Monitoring performance-related characteristics of athletes can reveal changes that facilitate training adaptations. Here, we examine the relationships between submaximal running, maximal jump performance (CMJ), concentrations of blood lactate, sleep duration (SD) and latency (SL), and perceived stress (PSS) in junior cross-country skiers during pre-season training. These parameters were monitored in 15 male and 14 females (17 ± 1 years) for the 12-weeks prior to the competition season, and the data was analysed using linear and mixed-effect models. An increase in SD exerted a decrease in both PSS (B = -2.79, p ≤ 0.01) and blood lactate concentrations during submaximal running (B = -0.623, p ≤ 0.05). In addition, there was a negative relationship between SL and CMJ (B = -0.09, p = 0.08). Compared to males, females exhibited higher PSS scores and little or no change in performance-related tests. A significant interaction between time and sex was present in CMJ with males displaying an effect of time on CMJ performance. For all athletes, lower PSS appeared to be associated with longer overnight sleep. Since the females experienced higher levels of stress, monitoring of their PSS might be beneficial. These findings have implications for the preparation of young athletes' competition season.

The mechanical efficiency of human locomotion has been studied extensively. The mechanical efficiency of the whole body occasionally exceeds muscle efficiency during bouncing type gaits. It is thought to occur due to elasticity and stiffness of the tendinomuscular system and neuromuscular functions, especially stretch reflexes. In addition, the lower limb joint kinetics affect mechanical efficiency. We investigated the impact of varying external work on mechanical efficiency and lower limb kinetics during repeated sledge jumping. Fifteen male runners performed sledge jumping for 4 minutes at 3 different sledge inclinations. Lower limb kinematics, ground reaction forces, and expired gases were analyzed. Mechanical efficiency did not differ according to sledge inclination. Mechanical efficiency correlated positively with the positive mechanical work of the knee and hip joints and the negative contribution of the hip joints. Conversely, it correlated negatively with both the positive and negative contributions of the ankle joint. This may be attributable to the greater workload in this study versus previous studies. To achieve greater external work, producing more mechanical energy at the proximal joint and transferring it to the distal joint could be an effective strategy for improving mechanical efficiency because of the greater force-generating capability of distal joint muscles.

This study examined the predictive quality of intervals performed at maximal sustainable effort to predict 3-km and 10-km running times. In addition, changes in interval performance and associated changes in running performance were investigated. Either 6-week (10-km group, n=29) or 2-week (3-km group, n=16) interval training periods were performed by recreational runners. A linear model was created for both groups based on the running speed of the first 6×3-min interval session and the test run of the preceding week (T1). The accuracy of the model was tested with the running speed of the last interval session and the test run after the training period (T2). Pearson correlation was used to analyze relationships between changes in running speeds during the tests and interval sessions. At T2, the mean absolute percentage error of estimate for 3-km and 10-km test times were 2.3% and 3.4%, respectively. The change in running speed of intervals and test runs from T1 to T2 correlated (r=0.75, p<0.001) in both datasets. Thus, the maximal sustainable effort intervals were able to predict 3-km and 10-km running performance and training adaptations with good accuracy, and current results demonstrate the potential usefulness of intervals as part of the monitoring process.

High-intensity functional training induces superior training adaptations compared with traditional military physical training. J Strength Cond Res 37(12): 2477-2483, 2023-This study examined the effectiveness of concurrent strength and endurance training with an emphasis on high-intensity functional training (HIFT) during military service. Voluntary male conscripts (aged 18-28 years) were placed in either an experimental (EXP: n = 50-66) or a control (CON: n = 50-67) group. The training for the EXP group included HIFT using body mass, sandbags, and kettlebells. The CON group trained according to the current practice. Physical performance and body composition were assessed at baseline (PRE), at week 10 (MID), and after (POST) the 19-week training period. Significance was set at p < 0.05. The total distance covered in a 12-minute running test increased in both groups, but the change in EXP was superior to the change in CON (11.6%, ES: 0.79 vs. 5.7%, ES: 0.33; p = 0.027). Maximal strength and power characteristics increased in EXP (3.1-5.0%), whereas no improvements were observed in CON. Conscripts with the highest initial fitness showed no improvements in physical performance in either group. Body mass and waist circumference decreased in EXP, whereas CON showed an increase in muscle mass. These findings suggest that HIFT is an effective and time-efficient approach to improve soldiers' aerobic fitness during military service. For the optimal development of strength, the training equipment used may not have provided sufficient and progressive loading to yield considerable strength adaptations. More focus should be placed on sufficient intensity and volume in both strength and endurance training, especially for the most fit soldiers.

The present study investigated performance changes during three runs (1 min recovery) of repeated military simulation task test (RMST) and its associations with physical performance. Voluntary male soldiers (N = 114) participated in a series of measurements of physical performance. Lower body explosive force production, anaerobic endurance and upper body strength endurance together explained 58% of the variance in the first RSMT (p < 0.001). The same variables explained the variance in the second and third runs of RSMT by 60% and 51%, but explosive force production was replaced with aerobic endurance, assessed by the 3.2 km loaded run (p < 0.001). This study demonstrated that the role of explosive power of the lower body decreased and military specific aerobic endurance increased when occupational performance was assessed under acute fatigue even during a short high-intensity test. These results may benefit tactical strength and conditioning coaches in training optimisation for improved occupational performance in military.Practioners summary: Soldiers are required to perform occupational tasks in a repeated manner with short recovery time. In the present study, the role of explosive power of the lower body decreased and military specific aerobic endurance increased when occupational performance was assessed with a repeated high-intensity task specific performance test.

The aim of this study was to investigate effects of a single infrared sauna (IRS) session on post-exercise recovery of neuromuscular performance, autonomic nervous system function, subjective sleep quality, and muscle soreness. Male basketball players (n = 16) performed two trials consisting of a complex resistance exercise protocol (maximal strength with plyometrics), followed by either 20 min passive recovery (PAS) or IRS (temperature 43 ± 5°C), in a randomized crossover design, with trials separated by one week. Recovery of neuromuscular performance was assessed using 20 m maximal sprint, maximal countermovement-jump (CMJ), and isometric leg press tests, performed 14 hours after exercise. Heart rate (HR), heart rate variability (HRV), sleep diary, muscle soreness, and indirect muscle damage markers were measured pre and post exercise. The decrease in CMJ performance from pre- to post-exercise was attenuated after IRS compared to PAS (p < 0.01). The IRS session resulted in higher HR and lower root mean square of successive differences between normal heartbeats (RMSSD), and high and low frequency power, compared to PAS (p < 0.002). Post-exercise night-time HR and HRV did not differ following IRS vs. PAS. Muscle soreness was less severe, and perceived recovery was higher after IRS compared to PAS (p < 0.01). Post-exercise IRS attenuated the drop in explosive performance and decreased subjective muscle soreness after resistance training, which may enhance mood, readiness, and physical performance of an athlete. A single IRS session had no detrimental effects on recovery of the autonomic nervous system.

Effects of combined strength and endurance training on body composition, physical fitness, and serum hormones during a 6-month crisis management operation. J Strength Cond Res 36(9): 2361-2370, 2022-Very few studies have examined the impact of training interventions on soldier readiness during an international military operation. Therefore, the present study investigated the effects of combined strength and endurance training on body composition, physical performance, and hormonal status during a 6-month international military deployment consisting of typical peacekeeping tasks, e.g., patrolling, observation, and on-base duties. Soldiers ( n = 78) were randomly allocated to a control group (C) or one of 3 combined whole-body strength and endurance training groups with varying strength-to-endurance training emphasis (Es = 25/75%, SE = 50/50% or Se = 75/25% of strength/endurance training). Body composition, physical performance (3000-m run, standing long jump [SLJ], isometric maximal voluntary contraction of the lower [MVC lower] and upper extremities [MVC upper ], muscle endurance tests), and selected serum hormone concentrations were determined prior to training (PRE), and after 9 (MID) and 19 (POST) weeks of training. Within- and between-group changes were analyzed using linear regression models. The average combined strength and endurance training frequency of the total subject group was 3 ± 2 training sessions per week. No changes were observed in physical performance variables in the intervention groups, whereas SLJ decreased by 1.9% in C ( p < 0.05). Maximal voluntary contraction lower increased by 12.8% in the combined intervention group ( p < 0.05), and this was significantly different to C ( p < 0.05). Testosterone-to-cortisol ratio increased in SE and Se ( p < 0.05), whereas no change was observed in C. The intervention groups maintained or improved their physical performance during deployment, which is beneficial for operational readiness. However, the high interindividual variation observed in training adaptations highlights the importance of training individualization during prolonged military operations.

This study investigated acute responses and post 24-h recovery to four running sessions performed at different intensity zones by supine heart rate variability, countermovement jump, and a submaximal running test. A total of 24 recreationally endurance-trained male subjects performed 90 min low-intensity (LIT), 30 min moderate-intensity (MOD), 6×3 min high-intensity interval (HIIT) and 10×30 s supramaximal-intensity interval (SMIT) exercises on a treadmill. Heart rate variability decreased acutely after all sessions, and the decrease was greater after MOD compared to LIT and SMIT (p<0.001; p<0.01) and HIIT compared to LIT (p<0.01). Countermovement jump decreased only after LIT (p<0.01) and SMIT (p<0.001), and the relative changes were different compared to MOD (p<0.01) and HIIT (p<0.001). Countermovement jump remained decreased at 24 h after SMIT (p<0.05). Heart rate during the submaximal running test rebounded below the baseline 24 h after all sessions (p<0.05), while the rating of perceived exertion during the running test remained elevated after HIIT (p<0.05) and SMIT (p<0.01). The current results highlight differences in the physiological demands of the running sessions, and distinct recovery patterns of the measured aspects of performance. Based on these results, assessments of performance and recovery from multiple perspectives may provide valuable information for endurance athletes, and help to improve the quality of training monitoring.

Heart rate (HR) and heart rate variability (HRV) can be monitored with wearable devices throughout the day. Resting HRV in particular, reflecting cardiac parasympathetic activity, has been proposed to be a useful marker in the monitoring of health and recovery from training. This study examined the validity of the wrist-based photoplethysmography (PPG) method to measure HR and HRV at rest. Recreationally endurance-trained participants recorded pulse-to-pulse (PP) and RR intervals simultaneously with a PPG-based watch and reference heart rate sensor (HRS) at a laboratory in a supine position (n = 39; 5-min recording) and at home during sleep (n = 29; 4-h recording). In addition, analyses were performed from pooled laboratory data (n = 11344 PP and RR intervals). Differences and correlations were analyzed between the HRS- and PPG-derived HR and LnRMSSD (the natural logarithm of the root mean square of successive differences). A very good agreement was found between pooled PP and RR intervals with a mean bias of 0.17 ms and a correlation coefficient of 0.993 (p < 0.001). In the laboratory, HR did not differ between the devices (mean bias 0.0 bpm), but PPG slightly underestimated the nocturnal recordings (mean bias -0.7 bpm, p < 0.001). PPG overestimated LnRMSSD both in the laboratory (mean bias 0.20 ms, p < 0.001) and nocturnal recordings (mean bias 0.17 ms, p < 0.001). However, very strong intraclass correlations in the nocturnal recordings were found between the devices (HR: 0.998, p < 0.001; LnRMSSD: 0.931, p < 0.001). In conclusion, PPG was able to measure HR and HRV with adequate accuracy in recreational athletes. However, when strict absolute values are of importance, systematic overestimation, which seemed to especially concern participants with low LnRMSSD, should be acknowledged.

J Strength Cond Res 35(2): 449-457, 2021-This study examined the effects of a 10-week period of high-intensity combined strength and endurance training on strength, endurance, body composition, and serum hormone concentrations in physically active women using hormonal contraceptives (HCs, n = 9) compared with those who had never used hormonal contraceptives (NHCs, n = 9). Training consisted of 2 strength training sessions and 2 high-intensity running interval sessions per week. Maximal bilateral isometric leg press (Isom), maximal bilateral dynamic leg press (one repetition maximum [1RM]), countermovement jump (CMJ), a 3,000-m running test (3,000 m), body composition, and serum hormone levels were measured before and after training between days 1-5 of each subject's menstrual cycle. Both groups increased 1RM and CMJ: HC = 13.2% (p < 0.001) and 9.6% (p < 0.05), and NHC = 8.3% (p < 0.01) and 8.5% (p < 0.001). Hormonal contraceptive improved 3,000 m by 3.5% (p < 0.05) and NHC by 1% (n.s.). Never used hormonal contraceptive increased lean mass by 2.1% (p < 0.001), whereas body fat percentage decreased from 23.9 ± 6.7 to 22.4 ± 6.0 (-6.0%, p < 0.05). No significant changes were observed in body composition in HC. No significant between-group differences were observed in any of the performance variables. Luteinizing hormone concentrations decreased significantly (p < 0.05) over 10 weeks in NHC, whereas other hormone levels remained statistically unaltered in both groups. It seems that the present training is equally appropriate for improving strength, endurance, and body composition in women using HC as those not using HC without disrupting hypothalamic-pituitary-gonadal axis function.

The aims of the current study were to examine the relationships between heart rate variability (HRV), salivary cortisol, sleep duration and training in young athletes. Eight athletes (16 ± 1 years) were monitored for 7 weeks during training and competition seasons. Subjects were training for endurance-based winter sports (cross-country skiing and biathlon). Training was divided into two zones (K1, easy training and K2, hard training). Heart rate and blood lactate during submaximal running tests (SRT), as well as cortisol, sleep duration and nocturnal HRV (RMSSD), were determined every other week. HRV and cortisol levels were correlated throughout the 7-week period (r = -0.552, P = 0.01), with the strongest correlation during week 7 (r = -0.879, P = 0.01). The relative changes in K1 and HRV showed a positive correlation from weeks 1-3 (r = 0.863, P = 0.006) and a negative correlation during weeks 3-5 (r = -0.760, P = 0.029). The relative change in sleep during weeks 1-3 were negatively correlated with cortisol (r = -0.762, P = 0.028) and K2 (r = -0.762, P = 0.028). In conclusion, HRV appears to reflect the recovery of young athletes during high loads of physical and/or physiological stress. Cortisol levels also reflected this recovery, but significant change required a longer period than HRV, suggesting that cortisol may be less sensitive to stress than HRV. Moreover, our results indicated that during the competition season, recovery for young endurance athletes increased in duration and additional sleep may be beneficial.

NEW FINDINGS: What is the central question of the study? What are the effects of caffeine on neuromuscular function in a non-fatigued state and during fatiguing exercise? What is the main finding and its importance? In a non-fatigued state, caffeine decreased the duration of the silent period evoked by transcranial magnetic stimulation. Caffeine-induced reduction of inhibitory mechanisms in the central nervous system before exercise was associated with an increased performance. Individuals who benefit from caffeine ingestion may experience lower perception of effort during exercise and an accelerated recovery of M-wave amplitude postfatigue. This study elucidates the mechanisms of action of caffeine and demonstrates that inter-individual variability of its effects on neuromuscular function is a fruitful area for further work. ABSTRACT: Caffeine enhances exercise performance, but its mechanisms of action remain unclear. In this study, we investigated its effects on neuromuscular function in a non-fatigued state and during fatiguing exercise. Eighteen men participated in this randomized, double-blind, placebo-controlled crossover trial. Baseline measures included plantarflexion force, drop jump, squat jump, voluntary activation of triceps surae muscle, soleus muscle contractile properties, M-wave, α-motoneuron excitability (H-reflex), corticospinal excitability, short-interval intracortical inhibition, intracortical facilitation, silent period evoked by transcranial magnetic stimulation (SP) and plasma potassium and caffeine concentrations. Immediately after baseline testing, participants ingested caffeine (6 mg·kg-1 ) or placebo. After a 1-h rest, baseline measures were repeated, followed by a fatiguing stretch-shortening cycle exercise (sets of 40 bilateral rebound jumps on a sledge apparatus) until task failure. Neuromuscular testing was carried out throughout the fatigue protocol and afterwards. Caffeine enhanced drop jump height (by 4.2%) and decreased the SP (by 12.6%) in a non-fatigued state. A caffeine-related decrease in SP and short-interval intracortical inhibition before the fatiguing activity was associated with an increased time to task failure. The participants who benefitted from an improved performance on the caffeine day reported a significantly lower sense of effort during exercise and had an accelerated postexercise recovery of M-wave amplitude. Caffeine modulates inhibitory mechanisms of the CNS, recovery of M-wave amplitude and perception of effort. This study lays the groundwork for future examinations of differences in caffeine-induced neuromuscular changes between those who are deemed to benefit from caffeine ingestion and those who are not.

INTRODUCTION: Physical fitness is strongly related to health and may offer valuable information about public health. We investigated trends in physical fitness, leisure-time physical activity (LTPA), and anthropometry of young healthy adult Finnish men in representative population-based samples between 2003 and 2015. METHODS: Three independent cross-sectional samples of 18- to 35-year-old Finnish men were assessed in 2003 (n = 889), 2008 (n = 803), and 2015 (n = 690). Cardiorespiratory (VO2 max) and muscular fitness (1-minute sit-ups and push-ups), body mass, and height were measured. Self-reported LTPA was assessed. RESULTS: After adjusting for age, education, and smoking, cardiorespiratory fitness was higher in 2003 (mean: 43.5, 95%CI: 42.9-44.1 mL/kg/min) compared to 2008 (41.3, 95%CI: 40.7-41.9 mL/kg/min) and 2015 (40.6, 95%CI: 40.0-41.2 mL/kg/min) (P < .001), whereas no difference was observed between 2008 and 2015. The lowest values in muscular fitness were observed in 2003, while no clear trends were further noticed. The adjusted BMI was higher in 2008 (25.1, 95%CI: 24.9-25.4) and 2015 (25.3, 95%CI: 25.3, 95%CI: 25.0-25.6) compared to 2003 (24.5, 95%CI: 24.3-24.8) (P < .005). In 2015, a higher proportion of individuals exercised at least four times per week compared to 2003 and 2008 (P < .05). CONCLUSION: The decrease in cardiorespiratory fitness that took place between 2003 and 2008 plateaued after 2008. The plateau is in accordance with the previously observed trend of 5-10 years younger Finnish men. Moreover, muscular fitness was for the most part higher in 2008 and 2015 compared to 2003. Efforts directed to promote regular physical activity and improve physical fitness are needed.

BACKGROUND: Despite numerous studies providing evidence for positive effects of physical activity and physical fitness, evidence for association between physical fitness and health-related quality of life (HRQoL) in young adults is limited. The aim of the present cross-sectional study was to investigate the association of cardiorespiratory and muscular fitness with HRQoL from the perspective of its physical and mental components among young adult Finnish males. METHODS: The sample consisted of 754 men, with the mean age of 26 years (SD 6.7 years), who participated in the military refresher training. HRQoL was measured using the Finnish RAND 36-item health survey. Cardiorespiratory fitness was determined by a bicycle ergometer test, and muscular fitness by various tests measuring maximal strength and muscular endurance. Logistic regression modelling was used to compare low, moderate and high physical and mental component of HRQoL scores to the respective levels of muscular and cardiorespiratory fitness. RESULTS: The findings of the adjusted (age, educational level, marital status, employment status, smoking, use of alcohol and BMI) analysis showed that cardiorespiratory and muscular fitness are positively associated with both physical and mental components of HRQoL. In terms of the physical component of HRQoL, even a moderate fitness level was positively associated with better HRQoL. In terms of the mental component of HRQoL, the impact was seen only in the group with the highest fitness level. CONCLUSIONS: The findings suggest a positive contribution of physical fitness to mental health and highlight the importance of both muscular and cardiorespiratory fitness in the promotion of HRQoL. Even lighter forms of physical activity that result in moderate physical fitness could contribute to the physical component of HRQoL. In terms of the mental component of HRQoL, higher levels of physical fitness may be needed to gain higher levels of HRQoL among young males.

BACKGROUND: There is growing evidence on positive effects of physical activity (PA) on mental health. However, the focus of previous research on this relationship has typically been on mental health from the perspective of mental health problems rather than from the perspective of mental wellbeing. Further, previous research has commonly focused rather on leisure time PA without evidence on the role of other domains of PA. The aim of the present cross-sectional study was to investigate the relationship between positive mental health (PMH) and different domains of PA in young Finnish men. The secondary aim was to examine the reasons for physical inactivity among individuals with a low level of PMH. METHODS: Positive mental health (measured with Short Warwick-Edinburgh Mental Wellbeing Scale, SWEMWBS), self-reported leisure time, occupational and commuting PA as well as reasons for physical inactivity were measured using questionnaires (n = 456, mean age 29 years) among young Finnish males. Logistic regression modelling was used to generate odds for low and high levels of positive mental health for different levels of PA and sociodemographic variables. RESULTS: A weak positive association between leisure time PA and PMH was found in men with a low level of PMH (OR = 0.33, 95% CI 0.13-0.86). No association was found in the domains of commuting and occupational PA. Multivariate logistic regression analysis showed lower level of leisure time PA, unemployment and being single independently predicting low level of PMH. No associations were found between any domains of PA and high level of PMH. The most common reasons for physical inactivity among men with a low level of PMH were lack of interest (28%) and unwillingness to practise sports alone (27%). CONCLUSIONS: The relationship between physical activity and positive mental health seems to vary between different domains of physical activity. The findings highlight the important role of leisure time physical activity, particularly in men with a low level of positive mental health. Strategies aimed at increasing physical activity for mental health benefits should focus particularly on providing opportunities for leisure time physical activity involving social interactions for men with lower mental wellbeing.

This study aimed to examine the characteristics of electromyography (EMG) and kinematics of the supporting leg affecting energy cost while running at incline, level, and decline slopes. Twelve male Japanese middle- and long-distance runners volunteered for this study. The subjects were asked to run at 13.5 km·h-1 on a treadmill under three slope conditions. Sagittal plane kinematics and the EMG of the lower limb muscles, respiratory gases were recorded. Energy cost differed significantly between slopes, being the lowest in decline slope and the greatest in incline slope. Integrated EMG (iEMG) of leg extensor muscles was greater in the incline slope than in the decline slope, and iEMG of the gastrocnemius and soleus muscles correlated positively with energy cost. The knee and ankle joint kinematics were associated with energy cost during running. In incline slope, the knee and ankle joints were more extended (plantarflexed) to lift the body. These movements may disturb the coordination between the ankle and knee joints. The gastrocnemius muscle would do greater mechanical work to plantarflex the ankle joint rather than transfer mechanical energy as well as greater mechanical work of mono-articular muscles. These muscular activities would increase energy cost.

J Strength Cond Res 34(12): 3369-3376, 2020-Differences in body composition and performance were investigated between elite and U20 male ice hockey players of 2 different national standards. One hundred seventy-nine players were recruited from the highest Finnish (n = 82) and Danish (n = 61) national level, as well as from 1 U20 team from Finland (n = 19) and Denmark (n = 17). Body composition and countermovement jump performance (CMJ) were measured off-ice in addition to on-ice assessments of agility, 10- and 30-m sprint performance, and endurance capacity (the maximal Yo-Yo Intermittent Recovery Level 1 Ice Hockey Test, Yo-Yo IR1-IHmax). Large differences in on-ice performances were demonstrated between Finnish and Danish elite players for agility, 10- and 30-m sprint performance (2-3%, P ≤ 0.05), and Yo-Yo IR1-IHmax performance (15%, P ≤ 0.05). By contrast, no differences (P > 0.05) were present between elite players for CMJ ability or body composition. However, elite players possessed more body and muscle mass than U20 players. Finally, the Finnish U20 cohort had a similar performance level as the Danish elite players and superior 10-m sprint performance, whereas the Danish U20 level was inferior to the other groups in every performance assessment (P ≤ 0.05). In conclusion, on-ice speed and endurance differ markedly between elite players of 2 different national standards with no distinction in body composition or CMJ ability. Moreover, the most consistent difference between U20 and senior elite players was related to body and muscle mass. These results highlight the usefulness of on-ice assessments and suggest the importance of on-ice high-intensity training in elite players in addition to training targeted the development of lean body mass in youth prospects.

BACKGROUND: Bone stress fractures are overuse injuries commonly encountered in sports and military medicine. Some fatigue fractures lead to morbidity and loss of active, physically-demanding training days. We evaluated the incidence, anatomical location, risk factors, and preventive measures for fatigue fractures in young Finnish male conscripts. METHODS: Five cohorts of 1000 men performing military service, classified according to birth year (1969, 1974, 1979, 1984, 1989), were analysed. Each conscript was followed for his full military service period (180 days for conscripts with rank and file duties, 270 days for those with special training, 362 days for officers and highly trained conscripts). Data, including physical activity level, were collected from a standard pre-information questionnaire and from the garrisons' healthcare centre medical reports. Risk factor analysis included the conscripts' service class (A, B), length of military service, age, height, weight, body mass index, smoking, education, previous diseases, injuries, and subjective symptoms, as well as self-reports of physical activity before entering the service using a standard military questionnaire. RESULTS: Fatigue fractures occurred in 44 (1.1%) of 4029 men, with an incidence of 1.27 (95% confidence interval: 0.92-1.70) per 1000 follow-up months, and mostly (33/44, 75%) occurred at the tibial shaft or metatarsals. Three patients experienced two simultaneous stress fractures in different bones. Most fatigue fractures occurred in the first 3 months of military service. Conscripts with fatigue fractures lost a total of 1359 (range 10-77) active military training days due to exemptions from duty. Conscripts reporting regular (> 2 times/week) physical activity before entering the military had significantly fewer (p = 0.017) fatigue fractures. Regular physical activity before entering the service was the only strong explanatory, protective factor in the model [IRR = 0.41 (95% CI: 0.20 to 0.85)]. The other measured parameters did not contribute significantly to the incidence of stress fractures. CONCLUSION: Regular and recurrent high-intensity physical activity before entering military service seems to be an important preventive measure against developing fatigue fractures. Fatigue fractures should be considered in conscripts seeking medical advice for complaints of musculoskeletal pain, and taken into consideration in planning military and other physical training programs.

BACKGROUND: A small cross sectional area (CSA) of the paraspinal muscles may be related to low back pain among military aviators but previous studies have mainly concentrated on spinal disc degeneration. Therefore, the primary aim of the study was to investigate the changes in muscle CSA and composition of the psoas and paraspinal muscles during a 5-year follow up among Finnish Air Force (FINAF) fighter pilots. METHODS: Study population consisted of 26 volunteered FINAF male fighter pilots (age: 20.6 (±0.6) at the baseline). The magnetic resonance imaging (MRI) examinations were collected at baseline and after 5 years of follow-up. CSA and composition of the paraspinal and psoas muscles were obtained at the levels of 3-4 and 4-5 lumbar spine. Maximal isometric strength tests were only performed on one occasion at baseline. RESULTS: The follow-up comparisons indicated that the mean CSA of the paraspinal muscles increased (p < 0.01) by 8% at L3-4 level and 7% at L4-5 level during the 5-year period. There was no change in muscle composition during the follow-up period. The paraspinal and psoas muscles' CSA was positively related to overall maximal isometric strength at the baseline. However, there was no association between LBP and muscle composition or CSA. CONCLUSIONS: The paraspinal muscles' CSA increased among FINAF fighter pilots during the first 5 years of service. This might be explained by physically demanding work and regular physical activity. However, no associations between muscle composition or CSA and low back pain (LBP) experienced were observed after the five-year follow-up.

J Strength Cond Res 33(6): 1488-1495, 2019-The aim of this study was to compare the effectiveness of 3 water immersion interventions performed after active recovery compared with active recovery only on physical and mental performance measures and physiological responses. The subjects were physically active men (age 20-35 years, mean ± SD 26 ± 3.7 years). All subjects performed a short-term exercise protocol, including maximal jumps and sprinting. Four different recovery methods (10 minutes) were used in random order: cold water immersion (CWI, 10° C), thermoneutral water immersion (TWI, 24° C), and contrast water therapy (CWT, alternately 10° C and 38° C). All these methods were performed after an active recovery (10-minute bicycle ergometer; heart rate [HR] 120-140 b·min, 60-73% from age-calculated maximum HR), and the fourth method was active recovery (ACT) only. Within 96 hours after exercise bouts, recovery was assessed through a 30-m maximal sprint test, maximal countermovement jump (CMJ), self-perceived muscle soreness and relaxation questionnaires, and blood lactate, creatine kinase, testosterone, cortisol, and catecholamine levels. The self-perceived feeling of relaxation after 60-minute recovery was better (p < 0.05) after CWI and CWT than ACT and TWI. Statistically significant differences were not observed between the recovery methods in any other marker. In the 30-m sprint test, however, slower running time was found in ACT (p < 0.001) and CWT (p = 0.005), and reduced CMJ results (p < 0.05) were found in ACT when the results were compared with baseline values. Based on these findings, it can be concluded that CWI and CWT improve the acute feeling of relaxation that can play a positive role in athletes' performance and well-being.
