oral candidiasis 34.4%; hairy leucoplakia 26.6%; xerostomia 15.5%; herpes simplex labialis 1.9%; HIV/periodontitis-gingivitis 0.6%. No cases of Kaposi's sarcoma were observed. The highest prevalence of HIV-ROL was found in the IDU group, and in patients with viral load more than 10,000 copies and CD4(+) cell count less than 200. Using our historical controls, this suggests that the prevalence of all oral lesions, particularly oral candidiasis, herpes simplex labiali, Kaposi's sarcoma, and periodontal disease has decreased more than 30% after the institution of HAART.

Introducción: La consulta pediátrica pretrasplante es fundamental en un programa de trasplante hepático, allí se evalúan diferentes aspectos como, la indicación del trasplante, el estadio de la enfermedad, comorbilidades asociadas y nos permite corregir las mismas ya que estas influirán en la sobrevida postrasplante. El objetivo fue conocer las características de la población pediátrica evaluada como posibles candidatos a trasplante. Pacientes y Métodos: Se realizó estudio retrospectivo. Se incluyeron todos los pacientes pediátricos que fueron referidos para valoración como posibles candidatos a trasplante hepático desde 2005 al 2010. Resultados: Se estudiaron 152 pacientes, 65 (43%) del sexo masculino y 87 (57%) del sexo femenino, con edad promedio de 6,16 + 5,24 años. Al momento del estudio 61 pacientes no tenían indicación de trasplante, 35 se encontraban en evaluación, 28 se trasplantaron, 21 fallecieron y 7 fueron referidos. Las indicaciones de trasplante fueron: disminución de la síntesis hepática 20, síndrome hepatopulmonar 2, hemorragia digestiva 3, síndrome hepatorrenal 2 y un fallo hepático subagudo. De los pacientes trasplantados 16 presentaban algún grado de desnutrición previa, 3 presentaban ascitis, 7 habían presentado hemorragias digestivas, 6 presentaban infecciones (urinarias, respiratorias y absceso dentarios), 24 tenían patologías odontológicas. Los 28 pacientes tenían esquema de inmunización incompleto durante la evaluación. Todos estos problemas fueron tratados previo al trasplante. Conclusión: La consulta pediátrica de hígado pretrasplante es de suma importancia ya que en ella se identifican los pacientes con indicación de trasplante además de valorar de forma integral al paciente, lo que nos permite conocer aquellas morbilidades asociadas a la enfermedad hepática terminal y resolverlas previo al trasplante, mejorando su posterior sobrevida.


Introduction: Pediatric liver pre-transplantation consultation is very important in a liver transplant program, there different aspects are assessed, such as the indication the transplant, disease stage, associated comorbidities and it allows us to correct them as they will affect the survival after transplantation. The objective was to determine the characteristics of the pediatric population evaluated as potential transplant candidates. Patients And Methods: A retrospective study was performed. All pediatric patients who were referred for evaluation as potential candidates for liver transplantation from 2005 to 2010 were included. RESULTS: 152 patients were studied, 65 (43%) were males and 87 (57%) females, mean age 6.16 ± 5.24 years. At the time of the study 61 patients had no indication for transplant, 35 were been evaluated, 28 were transplanted, 21 died and 7 were referred. The indications for transplantation were: decreased hepatic synthesis 20, hepatopulmonary syndrome 2, gastrointestinal bleeding 3, hepatorenal syndrome 2 and one subacute liver failure. Of the transplanted patients 16 had some degree of previous malnutrition, 3 had ascites, 7 had presented gastrointestinal bleeding, 6 had infections (urinary, respiratory and dental abscess), and 24 had dental pathologies. The 28 patients had an incomplete immunization schedule during the evaluation. All these problems were treated prior to transplantation. Conclusion: the pediatric liver pretransplantation consultation is of most importance since in it we identify patients with indication for transplantation; in addition, we can completely asses the patient allowing us to recognize morbidities associated with the end stage liver disease and resolve them before transplantation, improving subsequent survival.

El consumo de alcohol en nuestro país constituye un problema de salud pública relacionado con el desarrollo de eventos que afectan la esfera biopsicosocial del individuo. Objetivo: Caracterizar los patrones de consumo de alcohol en personas de 14 años y más que consultaron a los servicios de gastroenterología, Medicina Interna, Endocrinología y Cirugía del Hospital Vargas de Caracas. Pacientes y Métodos: El levantamiento de los datos se realizó durante el trimestre marzo-mayo 2008, en horario matutino y vespertino, de lunes a viernes. Se entrevistó una muestra intencional, heterogénea de 450 pacientes. Se aplicó una encuesta de preguntas abiertas en la cual se incluyeron los datos de identificación (nombre, apellido, sexo, edad, teléfono), consumo de alcohol (edad de inicio, número de tragos, frecuencia) y las preguntas del Test de Cage. Para medir el consumo de alcohol se utilizó una clasificación que permitió conocer el riesgo de Alcoholismo. Resultados: Se realizaron un total de 450 encuestas, en 149 pacientes masculinos y 301 femeninos. Del total de la muestra se identificaron 186 pacientes que consumen alcohol y 264 que no consumen, con un 41,55% y 58,45% respectivamente. El riesgo alto de consumo de alcohol estuvo presente en 16 pacientes y el consumo perjudicial en 50 pacientes, con un 8,60% y 26,88% respectivamente. Cabe destacar, que el riesgo alto de consumo fue mas elevado en el sexo femenino 10,34%, mientras que el consumo perjudicial en el sexo masculino con 43,87%. Conclusiones: El consumo de alcohol en los pacientes que acudieron a las Consultas Externas del Hospital Vargas de Caracas es alto, en un 41,55 % de los encuestados, con un riesgo de consumo de alcohol de 35,48 %.


Alcohol intake in our country is a public health problem related with the development of many events that affect the bio - psychosocial area of the patients. Objective: Characterize the patterns of alcohol intake in people from 14 years old on; that attended the gastroenterology, Internal Medicine, Endocrinology and Surgery Divisions at the Hospital Vargas in Caracas. Patients and Methods: the data was collected during March-May 2008, in the morning and afternoon, from Monday through Friday. Interviews were done in an intentional, heterogeneous sample of 450 patients. An opened questions survey was applied, in which the identification data (name, last name, sex, age, telephone), alcohol intake were included (age of beginning, number of drinks, frequency), as well as the Cage Test questions. In order to measure alcohol intake, we used a classification that allowed knowing the risk of Alcoholism. Results: A total of 450 surveys were made, in 149 male patients and 301 females. Of the total of the sample, 186 patients consume alcohol and 264 do not, 41.55% and 58.45% respectively. 16 patients, presented with high risk of alcohol intake and 50 patients with severe intake, 8.60% and 26.88% respectively. It is important to stress out that females presented a high risk of intake (10.34%), whereas the detrimental intake in males was 43.87%. Conclusions: Alcohol intake in patients who attended "Hospital Vargas de Caracas" outpatient clinic is high, with a 41.55% of the patients, and an alcohol intake risk of 35.48 %.

La enfermedad por reflujo gastroesofágico (ERGE) es una enfermedad crónica recurrente, que constituye uno de los trastornos gastrointestinales que afecta con mayor frecuencia a población mundial. Objetivo: Determinar la prevalencia de enfermedad por reflujo gastroesofágico en la población adulta venezolana. Pacientes y Métodos: Se realiza un estudio descriptivo, de corte transversal, multicéntrico durante los meses de abril y mayo de 2008. Se entrevistaron a individuos en edades comprendidas entre 15 - 65 años de diferentes estados del país (Aragua, Anzoátegui, Barinas, Carabobo, Delta Amacuro, Distrito Capital, Mérida, Miranda, Sucre, Zulia) escogidos a través del azar simple, realizándose un muestreo por conglomerados, de donde, en cada estado se seleccionó un Municipio y de éste, lugares como iglesias, centro comerciales, cines y paradas de autobuses. Se tomó en consideración la población de cada municipio según el Censo Nacional 2001. Resultados: Un total de 1318 individuos fueron encuestados en los estados incluidos, 814 mujeres (61,76%) y 504 Hombres (38,24%). En cuanto a la distribución por categoría de edad, el grupo de 25-35 años constituyó el de mayor predominio. Se obtuvo una Prevalencia Nacional de 11,54%, donde los síntomas cardinales pirosis y regurgitación ácida representaron 16,62% y 12,75% respectivamente. Conclusión: La prevalencia nacional de enfermedad por reflujo gastroesofágico en la población venezolana es del 11,54%.


The gastroesophageal reflux disease (GERD) is a chronic recurrent illness that represents one of the most common gastrointestinal disorders that affects worldwide population. Aim: To determine the prevalence of gastroesophageal reflux disease in the Venezuelan adult population. Patients and Methods: A Descriptive, cross section, multicentric study was made. A survey was done to people in ages between 15 - 65 years of different states of the country (Aragua, Anzoátegui, Barinas, Carabobo, Delta Amacuro, Distrito Capital, Mérida, Miranda, Sucre, Zulia) chosen randomly. Cluster sampling was performed, where in each State was selected a municipality and, of which one places like churches, malls, cinemas and bus stops. To calculate de amount of people The National Census 2001 was taken in account in each municipality. Results: A total of 1318 individuals were surveyed in the mentioned States including, 814 women (61.76%) and 504 men (38.24%). The predominant distribution by age was the group of 25-35 years old. A National Prevalence of 11.54% was obtained, where the cardinal symptoms were heartburn and acid regurgitation represented 16.62% and 12.75% respectively. Conclusion: The national prevalence of gastroesophageal reflux disease in the Venezuelan adult population is 11.54%.

La enfermedad por reflujo gastroesofágico (ERGE) es uno de los trastornos gastrointestinales con mayor prevalencia; afecta del 10 a 15% de la población general constituyendo una causa frecuente de consulta. Sin embargo, existen pocos reportes acerca de la prevalencia de ERGE en la población general. Objetivos: Determinar la prevalencia de ERGE en la población general adulta del área metropolitana de Caracas y estudiar algunos factores asociados a esta enfermedad. Materiales y métodos: Estudio de corte transversal, realizado en la población general adulta del área metropolitana de Caracas en abril de 2007. Se utilizó una encuesta basada en criterios clínicos epidemiológicos de acuerdo a consensos latinoamericanos y europeos sobre la ERGE. Para el análisis de los resultados se calcularon promedios, medianas y porcentajes. Resultados: De 389 sujetos encuestados, 223 fueron mujeres (57,3%) y 166 hombres (42,7%). El promedio de edad fue 33,89 años. La prevalencia de síntomas cardinales en la ERGE fue 19,02%. En los sujetos con ERGE encontramos que 47,3% ingerían licor al menos un vez al mes, y 21,6% referían hábitos tabáquicos. Conclusión: Se determinó que la prevalencia de la ERGE en la población general adulta del área Metropolitana de Caracas fue de 19,02% encontrándose asociación con el hábito alcohólico.


Gastroesophageal reflux disease (GERD) is one of the gastrointestinals problems with higher prevalence; it affects 10 to 15% of the general population being a frequent cause of medical visit. Nevertheless, there is low number of reports about GERD prevalence. Objectives: to determine GERD prevalence in adult population of the Caracas Metropolitan Area and study some related factors to this disease. Materials and methods: Study of transversal cut, made in adult population in Caracas Metropolitan Area in April 2007. A poll based in clinical and epidemiological criteria in accordance with a Latin- American and European consensus about GERD was used. To analize the results averages, medians and percentages were calculated. Results: of 389 subjects surveyed, 223 were women (57,3%) and 166 were men (42,7%). The mean age of the patients was 33 years old. The cardinals symptoms prevalence was 19,02%. In subjects with GERD we found that 47,3% drank alcohol at least once a week, and 21,6% were smokers. In Conclusion, we found that GERD prevalence in adult population of the Caracas Metropolitan Area was 19, 02% finding a relationship with alcoholic habit.

Desde hace dos décadas esta descrita la utilidad de las prótesis endoscópicas, en patología biliopancreática. Objetivo: Mostrar la experiencia del Servicio de Gastroenterología del Hospital Vargas de Caracas en la colocación de prótesis endoscópicas en la patología biliopancreática, describiendo características demográficas, patologías más frecuentes, tasa de éxito en su colocación y complicaciones. Pacientes y Métodos: Estudio descriptivo, retrospectivo. Se revisaron datos de los informes de CPRE de pacientes hospitalizados y ambulatorios evaluados en la institución desde enero 1998 hasta abril 2007. Resultados: Se incluyeron 248 procedimientos de 110 mujeres y 73 hombres, 242 correspondieron a colocación de prótesis biliares (234 de teflón o plástico y 8 autoexpansibles metálicas) y 6 prótesis pancreáticas. Las indicaciones mas frecuentes para la colocación de prótesis fueron: patologías neoplásicas (47%), lesiones de la vía biliar post-quirúrgicas o traumáticas (22%) y litiasis biliar (17%). El éxito de la colocación de la prótesis fue del 99,1%. Se reportaron complicaciones en 4% de los procedimientos, con mayor frecuencia la colangitis (2%) y una mortalidad del 0%. Conclusión: La colocación de prótesis endoscópicas en patologías biliopancreáticas es un procedimiento seguro, con un bajo porcentaje de complicaciones y mortalidad.


The utility of the endoscopic stent have been described for two decades in the biliopancreatic diseases. Aims: To show the Gastroenterology Division of the Hospital Vargas de Caracas experience in placing endoscopic stents in biliopancreatic diseases, describing demographic characteristics more frequent diseases, success rates and complications. Patients and Methods: descriptive and retrospective trial. Features of the reports of PEPR of hospitalized and ambulatory patients seen in the institution from January 1998 to april 2007 were reviewed. Results: 248 procedures from 110 women and 73 men were included, 242 had a biliar stent (234 of Teflon or plastic and 8 metallic selfexpanded) and 6 had pancreatic stents. The more frequent indications for placing the stents were: Tumoral disease, (47%), traumatic or post-surgical lesions of the biliary tract (22%) and biliary stones (17%). The success rate of the placing of stents was 99.1%. The procedureÊs complication rate reported was 4%, being colangitis the most frequent with 2% and a 0% mortality. Conclusion: the placing of endoscopic stents in biliopancreatic diseases is a safe procedure, with a low rate of complications and mortality.
