RESUMO

The aim of this study was to review and synthesize the existing knowledge of the effects of ultrafine particles [UFPs] with a specific focus on children's health. An extensive literature search identified 16 studies fulfilling the criteria set for the review. One of the most important findings of the review was that, in general, there is an association between children's health and exposure to UFPs, especially among children with respiratory diseases, who commonly experience alterations in inflammatory biomarkers and deterioration in lung function as a result of UFP exposure. Notably, the health effects of UFPs are related to their ability to penetrate through different systems of the body due to their small size.

The aim of this study was to investigate the autonomic modulation of heart rate in healthy individuals exposed to long-term air pollution through geometric methods. We analyzed data from 109 healthy adults aged 18 to 49, divided into three groups according to the exposure time: period 0 to 15 years of exposure (n = 29), more than 15 years of exposure (n = 31), and control group (n = 49). For the analysis of heart rate variability (HRV), heart rate was recorded beat-to-beat for 20 min in the sitting position. The RR intervals were transformed into geometric indexes, and from them, we calculated the RRTri (triangular index), TINN (triangle interpolation of histogram of intervals NN), and Poincaré plot (SD1, SD2, and SD1/SD2). Significantly lower values were observed in the group of individuals exposed to air pollution for more than 15 years compared with the group of individuals exposed to air pollution for a period of 0-15 years and those not exposed for the RRTri (11.5 vs 13.8 vs 14.0), SD1 (16.4 vs 20.5 vs 20.6), SD2 (60.5 vs 68.1 vs 72.5), and SD1/SD2 (0.27 vs 0.34 vs 0.31), with the effect of this difference being considered large (RRTri), medium (SD1, SD1/SD2), and small (SD2). TINN was not significantly different among groups (198.2 vs 223.1 vs 233.6). Healthy individuals exposed to air pollution for more than 15 years present an autonomic imbalance, characterized by lower parasympathetic modulation and overall HRV.

There are several papers in literature that prove in a conclusive and incontestable way, that the left branch of the His bundle, in most instances (85% of the cases) splits into three fascicles of variable morphological pattern, and not into two: left anterior fascicle (LAF), left posterior fascicle (LPF), and left septal fascicle (LSF). The abovementioned papers have anatomical, histological, anatomo-pathological, electrocardiographic, and vectocardiographic, body surface potential mapping or ECG potential mapping and electrophysiological foundation.Additionally, the mentioned papers have been performed both in animal models (dogs) and in the human heart.Several clinical papers have shown that the left septal fascicular block (LSFB) may occur intermittently or transitorily as a consequence of a temporary dromotropic alteration, constituting an aberrant ventricular conduction, rate-dependent or by the application of atrial extra-stimuli, or naturally during the acute phase of infarction when this involves the anterior descending artery, before the septal perforating artery that supplies the central portion of the septum, where the mentioned LSF runs.The ECG/VCG manifestation of LSFB consists in anterior shift of electromotive forces, known as Prominent Anterior Forces (PAF), which can hardly be diagnosed in the clinical absence of other causes capable of causing PAF, such as the normal variant by counterclockwise rotation of the heart on its longitudinal axis, in right ventricular enlargement, in the dorsal or lateral infarction of the new nomenclature, in type-A WPW, in CRBBB, and others. In this historical manuscript, we review in a sequential fashion, the main findings that confirmed the unequivocal existence of this unjustifiably "forgotten" dromotropic disorder.In the developed countries, its most important cause is coronary insufficiency, particularly the proximal involvement of the left anterior descending coronary artery, and in Latin America, Chagas disease.

Until the mid-1980s, it was believed that the vectorcardiogram presented a greater specificity, sensitivity and accuracy in comparison to the conventional electrocardiogram, in the diagnosis of the different heart diseases. Recent studies revealed that the vectorcardiogram still is superior to the electrocardiogram in very specific situations, such as in the evaluation of electrically inactive areas, in intraventricular conduction disorders combined and/or in association to inactive areas, in the identification and location of ventricular preexcitation, in the differential diagnosis of patterns varying from normal of electrical axis deviation, in the evaluation of particular aspects of Brugada syndrome, and in the estimation of the severity of some enlargements, among others. With the advent of computerized vectorcardiography, a technology that improves the processing and recording method; a future still promising is expected for this methodology. In the fields of education and research, vectorcardiography provided a better and more rational insight into the electrical phenomena that occurs spatially, and represented an important impact on the progress of electrocardiography. Although a few medical centers still use the method as a routine, we hope that the use of this resource will not get lost over time, since vectorcardiography still represents a source to enrich science by enabling a better morphological interpretation of the electrical phenomena of the heart.

BACKGROUND: Burnout syndrome can be defined as long-term work stress resulting from the interaction between constant emotional pressure associated with intense interpersonal involvement for long periods of time and personal characteristics. We investigated the prevalence/propensity of Burnout syndrome in clinical nurses, and the factors related to Burnout syndrome-associated such as socio-demographic characteristics, work load, social and family life, leisure activities, extra work activities, physical activities, and work-related health problems. METHOD: We conducted a cross-sectional, quantitative, prospective epidemiological study with 188 surgical clinic nurses. We used the Maslach Burnout Inventory (MBI), which is a socio-demographic questionnaire and the most widely used instrument to assess Burnout syndrome (three basic dimensions: emotional exhaustion, despersonalization and professional underachievement). The socio-demographic profile questionnaire wascomposed of questions regarding identification, training, time at work, work characteristics and personal circumstances. RESULTS: The prevalence of Burnout syndrome was higher (10.1%) and 55, 4% of subjects had a propensity to develop this syndrome. The analysis of the socio-demographic profile of the nurse sample studied showed that most nurses were childless married women, over 35 years of age, working the day shift for 36 hours weekly on average, with 2-6 years of post-graduation experience, and without extra employments. Factors such as marital status, work load, emotion and work related stress aggravated the onset of the syndrome. CONCLUSION: The prevalence and propensity of Burnout syndrome were high. Some factors identified can be useful for the adoption of preventive actions in order to decrease the prevalence of the clinical nurses Burnout syndrome.

BACKGROUND: Memantine attenuates heart stress due cold stress, however, no study focused its effects on liver and adrenal gland. We evaluated its effects on lipid depletion in adrenal gland and glycogen depletion in liver of rats exposed to cold stress. METHODS: Male rats divided into 4 groups: 1)Control (CON); 2)Memantine (MEM); 3)Induced cold stress (IH) and; 4)Induced cold stress memantine (IHF). Memantine were administrated by gavage (20 mg/kg/day) during eight days. Cold stress were performed during 4 hours once at - 8°C. Lipid and glycogen depletion were presented as its intensity levels. RESULTS: Rats exposed to cold stress presented the highest glycogen (p < 0.001) and lipid depletion (p < 0.001) in liver and adrenal gland, respectively. We noted that memantine significantly reduced lipid depletion in adrenal gland and glycogen depletion in liver. CONCLUSION: Memantine prevented glycogen depletion in liver and lipid depletion in adrenal gland of rats under a cold stress condition.

This anesthetic drug may cause a rare condition named propofol infusion syndrome, characterized by unexplained lactic acidosis, lipemia, rhabdomyolysis, cardiovascular collapse and Brugada-like electrocardiographic pattern or Brugada electrocardiographic phenocopy changes following high-dose propofol infusion over prolonged periods of time. Several articles have contributed to our understanding of the cause of the syndrome, and the growing number of case reports has made it possible to identify several risk factors. Uncertainty remains as to whether a genetic susceptibility exists. The favorable recovery profile associated with propofol offers advantages over traditional anesthetics in clinical situations in which rapid recovery is important. Propofol is a safe anesthetic agent, but propofol infusion syndrome is a rare lethal complication.

Previous events evidence that sudden cardiac death (SCD) in athletes is still a reality and it keeps challenging cardiologists. Considering the importance of SCD in athletes and the requisite for an update of this matter, we endeavored to describe SCD in athletes. The Medline (via PubMed) and SciELO databases were searched using the subject keywords "sudden death, athletes and mortality". The incidence of SCD is expected at one case for each 200,000 young athletes per year. Overall it is resulted of complex dealings of factors such as arrhythmogenic substrate, regulator and triggers factors. In great part of deaths caused by heart disease in athletes younger than 35 years old investigations evidence cardiac congenital abnormalities. Athletes above 35 years old possibly die due to impairments of coronary heart disease, frequently caused by atherosclerosis. Myocardial ischemia and myocardial infarction are responsible for the most cases of SCD above this age (80%). Pre-participatory athletes' evaluation helps to recognize situations that may put the athlete's life in risk including cardiovascular diseases. In summary, cardiologic examinations of athletes' pre-competition routine is an important way to minimize the risk of SCD.

A 32 year-old Caucasian male, an elite athlete, was admitted to the emergency department because of a sudden onset of palpitations which had lasted more than 12 hours and were associated with chest discomfort. He had a two-year history of recurrent stress-induced palpitations. He denied either episodes of syncope or any family history of sudden death. Physical examination was normal. He had no evidence of structural heart disease. The electrocardiography (ECG) documented during the event supported the diagnosis of idiopathic reentrant left ventricular tachycardia. Ventricular tachycardia ablation was successful. This case demonstrates that a careful physical examination and correct ECG diagnosis can lead to an appropriate arrhythmia management.

RESUMO

The U wave is the last, inconstant, smallest, rounded and upward deflection of the electrocardiogram. Controversial in origin, it is sometimes seen following the T wave with the TU junction along the baseline or fused with it and before P of the following cycle on the TP segment. In this review we will study its temporal location related to monophasic action potential, cardiac cycle and heart sounds, polarity, voltage or amplitude, frequency and shapecontour. We will analyze the clinical significance of negative, alternant, prominent U wave, and the difference between T wave with two peaks (T1-T2) and true U wave. Finally we will analyze the four main hypotheses about the source of U wave: repolarization of the intraventricular conducting system or Purkinje fibers system, delayed repolarization of the papillary muscles, afterpotentials caused by mechanoelectrical hypothesis or mechanoelectrical feedback, and the prolonged repolarization in the cells of the mid-myocardium ("M-cells").

RESUMO

O aumento da incidencia de doenças imunodeficientes vem sendo responsavel pelo aparecimento de diversas afecçoes com evoluçao pior e diferente da habitualmente conhecida. E apresentado um caso de histoplasmose em paciente HIV+ que comprometeu o colon em dois segundos, levando a quadro suboclusivo, cuja clinica, propedeutica e aspecto per-operatorio ou macroscopico sugeriam cancer em fase avançada com comprometimento de ganglios peritumorais e a distancia. Foi realizado estudo microscopico com coloraçao especial, que revelou presença do Histoplasma capsulatum dentro das celulas
