RESUMO

Few long-term studies assess the discoloration induced by hydraulic calcium silicate-based cement on dental structures. In addition, as far as we know, no long-term study has assessed the discoloration induced by these cement on composite resin. This in vitro study aimed to assess, during a period of two years, the discoloration potential of different hydraulic calcium silicate-based cements (hCSCs) on the enamel/dentin structure and composite resin restoration. A total of 40 enamel/dentin discs were obtained from bovine incisors, and 40 composite resin discs (10 mm in diameter × 2 mm thick) were fabricated. A 0.8 mm-deep cavity was made in the center of each disc and filled with the following hCSCs (n=10): Original MTA (Angelus); MTA Repair HP (Angelus); NeoMTA Plus (Avalon); and Biodentine (Septodont). An initial color measurement was performed (T0 - baseline). After 7, 15, 30, 45, 90, 300 days, and two years, new color measurements were performed to determine the color (ΔE00), lightness (ΔL'), chroma (ΔC'), hue differences (ΔH'), and whiteness index (WID). For enamel/dentin, the ΔE00 was significant among groups and periods (p<0.05). NeoMTA Plus had the greatest ΔE00. The NeoMTA Plus group had the greatest ΔE00 after two years for composite resin. Significant reduction in lightness was observed for all groups after two years (p<0.05). The most significant WID values were observed after 30 days for Biodentine (enamel/dentin) and MTA Repair HP groups (composite resin) (p<0.05). The hCSCs changed the colorimetric behavior of both substrates, leading to greater darkening over time. The Bi2O3 in the Original MTA seems relevant in the short periods of color change assessment.

OBJECTIVE: Dornase alfa (rhDNase) reduces the viscosity of purulent sputum in the lungs. The use in patients with cystic fibrosis (CF) is proven. However, the evidence of its applicability to other conditions is limited. This study aims to present the authors' experience with the use of rhDNase in non-CF patients admitted to the Pediatric Intensive Care Unit (PICU). At the study center, rhDNase was used during flexible bronchoscopies in 24 cases, of which 20 (83%) had atelectasis and seven (29%) were admitted to PICU. Four patients (57%) were on invasive mechanical ventilation (MV). CASE DESCRIPTION: Two cases of daily rhDNase administration at PICU are presented: patient A was an 8-year-old boy admitted with septic shock and acute respiratory distress syndrome (ARDS). The patient required mechanical ventilation with aggressive settings and experienced several clinical complications. On D50, he started rhDNase treatment with an improvement in FiO2, PaCO2 and PaO2/FiO2 ratio according to radiologic findings. He was extubated on D23 of treatment.Patient B was a 17-month-old girl admitted with a convulsive status epilepticus who experienced respiratory complications (infectious and barotrauma) with ARDS, requiring aggressive ventilation. She initiated rhDNase treatment on D60. During the treatment an improvement in FiO2, PaO2/FiO2 ratio and a tendency of PaCO2 decrease were found. She had radiological improvement. No complications were described. COMMENTS: RhDNase may be a helpful and safe tool to use in PICU prolonged intubated patients with ventilator-induced lung injury. Further studies are needed to assess and propose valid indications.

INTRODUCTION: Fibrous dysplasia (FD) and aneurysmal bone cyst (ABC) are uncommon non-neoplastic intraosseous lesions, and the occurrence of concomitant FD and ABC is extremely rare. CASE REPORT: An 11-year-old boy presented with gradual progressive enlargement of his right zygomatic bone over 4 years prior to presentation. Computed tomography revealed a lesion with a central bony area showing a ground-glass appearance surrounded by a well-defined expansile lesion with internal septations. An incisional biopsy was performed and suggested a fibro-osseous lesion. Resection was performed, followed by immediate reconstruction using autogenous bone graft from the iliac crest. Histopathological examination revealed irregularly shaped trabeculae comprising immature woven bone in a fibroblastic cell-rich stroma. Blood-filled sinusoidal spaces lined by fibrous septa containing scattered multinucleated giant cells were observed peripherally. These findings were compatible with concomitant FD and ABC. The patient was disease-free at the time of his 10-month follow-up. CONCLUSIONS: Based on our literature search, this report is the first to describe concomitant monostotic FD and ABC in the zygomatic bone. Accurate diagnosis requires careful investigation and examination of clinical, radiographical, and histopathological features. The treatment of choice should provide an esthetic and functional improvement in the patient.

Two unrelated infants were diagnosed with and initially treated for hemophagocytic lymphohistiocytosis (HLH), but progressed to cholestasis and liver failure. Early onset lysosomal acid lipase deficiency (EO-LAL-D) was suspected due to lymphocytes with cytoplasmic vacuolation and/or adrenal calcifications and confirmed by enzymatic and genetic analysis. Enzyme replacement therapy with sebelipase alfa was implemented, but both children died, despite initial improvement. Since this inborn error of metabolism progresses rapidly in infants, early diagnosis is crucial, and appropriate treatment should be started as soon as possible. The authors suggest that the diagnosis of EO-LAL-D should be considered in infants with symptoms of HLH.

BACKGROUND: The clinical spectrum of congenital cystic adenomatoid malformation of the lung (CCAML) ranges from asymptomatic lesions to neonatal respiratory distress and hydrops fetalis. AIM: To review our experience with CCAML, emphasising natural history, management and outcome. MATERIAL AND METHODS: A retrospective chart review of all CCAML-diagnosed neonates admitted to the neonatal intensive care units of five tertiary medical centres in the north of Portugal between 1996 and 2005. RESULTS: Fifteen neonates with CCAML were identified, 9F/6M, birth weight 3100 g (645-3975), gestational age 38 weeks (24-40). The incidence of CCAML was 1: 9300 births. There were 11 (73%) cases of cystic lung lesion diagnosed during pregnancy, median age 22 weeks (19-30). The lesion was right sided in six (40%) and left sided in nine (60%) cases. In utero spontaneous regression of the lesion was observed in two cases. Antenatal intervention (pleurocentesis and thoracoamniotic shunting) was performed in one foetus with impending hydrops. Normal lung radiographic findings at birth were present in five cases, with an abnormal CT scan. Three (20%) neonates became symptomatic during the neonatal period (respiratory distress) and one (70%) after the neonatal period (spontaneous pneumothorax). Two neonates (13%) died. Six (40%) patients underwent thoracotomy and appropriate excisional surgery. Histological examination showed definitive features of CCAML (Stocker classification: type I = 4; type II = 1; type III = 2). Eight (53%) patients remain asymptomatic and did not undergo surgery. CONCLUSIONS: Antenatally diagnosed CCAML has a good prognosis in the absence of severe foetal distress; normal radiographic findings at birth do not rule out CCAML; treatment of asymptomatic CCAML is controversial; surgery may be advocated because of its low morbidity and the prevention of late complications such as malignancy.

Introdução: Na sociabilidade capitalista é necessário que o capital atenda a alguns interesses da classe trabalhadora, para que ela esteja em condições objetivas e subjetivas de ser explorada. As políticas públicas aparecem como uma forma de satisfazer alguns desses interesses, e são apresentados como direitos do cidadão. Objetivo: Este artigo empírico teve como objetivo refletir sobre os limites da política pública de ressocialização a partir do sistema prisional mineiro. Métodos: Os dados quantitativos e qualitativos foram coletados por pesquisa documental e observação participante com diário de campo construído in loco durante oito meses. Um levantamento bibliométrico foi realizado para averiguar o que a ciência expressa sobre o fenômeno em tela e o exame do desenvolvimento das categorias. Resultados: Apresentamos as limitações da política pública em cada um dos pilares para a ressocialização, e concluímos que as atividades no cárcere visam alterar as subjetividades das pessoas privadas de liberdade para que elas: i) se comportem como bons trabalhadores e estejam aptos à exploração direta ou indireta do Capital no cárcere; ii) sejam explorados pelo capital em condição de escravidão com a mediação do Estado. Conclusão: Por fim, reivindicamos a participação dos profissionais da área de Terapia Ocupacional que esteja radicalmente comprometido com o processo de ressocialização nesses espaços, pois sabemos que não é suficiente para superar as limitações da política pública, mas se torna uma forma de potencializar a quota parte do mais valor que está sendo devolvida à classe trabalhadora.


Introduction: In capitalist sociability it is necessary that the capital meets some interests of the working class, so that it is in objective and subjective conditions to be exploited. Public policies appear as a way to satisfy some of these interests and are presented as citizens' rights. Objective: This empirical article aimed to reflect on the limits of the public policy of resocialization from the prison system in Minas Gerais. Method: Quantitative and qualitative data were collected by documentary research and participant observation with a field diary built in loco during eight months. A bibliometric survey was conducted to ascertain what science expresses about the phenomenon in question and the examination of the development of the categories. Results: We present the limitations of public policy in each of the pillars for resocialization, and conclude that the activities in prison aim to alter the subjectivities of people deprived of freedom so that they: i) behave as good workers and are fit for direct or indirect exploitation of Capital in prison; ii) are exploited by Capital in a condition of slavery with the mediation of the State. Conclusion: Finally, we claim the participation of professionals in the field of Occupational Therapy who are radically committed to the process of resocialization in these spaces, because we know that it is not enough to overcome the limitations of public policy, but it becomes a way to potentiate the share of the most value that is being returned to the working class.


Introducción: En la sociabilidad capitalista es necesario que el capital responda a algunos intereses de la clase obrera, para que esté en condiciones objetivas y subjetivas de ser explotada. Las políticas públicas aparecen como una forma de satisfacer algunos de estos intereses, y se presentan como derechos de los ciudadanos. Objetivo: Este artículo empírico tiene como objetivo reflexionar sobre los límites de la política pública de resocialización desde el sistema penitenciario de Minas Gerais. Método: Los datos cuantitativos y cualitativos se recogieron mediante una investigación documental y una observación participante con un diario de campo construido in situ durante ocho meses. Se realizó una encuesta bibliométrica para conocer lo que la ciencia expresa sobre el fenómeno en pantalla y el examen del desarrollo de las categorías. Resultados: Presentamos las limitaciones de la política pública en cada uno de los pilares para la resocialización, y concluimos que las actividades en la cárcel tienen como objetivo alterar las subjetividades de las personas privadas de libertad para que: i) se comporten como buenos trabajadores y sean aptos para la explotación directa o indirecta del Capital en la cárcel; ii) sean explotados por el Capital en condición de esclavitud con la mediación del Estado. Conclusión: Por último, reivindicamos la participación de los profesionales del ámbito de la Terapia Ocupacional que están radicalmente comprometidos con el proceso de resocialización en estos espacios, porque sabemos que no basta con superar las limitaciones de las políticas públicas, sino que se convierte en una forma de potenciar la cuota de mayor valor que se está devolviendo a la clase trabajadora.

Neste texto, objetivamos analisar os conteúdos presentes na definição de sentido(s) do trabalho que possuem maior influência no campo das ciências sociais aplicadas no Brasil. Para tanto, efetuamos uma análise imanente dos artigos mais referenciados na área, identificados por meio de um estudo bibliométrico nos portais CAPES, Spell e SciELO, entre os anos de 2000 a 2017. A partir do levantamento bibliométrico, foi constatado que os estudos acerca do sentido do trabalho possuem dois autores basilares: Estelle Morin e Ricardo Antunes. Para Morin, o trabalho só adquire sentido se houver a intensificação do trabalho e ele for produtivo para o capital; para Antunes, o trabalho só tem sentido para além do capital. Por isso, é epistemologicamente incoerente dialogar os dois autores em um mesmo trabalho, pois um afirma o status quo e o outro o nega. A compreensão emanada dos textos analisados permitiu reafirmarmos a necessidade de resgatar a categoria "trabalho" em sua particularidade histórica, defendermos a postura filosófica da determinação ontoprática do pensamento e propormos reflexões sobre uma agenda de pesquisa.


In this text, we aim to analyze the contents present in the definition of meaning of work that have the greatest influence in the field of applied social sciences in Brazil. We performed an immanent analysis of the most referenced papers in the area identified through a bibliometric study in the portals CAPES, Spell and SciELO, between the years of 2000 to 2017. By the bibliometric survey it was verified that the studies about the "Meaning(s) of the Work" have two basic authors: Morin and Antunes. For Morin, the meanings of work only make sense if there is an intensification of labor and it is productive for capital; for Antunes, the meanings of labor only make sense beyond capital. Therefore, it is epistemologically impossible to discuss the two authors in the same work, as one affirms the status quo and the other denies it. The understanding emanating from the analyzed texts allowed us to reaffirm the need to rescue the "work" category from the perspective of political economy, to defend the philosophical stance of the ontopractic determination of thought and to propose a research agenda.

Este artigo tem o objetivo de realizar um levantamento da produção bibliográfica relativa ao tema "Trabalho prisional", que vem ganhando espaço na área de estudos organizacionais. Para tanto, foi realizada uma consulta ao Portal Capes, Scielo, Spell e Anpad, sendo incluídos artigos de revistas, bancos de dissertações, teses, notícias de jornais, livros, capítulos de livros e monografias, todos escritos e publicados nas Ciências Sociais aplicadas. Após os recortes empíricos, restaram 27 artigos para analisar e para compreender o trabalho prisional em sua totalidade. Na tentativa de apresentar os sentidos do labor encarcerado, o tema foi reorganizado em subtemas, quais sejam: o trabalho como meio de sobrevivência, o binômio da educação e trabalho, os egressos e o mercado de trabalho, e, por fim, os trabalhadores do sistema prisional brasileiro. A revisão da literatura permite concluir que a produção acadêmica, voltada para a compreensão dos significados do trabalho dentro do sistema prisional, destaca três elementos bastante proeminentes na teoria marxista: a humanização, a exploração e a alienação dos trabalhadores.


This article aims to carry out a review of the academic production related to the theme "Prison Work", a subject that has been gaining space in organizational studies. In order to fulfill this aim, a query was made in Portal Capes, Scielo, Spell and Anpad in search for journal articles, dissertations, theses, newspaper articles, books, book chapters and monographs, all written and published in applied social sciences. After reviewing the material gathered, we have discovered 27 articles to analyze in order to understand prison work as a whole. To present the meanings of the incarcerated labor, the theme was reorganized into sub-themes, namely: work as a means of survival, the binomial of education and work, graduates and the labor market, and, finally, prison system. The review of the literature allows us to conclude that the academic production aimed at understanding the meanings of work within the prison system highlights three very prominent elements of Marxist theory, namely: humanization, exploitation and alienation of workers.

Respiratory syncytial virus (RSV) lower respiratory tract infection is the most common viral respiratory infection in infants. Several authors have sought to determine which risk factors are the best predictors for severe RSV disease. Our aim was to evaluate if a specific chest radiographic pattern in RSV disease can predict the disease severity. We conducted a multicenter retrospective cohort study in term and preterm neonates with confirmed lower respiratory tract RSV infection, admitted to neonatal intensive care units (NICU) from 2000 to 2010. To determine which factors independently predicted the outcomes, multivariate logistic regression analysis was performed. A total of 259 term and preterm neonates were enrolled. Patients with a consolidation pattern on the chest radiograph at admission (n = 101) had greater need for invasive mechanical ventilation (OR: 2.5; P = .015), respiratory support (OR: 2.3; P = .005), supplemental oxygen (OR: 3.0; P = .008), and prolonged stay in the NICU (>7 days) (OR: 1.8; P = .025). Newborns with a consolidation pattern on admission chest radiograph had a more severe disease course, with greater risk of invasive mechanical ventilation, respiratory support, supplemental oxygen, and prolonged hospitalization.
