BACKGROUND: Traditional clinical trials are conducted at investigator sites. Participants must visit healthcare facilities several times for the trial procedures. Decentralized clinical trials offer an interesting alternative. They use telemedicine and other technological solutions (apps, monitoring devices or web platforms) to decrease the number of visits to study sites, minimise the impact on daily routine, and decrease geographical barriers for participants. Not much information is available on the use of decentralization in randomized clinical trials with vaccines. METHODS: A hybrid clinical trial may be assisted by parental recording of symptoms using electronic log diaries in combination with home collected nasal swabs. During two influenza seasons, children aged 12 to 35 months with a history of recurrent acute respiratory infections were recruited in 12 primary health centers of the Valencia Region in Spain. Parents completed a symptom diary through an ad hoc mobile app that subsequently assessed whether it was an acute respiratory infection and requested collection of a nasal swab. Feasibility was measured using the percentage of returned electronic diaries and the validity of nasal swabs collected during the influenza season. Respiratory viruses were detected by real-time PCR. RESULTS: Ninety-nine toddlers were enrolled. Parents completed 10,476 electronic diaries out of the 10,804 requested (97%). The mobile app detected 188 potential acute respiratory infections (ARIs) and requested a nasal swab. In 173 (92%) ARI episodes a swab was taken. 165 (95.4%) of these swabs were collected at home and 144 (87.3%) of them were considered valid for laboratory testing. Overall, 152 (81%) of the ARIs detected in the study had its corresponding valid sample collected. CONCLUSIONS: Hybrid procedures used in this clinical trial with the influenza vaccine in toddlers were considered adequate, as we diagnosed most of the ARI cases on time, and had a valid swab in 81% of the cases. Hybrid clinical trials improve participant adherence to the study procedures and could improve recruitment and quality of life of the participants and the research team by decreasing the number of visits to the investigator site. This report emphasises that the conduct of hybrid CTs is a valid alternative to traditional CTs with vaccines. This hybrid CT achieved high adherence of participant to the study procedures. Lower-limb dynamics of muscle oxygen saturation during the back-squat exercise: effects of training load and effort level. J Strength Cond Res 34(5): 1227-1236, 2020-The aim of this study was to analyze the effect of strength training on lower limb muscle oxygenation. The sample consisted of 12 male subjects (22.4 ± 1.73 years; 1.81 ± 0.08 cm height and 77.76 ± 8.77 kg body mass). Six different strength training stimuli were analyzed, based on the training variables: load (60-75% 1 repetition maximum [1RM]) and level of effort (LE) (E1: 4 × 8 [20RM], E2: 4 × 12 [20RM], E3: 4 × 16 [20RM], E4: 4 × 4 [10RM], E5: 4 × 6 [10RM], and E6: 4 × 8 [10RM]) in the squat exercise up to 90° with a 2-second stop between repetitions to avoid the myotatic reflex. Oxygen saturation at the beginning of the series (SmO2start), oxygen saturation at the end of the series (SmO2stop), percentage of oxygen saturation loss (▽%SmO2), and reoxygenation time (SmO2recT) were assessed using a near-infrared spectroscopy device. In addition, the percentage of mean propulsive velocity loss (%MPVL) was recorded using a linear transducer. The results suggested an influence of LE and training load on muscle oxygenation. A greater LE was directly associated with SmO2recT (r = 0.864), ▽%SmO2 (r = 0.873), and %MPVL (r = 0.883) and inversely with SmO2stop (r = -0.871). When the same LE was used (E1 vs. E4, E2 vs. E5, and E3 vs. E6), it was found that the stimuli with a higher load had a lower SmO2recT, ▽%SmO2, and %MPVL and a higher SmO2stop. Muscle oxygen saturation was found to be minimal (%SmO2 = 0) in stimuli with a LE greater than 60% (E3 and E6). The SmO2 variables studied in the present research could be considered as an easier and more useful method for understanding skeletal muscle fatigue during resistance training.

OBJECTIVES: (1) To describe the fast Fourier transform (FFT) multijoint as monopodal postural stability measurement in well-trained athletes, (2) to compare the within-subject FFT between laterality, joints, and body segments, and (3) to establish the within- and between-subject relationship between joints. METHODS: Twelve national-level basketball players participated voluntarily in this investigation. The participants performed two 60-second repetitions of a monopodal stability test (1 repetition with each lower limb), separated by 3 minutes of active recovery. All tests were recorded by 4 WIMU PRO™ inertial devices located on the ankle, knee, lumbar spine, and thoracic spine. The main variable was total acceleration, where the FFT was applied. RESULTS: The higher instability results were found in the ankle and in the nondominant lower limb (dominant = 1.131 [0.122] a.u. (arbitrary units); nondominant = 1.141 [0.172] a.u). In the body segment analysis, the greater percentage of differences (%diff) were shown between lumbar spine and knee in the dominant (%diff = -2.989%; d = 0.87) and nondominant (%diff = -3.243%; d = 0.90) lower limb. Finally, very large between-subjects variability was found in all joints and body segments. CONCLUSIONS: The described protocol is proposed for monopodal postural stability assessment, being useful to provide information about the stability of joints and the body segment between joints. Besides, a within-subject analysis is recommended, and the FFT calculation will enable a linear analysis of each test.

UNLABELLED: Aging is directly related with loss of physical independency. Composite Physical Function questionnaire (CPF) assess, throw 12 items, a range of daily life activities in order to determine dependency levels in elderly. However, there is not a Spanish version of this instrument. AIM: To translate and culturally adapt the CPF to Spanish for its use in Chilean elderly. METHOD: Standardized international methodology was used in this study, which consisted in double direct translation to Spanish, harmonization of versions and back-translation to English. Acceptability and familiarity of the obtained version was analyzed using probing and paraphrasing methods using a sample of 20 older adults aged from 65 to 80 years old. RESULTS: All items were clear and understandable, although minor adaptations needed to be done in order to improve the understandability of two items. These adaptations consisted in adding information in brackets at the end of the sentence. CONCLUSION: Spanish version of the CPF questionnaire was obtained to its use in Chile. This questionnaire has been proved to be understandable and adapted to its use in Chilean older adults. Its ease of use makes this questionnaire potentially useful in future researches and surveys.

The unprecedented COVID-19 health crisis severely disrupted global sports in 2020, prompting lengthy suspensions followed by resumed competitions under abnormal behind-closed-doors conditions without fans. These disruptions necessitated tactical adaptations by coaches and teams, attempting to still achieve successful outcomes. This study investigates the pandemic's impacts on performance metrics and indicators within Spanish professional soccer. Utilizing systematic notational analysis, 760 match cases from the 2019-2020 La Liga season were examined, comprising 27 matchdays from the pre-COVID context and 11 after resumption. Multivariate tests identified significant pre/post differences and interactions for various technical indicators including shots, cards, corners, and offside calls. The pandemic was associated with a reduction from 12 to just 5 identifiable playing styles, suggestive of increased conservatism featuring more passive play, limited attacking depth, and horizontal ball movement. Such tactical changes appear provoked by condensed fixture scheduling post-lockdown, the lack of supportive crowds, and compromised player fitness/recovery. By quantifying these COVID-precipitated changes, the analysis provides tangible evidence for coaches to make informed adjustments in training and preparation for functioning effectively in disrupted environments. The findings emphasize that versatility and flexibility will be vital to optimize performance during times of unprecedented uncertainty.

The menstrual cycle can be seen as a potential determinant of performance. This study aims to analyze the influence of the menstrual cycle in women on sports performance, more specifically on the internal and external load of professional women basketball players. The sample consisted of 16 women players and 14 training sessions were recorded. A descriptive analysis of the mean and standard deviation of the variables according to the different phases of the menstrual cycle was performed, as well as an ANCOVA, partial Eta2 effect size criteria, and Bonferroni's Post Hoc test to identify differences among phases. The results establish that ovulation is the phase in which higher values of external load are recorded and, therefore, the late follicular phase is the time of the cycle where a greater intensity in explosive distance, accelerations and decelerations are recorded. Considering women's hormonal cycles, understanding their function and the individual characteristics of each athlete is essential since it allows for the development of specific training, the prevention of injuries and therefore positively affects the performance of women players. To this end, individual training profiles should be created in specific contexts, not following general rules. In addition, psychological factors and the specific position of the athletes should be monitored.

To carry out research that analyzes performance indicators through observational methodology, it is necessary to have validated tools. The purpose of this study was to design and validate a tool to ascertain the characteristics of the strokes that padel pairs use to reach the net and their consequences in the two subsequent shots of the game. Eleven experts, who had to meet four of the five inclusion criteria established by the researchers, participated in the process. Aiken's V coefficient and confidence intervals were used to analyze content validity and Cronbach's α coefficient to calculate reliability. The adequacy and wording of the sixteen variables initially designed were evaluated. Four variables were eliminated due to obtaining values <0.90 in Aiken's V coefficient in adequacy. The rest of the variables were modified in their wording, according to the qualitative evaluations of the experts, or were considered correct. The reliability of the instrument was acceptable (α = 0.89). The NAPOA instrument is novel, as it is of interest to analyze the characteristics of the strokes that padel players use to achieve the offensive position.

The load in tasks planned for sports teaching in physical education classes has received little attention. The purpose of this study was therefore to analyze the external load, eTL, in the tasks designed by physical education teachers from the in-service and pre-service stages, for teaching handball in primary education, and to compare them with the tasks included in the lesson plans designed for handball using the tactical games teaching model. An associative, comparative and cross-sectional methodology was used. Twenty-three teachers, five in the in-service phase and eighteen in the pre-service phase, designed lesson plans for teaching handball, which were compared with lesson plans validated by a panel of experts. The analysis was performed on 1,232 tasks or analysis units. eTL was categorized using the Integrated analysis system of training tasks (SIATE) instrument. A descriptive and associative analysis was made of the variables that make up the eTL and an inferential analysis of the eTL using non-parametric tests. The total eTL of the tasks planned by the in-service and pre-service teachers was low, and significantly lower than the tasks planned using the tactical games model, which showed a high level.

Assessment of the physical dimension implicit in the soccer match is crucial for the improvement and individualization of training load management. This study aims to: (a) describe the external match load at the amateur level, (b) analyze the differences between playing positions, (c) verify whether the home/away matches and if (d) the phase (first or second) of the championship influence the external load. Twenty amateur soccer players (21.5 ± 1.9 years) were monitored using the global positioning system. The external load was assessed in 23 matches, where 13 were part of the first phase of the competition (seven home and six away matches) and the other 10 matches belonged to the second (and final) phase of the championship (five home and five away matches). A total of 173 individual match observations were analyzed. The results showed significant differences between playing positions for all the external load measures (p < 0.001). There were higher values observed in the total distance covered for central defenders (p = 0.037; ES = 0.70) and in high-intensity decelerations for forwards (p = 0.022; ES = 1.77) in home matches than in away matches. There were higher values observed in the total distance (p = 0.026; ES = 0.76), relative distance (p = 0.016; ES = 0.85), and moderate-intensity accelerations (p = 0.008; ES = 0.93) for central defenders, in very high-speed running distance for forwards (p = 0.011; ES = 1.97), and in high-intensity accelerations (p = 0.036; ES = 0.89) and moderate-intensity decelerations (p = 0.006; ES = 1.11) for wide midfielders in the first phase than in the second phase of the championship. Match location and championship phase do not appear to be major contributing factors to influence the external load while the playing position should be used as the major reference for planning the external training load.

Ensuring adequate levels of training and recovery to maximize player performance is critical; however, there are methodological challenges in designing a periodized training program for soccer teams. This study aims to describe and characterize the daily and weekly external load in an amateur soccer team and based on the weighting factors determined by the match reference, compare the external loads between playing positions. Twenty-four amateur soccer players (22.3 ± 1.7 years) were monitored using a global positioning system. Data collected comprises 19 competitive microcycles with a standard structure composed of 3 training sessions (matchday-5, matchday-3, and matchday-2) and one match. Match-reference values were calculated as the mean of the five best values recorded during official matches. The results show, on matchday-5 session, the existence of significant differences between playing positions to relative total distance covered (p = 0.050), relative sprint distance (p = 0.001), relative moderate-intensity accelerations (p < 0.001), relative high-intensity accelerations (p = 0.003), relative moderate-intensity decelerations (p < 0.001), and relative high-intensity decelerations (p = 0.017). On matchday-3 session, there are significant differences to relative very high-speed running distance (p = 0.017) and relative moderate-intensity decelerations (p = 0.014). On matchday-2 session, there are significant differences to relative high-speed running distance (p = 0.025), relative very high-speed running distance (p = 0.008), and relative moderate-intensity decelerations (p < 0.001). Weekly significant differences are observed between the playing positions to relative moderate-intensity accelerations (p = 0.002), relative high-intensity accelerations (p < 0.001), and relative moderate-intensity decelerations (p < 0.001). The weekly load is characterized by a greater weighting on accelerations and decelerations, compared to distances at very-high speed and sprint. The training loads must respect a standard training model that contemplates the individualization of the physical demands of the match, for each playing position, as for each individual.

The implications of relative age grouping in sport are known as the Relative Age Effect (RAE). This study has the twofold purpose of analyzing RAE in Spanish youth national soccer teams and examining the prediction value of being selected for national youth teams to be a professional. The sample was composed of 548 players divided into five groups. A descriptive analysis of distribution and participation, frequencies, mean and standard deviation, crosstabs, Sankey charts, coefficient correlation and Cohen's effect size criteria and two regression analyses were performed. Results established that the RAE is present in U'17 to U'21 Spanish youth national teams. Talent detection and selection programs are more reliable the closer they are to adulthood, reaching a success rate of almost 100% at the U'21 stage. The selection of players for such programs should be delayed as much as possible, thus, preventing younger players from dropping out and those selected from thinking they have already reached their goal. To this end, they should focus on long-term improvement, not short-term performance. In addition, factors such as the RAE or the maturity level of the athletes should be monitored.

BACKGROUND: RSV is the leading cause of hospital admissions in infants and the principal cause of bronchiolitis in young children. There is a lack of granular data on RSV-associated hospitalization per season using laboratory confirmed results. Our current study addresses this issue and intends to fill this gap. METHODS: The study was conducted from 2014 through 2018, in 4 to 10 hospitals in the Valencia Region, Spain. Infants included in this study were admitted in hospital through the Emergency Department with a respiratory complaint and tested by RT-PCR for RSV in a central laboratory. RESULTS: Incidence rates of RSV-associated hospitalization varied by season and hospital. Overall, the highest incidence rates were observed in 2017/2018. RSV-associated hospitalization was highest in infants below 3 months of age and in those born before or at the beginning of the RSV season. Almost 54% of total infants hospitalized with laboratory confirmed RSV were found to be born outside the season, from April to October. The RSV positivity rate by ICD-10 discharged codes varied by season and age with results from 48% to 57% among LRI (J09-J22). CONCLUSION: The study was instrumental in bringing forth the time unpredictability of RSV epidemics, the critical impact of age, and the comparable distribution of RSV-associated hospitalization in infants born on either side of the RSV season. These data could help in better characterization of the population that drives the healthcare burden and is crucial for the development of future immunization strategies, especially with upcoming vaccines in against RSV.

On 9 March 2020, the World Health Organization (WHO) Global Influenza Programme (GIP) asked participant sites on the Global Influenza Hospital Surveillance Network (GIHSN) to contribute to data collection concerning severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). We re-analysed 5833 viral RNA archived samples collected prospectively from hospital admissions for influenza-like illness (ILI) in the Valencia Region of Spain by the Valencia Hospital Surveillance Network for the Study of Influenza and Other Respiratory Viruses (VAHNSI) network (four hospitals, catchment area population 1 118 732) during the pre-pandemic 2018/2019 (n = 4010) and pandemic 2019/2020 (n = 1823) influenza seasons for the presence of SARS-CoV-2. We did not find evidence for community-acquired SARS-CoV-2 infection in hospital admissions for ILI in our region before early March 2020.

This study was to analyze and compare the learning acquired by the students in the sport of basketball in two different methodologies. The sample was composed of 40 students divided into two groups. The intervention programs had previously been validated. A descriptive analysis of the learning indicators, a t-test for independent samples to identify the differences between the methods, and a t-test for related samples to analyze the differences in each group were performed. There are differences between the performance profiles of students in the Direct Instruction in Basketball program and those in the Tactical Game in Basketball program in nine variables. Significant differences are found in the situations of dribbling, shooting, passing and movement, spacing, off-ball defense, and help and in the performance indicator for decision making, execution, and total, which are favorable to the Tactical Game in Basketball program. The students of the Direct Instruction in Basketball program only improved in three variables after the program, while the Tactical Game in Basketball students improved in thirteen variables. It is recommended that the teachers at the schools use the Tactical Game in Basketball methodology for their basketball teaching lessons, because student learning is better than in the Direct Instruction in Basketball program.

In soccer, the assessment of the load imposed by training and a match is recognized as a fundamental task at any competitive level. The objective of this study is to carry out a systematic review on internal and external load monitoring during training and/or a match, identifying the measures used. In addition, we wish to make recommendations that make it possible to standardize the classification and use of the different measures. The systematic review was carried out according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. The search was conducted through the electronic database Web of Science, using the keywords "soccer" and "football", each one with the terms "internal load", "external load", and "workload". Of the 1223 studies initially identified, 82 were thoroughly analyzed and are part of this systematic review. Of these, 25 articles only report internal load data, 20 report only external load data, and 37 studies report both internal and external load measures. There is a huge number of load measures, which requires that soccer coaches select and focus their attention on the most useful and specific measures. Standardizing the classification of the different measures is vital in the organization of this task, as well as when it is intended to compare the results obtained in different investigations.

A better understanding of the demands of in-game competition demands may improve coaching strategies, training designs, and injury prevention programs. However, there is limited information regarding performance analysis in professional padel. This study aimed to analyse performance indicators and their influence on match outcomes regarding sex, tournament round, and set number. The sample contained 1070 sets from 532 matches of the 2016 to 2019 World Padel Tour seasons. Variables including sex, round, game result, stroke effectiveness, and break points were registered through systematic observation. A non-parametric approach was applied to evaluate differences between sex, match outcome, and tournament round. The results showed significant differences between winners and losers regarding sex in break points (male d = 2.13, p = 0.00; female d = 2.22, p = 0.00), smash winners (male d = 0.85, p = 0.00; female d = 0.69, p = 0.00), groundstroke winners (male d = 1.01, p = 0.00; female d = 1.18, p = 0.00), volley winners (male d = 1.08, p = 0.00; female d = 0.91, p = 0.00), and errors (male d = 0.76, p = 0.00; female d = 0.65, p = 0.00). Furthermore, differences in shot effectiveness between winners and errors increased in the last set of the match and in the last round of the tournament (p < 0.05). Therefore, shot effectiveness seems to be a key factor in professional padel that distinguishes between winning and losing players. Such knowledge may have implications in the design of appropriate game strategies and specific training sessions to improve performance and to prevent sport injuries.

Team sports are in continuous evolution, and physical performance is acquiring greater importance in the game. The assessment of physical fitness is the most reliable method for knowing if the athlete's physical fitness is appropriate. Therefore, the objectives of this research were to identify profiles of physical-physiological demands with different specific tests of physical fitness related to the maximum strength of the lower body and speed. Moreover, some differences based on the sex and age of the players were identified. One hundred and forty-nine basketball players were analyzed (men n = 103 vs. women n = 46, weight: 74.74 ± 11.65 kg vs. 56.89 ± 3.71 kg, height: 184.66 ± 11.67 vs. 164 ± 4 and BMI: 21.7 ± 0.83 vs. 21.6 ± 0.90). The players performed an Abalakov test, a multi-jump test, and a repeat sprint ability test. Each player was equipped with a WIMU PRO device, and photoelectric cells were used. A MANOVA was performed to discover the differences between ages, and a t-test was used regarding sex. The results showed significant differences based on age and sex in variables related to time and Player Load/min (p < 0.001). The variables related to steps (contact, step, flight) also showed significant differences as a function of age (p < 0.001) and sex (p < 0.05). The multi-jump test showed differences based on age (p < 0.05 except in between jumps), but not on sex. These results confirm the importance of working together on lower body strength and speed skills. In addition, the planning of the work on these abilities must be individualized and according to the characteristics of the athlete.

Influenza vaccination is annually recommended for specific populations at risk, such as older adults. We estimated the 2018/2019 influenza vaccine effectiveness (IVE) overall, by influenza subtype, type of vaccine, and by time elapsed since vaccination among subjects 65 years old or over in a multicenter prospective study in the Valencia Hospital Surveillance Network for the Study of Influenza and other Respiratory Viruses (VAHNSI, Spain). Information about potential confounders was obtained from clinical registries and/or by interviewing patients and vaccination details were only ascertained by registries. A test-negative design was performed in order to estimate IVE. As a result, IVE was estimated at 46% (95% confidence interval (CI): (16%, 66%)), 41% (95% CI: (-34%, 74%)), and 45% (95% CI: (7%, 67%)) against overall influenza, A(H1N1)pdm09 and A(H3N2), respectively. An intra-seasonal not relevant waning effect was detected. The IVE for the adjuvanted vaccine in ≥75 years old was 45% (2%, 69%) and for the non-adjuvanted vaccine in 65-74 years old was 59% (-16%, 86%). Thus, our data revealed moderate vaccine effectiveness against influenza A(H3N2) and not significant against A(H1N1)pdm09. Significant protection was conferred by the adjuvanted vaccine to patients ≥75 years old. Moreover, an intra-seasonal not relevant waning effect was detected, and a not significant IVE decreasing trend was observed over time.

Performance profiles have begun to be identified as extremely useful in order to help coaches individualize training according to the age and gender of athletes. Therefore, the aim of this study was to determine the activity demands and speed profile of U18 female basketball players during competitive matches. Time variables (real and playing time), distance variables (distance performed, distance in speed zones, high intensity distance and distance covered sprinting) and speed variables (number of sprints, sprint duration, maximum speed and average speed) were recorded from forty-eight players belonging to four teams (13 guards, 22 forwards and 13 centers). WIMUPROTM inertial measurement units with ultra-wide band (UWB) indoor-tracking technology recorded six matches during final four in the season 2018/2019. A one factor ANOVA with Cohen's effect sizes (d) were used to identify the differences between groups (playing position and match day). Distance per minute (123.96 vs 112.67 m), high intensity distance per minute (15.48 vs 14 m), running distance (403.2 vs 541.28 m) and average speed (5.05 vs 5.41 km/h) were significantly higher on day 3 than 1, respectively. About playing position, forwards played more minutes during games, so covered a greater distance, more sprints and high intensity actions than the rest. In spite of fatigue, day 3 showed a greater intensity than day 1, therefore, the last day was the crucial one for the teams in the tournament. Forwards when playing more minutes obtain higher absolute values ​​but not per minute which could mean a lower performance of the team.
