BACKGROUND: During robotic gynecologic pneumoperitoneum surgery in the Trendelenburg position, aeration loss leads to perioperative atelectasis. Recently developed ventilator mode pressure-controlled ventilation volume-guaranteed (PCV-VG) mode could provide adequate ventilation with lower inspiratory pressure compared to volume-controlled ventilation (VCV); we hypothesized that PCV-VG mode may be beneficial in reducing perioperative atelectasis via low tidal volume (VT) of 6 mL/kg ventilation during robotic gynecologic pneumoperitoneum surgery in the Trendelenburg position. We applied lung ultrasound score (LUS) for detecting perioperative atelectasis. We aimed to compare perioperative atelectasis between VCV and PCV-VG with a low VT of 6 mL/kg during pneumoperitoneum surgery in the Trendelenburg position using LUS. METHODS: Patients scheduled for robotic gynecologic surgery were randomly allocated to the VCV (n = 41) or PCV-VG group (n = 41). LUS, ventilatory, and hemodynamic parameters were evaluated at T1 (before induction), T2 (10 minutes after induction in the supine position), T3 (10 minutes after desufflation of CO2 in the supine position), and T4 (30 minutes after emergence from anesthesia in the recovery room). RESULTS: Eighty patients (40 with PCV-VG and 40 with VCV) were included. Demographic data showed no significant differences between the groups. The total LUS has changed from baseline to T4, 0.63 (95% confidence interval [CI], 0.32, 0.94) to 1.77 (95% CI, 1.42, 2.21) in the VCV group and 0.86 (95% CI, 0.56, 1.16) to 1.43 (95% CI, 1.08, 1.78) in the PCV-VG group (P = 0.170). In both groups, total LUS increased significantly compared to the baseline values. CONCLUSION: Using a low VT of 6 mL/kg during pneumoperitoneum surgery in the Trendelenburg position, our study showed no evidence that PCV-VG ventilation was superior to VCV in terms of perioperative atelectasis. While controlling a lower limb exoskeleton providing walking assistance to wearers, the walking terrain is an important factor that should be considered for meeting performance and safety requirements. Therefore, we developed a method to estimate the slope and elevation using the contact points between the limb exoskeleton and ground. We used the center of pressure as a contact point on the ground and calculated the location of the contact points on the walking terrain based on kinematic analysis of the exoskeleton. Then, a set of contact points collected from each step during walking was modeled as the plane that represents the surface of the walking terrain through the least-square method. Finally, by comparing the normal vectors of the modeled planes for each step, features of the walking terrain were estimated. We analyzed the estimation accuracy of the proposed method through experiments on level ground, stairs, and a ramp. Classification using the estimated features showed recognition accuracy higher than 95% for all experimental motions. The proposed method approximately analyzed the movement of the exoskeleton on various terrains even though no prior information on the walking terrain was provided. The method can enable exoskeleton systems to actively assist walking in various environments.

The feasibility of a carbon recycling system that transforms algal residue to volatile fatty acids (VFAs) for re-cultivating microalgae was evaluated based on a carbon balance analysis of major VFAs consisting of acetate (HAc), propionate (HPr), and butyrate (HBu). This system largely involves two processes: (i) bioconversion of algal residue to VFAs by anaerobic fermentation, and (ii) cultivation of microalgae using the produced VFAs. The carbon balance for each unit process was examined to assess how much carbon in algal residue can be converted to these major VFAs and then assimilated to microalgae biomass. First, the yield and the profile of VFAs from raw algae (RA) and lipid-extracted algae (LEA) at psychrophilic (15 °C), mesophilic (35 °C), and thermophilic conditions (55 °C) were compared. When digesting the LEA under the thermophilic condition, the highest conversion yield, 0.36 (g carbon in VFAs/g carbon in biomass), with a compositional ratio of 6:1:3 (HAc: HPr: HBu) was obtained. Consumption of VFAs for microalgal growth reached a maximum value of 0.66 (g VFAs assimilated to biomass/g VFAs provided) at the compositional ratio of 6:1:3. Consequently, the maximum total carbon recycling ratio was 23.8% when fermenting LEA at the thermophilic condition. Our findings comprehensively revealed that establishing conditions that convert LEA to higher content of acetate is a decisive factor. It was estimated that around 40% of the total carbon from the LEA can be recovered for the production of algal biomass, when increasing the VFA conversion yield beyond 60% by adopting pretreatment methods.

Lactic acid is a platform chemical for the sustainable production of various materials. To develop a robust yeast platform for low-pH production of d-lactic acid (LA), an acid-tolerant yeast strain was isolated from grape skins and named Pichia kudriavzevii NG7 by ribosomal RNA sequencing. This strain could grow at pH 2.0 and 50°C. For the commercial application of P. kudriavzevii NG7 as a lactic acid producer, the ethanol fermentation pathway was redirected to lactic acid by replacing the pyruvate decarboxylase 1 gene (PDC1) with the d-lactate dehydrogenase gene (d-LDH) derived from Lactobacillus plantarum. To enhance lactic acid tolerance, this engineered strain was adapted to high lactic acid concentrations, and a new transcriptional regulator, PAR1, responsible for acid tolerance, was identified by whole-genome resequencing. The final engineered strain produced 135 g/L and 154 g/L of d-LA with productivity over 3.66 g/L/hr at pH 3.6 and 4.16 g/L/hr at pH 4.7, respectively.

Microalgae are regarded as a promising source of biofuels, and the concept of a microalgae-based biorefinery has attracted increasing attention in recent years. From an economic perspective, however, the process remains far from competitive with fossil fuels. This is particularly true of lipid extraction, due in part to the energy-intensive drying step. As a result, wet extraction methods have been studied as an economic alternative. In the present study, a novel extraction approach which utilizes high shear stress mixing was adopted and demonstrated for simultaneous lipid extraction and cell disruption to enable the retrieval of lipids directly from concentrated wet biomass. When a high shear mixer (HSM) was used to extract lipid from a dense biomass (> 350 g/L) of the oleaginous algae Aurantiochytrium sp., it exhibited a yield of esterifiable lipids which exceeded 80% in 10 min at 15,000 rpm with various solvent types. The HSM was found to improve the lipid yields substantially with solvents less miscible with either lipids or water, such that the range of Hansen solubility parameters for the usable solvents became 3.3 times wider (14.9-26.5 MPa1/2). The HSM, which appeared effectively to loosen the water barrier that prevents solvent molecules from penetrating through the cell envelope, was found to be more efficient with hexane, hexane/isopropanol, and ethanol, all of which showed nearly identical lipid yields compared to the dry extraction process. The HSM can, indeed, offer a powerful mechanical means of lipid extraction with non-polar and less toxic solvents from wet biomass.

As an alternative to high-cost shoe insole pressure sensors that measure the insole pressure distribution and calculate the center of pressure (CoP), researchers developed a foot sensor with FSR sensors on the bottom of the insole. However, the calculations for the center of pressure and ground reaction force (GRF) were not sufficiently accurate because of the fundamental limitations, fixed coordinates and narrow sensing areas, which cannot cover the whole insole. To address these issues, in this paper, we describe an algorithm of virtual forces and corresponding coordinates with an artificial neural network (ANN) for low-cost flexible insole pressure measurement sensors. The proposed algorithm estimates the magnitude of the GRF and the location of the foot plantar CoP. To compose the algorithm, we divided the insole area into six areas and created six virtual forces and the corresponding coordinates. We used the ANN algorithm with the input of magnitudes of FSR sensors, 1st and 2nd derivatives of them to estimate the virtual forces and coordinates. Eight healthy males were selected for data acquisition. They performed an experiment composed of the following motions: standing with weight shifting, walking with 1 km/h and 2 km/h, squatting and getting up from a sitting position to a standing position. The ANN for estimating virtual forces and corresponding coordinates was fitted according to those data, converted to c script, and downloaded to a microcontroller for validation experiments in real time. The results showed an average RMSE the whole experiment of 31.154 N for GRF estimation and 8.07 mm for CoP calibration. The correlation coefficients of the algorithm were 0.94 for GRF, 0.92 and 0.76 for the X and Y coordinate respectively.

In the present work, a two-stage anaerobic digestion system (TSADS) was newly designed to produce biogas with a greatly reduced H2 S content. The role of first (sulfidogenic)-stage digester was not only acidogenesis but also sulfidogenesis (sulfate reduction to H2 S), which would minimize the input of H2 S-producing source in the followed second (methanogenic)-stage digester. For the coexistence of acidogens and sulfate reducing bacteria (SRB) in the sulfidogenic-stage digester, it was found that pH played a crucial role. The acidogenic activity was not affected within a pH range of 4.5-6.0, while it was important to maintain a pH at 5.5 to achieve a sulfate removal efficiency over 70%. The highest sulfate removal attained was 78% at a hydraulic retention time (HRT) of 5 h at pH 5.5 ± 0.1. The H2 S content in the biogas produced in the conventional single-stage digester (SSAD), used as a control, reached 1,650 ± 25 ppmv . In contrast, the biogas produced in the methanogenic-stage digester of the developed process had an H2 S content of 200 ± 15 ppmv . Microbial analysis, done by the next generation sequencing technique, clearly showed the changes in community under different operating conditions. Desulfovibrio bastinii (4.9%) played a key role in sulfate removal in the sulfidogenic-stage of the TSADS owing to its characteristics of a short doubling time and growth in an acidic environment. BACKGROUND: Dexmedetomidine as a sole agent showed limited use for painful procedures due to its insufficient sedative/analgesic effect, pronounced hemodynamic instability and prolonged recovery. The aim of this study was to compare the effects of dexmedetomidine-ketamine (DK) versus dexmedetomidine-midazolam-fentanyl (DMF) combination on the quality of sedation/analgesia and recovery profiles for monitored anesthesia care (MAC). METHODS: Fifty six patients undergoing chemoport insertion were randomly assigned to group DK or DMF. All patients received 1 µg.kg(-1) dexmedetomidine over 10 min followed by 0.2-1.0 µg.kg(-1)h(-1) in order to maintain 3 or 4 of modified Observer's Assessment of Analgesia and Sedation score checked every 3 min. At the start of dexmedetomidine infusion, patients in group DK or DMF received 0.5 mg.kg(-1) ketamine or 0.05 mg.kg(-1) midazolam + 0.5 µg.kg(-1) fentanyl intravenously, respectively. When required, rescue sedatives (0.5 mg.kg-1 of ketamine or 0.05 mg.kg-1 of midazolam) and analgesics (0.5 mg.kg-1 of ketamine or 0.5 µg.kg-1 of fentanyl) were given to the patients in DK or DMF group, respectively. The primary outcome of this study was the recovery parameters (time to spontaneous eye opening and the length of the recovery room stay). The secondary outcomes were parameters indicating quality of sedation/analgesia, cardiorespiratory variables, and satisfaction scores. RESULTS: There were no significant differences in the onset time, time to spontaneous eye opening, recovery room stay, the incidences of inadequate analgesia, hypotension and bradycardia between the two groups. Despite lower infusion rate of dexmedetomidine, more patients in the DMF group had bispectral index (BIS) < 60 than in the DK group and vice versa for need of rescue sedatives. The satisfaction scores of patients, surgeon, and anesthesiologist in the DMF group were significantly better than the DK group. CONCLUSIONS: The DK and DMF groups showed comparable recovery time, onset time, cardiorespiratory variables, and analgesia. However, the DMF group showed a better sedation quality and satisfaction scores despite the lower infusion rate of dexmedetomidine, and a higher incidence of BIS < 60 than the DK group. Rice straw is one of the most abundant renewable biomass sources and was selected as the feedstock for the production of volatile fatty acids (VFAs) from which microbial biodiesel can be produced. Two kinds of chemical pretreatments involving nitric acid and sodium hydroxide were investigated at 150 °C with 20 min of reaction time. The nitric acid pretreatment generated the most hemicellulose hydrolyzate, while significant reduction of the lignin occurred with sodium hydroxide pretreatment. Anaerobic digestion of 20 g/L rice straw yielded 6.00 and 7.09 g VFAs/L with 0.5% HNO3 and 2% NaOH, respectively. The VFAs yield with 2% NaOH was 0.35 g/g.

BACKGROUND: Craniotomy patients have a high incidence of postoperative nausea and vomiting (PONV). This prospective, randomized, double-blind, multi-center study was performed to evaluate the efficacy of prophylactic ramosetron in preventing PONV compared with ondansetron after elective craniotomy in adult patients. METHODS: A total of 160 American Society of Anesthesiologists physical status I-II patients aged 19-65 years who were scheduled to undergo elective craniotomy for various intracranial lesions were enrolled in this study. All patients received total intravenous anesthesia (TIVA) with propofol and remifentanil. Patients were randomly allocated into three groups to receive ondansetron (4 mg; group A, n  =  55), ondansetron (8 mg; group B, n  =  54), or ramosetron (0.3 mg; group C, n  =  51) intravenously at the time of dural closure. The incidence of PONV, the need for rescue antiemetics, pain score, patient-controlled analgesia (PCA) consumption, and adverse events were recorded 48 h postoperatively. RESULTS: Among the initial 160 patients, 127 completed the study and were included in the final analysis. The incidences of PONV were lower (nausea, 14% vs. 59% and 41%, respectively; P  <  0.001; vomiting, P  =  0.048) and the incidence of complete response was higher (83% vs. 37% and 59%, respectively; P  <  0.001) in group C than in groups A and B at 48 h postoperatively. There were no significant differences in the incidence of PONV or need for rescue antiemetics 0-2 h postoperatively, but significant differences were observed in the incidence of PONV and complete response among the three groups 2-48 h postoperatively. No statistically significant intergroup differences were observed in postoperative pain, PCA consumption, or adverse events. CONCLUSION: Intravenous administration of ramosetron at 0.3 mg reduced the incidence of PONV and rescue antiemetic requirement in craniotomy patients. Ramosetron at 0.3 mg was more effective than ondansetron at 4 or 8 mg for preventing PONV in adult craniotomy patients. Ketamine has anti-inflammatory, analgesic and antihyperalgesic effect and prevents pain associated with wind-up. We investigated whether low doses of ketamine infusion during general anesthesia combined with single-shot interscalene nerve block (SSISB) would potentiate analgesic effect of SSISB. Forty adult patients scheduled for elective arthroscopic shoulder surgery were enrolled and randomized to either the control group or the ketamine group. All patients underwent SSISB and followed by general anesthesia. During an operation, intravenous ketamine was infused to the patients of ketamine group continuously. In control group, patients received normal saline in volumes equivalent to ketamine infusions. Pain score by numeric rating scale was similar between groups at 1, 6, 12, 24, 36, and 48 hr following surgery, which was maintained lower than 3 in both groups. The time to first analgesic request after admission on post-anesthesia care unit was also not significantly different between groups. Intraoperative low dose ketamine did not decrease acute postoperative pain after arthroscopic shoulder surgery with a preincisional ultrasound guided SSISB. The preventive analgesic effect of ketamine could be mitigated by SSISB, which remains one of the most effective methods of pain relief after arthroscopic shoulder surgery.

Advances in harvesting of microalgae are needed for the efficient and economical production of microalgal biodiesel. In addition to improvements in recovery efficiency, developments in harvest technology should focus on reducing the adverse impact of subsequent processes, and should also allow water recycling. We investigated a continuous electrochemical approach for microalgal biodiesel production. Instead of conventional DC, pulsed DC was applied as a method of polarity exchange and its performance was analyzed in terms of recovery efficiency, electricity consumption, and residual Al concentration. Under optimized pulsed-DC conditions, 32 % less electricity was required and 7 % less Al was remained after continuous harvesting and there was no decrease in recovery efficiency compared to the continuous harvesting by conventional DC. We also examined the effect of this new protocol on biodiesel quality and water reusability. There were no differences in the microalgal oil composition before and after electrolytic harvesting. In addition, the harvested oil quality, based on four key parameters, was superior to that produced by other terrestrial crops. Lastly, there was no retardation of growth in recycled medium relative to that in fresh medium.

Developing electroactive membranes for filtration has gained importance owing to their effectiveness in mitigating the long-lasting issue of fouling faced with traditional membranes. Here, we developed thin electroactive metallic films on to stainless steel mesh (SSM) using electrodeposition method and evaluated their performance for microalgae harvesting via electro filtration. The effect of electrodeposition parameters on membrane formulation and operating parameters for electro filtration, both in continuous and intermittent modes, were evaluated and optimum values were obtained using response surface methodology (RSM). The optimal combination of electrodeposition parameters is 1000 µA/cm2 and 5 min for deposition current density and time, respectively. Whereas the electric field strength of 20 V/mm with an application time of 1 min is suggested to be the optimal combination of electro filtration parameters for maximized flux recovery and corresponding experimental rejection efficiency of more than 90%. Overall, this research contributes to a better understanding of the parameters governing electro-filtration and offers insights for improving the performance of membrane-based microalgae harvesting systems.

Microalgae, a group of microorganisms that grow using sunlight as the sole energy source and carbon dioxide as an only carbon source, have been considered as a feedstock of choice for the production of biofuels such as biodiesel. To explore the economic feasibility of such application, however, many technical hurdles must first be overcome; the selection and/or screening of competent species are some of the most important and yet challenging tasks. To greatly accelerate this rather slow and laborious step, we developed a droplet-based microfluidic system that uses alginate hydrogel microcapsules with a mean diameter of 26 µm, each of which is able to encapsulate a single microalgal cell. This novel device was successfully demonstrated using three microalgae species, namely, Chlorella vulgaris , Chlamydomonas sp., and Botryococcus braunii . In situ analysis of the lipid content of individual microalgal cells by nondestructive fluorescence staining using BODIPY (4,4-difluoro-1,3,5,7,-tetramethyl-4-bora-3a,4a-diaza-s-indacene) was possible. In all cases, we confirmed that the lipid content of microalgal species in alginate hydrogel microcapsules was comparable to that of free-living cells. Stochastic heterogeneity in the lipid content was verified under a highly viable physiological condition, implying that other analyses were possible after the determination of lipid content. Furthermore, the designed microwell arrays enabled us to distinguish the BODIPY fluorescence response of a single live alga within the microcapsules.

OBJECTIVE: The aim of this study was to evaluate the pretreatment effect of simvastatin on spinal cord ischemia-reperfusion injury. DESIGN: Prospective, interventional study. SETTING: University research laboratory. PARTICIPANTS: Forty-five male Sprague-Dawley rats. INTERVENTIONS: Rats were treated with oral simvastatin, 10 mg/kg (simvastatin group; n = 15) or saline (control group; n = 15) for 5 days before ischemia. Spinal cord ischemia was induced using a balloon-tipped catheter placed in the proximal descending aorta in the control and simvastatin groups, but not in the sham group (n = 15). MEASUREMENTS AND MAIN RESULTS: Neurologic function was assessed daily using the motor deficit index until 7 days after reperfusion. After the last neurologic evaluation, a histologic examination of the spinal cord was performed. At day 1 after reperfusion, the simvastatin group showed a significantly lower motor deficit index compared with the control group (2.0, 2.0-2.0, v 4.0, 3.5-5.0; p < 0.001). This trend was sustained at day 7 (2.0, 1.5-2.0, v 4.0, 3.0-4.0; p < 0.001). The simvastatin group displayed a significantly larger number of normal motor neurons compared with the control group (mean ± SD, 31.7 ± 6.1 v 20.4 ± 4.4; p < 0.001). However, compared with the sham group, the simvastatin group displayed fewer intact motor neurons (sham group, 38.5 ± 5.1; p = 0.005). CONCLUSIONS: Pretreatment with simvastatin, 10 mg/kg, given orally for 5 days before the ischemia-reperfusion insult, improved the neurologic outcome and preserved more normal motor neurons compared with the control group in a rat model of spinal cord ischemia-reperfusion.

The effects of light, pH and organic carbon sources were investigated on hydrogen production by algae. An optical fiber was examined as an internal light source. The optical fiber rendered prolonged lag time and total time of hydrogen production. The optimal pH to produce hydrogen for Chlorella sp. was 8.0. Glucose, fructose, sucrose and malt extract were compared as organic carbon sources. The optimal dose of each carbon source was 5 g/L for maximum hydrogen yield. Sucrose produced the largest hydrogen volume (1,315 ml/L), while the highest production rate (24 ml/L/h) was observed in the presence of fructose.

Escherichia coli is an important microbial indicator of fecal contamination, making accurate quantitative detection of E. coli a key to ensuring public health. In this study, a microbial fuel cell (MFC) was used as a detection unit of an E. coli sensor, and specific enzymes expressed in E. coli, such as ß-D-galactosidase (GAL) and ß-D-glucuronidase (GUS), were exploited as biological detection elements. As substrates, 4-aminophenyl-ß-D-galactopyranoside (4-APGal) were used for GAL detection, whereas 8-hydroxyquinoline glucuronide (8-HQG) and 4-nitrophenyl ß-D-glucuronide (PNPG) were used for GUS detection. Once these substrates were hydrolyzed by GAL or GUS, they became electrochemically active products, which were, in turn, oxidized on the anode of the MFC reactor. The power output of the MFC reactor increased sharply when E. coli in the reactor reached the critical concentration. Accordingly, the time required to reach the highest voltage output was recorded as a detection time (DT), and a negative linear relationship was established between DT and the logarithm of the initial concentration of E. coli in the samples studied. The DTs of laboratory samples were 140 min and 560 min for initial concentrations of 1.9 × 10(7) CFU/mL and 42 CFU/mL at 44.5 °C. Moreover, the DTs for GUS assays were further shortened by induction with methyl ß-D-glucuronide sodium salt (MetGlu). The quantitative relationship between DTs and initial E. coli concentrations established from replicate laboratory sample assays allowed estimation of the E. coli concentration in environmental samples, but with approximately 100 min of lag time. The lag time was also observed with E. coli samples that were prepared by starving cells in a laboratory.

Valorization of nitric oxide is a promising solution for addressing the environmental and resource issues related to the nitrogen cycle. However, low concentrations of nitric oxide combined with impurities in exhaust streams limit its potential, and it requires extensive energy to produce high-purity nitric oxide. Here, we propose a synergistic reactive separation system that combines iron-chelate selective absorption with an electrochemical reaction to convert nitric oxide to nitrate. Among the iron-based chelates tested, EDTA was found to be the most effective in capturing gas-phase nitric oxide. Direct electrochemical oxidation of Fe-EDTA-NO solution exhibited Faradaic efficiency and a partial current density toward nitrate of 70% and 30.1 mA cm-2 at 2.2 V vs RHE and pH 7, resulting in a 43-fold enhancement of nitrate partial current density and a 2-fold improvement in Faradaic efficiency compared to simple purging without selective absorbent. Nitrate was then selectively recovered from the Fe-EDTA system using simple polarity reversal following electrooxidation with a separation factor of 13 over background sulfate. This study offers a new approach to gas-phase NO remediation and valorization using an electrified means.

Alzheimer's disease (AD), one of the most prevalent neurodegenerative diseases is associated with pathological autophagy-lysosomal pathway dysfunction. Dexmedetomidine (Dex) has been suggested as an adjuvant to general anesthesia with advantages in reducing the incidence of postoperative cognitive dysfunction in Dex-treated patients with AD and older individuals. Several studies reported that Dex improved memory; however, evidence on the effects of Dex on neuronal autophagy dysfunction in the AD model is lacking. We hypothesized that Dex administration would have neuroprotective effects by improving pathological autophagy dysfunction in mice that received an intracerebroventricular (i.c.v.) injection of amyloid ß-protein fragment 25-35 (Aß25-35) and in an autophagy-deficient cellular model. In the Y-maze test, Dex reversed the decreased activity of Aß25-35 mice. Additionally, it restored the levels of two memory-related proteins, phosphorylated Ca2+/calmodulin-dependent protein kinase II (p-CaMKII) and postsynaptic density-95 (PSD-95) in Aß25-35 mice and organotypic hippocampal slice culture (OHSC) with Aß25-35. Dex administration also resulted in decreased expression of the autophagy-related microtubule-associated proteins light chain 3-II (LC3-II), p62, lysosome-associated membrane protein2 (LAMP2), and cathepsin D in Aß25-35 mice and OHSC with Aß25-35. Increased numbers of co-localized puncta of LC3-LAMP2 or LC3-cathepsin D, along with dissociated LC3-p62 immunoreactivity following Dex treatment, were observed. These findings were consistent with the results of western blots and the transformation of double-membrane autophagosomes into single-membraned autolysosomes in ultrastructures. It was evident that Dex treatment alleviated impaired autolysosome formation in Aß mice. Our study demonstrated the improvement of memory impairment caused by Dex and its neuroprotective mechanism by investigating the role of the autophagy-lysosomal pathway in a murine Aß25-35 model. These findings suggest that Dex could be used as a potential neuroprotective adjuvant in general anesthesia to prevent cognitive decline.

Dexmedetomidine prevents postoperative cognitive dysfunction by inhibiting high-mobility group box 1 (HMGB1), which acts as an inflammatory marker. This study investigated the HMGB1 levels and the cognitive function using a Mini-Cog© score in elderly patients undergoing orthopedic surgery with dexmedetomidine infusion. In total, 128 patients aged ≥ 65 years were analyzed. The patients received saline in the control group and dexmedetomidine in the dexmedetomidine group until the end of surgery. Blood sampling and the Mini-Cog© test were performed before the surgery and on postoperative days 1 and 3. The primary outcomes were the effect of dexmedetomidine on the HMGB1 levels and the Mini-Cog© score in terms of postoperative cognitive function. The Mini-Cog© score over time differed significantly between the groups (p = 0.008), with an increase in the dexmedetomidine group. The postoperative HMGB1 levels increased over time in both groups; however, there was no significant difference between the groups (p = 0.969). The probability of perioperative neurocognitive disorders decreased by 0.48 times as the Mini-Cog© score on postoperative day 3 increased by 1 point. Intraoperative dexmedetomidine has shown an increase in the postoperative Mini-Cog© score. Thus, the Mini-Cog© score is a potential tool for evaluating cognitive function in elderly patients.
