Oocytes are among the longest-lived cells in the body and need to preserve their cytoplasm to support proper embryonic development. Protein aggregation is a major threat for intracellular homeostasis in long-lived cells. How oocytes cope with protein aggregation during their extended life is unknown. Here, we find that mouse oocytes accumulate protein aggregates in specialized compartments that we named endolysosomal vesicular assemblies (ELVAs). Combining live-cell imaging, electron microscopy, and proteomics, we found that ELVAs are non-membrane-bound compartments composed of endolysosomes, autophagosomes, and proteasomes held together by a protein matrix formed by RUFY1. Functional assays revealed that in immature oocytes, ELVAs sequester aggregated proteins, including TDP-43, and degrade them upon oocyte maturation. Inhibiting degradative activity in ELVAs leads to the accumulation of protein aggregates in the embryo and is detrimental for embryo survival. Thus, ELVAs represent a strategy to safeguard protein homeostasis in long-lived cells.

Oocytes form before birth and remain viable for several decades before fertilization1. Although poor oocyte quality accounts for most female fertility problems, little is known about how oocytes maintain cellular fitness, or why their quality eventually declines with age2. Reactive oxygen species (ROS) produced as by-products of mitochondrial activity are associated with lower rates of fertilization and embryo survival3-5. Yet, how healthy oocytes balance essential mitochondrial activity with the production of ROS is unknown. Here we show that oocytes evade ROS by remodelling the mitochondrial electron transport chain through elimination of complex I. Combining live-cell imaging and proteomics in human and Xenopus oocytes, we find that early oocytes exhibit greatly reduced levels of complex I. This is accompanied by a highly active mitochondrial unfolded protein response, which is indicative of an imbalanced electron transport chain. Biochemical and functional assays confirm that complex I is neither assembled nor active in early oocytes. Thus, we report a physiological cell type without complex I in animals. Our findings also clarify why patients with complex-I-related hereditary mitochondrial diseases do not experience subfertility. Complex I suppression represents an evolutionarily conserved strategy that allows longevity while maintaining biological activity in long-lived oocytes.

Oocytes spend the majority of their lifetime in a primordial state. The cellular and molecular biology of primordial oocytes is largely unexplored; yet, it is necessary to study them to understand the mechanisms through which oocytes maintain cellular fitness for decades, and why they eventually fail with age. Here, we develop enabling methods for live-imaging-based comparative characterization of Xenopus, mouse and human primordial oocytes. We show that primordial oocytes in all three vertebrate species contain active mitochondria, Golgi and lysosomes. We further demonstrate that human and Xenopus oocytes have a Balbiani body characterized by a dense accumulation of mitochondria in their cytoplasm. However, despite previous reports, we did not find a Balbiani body in mouse oocytes. Instead, we demonstrate that what was previously used as a marker for the Balbiani body in mouse primordial oocytes is in fact a ring-shaped Golgi that is not functionally associated with oocyte dormancy. This study provides the first insights into the organization of the cytoplasm in mammalian primordial oocytes, and clarifies the relative advantages and limitations of choosing different model organisms for studying oocyte dormancy.

The in vitro activity of imipenem-relebactam, meropenem-vaborbactam, ceftazidime-avibactam, and cefiderocol was evaluated against both clinical and isogenic enterobacterial isolates producing carbapenemases of the SME, NmcA, FRI, and IMI types. Ceftazidime-avibactam and meropenem-vaborbactam showed the highest activity against all tested isolates; imipenem-relebactam showed only moderate activity. All isolates remained susceptible to cefiderocol. Furthermore, avibactam and vaborbactam have greater inhibitory activity than relebactam against the tested carbapenemases. Overall, ceftazidime-avibactam, meropenem-vaborbactam, and cefiderocol were the most effective therapeutic options for treating infections caused by the tested minor carbapenemase producers.

INTRODUCTION: Latin American Initiative for Lifestyle Intervention to Prevent Cognitive Decline (LatAm-FINGERS) is the first non-pharmacological multicenter randomized clinical trial (RCT) to prevent cognitive impairment in Latin America (LA). Our aim is to present the study design and discuss the strategies used for multicultural harmonization. METHODS: This 1-year RCT (working on a 1-year extension) investigates the feasibility of a multi-domain lifestyle intervention in LA and the efficacy of the intervention, primarily on cognitive function. An external harmonization process was carried out to follow the FINGER model, and an internal harmonization was performed to ensure this study was feasible and comparable across the 12 participating LA countries. RESULTS: Currently, 1549 participants have been screened, and 815 randomized. Participants are ethnically diverse (56% are Nestizo) and have high cardiovascular risk (39% have metabolic syndrome). DISCUSSION: LatAm-FINGERS overcame a significant challenge to combine the region's diversity into a multi-domain risk reduction intervention feasible across LA while preserving the original FINGER design.

The Tear Film Lipid Layer (TFLL) acts primarily as an interface between the aqueous layer and air. Tear film lipid is composed of a thin layer of polar lipids that interact with the secretory layer of the underlying mucosa and a thicker layer of non-polar lipids at the air interface. The tear film has a complex structure and composition that protects the cornea, promotes wound healing, and maintains high-quality vision. Plasma Rich in Growth Factor (PRGF) eye drops emerged as an exciting new treatment for corneal epitheliopathies, including aqueous deficient dry eye. The purpose of this study was to compare the lipidomic profile of eye drops obtained from PRGF with tear lipidome to determine whether PRGF drops could be an adequate complement to tears in patients with impaired TFLL. To address this study, tears and blood was collected and processed from healthy donors to obtain PRGF eye drops. Samples were aliquoted and stored at -80 °C until use. The lipid profiles of these samples were analysed by Ultrahigh Performance Liquid Chromatography (UHPLC) using a Vanquish UHPLC system to obtain untargeted lipidome profiles on a Q-Exactive HF-X hybrid quadrupole-Orbitrap mass spectrometer. In PRGF eye drops, 408 lipids were identified in ESI+ mode and 183 in ESI- mode, and they were grouped into 15 different lipid classes from four distinct categories. By contrast, 112 lipid species were identified from tear samples in ESI+ mode and 36 in ESI- mode, belonging to 12 lipid classes from six different categories. The relative abundance of most lipid species was much greater in the PRGF eye drops than in the tear, although there were some lipids present in tears that were not found in the PRGF, such as wax esters and (O-acyl)-ω-hydroxy fatty acids. In summary, these results suggest that the lipids present in PRGF eye drops could serve as a tear supplement in individuals in whom tear lipid composition is altered, although there are differences in the lipid profile of these two fluids.

RESUMO

Medical AI is increasingly being developed and tested to improve medical diagnosis, prediction and treatment of a wide array of medical conditions. Despite worries about the explainability and accuracy of such medical AI systems, it is reasonable to assume that they will be increasingly implemented in medical practice. Current ethical debates focus mainly on design requirements and suggest embedding certain values such as transparency, fairness, and explainability in the design of medical AI systems. Aside from concerns about their design, medical AI systems also raise questions with regard to physicians' responsibilities once these technologies are being implemented and used. How do physicians' responsibilities change with the implementation of medical AI? Which set of competencies do physicians have to learn to responsibly interact with medical AI? In the present article, we will introduce the notion of forward-looking responsibility and enumerate through this conceptual lens a number of competencies and duties that physicians ought to employ to responsibly utilize medical AI in practice. Those include amongst others understanding the range of reasonable outputs, being aware of own experience and skill decline, and monitoring potential accuracy decline of the AI systems.

Crops and ecosystems constantly change, and risks are derived from heavy rains, hurricanes, droughts, human activities, climate change, etc. This has caused additional damages with economic and social impacts. Natural phenomena have caused the loss of crop areas, which endangers food security, destruction of the habitat of species of flora and fauna, and flooding of populations, among others. To help in the solution, it is necessary to develop strategies that maximize agricultural production as well as reduce land wear, environmental impact, and contamination of water resources. The generation of crop and land-use maps is advantageous for identifying suitable crop areas and collecting precise information about the produce. In this work, a strategy is proposed to identify and map sorghum and corn crops as well as land use and land cover. Our approach uses Sentinel-2 satellite images, spectral indices for the phenological detection of vegetation and water bodies, and automatic learning methods: support vector machine, random forest, and classification and regression trees. The study area is a tropical agricultural area with water bodies located in southeastern Mexico. The study was carried out from 2017 to 2019, and considering the climate and growing seasons of the site, two seasons were created for each year. Land use was identified as: water bodies, land in recovery, urban areas, sandy areas, and tropical rainforest. The results in overall accuracy were: 0.99% for the support vector machine, 0.95% for the random forest, and 0.92% for classification and regression trees. The kappa index was: 0.99% for the support vector machine, 0.97% for the random forest, and 0.94% for classification and regression trees. The support vector machine obtained the lowest percentage of false positives and margin of error. It also acquired better results in the classification of soil types and identification of crops.

In this paper, we study the shear-induced flow of magneto-polymer composites, consisting of dispersions of magnetic particles in solutions of polymers, as a competition between the colloidal forces amid particles and their bulk transport induced by the hydrodynamic forces. For this aim, we analyse the role of different experimental parameters. Firstly, by using only solutions of a well-known anionic polymer (sodium alginate), we provoke a moderate hindering of particle movement, but keeping the liquid-like state of the samples. On the contrary, a gel-like behaviour is conferred to the samples when a cationic polymer (chitosan) is additionally added, which further reduces the particle movement. We analyse the effect of an applied magnetic field, which is opposed to particle transport by hydrodynamic forces, by inducing magnetic attraction between the particles. We perform the analysis under both stationary and oscillatory shear. We show that by using dimensionless numbers the differences between samples and experimental conditions are emphasized. In all cases, as expected, the transport of particles driven by bulk hydrodynamic forces dominates at high values of the shear rate. OBJECTIVES: The disease caused by severe acute respiratory syndrome coronavirus 2, known as coronavirus disease 2019, has resulted in a global pandemic. Reports are emerging of a new severe hyperinflammatory syndrome related to coronavirus disease 2019 in children and adolescents. The Centers for Disease Control and Prevention has designated this disease multisystem inflammatory syndrome in children. Our objective was to develop a clinical inpatient protocol for the evaluation, management, and follow-up of patients with this syndrome. DATA SOURCES: The protocol was developed by a multidisciplinary team based on relevant literature related to coronavirus disease 2019, multisystem inflammatory syndrome in children, and related inflammatory syndromes, as well as our experience caring for children with multisystem inflammatory syndrome in children. Data were obtained on patients with multisystem inflammatory syndrome in children at our institution from the pre-protocol and post-protocol periods. DATA SYNTHESIS: Our protocol was developed in order to identify cases of multisystem inflammatory syndrome in children with high sensitivity, stratify risk to guide treatment, recognize co-infectious or co-inflammatory processes, mitigate coronary artery abnormalities, and manage hyperinflammatory shock. Key elements of evaluation include case identification using broad clinical characteristics and comprehensive laboratory and imaging investigations. Treatment centers around glucocorticoids and IV immunoglobulin with biologic immunomodulators as adjuncts. Multidisciplinary follow-up after discharge is indicated to manage continued outpatient therapy and evaluate for disease sequelae. In nearly 2 months, we admitted 54 patients with multisystem inflammatory syndrome in children, all of whom survived without the need for invasive ventilatory or mechanical circulatory support. After institution of this protocol, patients received earlier treatment and had shorter lengths of hospital stay. CONCLUSIONS: This report provides guidance to clinicians on evaluation, management, and follow-up of patients with a novel hyperinflammatory syndrome related to coronavirus disease 2019 known as multisystem inflammatory syndrome in children. It is based on the relevant literature and our experience. Instituting such a protocol during a global pandemic is feasible and is associated with patients receiving treatment and returning home more quickly.

The use of black box algorithms in medicine has raised scholarly concerns due to their opaqueness and lack of trustworthiness. Concerns about potential bias, accountability and responsibility, patient autonomy and compromised trust transpire with black box algorithms. These worries connect epistemic concerns with normative issues. In this paper, we outline that black box algorithms are less problematic for epistemic reasons than many scholars seem to believe. By outlining that more transparency in algorithms is not always necessary, and by explaining that computational processes are indeed methodologically opaque to humans, we argue that the reliability of algorithms provides reasons for trusting the outcomes of medical artificial intelligence (AI). To this end, we explain how computational reliabilism, which does not require transparency and supports the reliability of algorithms, justifies the belief that results of medical AI are to be trusted. We also argue that several ethical concerns remain with black box algorithms, even when the results are trustworthy. Having justified knowledge from reliable indicators is, therefore, necessary but not sufficient for normatively justifying physicians to act. This means that deliberation about the results of reliable algorithms is required to find out what is a desirable action. Thus understood, we argue that such challenges should not dismiss the use of black box algorithms altogether but should inform the way in which these algorithms are designed and implemented. When physicians are trained to acquire the necessary skills and expertise, and collaborate with medical informatics and data scientists, black box algorithms can contribute to improving medical care.

Magnetic hydrogels (ferrogels) are soft materials with a wide range of applications, especially in biomedicine because (i) they can be provided with the required biocompatibility; (ii) their heterogeneous structure allows their use as scaffolds for tissue engineering; (iii) their mechanical properties can be modified by changing different design parameters or by the action of magnetic fields. These characteristics confer them unique properties for acting as patterns that mimic the architecture of biological systems. In addition, and (iv) given their high porosity and aqueous content, ferrogels can be loaded with drugs and guided towards specific targets for local (non-systemic) pharmaceutical treatments. The ferrogels prepared in this work contain magnetic particles obtained by precipitation of magnetite nanoparticles onto the porous surface of bentonite platelets. Then, the particles were functionalized by adsorption of alginate molecules and dispersed in an aqueous solution of sodium alginate. Finally, the gelation was promoted by cross-linking the alginate molecules with Ca2+ ions. The viscoelastic properties of the ferrogels were measured in the absence/presence of external magnetic fields, showing that these ferrogels exhibited a strong enough magnetorheological effect. This behaviour is explained considering the field-induced strengthening of the heterogeneous (particle-polymer) network generated inside the ferrogel. Even in the absence of cross-linking, at large enough concentration, long polymer strands have a strong influence on the rheology of aqueous systems. In this work, we show that solutions of medium molecular weight (120 000-190 000 g mol-1) alginate polymer retained a liquid-like behaviour even for concentrations as large as 20% w/v. On the contrary, solutions of alginate polymer of larger (and also polydisperse) molecular weight (up to 600 000 g mol-1) presented a gel-like behaviour already at concentrations of 7% w/v. We dispersed micrometre-sized iron particles at a concentration of 5% v/v in these solutions, which resulted in either stable magnetic fluids or gels, depending on the type of alginate polymer employed (medium or large molecular weight, respectively). These magneto-polymer composites presented a shear-thinning behaviour that allowed injection through a syringe and recovery of the original properties afterwards. More interestingly, application of a magnetic field resulted in the formation of particle clusters elongated along the field direction. The presence of these clusters intensely affected the rheology of the systems, allowing a reversible control of their stiffness. We finally developed theoretical modelling for the prediction of the magnetic-sensitive rheological properties of these magneto-polymer colloids. The Industrial Internet of Things (IIoT) network generates great economic benefits in processes, system installation, maintenance, reliability, scalability, and interoperability. Wireless sensor networks (WSNs) allow the IIoT network to collect, process, and share data of different parameters among Industrial IoT sense Node (IISN). ESP8266 are IISNs connected to the Internet by means of a hub to share their information. In this article, a light-diffusion algorithm in WSN to connect all the IISNs is designed, based on the Peano fractal and swarm intelligence, i.e., without using a hub, simply sharing parameters with two adjacent IINSs, assuming that any IISN knows the parameters of the rest of these devices, even if they are not adjacent. We simulated the performance of our algorithm and compared it with other state-of-the-art protocols, finding that our proposal generates a longer lifetime of the IIoT network when few IISNs were connected. Thus, there is a saving-energy of approximately 5% but with 64 nodes there is a saving of more than 20%, because the IIoT network can grow in a 3 n way and the proposed topology does not impact in a linear way but log 3 , which balances energy consumption throughout the IIoT network.

There are several pathologies attacking the central nervous system and diverse therapies for each specific disease. These therapies seek as far as possible to minimize or offset the consequences caused by these types of pathologies and disorders in the patient. Therefore, comprehensive neurological care has been performed by neurorehabilitation therapies, to improve the patients' life quality and facilitating their performance in society. One way to know how the neurorehabilitation therapies contribute to help patients is by measuring changes in their brain activity by means of electroencephalograms (EEG). EEG data-processing applications have been used in neuroscience research to be highly computing- and data-intensive. Our proposal is an integrated system of Electroencephalographic, Electrocardiographic, Bioacoustic, and Digital Image Acquisition Analysis to provide neuroscience experts with tools to estimate the efficiency of a great variety of therapies. The three main axes of this proposal are: parallel or distributed capture, filtering and adaptation of biomedical signals, and synchronization in real epochs of sampling. Thus, the present proposal underlies a general system, whose main objective is to be a wireless benchmark in the field. In this way, this proposal could acquire and give some analysis tools for biomedical signals used for measuring brain interactions when it is stimulated by an external system during therapies, for example. Therefore, this system supports extreme environmental conditions, when necessary, which broadens the spectrum of its applications. In addition, in this proposal sensors could be added or eliminated depending on the needs of the research, generating a wide range of configuration limited by the number of CPU cores, i.e., the more biosensors, the more CPU cores will be required. To validate the proposed integrated system, it is used in a Dolphin-Assisted Therapy in patients with Infantile Cerebral Palsy and Obsessive-Compulsive Disorder, as well as with a neurotypical one. Event synchronization of sample periods helped isolate the same therapy stimulus and allowed it to be analyzed by tools such as the Power Spectrum or the Fractal Geometry.

When one wants to use citizen input to inform policy, what should the standards of informedness on the part of the citizens be? While there are moral reasons to allow every citizen to participate and have a voice on every issue, regardless of education and involvement, designers of participatory assessments have to make decisions about how to structure deliberations as well as how much background information and deliberation time to provide to participants. After assessing different frameworks for the relationship between science and society, we use Philip Kitcher's framework of Well-Ordered Science to propose an epistemic standard on how citizen deliberations should be structured. We explore what potential standards follow from this epistemic framework focusing on significance versus scientific and engineering expertise. We argue that citizens should be tutored on the historical context of why scientific questions became significant and deemed scientifically and socially valuable, and if citizens report that they are capable of weighing in on an issue then they should be able to do so. We explore what this standard can mean by looking at actual citizen deliberations tied to the 2014 NASA ECAST Asteroid Initiative Citizen forums. We code different vignettes of citizens debating alternative approaches for Mars exploration based upon what level of information seemed to be sufficient for them to feel comfortable in making a policy position. The analysis provides recommendations on how to design and assess future citizen assessments grounded in properly conveying the historical value context surrounding a scientific issue and trusting citizens to seek out sufficient information to deliberate.

Purpose: The purpose of this work was to analyze the expressions of matrix metalloproteinase 9 (MMP-9), calcyclin (S100A6), and cystatin S (CST4) in the tears of keratoconus (KC) patients. The correlations between the expressions of these proteins and the values of various ocular surface parameters were examined after accelerated corneal crosslinking (A-CXL) with pulsed ultraviolet light. Methods: This prospective, observational study enrolled patients with different grades of KC, scheduled to undergo the A-CXL procedure, as well as healthy subjects. Tear samples were analyzed by employing customized antibody microarray assays for MMP-9, S100A6, and CST4 proteins. The keratometry readings at the maximum keratometry (Kmax) and the simulated keratometry (SimK) values were obtained for examining the postoperative evolution of corneal topography. The state of the ocular surface was evaluated using the results of the Ocular Surface Disease Index (OSDI) questionnaire, tear osmolarity (OSM) test, Schirmer test (SCH), Tear Break Up Time (TBUT), tear clearance (CLR), and fluorescein (FLUO) and lissamine green (LG) corneal staining. Results: A total of 18 patients (22 eyes) and 10 healthy subjects were studied. The concentrations of MMP-9 and S100A6 decreased in tears, from 104.5 ± 78.98 ng/ml and 350.20 ± 478.08 ng/ml before the surgery to 48.7 ± 24.20 ng/ml and 55.70 ± 103.62 ng/ml, respectively, after 12 months of follow up. There were no changes in the CST4 concentration after 12 months of follow up (2202.75 ± 2863.70 versus 2139.68 ±2719.89 ng/ml). When the patients were divided into three groups according to the evolutive stage of KC, the trends for the three biomarkers in each group were the same as in the general group. Basal concentrations of MMP-9 and S100A6 from healthy subjects and KC patients were compared. The levels of MMP-9 and S100A6 in tears were (9.8 ± 5.11 and 104.55 ± 78.98 ng/ml, p<0.01; and 11.35 ± 3.18 and 350.26 ± 478.06 ng/ml, respectively, p<0.01). This was not the case for CST4, which did not exhibit statistically significant differences between the two groups (2261.94 ± 510.65 and 2176.73 ± 2916.27 ng/ml respectively, p=0.07). Conclusions: A-CXL promoted a decrease in the concentrations of MMP-9 and S100A6 in the tear film. This effect may be related to the restoration of corneal homeostasis and the consequent repair of the tissue damage caused by KC. Moreover, the A-CXL treatment did not produce lasting alterations in the ocular surface, and the values of the evaluated clinical parameters did not change significantly.

Purpose: The purpose was to select a simple and reproducible method for lipid measurements of human tears with ultrahigh-performance liquid chromatography-mass spectrometry (UHPLC-MS). Two sample preparation procedures were evaluated and compared: the Bligh and Dyer (BD) liquid-liquid extraction method with chloroform and methanol and protein precipitation with isopropanol (IPA). Methods: Reproducibility and recovery efficiencies of 20 non-endogenous internal lipid standards were tested in 10-µl tear samples from healthy subjects. The lipid coverage and the simplicity of execution were also assessed. Lipid profiles of the tear extracts were acquired with UHPLC-MS, uhpland the lipids were identified using SimLipid software. Results: Both methods were robust producing good lipid coverage and reproducibility and high recovery efficiencies. The two protocols identified a 69-feature tear lipidome that covered 11 lipid classes from six different lipid categories. The main differences in recovery were due to the intrinsic lipid selectivity of each solvent. Although both methods were similarly efficient in recovering O-acyl-ω-hydroxy fatty acid (OAHFAs) and non-polar lipids, polar lipids were more efficiently recovered with IPA precipitation, which, in turn, exhibited higher reproducibility. In addition, IPA precipitation is automatable and simpler than the BD approach. Conclusions: IPA precipitation is an excellent procedure for extracting lipids from small tear volumes for quantitative large-scale, untargeted lipid profiling, which may be useful for identifying lipid biomarkers in tears from patients with different ocular surface pathologies, allowing personalized therapies to be designed.

Magnetic gels (ferrogels) are heterogeneous systems structured at the nanoscale that contains magnetic particles dispersed in three-dimensional networks of polymer chains. In the present work, the magnetic particles were synthesized with a core-shell structure, consisting of sepiolite particles covered by magnetite nanoparticles. These composite particles had a rod-like shape with a high aspect ratio. The obtained sepiolite-magnetite particles showed a high enough susceptibility and saturation magnetization. The magneto-rheological (MR) properties, and the intensity of the MR effect, of aqueous suspensions of the synthesized particles were studied. The particles, functionalized by adsorption of alginate molecules, were imbedded in alginate hydrogels to get homogeneous soft materials. The particles were linked to the polymer chains as the knots in a network and dominated in a great extent the mechanical properties of the materials. After determining the optimal compositions of the ferrogels, their viscoelastic properties were measured in the absence/presence of magnetic fields. The results pointed out that the MR effect provided by the clay-magnetite particles was considerably more intense than those achieved in ferrogels that contain spherical magnetic microparticles. Therefore, the imbedding of rod-shaped magnetic particles in hydrogels allows controlling the mechanical properties in a wider range than in conventional ferrogels. Anisotropy is an intrinsic feature of most of the human tissues (e.g. muscle, skin or cartilage). Because of this, there has been an intense effort in the search of methods for the induction of permanent anisotropy in hydrogels intended for biomedical applications. The dispersion of magnetic particles or beads in the hydrogel precursor solution prior to cross-linking, in combination with applied magnetic fields, which gives rise to columnar structures, is one of the most recently proposed approaches for this goal. We have gone even further and, in this paper, we show that it is possible to use magnetic particles as actuators for the alignment of the polymer chains in order to obtain anisotropic hydrogels. Furthermore, we characterize the microstructural arrangement and mechanical properties of the resulting hydrogels.
