RESUMO

BACKGROUND: The conventional IMCI training for healthcare providers is delivered in 11 days, which can be expensive and disruptive to the normal clinical routines of the providers. An equally effective, shorter training course may address these challenges. METHODS: We conducted a quasi-experimental study in two provinces (Sindh and Punjab) of Pakistan. 104 healthcare providers were conveniently selected to receive either the abridged (7-day) or the standard (11-day) training. Knowledge and clinical skills of the participants were assessed before, immediately on conclusion of, and six months after the training. RESULTS: The improvement in mean knowledge scores of the 7-day and 11-day training groups was 31.6 (95% CI 24.3, 38.8) and 29.4 (95% CI 23.9, 34.9) respectively, p = 0.630 while the improvement in mean clinical skills scores of the 7-day and 11-day training groups was 23.8 (95% CI: 19.3, 28.2) and 23.0 (95% CI 18.9, 27.0) respectively, p = 0.784. The decline in mean knowledge scores six months after the training was - 12.4 (95% CI - 18.5, - 6.4) and - 6.4 (95% CI - 10.5, - 2.3) in the 7-day and 11-day groups respectively, p = 0.094. The decline in mean clinical skills scores six months after the training was - 6.3 (95% CI - 11.3, - 1.3) in the 7-day training group and - 9.1 (95% CI - 11.5, - 6.6) in the 11-day group, p = 0.308. CONCLUSION: An abridged IMNCI training is equally effective as the standard training. However, training for certain illnesses may be better delivered by the standard course.

Introduction Globally, prematurity accounts for 12.7% of all live births while late preterm accounts for around three-fourth (73%) of these premature births. In Pakistan, the prevalence of prematurity is approximately 18.89%. Late preterm infants often have weight and size similar to some term infants, but they are still metabolically and physiologically immature. Hence, these infants, as compared to term infants, are at a higher risk of developing medical complications, which results in higher morbidity and mortality during the birth hospitalization. We aim to determine the frequency of early complications in late preterm infants during their stay at Aga Khan Secondary-care Hospitals, Karachi. Methods A prospective descriptive study was conducted via the nonprobability sampling technique from March 22, 2016, to March 22, 2017, at secondary-care hospitals of The Aga Khan University Hospital; The Aga Khan Hospital for Women, Karimabad, and The Aga Khan Hospital for Women and Children, Garden. All late-preterm infants, i.e. those born between the 340/7 through 366/7 weeks gestation were included in this study and observed for 72 hours after birth for early complications, including hypothermia, sepsis, hypoglycemia, respiratory distress, and hyperbilirubinemia. Descriptive analysis was done using SPSS Version 19.0 (IBM Corp., Armonk, NY, US) and frequency and percentages were calculated. Results Throughout the period of study, a total of 1696 infants were born in secondary-care hospitals, of which 86.67% (n=1470) were term and 13.3% (n=226) were preterm. Late preterm infants constituted 95.5% (n=217) of preterm births and 12.7% of all newborns delivered at study sites. Among them, respiratory distress was diagnosed in 23.5%, hyperbilirubinemia in 17.5%, hypoglycemia in 13.8%, sepsis in 9.2%, and hypothermia in 6%. Conclusion Late preterm neonates form the major subgroup of preterm infants delivered at secondary-care hospitals. They have a significant risk of morbidity and birth hospitalizations. We propose that late preterm infants, regardless of their physical dimensions, be given medical attention similar to all preterms.

RESUMO

A pre-term baby girl was born following a pregnancy complicated by severe polyhydramnios at a gestational age of 36 weeks. She was initially suffering from respiratory distress consistent with idiopathic respiratory distress syndrome, and altered electrolyte imbalance with hyponatremia, hypokalemia and hypochloremic metabolic alkalosis. However, during the third week of life when she had dehydration along with significant electrolyte imbalance, Bartter's syndrome was considered which was supported by findings of high renin and aldosterone levels. Treatment was done by correction of electrolytes and dehydration along with indomethacin. The drug was well tolerated. The infant showed correction of electrolyte imbalance. The features of this case suggest an extreme form of Bartter's syndrome presenting from the early days of life. The syndrome is reported because of it's rarity and alerts pediatricians to the antenatal and neonatal variant of Bartter's syndrome.
