RESUMO

ABSTRACT: Ultramarathon open water swimming (OWS) events are one of the toughest endurance challenges in the world. The sport has gained notoriety with athletes swimming across the English Channel, Diana Nyad swimming from Cuba to Florida, and the 5- and 10-km OWS in the Olympic schedule. The athletes who participate are exposed to dangerous conditions that expose risks inherent to the sport. The optimal time to prepare for an emergency is before it happens. The aim of this paper is to present an emergency action plan (EAP) designed for the "Swim Tuff" event, a record-breaking ultramarathon swim that took place in Rhode Island, United States. This article identifies an overview of Swim Tuff, the challenges experienced, and how the team designed and implemented risk mitigation strategies. The professionals looking over athletes completing an OWS should be educated and aware of the unique circumstances inherent to the sport.

OBJECTIVE: Minority student-athletes have a lower survival rate from sudden cardiac arrest (SCA) than non-minority student-athletes. This study examined the relationship between high school indicators of socioeconomic status (SES) and survival in student-athletes with exercise-related SCA. METHODS: High school student-athletes in the USA with exercise-related SCA on school campuses were prospectively identified from 1 July 2014 to 30 June 2018 by the National Center for Catastrophic Sports Injury Research. High school indicators of SES included the following: median household and family income, proportion of students on free/reduced lunch and percent minority students. Resuscitation details included witnessed arrest, presence of an athletic trainer, bystander cardiopulmonary resuscitation and use of an on-site automated external defibrillator (AED). The primary outcome was survival to hospital discharge. Differences in survival were analysed using risk ratios (RR) and univariate general log-binomial regression models. RESULTS: Of 111 cases identified (mean age 15.8 years, 88% male, 49% white non-Hispanic), 75 (68%) survived. Minority student-athletes had a lower survival rate compared with white non-Hispanic student-athletes (51.1% vs 75.9%; RR 0.67, 95% CI 0.49 to 0.92). A non-significant monotonic increase in survival was observed with increasing median household or family income and with decreasing percent minority students or proportion on free/reduced lunch. The survival rate was 83% if an athletic trainer was on-site at the time of SCA and 85% if an on-site AED was used. CONCLUSIONS: Minority student-athletes with exercise-related SCA on high school campuses have lower survival rates than white non-Hispanic athletes, but this difference is not fully explained by SES markers of the school.

J Strength Cond Res 35(10): 2775-2782, 2021-Our objective was to determine the impact of environmental conditions on player loads during preseason training sessions in women's soccer athletes. Eleven women's NCAA Division III soccer players (age = 20 ± 1 year, height = 167.28 ± 8.65 cm, body mass = 60.18 ± 5.42 kg, V̇o2max = 43.70 ± 3.95 ml·kg-1·min-1) volunteered to wear Global Positioning System (GPS) devices (Sports Performance Tracking, Melbourne, Australia) that provided measures of training session external intensity throughout all preseason practices (n = 15). We recorded wet-bulb globe temperature (WBGT), session Rating of Perceived Exertion-Training Load (sRPE-TL), and ΔBM during each preseason training session and set α ≤ 0.05. The combination of WBGT, sRPE-TL, and ΔBM explained 34% of the variance in GPS-based intensity score (proprietary measure) (F3,153 = 26.25, p < 0.001). Wet-bulb globe temperature (t156 = -2.58, p = 0.01), sRPE (t156 = 8.24, p < 0.001), and ΔBM (t156 = 2.39, p = 0.02) were significantly associated with intensity. The ΔBM from prepractice (60.00 ± 5.21 kg) to postpractice (59.61 ± 5.10 kg) was statistically significant (p < 0.001); however, ΔBM from the beginning of preseason (59.87 ± 5.31 kg) to the end of preseason (59.91 ± 5.58 kg) was not significant (p = 0.89). Despite relatively low to moderate environmental conditions, increases in WBGT were associated with reductions in GPS intensity and elevated internal load via sRPE-TL. Our findings support the association between exercise intensity and WBGT, internal load, and hydration status; thus, coaches and exercise scientists should take these factors into account when monitoring or interpreting intensity metrics. Furthermore, these findings support the continued use of environmental monitoring and hydration best-practice policies to limit exercise intensity in the heat so as to mitigate excessive heat stress.

J Strength Cond Res 35(4): 1103-1109, 2021-The purpose of this study was twofold: (a) to examine the relationship between heart rate variability (HRV) and acute:chronic workload ratio (ACWR)-based training load (TL) metrics and (b) to examine relationships across various A:C ratio-based TL metrics. Heart rate variability in 23 male college soccer players (mean ± SD; age, 21 ± 1 years; body mass, 80.3 ± 5.8 kg; height, 181.9 ± 6.5 cm; %body fat, 11.9 ± 2.0%; and V̇o2max, 51.9 ± 5.0 ml·kg-1·min-1) was measured at 5 time points: week(W)1, W3, W7, W12, and W14 during the 2015 NCAA men's soccer season. Heart rate variability was calculated from beat to beat intervals using a heart rate monitor. Players donned a global position satellite-enabled device that measured the following TL metrics: session time (ST), Player Load (PL), PL·min-1, and total distance (TD). Acute:chronic workload ratio was calculated for each TL metric: ACWR-based ST (ACWRST), ACWR-based PL (ACWRPL), ACWR-based PL·min-1 (ACWRPLM), and ACWR-based TD (ACWRTD): ACWR = week average TLs/mo average (30 ± 1 days) TLs. Relationships between HRV and ACWR-based each TL metric were evaluated using mixed effects models. Tukey pairwise comparisons were used to examine differences between types of ACWR-based TL metrics. An increase in ACWRST significantly reduced HRV throughout a season (-7.4 ± 3.6 m·s-1; p = 0.04). There were significant differences between ACWRPLM and ACWRST, ACWRPL and ACWRTD at W1, ACWRPLM and ACWRST at W3 (p < 0.05). In conclusion, ACWRST, ACWRPL, and ACWRTD were significantly different from ACWRPLM. ACWRST was found to significantly predict HRV; higher ACWRST was significantly associated with lower HRV. Therefore, tracking of the ACWR using ST may help to optimize athlete's physiological state throughout a season.

Seasonal accumulated workloads in collegiate men's soccer: a comparison of starters and reserves. J Strength Cond Res 35(11): 3184-3189, 2021-The purpose of this investigation was to quantify and compare player's season total-, match-, and training-accumulated workload by player status characteristics (i.e., starter vs. reserve) in American collegiate men's soccer. Global positioning system (GPS) and heart rate (HR)-derived workloads were analyzed from 82 collegiate male soccer athletes from 5 separate teams over the 2016 and 2017 seasons. Differences in total physical and physiological workloads (i.e., total distance, accelerations, and weighted HR-zone training impulse [TRIMP] score) as well as workloads over a range of intensity zones were examined using multilevel mixed models, with mean difference (MD) and effect size (ES) reported. Starters accumulated substantially more total distance (MD = 82 km, ES = 1.23), TRIMP (MD = 2,210 au, ES = 0.63), and total accelerations (MD = 6,324 n, ES = 0.66) over the season. Total accumulated distance in all velocity zones (ES [range] = 0.87-1.08), all accelerations zones (ES [range] = 0.54-0.74), and time spent at 70-90% HRmax (ES [range] = 0.60-1.12) was also greater for starters. Reserves accumulated substantially more total distance (MD = 20 km, ES = 0.43) and TRIMP (MD = 1,683 au, ES = 0.79) during training. Although reserves show elevated physical and physiological loads during training compared with starters, there is an imbalance in overall workloads between player roles, with starters incurring substantially more match and total seasonal workloads. These results indicate managing player workloads in soccer requires attention to potential imbalances between players receiving variable match times. Coaches and practitioners in collegiate men's soccer may consider implementing strategies to reduce discrepancies in loading between starters and reserves. Individualized monitoring of training and match workloads may assist in the implementation of more balanced load management programs.

The relationships between perceived wellness of, sleep of, and acute: chronic training load on National Collegiate Athletics Association division I male soccer players. J Strength Cond Res 35(5): 1326-1330, 2021-The purpose of this study was to investigate relationships between perceived wellness, sleep, and acute: chronic workload ratio (ACWR) throughout a collegiate men's soccer season. Sixty male collegiate soccer players (mean[M] ± SD; age, 21±2 year; body mass, 77.6 ± 6.5 kg; height, 180.1 ± 6.4 cm; body fat%, 9.9 ± 3.9% ; and V̇o2max, 53.1 ± 5.0 ml·kg-1·min-1) participated in this study. During each session, players used a heart rate and global positioning satellite-enabled chest strap to measure training impulse and ACWR. The ACWR values were trichotomized at the individual level giving an equal number of observations within each ACWR category of low, moderate, and high ACWR (M ± SD; low, 0.658 ± 0.23; moderate, 0.92 ± 0.15; and high, 1.17 ± 0.16). Stress, fatigue, and soreness levels were collected using 1-10 Likert scales and sleep duration, and sleep quality were measured by the Karolinska Sleep Diary. Stress, fatigue, soreness levels, and sleep quality were transformed to corresponding z-scores at the individual level. Fatigue levels were significantly higher when ACWR was high compared with low (mean difference [95% confidence intervals], effect size, p-value; 0.31 [0.21, 0.42], 0.29, p < 0.001) and moderate (0.14 [0.03, 0.24], 0.13, p = 0.01). Fatigue levels were also significantly higher when the ACWR was moderate compared with low (0.18 [0.07, 0.28], 0.16, p = 0.001). Soreness levels were significantly higher when the ACWR was high compared with low (0.25 [0.14, 0.36], 0.23, p < 0.001). Stress levels were significantly greater when the ACWR was high compared with low (0.19, [0.08, 0.29], 0.18, p < 0.001) and compared with moderate (0.15, [0.05, 0.25], 0.14, p = 0.004). There were no differences in sleep duration or sleep quality in different ACWR. The ACWR may be a useful tool to achieve an appropriate balance between training and recovery to manage daily fatigue and soreness levels in athletes.

Contextual factors influencing external and internal training loads in collegiate men's soccer. J Strength Cond Res 34(2): 374-381, 2020-This study investigated factors influencing training loads (TL) in collegiate men's soccer. Total distance, high-speed running distance (>14.4 km·h), high-intensity heart-rate zone duration (HI HRZ, >70% heart rate relative to maximum), and session rating of perceived exertion were assessed daily from 107 male soccer players competing for 5 National Collegiate Athletics Association Division I teams. Differences between athlete role (starter and reserve), position (defender, midfielder, and forward), season phase (preseason, in-season, and postseason), days relative to match (MD-1 to MD-5+), days between matches (<4, 4-5, >5 days), previous match outcome (win, loss, and draw), and upcoming opponent relative ranking (weaker, trivial, and stronger) were examined. Mean differences (MD) and effect sizes (ESs) with 90% confidence intervals were reported. There were trivial and insignificant differences by player role, position, or upcoming opponent strength, and small-moderate increases in preseason TL compared with in-season (ES [range] = 0.4-0.9). TLs were lower for MD-1 and higher for MD-5+ (ES [range] = 0.4-1.3) when compared with MD-2-4. External loads (ES = -0.40 ± 0.20) were less after wins compared with losses. TLs are increased in the preseason, when training sessions occur greater than 5 days from a match and after losses. Contextualizing factors affecting TLs has implications for developing workload prescription and recovery strategies.

The validity and reliability of GPS units for measuring distance and velocity during linear and team sport simulated movements. J Strength Cond Res 34(11): 3070-3077, 2020-This experimental study aimed to assess the validity and reliability of shirt-mounted 10-Hz global positioning system (GPS) units (Polar Team Pro) for measuring total distance (TD), constant velocity (VelC), and instantaneous velocity (VelI) during linear running and a team sport simulation circuit (TSSC). Fifteen male soccer athletes completed linear tasks (40 and 100 m) at various velocities: walk (W) (4.8-7.9 km·h), jog (J) (8.0-12.7 km·h), run (R) (12.9-19.9 km·h), and sprint (S) (>20.0 km·h) and a 120-m TSSC. Global positioning system validity and reliability for TD, VelC, and VelI were compared with criterion measures using 2 methods (a and b) of GPS raw data extraction. When measuring TD for the Polar Team Pro device, validity and reliability measures were <5% error at all velocities during the 40-m (with the exception of the S [%CV = 8.03]) and 100-m linear trial (both extraction methods) and TSSC. The GPS mean difference (±SD) for TD during the TSSC using extraction methods (a) and (b) was 0.2 ± 1.2 and 2.2 ± 2.2 m, respectively. The validity of the device in measuring VelC was significantly different (p < 0.05) at all velocities during the 40 m (exception W) and the 100 m, with effect sizes ranging from trivial to small (exception of 100 m S). VelI was similar (p > 0.05) at all velocities, except for the W (p = 0.001). The reliability of the device when measuring VelC during the 40 and 100 m was <5% CV; however, during the 100 m, VelI ranged from 1.4 to 12.9%. Despite trivial to large effect sizes for validity of TD, this device demonstrated good reliability <5% CV during linear and TSSC movements. Similarly, effect sizes ranged from trivial to large for VelC, and yet VelI reliability was good for VelC, but good to poor for VelI.

Background and Objectives: Exertional heat stroke (EHS) survivors may be more susceptible to subsequent EHS; however, the occurrence of survivors with subsequent EHS episodes is limited. Therefore, the purpose of this study was to evaluate the incidence of participants with repeated EHS (EHS-2+) cases in a warm-weather road race across participation years compared to those who experienced 1 EHS (EHS-1). Materials and Methods: A retrospective observational case series design was utilized. Medical record data from 17-years at the Falmouth Road Race between 2003-2019 were examined for EHS cases. Incidence of EHS-2+ cases per race and average EHS cases per EHS-2+ participant were calculated (mean ± SD) and descriptive factors (rectal temperature (TRE), finish time (FT), Wet Bulb Globe Temperature (WBGT), age, race year) for each EHS was collected. Results: A total of 333 EHS patients from 174,853 finishers were identified. Sixteen EHS-2+ participants (11 males, 5 females, age = 39 ± 16 year) accounted for 11% of the total EHS cases (n = 37/333). EHS-2+ participants had an average of 2.3 EHS cases per person (range = 2-4) and had an incidence rate of 2.6 EHS per 10 races. EHS-2+ participants finished 93 races following initial EHS, with 72 of the races (77%) completed without EHS incident. Initial EHS TRE was not statistically different than subsequent EHS initial TRE (+0.3 ± 0.9 °C, p > 0.050). Initial EHS-2+ participant FT was not statistically different than subsequent EHS FT (-4.2 ± 7.0 min, p > 0.050). The years between first and second EHS was 3.6 ± 3.5 year (Mode: 1, Range: 1-12). Relative risk ratios revealed that EHS patients were at a significantly elevated risk for subsequent EHS episodes 2 years following their initial EHS (relative risk ratio = 3.32, p = 0.050); however, the risk from 3-5 years post initial EHS was not statistically elevated, though the relative risk ratio values remained above 1.26. Conclusions: These results demonstrate that 11% of all EHS cases at the Falmouth Road Race are EHS-2+ cases and that future risk for a second EHS episode at this race is most likely to occur within the first 2 years following the initial EHS incident. After this initial 2-year period, risk for another EHS episode is not significantly elevated. Future research should examine factors to explain individuals who are susceptible to multiple EHS cases, incidence at other races and corresponding prevention strategies both before and after initial EHS.

OBJECTIVE: To assess the diagnostic ability of urine reagent strips to identify hypohydration based on urine specific gravity (USG). DESIGN: This study examined the agreement of USG between strips and refractometry with Bland-Altman, whereas the diagnostic ability of the strips to assess hypohydration was performed by receiver operating characteristic analysis. SETTING: Arkansas high school football preseason practice. PARTICIPANTS: Four hundred fourteen fresh urine samples were analyzed. MAIN OUTCOME MEASURES: Urine specific gravity was assessed by both reagent strips and refractometry. Cutoffs of >1.020 and >1.025 were used for identifying hypohydration. RESULTS: Bland-Altman analysis showed agreement of the 2 methods. Overall diagnostic ability of the urine strip to identify hypohydration was fair (area under the curve 72%-78%). However, the sensitivity to correctly identify hypohydration was poor (63%-71%), and the specificity of correctly identifying euhydration was poor to fair (68%-83%). CONCLUSION: The urine strip method is not valid for assessing hypohydration.

Exertional heat illness (EHI) risk is a serious concern among athletes, laborers, and warfighters. US Governing organizations have established various activity modification guidelines (AMGs) and other risk mitigation plans to help ensure the health and safety of their workers. The extent of metabolic heat production and heat gain that ensue from their work are the core reasons for EHI in the aforementioned population. Therefore, the major focus of AMGs in all settings is to modulate the work intensity and duration with additional modification in adjustable extrinsic risk factors (e.g., clothing, equipment) and intrinsic risk factors (e.g., heat acclimatization, fitness, hydration status). Future studies should continue to integrate more physiological (e.g., valid body fluid balance, internal body temperature) and biometeorological factors (e.g., cumulative heat stress) to the existing heat risk assessment models to reduce the assumptions and limitations in them. Future interagency collaboration to advance heat mitigation plans among physically active population is desired to maximize the existing resources and data to facilitate advancement in AMGs for environmental heat.

J Strength Cond Res 33(11): 3065-3077, 2019-This observational study aimed to characterize the responses of a comprehensive panel of biomarkers, observed ranges, training load (TL) metrics, and performance throughout the collegiate soccer season (August-November). Biomarkers (n = 92) were collected before the start of pre-season (PS), in-season weeks (W)1, W4, W8, and W12 in NCAA Division I male soccer players (n = 20, mean ± SD; age = 21 ± 1 years, height = 180 ± 6 cm, body mass = 78.19 ± 6.3 kg, body fat = 12.0 ± 2.6%, VO2max 51.5 ± 5.1 ml·kg·min). Fitness tests were measured at PS, and W12 and TL was monitored daily. Changes in biomarkers and performance were calculated via separate repeated-measures analysis of variance. Despite similar fitness (p > 0.05), endocrine, muscle, inflammatory, and immune markers changed over time (p < 0.05). Total and free testosterone was lower in W1 vs. PS, whereas free cortisol remained unchanged at PS, W1, and W4 (>0.94 mg·dL). Oxygen transport and iron metabolism markers remained unchanged except for HCT (W1 vs. PS) and total iron binding capacity (W8-W12 vs. W1). Hepatic markers albumin, globulin, albumin:globulin, and total protein levels were elevated (p < 0.05) at W12 vs. W1, whereas aspartate aminotransferase and alanine aminotransferase levels were elevated at W1-W12 and W8-W12 vs. PS, respectively. Vitamin E, zinc, selenium, and calcium levels were elevated (p < 0.05) at W12 vs. W1, whereas Vitamin D was decreased (p < 0.05). Fatty acids and cardiovascular markers (omega-3 index, cholesterol:high-density lipoprotein [HDL], docosahexenoic acid, low-density lipoprotein [LDL], direct LDL, non-HDL, ApoB) were reduced at W1 vs. PS (p ≤ 0.05). Immune, lipid, and muscle damage biomarkers were frequently outside clinical reference ranges. Routine biomarker monitoring revealed subclinical and clinical changes, suggesting soccer-specific reference ranges. Biomarker monitoring may augment positive adaptation and reduce injuries from stressors incurred during soccer.

J Strength Cond Res 33(3): 727-735, 2019-Heat exposure and hypohydration induce physiological and psychological strain during exercise; however, it is unknown if the separate effects of heat exposure and hypohydration are synergistic when co-occurring during loaded exercise. This study compared separate and combined effects of heat exposure and hypohydration on physiological strain, mood state, and visual vigilance during loaded exercise. Twelve men (mean ± SD; age, 20 ± 2 years; body mass, 74.0 ± 8.2 kg; maximal oxygen uptake, 57.0 ± 6.0 ml·kg·min) completed 4 trials under the following conditions: euhydrated temperate (EUT), hypohydrated temperate (HYT), euhydrated hot (EUH), and hypohydrated hot (HYH). Exercise was 90 minutes of treadmill walking (∼50% V[Combining Dot Above]O2max, 5% grade) while carrying a 45-lb rucksack. Profile of Mood States and the Scanning Visual Vigilance Test were completed before and after exercise. The separate effects of heat exposure (EUH) and hypohydration (HYT) on post-exercise rectal temperature (Tre) were similar (38.25 ± 0.63°C vs. 38.22 ± 0.29°C, respectively, p > 0.05), whereas in combination (HYH), post-exercise Tre was far greater (39.32 ± 0.43°C). Increase in Tre per 1% body mass loss (BML) for HYH (vs. EUH) was greater than HYT (vs. EUT) (0.32 vs. 0.04°C, respectively, p = 0.02); heart rate increase per 1% BML for HYH (vs. EUH) was 7 b·min compared with HYT (vs. EUT) at 3 b·min (p = 0.30). Hypohydrated hot induced greater mood disturbance (post-exercise - pre-exercise) (35 ± 21 units) compared with other conditions (EUT = 3 ± 9 units; HYT = 3 ± 16 units; EUH = 16 ± 26 units; p < 0.001). No differences occurred in visual vigilance (p > 0.05). Independently, heat exposure and hypohydration induced similar physiological strain during loaded exercise; when combined, heat exposure with hypohydration, synergistically exacerbated physiological strain and mood disturbance.

To investigate the influence of estimated wet bulb globe temperature (WBGT) and the International Institute of Race Medicine (IIRM) activity modification guidelines on the incidence of exertional heat stroke (EHS) and heat exhaustion (HEx) and the ability of an on-site medical team to treat those afflicted. Medical records of EHS and HEx patients over a 17-year period from the New Balance Falmouth Road Race were examined. Climatologic data from nearby weather stations were obtained to calculate WBGT with the Australian Bureau of Meteorology (WBGTA) and Liljegren (WBGTL) models. Incidence rate (IR) of EHS, HEx, and combined total of EHS and HEx (COM) were calculated, and linear regression analyses were performed to assess the relationship between IR and WBGTA or WBGTL. One-way ANOVA was performed to compare differences in EHS, HEx, and COM incidence to four alert levels in the IIRM guidelines. Incidence of EHS, HEx, and COM was 2.12, 0.98, and 3.10 cases per 1000 finishers. WBGTA explained 48, 4, and 46% of the variance in EHS, HEx, and COM IR; WBGTL explained 63, 13, and 69% of the variance in EHS, HEx, and COM IR. Main effect of WBGTA and WBGTL on the alert levels were observed in EHS and COM IR (p < 0.05). The cumulative number of EHS patients treated did not exceed the number of cold water immersion tubs available to treat them. EHS IR increased as WBGT and IIRM alert level increased, indicating the need for appropriate risk mitigation strategies and on-site medical treatment.

J Strength Cond Res 32(10): 2907-2917, 2018-This study aimed to profile positional movement characteristics of National Collegiate Athletic Association (NCAA) Division I male soccer players. Eighteen Division I male soccer players were monitored using global positioning systems, inertial movement, and heart rate (HR) technology during 24 matches over a full competitive season (N = 235 observations). Positional groups were classified as either a forward (F), center midfielder (CM), wide midfielder (WM), or defender (D). Movement was profiled by locomotor (walking [0-7.19 km·h], jogging [7.20-14.39 km·h], running [14.40-21.59 km·h], and sprinting [>21.6 km·h]), and acceleration/deceleration characteristics (low intensity [0-1.99 m·s], moderate intensity [2-3.99 m·s], and high intensity [>4 m·s]). Players averaged distances of 9,367 ± 2,149 m per match at speeds of 91 ± 20 m·min and physiological intensities of 78 ± 8 %HRmax. Center midfielder demonstrated the highest average speeds (97 ± 20 m·min) and covered the most distance (9,941 ± 2,140 m). Wide midfielder accumulated the most sprint distance (391 ± 145 m) and high-intensity accelerations (129 ± 30 n)/decelerations (96 ± 24 n). Several practically meaningful differences exist between positions for internal and external load metrics. Match loads seen in NCAA Division I soccer vary from reports of professional soccer; however, the effects of match regulation, structure, and congestion, which are unique to NCAA soccer, require further investigation. Physical and physiological load monitoring of NCAA soccer may aid coaches and practitioners in the periodization of training programs leading up to and during a competitive soccer season. These data speak to the necessity for examining both internal and external loads by position.

J Strength Cond Res 32(11): 3080-3087, 2018-The purpose of this study was to use a mixed-methods design to describe the pacing strategy of the overall female winner of a 226.3-km Ironman triathlon. During the race, the triathlete wore a global positioning system and heart rate (HR)-enabled watch and rode a bike outfitted with a power and cadence meter. High-frequency (every km) analyses of mean values, mean absolute percent error (MAPE), and normalized graded running pace and power (accounting for changes in elevation) were calculated. During the bike, velocity, power, cadence, and HR averaged 35.6 km·h, 199 W, 84 rpm, and 155 b·min, respectively, with minimal variation except for velocity (measurement unit variation [MAPE]: 7.4 km·h [20.3%], 11.8 W [7.0%], 3.6 rpm [4.6%], 3 b·min [2.3%], respectively). During the run, velocity and HR averaged 13.8 km·h and 154 b·min, respectively, with velocity varying four-fold more than HR (MAPE: 4.8% vs. 1.2%). Accounting for elevation changes, power and running pace were less variable (raw [MAPE] vs. normalized [MAPE]: 199 [7.0%] vs. 204 W [2.7%]; 4:29 [4.8%] vs. 4:24 min·km [3.6%], respectively). Consistent with her planned pre-race pacing strategy, the triathlete minimized fluctuations in HR and watts during the bike and run, whereas velocity varied with changes in elevation. This case report provides observational evidence supporting the utility of a pacing strategy that allows for an oscillating velocity that sustains a consistent physiological effort in full Ironman races.

This study examined the separate and combined effects of heat acclimation and hand cooling on post-exercise cooling rates following bouts of exercise in the heat. Seventeen non-heat acclimated (NHA) males (mean ± SE; age, 23 ± 1 y; mass, 75.30 ± 2.27 kg; maximal oxygen consumption [VO2 max], 54.1 ± 1.3 ml·kg-1·min-1) completed 2 heat stress tests (HST) when NHA, then 10 days of heat acclimation, then 2 HST once heat acclimated (HA) in an environmental chamber (40°C; 40%RH). HSTs were 2 60-min bouts of treadmill exercise (45% VO2 max; 2% grade) each followed by 10 min of hand cooling (C) or no cooling (NC). Heat acclimation sessions were 90-240 min of treadmill or stationary bike exercise (60-80% VO2 max). Repeated measures ANOVA with Fishers LSD post hoc (α < 0.05) identified differences. When NHA, C (0.020 ± 0.003°C·min-1) had a greater cooling rate than NC (0.013 ± 0.003°C·min-1) (mean difference [95%CI]; 0.007°C [0.001,0.013], P = 0.035). Once HA, C (0.021 ± 0.002°C·min-1) was similar to NC (0.025 ± 0.002°C·min-1) (0.004°C [-0.003,0.011], P = 0.216). Hand cooling when HA (0.021 ± 0.002°C·min-1) was similar to when NHA (0.020 ± 0.003°C·min-1) (P = 0.77). In conclusion, when NHA, C provided greater cooling rates than NC. Once HA, C and NC provided similar cooling rates.

Dietary approaches for optimizing exercise performance have been debated in the literature for years. For endurance athletes, various position stands focus on recommendations for high-carbohydrate diets to maximize performance in events. However, theories of low-carbohydrate diets and their ability to provide more fuel may prove beneficial to ultraendurance athletes. Therefore, the purpose of this research was to observe the food and fluid consumption of successful recreational ultraendurance cyclists on the day before (ED-1), Event Day, and the day after (ED+1), a 162 km endurance event in a hot environment, and subsequently compare dietary intakes to recommendations and other observed dietary practices. Twenty men (age, 48 ± 8 years; mass, 85.1 ± 13.4 kg; height, 178.2 ± 7.4 cm) recorded all dietary items during ED-1, Event Day, and ED+1. Diet composition and the relationships between carbohydrate and caloric intake with finish time were examined. Results show athletes consumed a high-carbohydrate diet on ED-1 (384 g·d), Event Day (657 g·d), and ED+1 (329 g·d). However, there were no significant associations between carbohydrate (p > 0.05), or caloric intake (p > 0.05), and finish time. This study results great variation in diets of recreational endurance cyclists, although most consume within nationally recognized dietary recommendation ranges. Because there is great variation and lack of correlation with performance, these findings suggest that current high-carbohydrate dietary recommendations for general endurance athletes may not be sport specific or individualized enough for recreational ultraendurance cyclists, and that individualized dietary macronutrient composition manipulations may improve performance outcomes.

CONTEXT: Evidence-based best practices for the recognition and treatment of exertional heat stroke (EHS) indicate that rectal thermometry and immediate, aggressive cooling via cold-water immersion ensure survival from this medical condition. However, little is known about the recovery, medical follow-up, and return to activity after an athlete has suffered EHS. OBJECTIVE: To highlight the transfer of evidenced-based research into clinical practice by chronicling the treatment, recovery, and return to activity of a runner who suffered an EHS during a warm-weather road race. DESIGN: Case study. SETTING: Warm-weather road race. PARTICIPANT: 53-y-old recreationally active man. INTERVENTION: A runner's treatment, recovery, and return to activity from EHS and 2014 Falmouth Road Race performance. MAIN OUTCOMES: Runner's perceptions and experiences with EHS, body temperature, heart rate, hydration status, exercise intensity. RESULTS: The runner successfully completed the 2014 Falmouth Road Race without incident of EHS. Four dominant themes emerged from the data: predisposing factors, ideal treatment, lack of medical follow-up, and patient education. The first theme identified 3 predisposing factors that contributed to the runner's EHS: hydration, sleep loss, and lack of heat acclimatization. The runner received ideal treatment using evidence-based best practices. A lack of long-term medical care following the EHS with no guidance on the runner's return to full activity was observed. The runner knew very little about EHS before the 2013 race, which drove him to seek knowledge as to why he suffered EHS. Using this newly learned information, he successfully completed the 2014 Falmouth Road Race without incident. CONCLUSIONS: This case supports prior literature examining the factors that predispose individuals to EHS. Although evidence-based best practices regarding prompt recognition and treatment of EHS ensure survival, this case highlights the lack of medical follow-up and physician-guided return to activity after EHS.
