RESUMO

Providing knowledge and support for caring relatives with the smartphone - the MoCaB project Abstract. Background: Caring relatives often have a lack of knowledge of illness and care options. Surveys, as part of the MoCaB (Mobile Care Backup) project, showed that this is often experienced as a burden. Aim: The aim is to support caring relatives by increasing their knowledge. Therefore care-relevant knowledge and instructions for self-care should be provided according to their needs. Methods: The core element of the mobile application, personalized knowledge transfer in dialogue form, will be developed in a participative process with potential users. The relevant evidence-based nursing knowledge was reviewed by experts and written down for the target group. Results: So far, 86 relevant topics have been identified and formulated. First usability tests showed that the content itself, the used expressions and the presentation via the MoCaB app are well received. Outlook: In a next step, the app will be tested in the home setting with caring relatives and, in order to identify any further need and areas for improvement.

RESUMO

The development of new optical designs for intraocular lenses (IOL) is a major challenge for lens manufacturers, as postoperative patient satisfaction is difficult to assess a priori with new concepts. Until now, invasive clinical investigations were carried out for this purpose. In contrast, the simulated implantation of IOLs allows a risk-free determination of the achievable visual performance and a subjective evaluation of the visual impression of new optical concepts. For intraocular lens manufacturers, this offers, on the one hand, the possibility to take the subjective perception of test persons into account during the development and optimization of novel lens designs and, on the other hand, to carry out comparative performance tests with known products before the first implantation. By means of simulated implantation, subjectively optimized IOLs may lead to a better postoperative visual quality for the patients and minimize the risk of a cost-intensive development of products with clinically unacceptable visual perception achievements due to optical side effects. The application of the simulated implantation during the preoperative patient consultation may enable the determination of the patient's subjective visual preferences and result in a targeted IOL selection recommendation. In addition to an improved selection from an existing IOL portfolio, IOL manufacturers could also offer individualized multifocal IOLs that match the patient's previous preference pattern. This article explains the technical background and application scenarios of the simulated implantation and introduces current procedures and devices.

PURPOSE: To assess the reliability of a novel virtual IOL implantation device to predict the outcomes of a trifocal IOL. SETTING: Clinical sites in Spain and Germany. DESIGN: Prospective multicenter study. METHODS: Preoperative measurements obtained by the virtual implantation of AT LISA tri 839MP IOL (Carl Zeiss Meditec AG, Jena, Germany) with VirtIOL device (10Lens S.L.U., Terrassa, Spain) were compared to postoperative (month 3) outcomes following the IOL implantation. Evaluated metrics were defocus curve, contrast sensitivity (both in photopic conditions), and visual phenomena (visual angle size of burst and annular pattern) in mesopic conditions. RESULTS: The study included 45 presbyopic non-cataract patients (mean age 56.16±5.03 years). VirtIOL device reasonably predicted the shape of the defocus curve, especially in the intermediate vision range, but underestimated postoperative visual acuities mainly at the defocus range between +1.0D and -3.5D. The difference between predicted and achieved visual acuity was -0.11±0.13 logMAR (p<0.001) for 0.0D defocus and -0.16±0.15D logMAR (p<0.001) for -2.50D defocus. The shape of the contrast sensitivity curve was almost identical for predicted and achieved measurements, but the device underestimated the values by ≈0.3 logCS units for each spatial frequency. Visual phenomena were similar between the predicted and achieved outcome (predicted vs. achieved: burst pattern 2.65±0.97° vs. 2.74°±0.95°, p=0.63; annular pattern: 0.88±0.42° vs. 0.93±0.49°, p=0.59). CONCLUSION: The device reasonably predicted the pattern of measured variables. Visual acuity/contrast sensitivity reduction might be attributed to viewing the objects through a more complex optical system and would need to be accounted for in clinical studies.

RESUMO

Caregiving relatives have to manage very diverse tasks and need a lot of care-relevant knowledge. For most of them it is not easy to find the knowledge required. Thus, a personalized knowledge transfer for caregiving relatives is necessary. Against this background, methods to determine the personal relevance and importance of knowledge resources for caregiving relatives are developed. To evaluate these methods, an exemplary fictitious person is created by experts in Nursing Science. In this evaluation, the approach's results are compared with an expert opinion. The approach indicates that a personalized knowledge transfer is possible, providing caregiving relatives with necessary care knowledge according to their personal life situation.
