RESUMO

The relation between hypertension and cognitive impairment is an undisputable fact. The aims of this study were to determine the prevalence of cognitive impairment in hypertensive patients, to identify the most affected cognitive domain, and to observe the association with different parameters of hypertension and other vascular risk factors. A multicentre study was carried out, and 1281 hypertensive patients of both genders and ≥21 years of age were included. Data on the following parameters were obtained: cognitive status (Minimal Cognitive Examination), behavioural status (Hospital Anxiety and Depression Scale), blood pressure, anthropometry, and biochemical profile. The average age was 60.2±13.5 years (71% female), and the educational level was 9.9±5.1 years. Global cognitive impairment was seen in 22.1%, executive dysfunction in 36.2%, and semantic memory impairment in 48.9%. Cognitive impairment was higher in males (36.8% vs. 30.06%) within both the 70-79-year-old and the ≥80-year-old (50% vs. 40%) age groups. Abnormal Clock Drawing Test results were related to high pulse pressure (p<0.0036), and abnormal Mini-Boston Naming Test results to both high systolic blood pressure (p<0.052) and pulse pressure (p<0.001). The treated/uncontrolled hypertensive group showed abnormal results both in the Mini Mental State Examination (OR, 0.73; p=0.036) and the Mini-Boston Naming Test (OR, 1.36; p=0.021). Among patients without cognitive impairment (MMSE >24), 29.4% presented executive dysfunction, and 41.5% semantic memory impairment. Cognitive impairment was higher in hypertensive patients than in the general population. Executive functions and semantic memory were the most affected cognitive domains. High systolic blood pressure and pulse pressure were associated with abnormal results in cognitive tests.

Peritoneal carcinomatosis (PC) is a frequent terminal evolution from colorectal cancer. At the time of diagnosis of colon malignancies, PC affects approximately 10% of the patients [1]. Medical oncologists and gastrointestinal surgeons had considered it to be an untreatable condition for its unfavorable prognosis with a median survival of 6-9 months [1], suitable only for palliative treatment. Recurrence with carcinomatosis occurs in 25% of patients [2] and seems to be the only site of disease in 25-35% of these [3]. In a recent review of the literature on 12 trials, the incidence of peritoneal seeding during potentially curative surgery for PC from colorectal cancer varied from 3% to 28% [4]. Sugarbaker has suggested that peritoneal carcinomatosis is a locoregional cancer spread as a result of a molecular crosstalk between cancer cells and host elements [9]. Based on this concept and after Spratt first performed a debulking surgery (CR) followed by an intraperitoneal hyperthermic chemotherapy (HIPEC) in 1980 [5] and [6], numerous trials and investigations have been conducted on this technique [4]. Phase II studies on patients with PC of colorectal origin treated with CR plus HEPIC reported 5-year survival rates from 20% to 30% [7], while in a randomized controlled single-institution phase III trial this technique improved survival of patients affected by peritoneal carcinomatosis from colorectal malignancies (22.3 months versus 12.6 months) [8]. In all the series, an optimal cytoreduction is the most important prognostic factor for these patients [4]. The aim of this study is to demonstrate that peritoneum, as first line defense from carcinomatosis, is like an organ and so, the treatment of PC in selected patients should be a locoregional therapy with HIPEC in addition to CR [9].

BACKGROUND: Various intragastric balloons have been used in obese patients for temporary weight loss. Recently, a new balloon, the Heliosphere Bag, was proposed. In a preliminary study, we evaluated the safety and efficacy of this device. METHODS: The Heliosphere Bag was used in 10 patients, selected according to the guidelines for obesity surgery. The manufacturer's instructions were followed in positioning the device. Heliosphere Bag positioning was performed, after diagnostic endoscopy, under unconscious sedation. After placement, the balloon was slowly inflated with 840-960 cc of air, which gives the inflated final volume of 650-700 cc of air, as the air is compressed. On the first and second post-treatment day, intravenous saline (30-35 ml/kg/d) with omeprazole (20 mg/d), ondansetron (8 mg/d) and butylscopolamine bromide (20 mg t.i.d.) were given to all patients. All patients from day 3 after placement began liquid diet and were discharged home on day 4 on a 1000 kcal diet (carbohydrate 146 g, lipid 68 g, protein 1 g/kg ideal weight). After 6 months, the Heliosphere Bag was removed. The patients were followed monthly, and complications and their treatment, post-placement symptoms, BMI and %EWL were recorded. Data were expressed as mean +/- SD. RESULTS: From Sept-Dec 2004, 10 patients (5M/5F) underwent Heliosphere Bag placement, with age 35.2 +/- 15.7 years (17-49), BMI 43.3 +/- 8.1 kg/m(2) (35-51.2), and weight 126.8 +/- 23.7 kg (98.4-148). Heliosphere Bag positioning was quite difficult in all patients due to low pliancy and large size of the bag, causing patient discomfort. System failure at time of Heliosphere Bag positioning was observed in 5/10 patients (50%). At time of removal, the Heliosphere Bag was not found in the stomach in one patient. In 3 other patients, the balloon was found partially deflated. At the time of balloon removal after 6 months, BMI was 37.4 +/- 13.4 (28.9-42.1) and %EWL was 29.1 +/- 20.1 (9.0-57.4). BMI loss was 5.2 +/- 13.1 (1.9-11.2) and mean weight loss was 17.5 +/- 16.2 kg (5-33). CONCLUSIONS: Although weight loss was satisfactory, this device cannot be considered an advance for the temporary treatment of morbid obesity. This balloon still has some instrumental and technical problems that need to be solved: high rate of system failure at positioning, high rate of spontaneous deflation, absence of a marker such as methylene blue, and large size with low pliability that cause significant patient discomfort.
