RESUMO

Targeted antitumour therapy has revolutionized the treatment of several types of tumours. Among the validated targets, phosphatidylinositol-3 kinase (PI3K) deserves to be highlighted. Several PI3K inhibitors have been developed for the treatment of cancer, including gedatolisib (4). This inhibitor was elected as a prototype and molecular modifications were planned to design a new series of simplified gedatolisib analogues (5a-f). The analogues were synthesised, and the comparative cytotoxic activity profile was studied in phenotypic models employing solid and nonadherent tumour cell lines. Compound 5f (LASSBio-2252) stood out as the most promising of the series, showing good aqueous solubility (42.38 µM (pH = 7.4); 39.33 µM (pH = 5.8)), good partition coefficient (cLogP = 2.96), cytotoxic activity on human leukemia cell lines (CCRF-CEM, K562 and MOLT-4) and an excellent metabolic stability profile in rat liver microsomes (t1/2 = 462 min; Clapp = 0.058 mL/min/g). The ability of 5f to exert its cytotoxic effect through modulation of the PI3K pathway was demonstrated by flow cytometry analysis in a comparative manner to gedatolisib.

INTRODUCTION: Diabetic obese patients are susceptible to the development of cardiovascular disease, including hypertension and cardiac dysfunction culminating in diabetic cardiomyopathy (DC), which represents a life-threatening health problem with increased rates of morbidity and mortality. The aim of the study is to characterize the effects of a new benzofuran N-acylhydrazone compound, LASSBio-2090, on metabolic and cardiovascular alterations in Zucker diabetic fatty (ZDF) rats presenting DC. METHODS: Male non-diabetic lean Zucker rats (ZL) and ZDF rats treated with vehicle (dimethylsulfoxide) or LASSBio-2090 were used in this study. Metabolic parameters, cardiovascular function, left ventricle histology and inflammatory protein expression were analyzed in the experimental groups. RESULTS: LASSBio-2090 administration in ZDF rats reduced glucose levels to 85.0 ± 1.7 mg/dL (p < 0.05). LASSBio-2090 also lowered the cholesterol and triglyceride levels from 177.8 ± 31.2 to 104.8 ± 5.3 mg/dL and from 123.0 ± 11.4 to 90.9 ± 4.8 mg/dL, respectively, in obese diabetic rats (p < 0.05). LASSBio-2090 normalized plasma insulin, insulin sensitivity and endothelial function in aortas from diabetic animals (p < 0.05). It also enhanced systolic and diastolic left-ventricular function and reverted myocardial remodeling by blocking the threefold elevation of TNF-α levels in hearts from ZDF rats. CONCLUSION: LASSBio-2090 alleviates metabolic disturbance and cardiomyopathy in an obese and diabetic rat model, thus representing a novel strategy for the treatment of cardiovascular complications in obesity-associated type 2 diabetes mellitus.

BACKGROUND: Increased mortality due to type 2 diabetes mellitus (T2DM) has been associated with renal and/or cardiovascular dysfunction. Dipeptidyl dipeptidase-4 inhibitors (iDPP-4s) may exert cardioprotective effects through their pleiotropic actions via glucagon-like peptide 1-dependent mechanisms. In this study, the pharmacological profile of a new iDPP-4 (LASSBio-2124) was investigated in rats with cardiac and renal dysfunction induced by T2DM. METHODS: T2DM was induced in rats by 2 weeks of a high-fat diet followed by intravenous injection of streptozotocin. Metabolic disturbance and cardiac, vascular, and renal dysfunction were analyzed in the experimental groups. RESULTS: Sitagliptin and LASSBio-2124 administration after T2DM induction reduced elevated glucose levels to 319.8 ±â€¯13.2 and 279.7 ±â€¯17.8 mg/dL, respectively (p < 0.05). LASSBio-2124 also lowered the cholesterol and triglyceride levels from 76.8 ±â€¯8.0 to 42.7 ±â€¯3.2 mg/dL and from 229.7 ±â€¯25.4 to 100.7 ±â€¯17.1 mg/dL, in diabetic rats. Sitagliptin and LASSBio-2124 reversed the reduction of the plasma insulin level. LASSBio-2124 recovered the increased urinary flow in diabetic animals and reduced 24-h proteinuria from 23.7 ±â€¯1.5 to 13.3 ±â€¯2.8 mg (p < 0.05). It also reduced systolic and diastolic left-ventricular dysfunction in hearts from diabetic rats. CONCLUSION: The effects of LASSBio-2124 were superior to those of sitagliptin in the cardiovascular systems of T2DM rats. This new prototype showed promise for the avoidance of comorbidities in a T2DM experimental model, and thus may constitute an innovative therapeutic agent for the treatment of these conditions in the clinical field in future.

Abstract Biofilm has a primary role in the pathogenesis of diseases and in the attachment of multicellular organisms to a fouled surface. Because of that, the control of bacterial biofilms has been identified as an important target. In the present study, five lipid compounds isolated from soft coral Eunicea sp. and three terpenoids together with a mixture of sterols from Eunicea fusca collected at the Colombian Caribbean Sea showed different effectiveness against biofilm formation by three marine bacteria associated with immersed fouled surfaces, Ochrobactrum pseudogringnonense,Alteromona macleodii and Vibrio harveyi, and against two known biofilm forming bacteria, Pseudomonas aeruginosa ATCC 27853 and Staphylococcus aureus ATCC 25923. The pure compounds were characterized by NMR, HRESI-MS, HRGC-MS and optical rotation. The most effective compounds were batyl alcohol (1) and fuscoside E peracetate (6), acting against four strains without affecting their microbial growth. Compound 1 showed biofilm inhibition greater than 30% against A. macleodii, and up to 60% against O. pseudogringnonense,V. harveyi and S. aureus. Compound 6 inhibited O. pseudogringnonense and V. harveyi between 25 and 50%, and P. aeruginosa or S. aureus up to 60% at 0.5 mg/ml. The results suggest that these compounds exhibit specific biofilm inhibition with lower antimicrobial effect against the bacterial species assayed.

INTRODUCTION: Highly active antiretroviral therapy (HAART) has been administered to children infected with human immunodeficiency virus (HIV) since 1996. This kind of therapy is effective in achieving viral suppression and stopping disease progression but prolonged administration increases the risk of toxic effects, favours the onset of viral resistance and leads to decreased adherence. The aim of the present study was to determine prognostic factors among clinical, immunological and virological parameters at the beginning of HAART. POPULATION AND METHODS: We performed a prospective-retrospective observational analysis of a cohort or 564 HIV+ children assisted in Hospitals of Buenos Aires and Rosario, Argentina, treated with HAART since 1998 (media of treatment: 46.78 months. Range: 2-91 months). Patients were divided in groups according to age (younger or older than one year), and outcome (favourable or unfavourable). Stage, CD4 lymphocytes percentage, CD4 lymphocyte cell count and viral load at the beginning of treatment were analyzed with outcome by means of chi(2) tests, and logistic regression. RESULTS: No differences were observed on the percentage of CD4 T cells and viral load at baseline, between children under one year of age with good (n= 79) or bad outcomes (n= 4). Among older children (450 with good outcome, 31 with unfavourable), the following were identified as predictors of bad outcome: HAART initiation during stage C (p= 0.006), CD4 T-cell percentage below 15 percent (p< 0.001) and CD4 absolute value below 500 cells/mm(3) (p= 0.003). CONCLUSIONS: Children older than one year will have better outcome when HAART is initiated before stage C, with more than 15% CD4 or more than 500 cells/mm(3).

Introducción. Desde 1996, los niños infectados por el virus de la inmunodeficiencia humana reciben tratamientos antirretrovirales denominados de gran actividad (TARGA). El inicio tardío puede restarle eficacia; la administración prolongada aumenta el riesgo de episodios adversos y desarrollo de resistencia, y dificulta la adherencia. Nuestro objetivo fue establecer si existe relación entre los parámetros clínicos, inmunológicos y virológicos al inicio del tratamiento TARGA, y la evolución de los pacientes. Población, material y métodos. Estudio retrospectivo-prospectivo observacional de una cohorte de niños VIH positivos tratados con TARGA a partir de 1998 (n= 564) en hospitales de Buenos Aires y Rosario (promedio de tratamiento: 46,78 meses. Intervalo: 2-91 meses). Se los agrupó según edad (menor o mayor de un año) y evolución (favorable o desfavorable). Se correlacionaron el estadio clínico, porcentaje de linfocitos CD4 y carga viral al comienzo del tratamiento con la evolución. Resultados. No hubo diferencias entre porcentaje y recuento de CD4 y carga viral al inicio entre los niños menores de un año con buena (n= 79) o mala evolución (n: 4). Entre los niños mayores (450 con buena evolución, 31 con evolución desfavorable), fueron predictores de mala evolución al iniciar TARGA, el compromiso clínico grave (estadio C) (p= 0,006), CD4 menor 15 por ciento (p< 0,001) y recuento de CD4 menor de 500 células/mm3 (p= 0,003). Conclusiones. Los niños mayores de un año tienen mejor pronóstico cuando empiezan tratamiento en estadios previos al C, con CD4 > 15 por ciento o más de 500 células CD4/mm3.

Se estudiarion dieciocho lactantes menores de un añofallecidos por una infección respiratoria baja (IRB): neumonitis, neumonía o bronconeumonía (grupo de estudio) y un grupo control (n=18) con igual diagnóstico y sobrevivientes, apareados por edad y sexo, internados en el Hospital de Pediatría Garrahan entre 1987 y 1989. Se comprobó que la mayoría de los casos provenía de áreas con alto porcentaje (>30 por ciento) de familias con necesidades básicas insatisfechas (NBI). Hubo, en ambos grupos, elevada frecuencia de antecedentes adversos en la salud materno-infantil: embarazo no controlado (n=6), desnutrición (n=19), vacunación incompleta (n=15) e internaciones previas por infecciones respiratorias (n=9) siendo la frecuencia de las tres últimas variables desfavorables significativamente mayor en el grupo de estudio. En la mayoría de los casos (n=24) la evoluci'on ambulatoria de la IRB fue de 3 o más días; todos presentaron durante esta etapa dificultad respiratoria, pero sólo en el grupo fallecido tos cianozante (n=7) y apneas (n=4). A pesar de estos signos, la mayoría de los pacientes (n=29) fue atendida ambulatoriamente. Los pacientes fallecidos presentaron mayor gravedad al ingreso, expresada por una mayor frecuencia de hipercapnia (n=16), estadísticamente significativa. En la anatomía patológica predominó la bronconeumonía bacteriana inespecífica, asociada, en algunos casos, a sepsis y neumonitis viral. Estos resultados demuestran que en la población internada por IRB existe una elevada frecuencia de antecedentes adversos, siendo éstos más frecuentes y severos en el grupo fallecido. Asimismo se comprobó insuficiente atención de la enfermedad actual, no relacionada en sus acciones con los factores de riesgo ni con la severidad de los signos presentes. Los resultados señalan la necesidad de generar acciones preventivas y curativas que reorganicen y mejoren el sistema de atención materno-infantil a fin de disminuír la mortalidad infantil debida a IRB.

RESUMO

Estudo prospectivo de 157 recém-natos e suas mäes adolescentes, atnedidas no serviço de prinatologia do CHDSGP (agosto 1987 a março 1988). Observou-se que a gestäo da adolescente, tanto para ela como para seu concepto, é de igual risco em qualquer faixa etaria dentro da adolescência. Nesta amostra a maior parte dos casos teve assistência pré-natal adequada, sem intercorrência perinatais, com RN de peso normal, a termo e adequados para a idade gestacional
