RESUMO

The purpose of this study was to undertake a comprehensive assessment of each stage of the end-of-life process and the home care nursing needs of non-cancer patients. The total number of eligible patients was 117 in Korea and 121 in Japan, aged ≥ 40, receiving continuous home care nursing throughout the beginning, stable, and at final death stages and ultimately dying at home. The need for the 'management of physical symptoms' increased as patients progressed through the end-of-life stages to death. In both countries, the needs for 'loss and grief care' and 'coordination among care team members' were significantly higher in the stable stage than in the beginning or final death stages. Further research is needed to develop tailored nursing care programmes that meet the specific needs of patients in each stage of the end-of-life care at home.

RESUMO

OBJECTIVES: There are several hospitals in Korea that introduced the ICNP (International Classification for Nursing Practice) as the standard terminology for clinical and home care nursing. This research attempted to determine the appropriateness of ICNP in Korean, hospital based, home care nursing. METHODS: The data was collected from a home care nursing center from January 1, 2009 to September 21, 2012. The center is operated by a Korean teaching hospital equipped with an ICNP based electronic nursing record (ENR) system. Via a refining process, 40,082 simplified sentences of nursing intervention were acquired from 41,158 nursing records. Among them, 545 preferred nursing statements were extracted, then mapped, to ICNP 2011 at both axis and sentence levels. RESULTS: The mapping results were classified into three categories based on the axis of concept origin and the level of hierarchy. These categories were titled: complete, incomplete and no mapping. Out of 45 unique concepts in the action axis, 42 (93.33%) concepts were completely mapped. However, only 38 (15.08%), out of 252 unique concepts, were completely mapped in the focus axis. At the statement level, only 19.63% of statements were completely mapped. CONCLUSIONS: The ICNP is not useful as a tool for home care nursing in its present form. The granularity of ICNP has to be improved and more concepts, specific to home care nursing, need to be added in the focus and action axes. Also, a new measure needs to be introduced to prevent information loss during mapping.

RESUMO

PURPOSE: The purpose of this study was to explore the effects of a Taegyo program on parents-fetal attachment and parenthood in first pregnancy couples (mothers and spouses). METHODS: The research design was a nonequivalent control group pretest-posttest experiment. Study participants were 52 first pregnancy couples visiting two medium-scale obstetrics and gynecology clinics located in Gwangju. A total of 52 couples were assigned to the experimental group (25 couples) and the control group (27 couples). The experimental couples were provided with a Taegyo program for 4 weeks. Data were analyzed by chi square test, t-test, and ANCOVA using the SPSS program. RESULTS: Post-treatment maternal-fetal attachment, paternal-fetal attachment and motherhood significantly increased in the experimental group as compared to the control group, but post-treatment fatherhood, anxiety, blood pressure and pulse of participants in the experimental group showed no significant difference from those in the control group. CONCLUSION: From these results, it is suggested that the Taegyo program has beneficial effects in enhancing parent-fetal attachment and motherhood in first pregnancy couples. Therefore, a Taegyo program can be recommended as a nursing intervention program for first pregnancy couples.

RESUMO

The participatory approach has been widely used to improve the work environment. The purpose of this study was to develop an intervention to prevent work-related musculoskeletal disorders in hospital nurses using the participatory approach. Based on the Participatory Action Oriented Training (PAOT) approach, the multidisciplinary team conducted the trainer workshop to develop a comprehensive intervention protocol, which yielded several practical and low-cost solutions to reduce the risk factors for musculoskeletal disorders. Structured tools that were focused on the hospital environment were developed. The developed action checklist consisted of 43 items that were focused on five areas of nursing tasks (i.e., patient care and treatment; safe handling of drugs, medical devices, and equipment; workstation design; physical environment; and welfare facilities and administration). The final intervention protocol consisted of a series of structured participant workshop, follow-up visits, and presentation of achievements.

RESUMO

OBJECTIVES: The purpose of this study was to construct and test the feasibility and potential utility of a Participatory Action Oriented Training for Hospital Nurses (PAOTHN) program to prevent work-related musculoskeletal disorders. The PAOTHN program emphasized the active participation of nurses, resulting in practical and low-cost solutions for improving their work environment by reducing risk factors of musculoskeletal disorders. METHODS: The PAOTHN program was conducted in a hospital located in a suburb of Seoul, South Korea. Of the 24 units in the hospital, 16 units participated in the study. The main components of the intervention were a series of structured workshops, continuous technical assistance by the research team, and periodical forums for sharing "best practices" among the participants. RESULTS: Through the workshops, head nurses of the participating units identified a total of 46 strategic (23 short-term and 23 long-term) plans for reducing musculoskeletal disorders risks across five dimensions: (1) patient care and treatment, (2) safe handling of drugs, medical devices, and equipment, (3) workstation design, (4) physical environment, and (5) welfare facilities and administration. Over the course of the year-long project, 18 of the suggested plans were completed, for an overall completion rate of 39.1%. CONCLUSIONS: The PAOTHN program was found to be feasible and potentially useful in reducing the musculoskeletal disorder risks faced by hospital nurses, and in identifying both risk factors and improvement opportunities at the individual and organizational levels.
