RESUMO

Platypnea orthodeoxia is an uncommon condition but given its associations with severe lung disease and advancing age, it might be seen in the palliative care population. Presentation is classical in the association of symptoms with postural change. It should be considered when patients present with breathlessness or desaturation given the minimally invasive tests of echocardiography, and the potential for procedural resolution of the defect and thus symptoms. Further, there has not previously been reported a case presenting with nausea (rather than dyspnoea) secondary to platypnea orthodeoxia syndrome. This unusual presentation is a reminder that there is a broad range of potential subjective experiences of the same physiological events. Careful history and examination are vital in elucidating the underlying cause.

RESUMO

Despite the World Health Organization promoting breastfeeding as the optimal feeding method for infants, the breastfeeding initiation rates within these disadvantaged groups' remains low. It is important to identify the factors that prevent these groups from initiating and establishing successful breastfeeding. This review aims to identify the breastfeeding experiences of teenage mothers and mothers from low income groups. Qualitative research papers were identified using electronic and manual searches by following a systematic methodology. Nine relevant articles were critically analysed using a qualitative Critical Appraisal Skills Programme tool. Coding of the nine papers generated six themes. The benefits of breastfeeding known to these mothers were often superseded by the perceived barriers of breastfeeding, while the decision to breastfeed was frequently influenced by their social support network and prior exposure to breastfeeding. Disadvantaged mothers may require extra input and support to overcome any problems associated with breastfeeding. Developing healthcare professionals' capabilities to educate these disadvantaged groups, their social networks and the public about breastfeeding is crucial if it is to become established within our society.

RESUMO

This case report presents the importance of articulation between legal professionals with the expertise of those who work in different areas of animal science and the activists of animal cause. The report is based on the experience that took place in the interior of the State of Bahia, with a donkey herd, the target of foreign groups interested in donkey hide exploration. The animals were rescued from mistreatment and slaughter, thanks to efficient legal work, aided by several areas of the veterinary sciences, and supported politically by the movement of animal activists. The union between activism and technical knowledge in the areas of health, breeding, nutrition, animal welfare, and legal knowledge is a tool that should not be overlooked. On the contrary, it has proved effective, confirming a strong and innovative link capable of saving animals, promoting their welfare, generating technical knowledge, and new and promising proposals for intersectoral action.(AU)


Este caso relata a experiência da articulação entre profissionais da área jurídica com a expertise daqueles que atuam nas diversas áreas da ciência animal e os militantes do ativismo da causa animal. O relato de caso toma por base a experiência ocorrida no interior do estado da Bahia, no Brasil, com um rebanho de jumentos, alvo de grupos estrangeiros interessados na exploração de pele. Os animais foram salvos de maus-tratos e do abate graças ao eficiente trabalho jurídico, auxiliado por várias áreas da ciência animal e apoiado politicamente pelo movimento de ativistas da causa animal. A união entre o ativismo e o conhecimento técnico é poderosa ferramenta capaz de estabelecer relações, conexões e promover a divulgação do conhecimento na comunidade e, principalmente, salvar a vida de animais humanos e animais não humanos.(AU)

RESUMO

OBJECTIVE: To develop a reduced-sodium bread in which salt (NaCl) is partially replaced with potassium, magnesium and calcium salts. METHODS: Experimental bread was compared against standard brown bread, after a drop test (to assess whether bread quality deteriorates with abuse) and after the usual baking practices for baking properties (volume, crust colour, crumb colour and cell structure), sensory properties and nutritional composition. Plant production feasibility was evaluated in an industrial plant. Breads produced there were subjected to sensory evaluation using triangulation tests in a panel of 122 consumers. Twenty-four samples of both standard and experimental bread were laboratory-analysed for sodium, potassium, magnesium and calcium content. RESULTS AND CONCLUSION: A 32.3% reduced-sodium brown bread was developed that was acceptable in terms of baking qualities, appearance, texture and taste. The potassium, magnesium and calcium contents of the bread were increased by 55.2%, 69.0%, and 34.8%, respectively.
