RESUMO

CLINICAL RELEVANCE: A well-polished cement surface increases the viability and spreading of gingival fibroblasts. The tested resin composite cements did not reveal any cytotoxic effects. SUMMARY: Objective: This in vitro study aimed to investigate the effect of cement type and roughness on the viability and cell morphology of human gingival fibroblasts (HGF-1).Methods and Materials: Discs of three adhesive (Panavia V5 [PV5], Multilink Automix [MLA], RelyX Ultimate [RUL] and three self-adhesive (Panavia SA plus [PSA], SpeedCem plus [SCP], RelyX Unicem [RUN]) resin composite cements were prepared with three different roughnesses using silica paper grit P180, P400, or P2500. The cement specimens were characterized by surface roughness and energy-dispersive X-ray spectroscopic mapping. A viability assay was performed after 24 hours of incubation of HGF-1 cells on cement specimens. Cell morphology was examined with scanning electron microscopy.Results: The roughness of the specimens did not differ significantly among the different resin composite cements. Mean Ra values for the three surface treatments were 1.62 ± 0.34 µm for P180, 0.79 ± 0.20 µm for P400, and 0.17 ± 0.08 µm for P2500. HGF-1 viability was significantly influenced by the cement material and the specimens' roughness, with the highest viability for PSA ≥ RUN = MLA ≥ SCP = PV5 > RUL (p<0.05) and for P2500 = P400 > P180 (p<0.001). Cell morphology did not vary among the materials but was affected by the surface roughness.Conclusion: The composition of resin composite cements significantly affects the cell viability of HGF-1. Smooth resin composite cement surfaces with an Ra of 0.2-0.8 µm accelerate flat cell spreading and formation of filopodia.

OBJECTIVE:: The purpose of this study was to establish a clinically appropriate light-curing moment for resin composite cements while achieving the highest indirect tensile strength and lowest polymerization shrinkage. METHODS AND MATERIALS:: Polymerization shrinkage of seven resin composite cements (Multilink Automix, Multilink Speed Cem, RelyX Ultimate, RelyX Unicem 2 Automix, Panavia V5, Panavia SA plus, VITA Adiva F-Cem) was measured at ambient temperatures of 23°C and 37°C. Testing was done for autopolymerized and light-cured specimens after light application at either 1, 5, or 10 minutes after mixing. Indirect tensile strength of all cements was measured after 24 hours of storage at temperatures of 23°C and 37°C, for autopolymerized and light-cured specimens after light application 1, 5, or 10 minutes after mixing. To illustrate filler size and microstructures, SEM images of all cements were captured. Statistical analysis was performed with one-way ANOVA followed by post hoc Fisher LSD test ( α=0.05). RESULTS:: Final polymerization shrinkage of the resin composite cements ranged from 3.2% to 7.0%. An increase in temperature from 23°C to 37°C as well as the light-curing moment resulted in material dependent effects on the polymerization shrinkage and indirect tensile strength of the cements. Polymerization shrinkage of the cements did not correlate with the indirect tensile strength of the cement in the respective groups. Highest indirect tensile strengths were observed for the materials containing a homogeneous distribution of fillers with a size of about 1 µm (Multilink Automix, Panavia V5, VITA Adiva F-Cem). CONCLUSION:: The magnitude of the effect of light-curing moment and temperature increase on polymerization shrinkage and indirect tensile strength of resin composite cements is material dependent and cannot be generalized.

A total of 1303 fine needle aspiration biopsies (FNABs) were performed in all 105 kidney transplanted patients at our institution during 1990-1992: 838 were in patients who never had an acute cellular rejection (ACR), and 465 were in patients who had experienced an ACR; 393 of these FNABs were performed in first rejections and 72 in second rejections. The immunosuppressive protocol included monotherapy with cyclosporin A and an initial, additional course of Minnesota-ALG during the first ten days after transplantation (Tx). OKT3 was first-line antirejection therapy accompanied for a short period with prednisolone and azathioprine to oppose the development of anti-OKT3 antibodies. FNABs were taken each day for the first three days after Tx, then twice a week for the first month, and then at each visit to the outpatient clinic for the next three months. In non-rejecting grafts the total corrected increment (median) of the inflammatory response (TCI) increased from 1 to a maximum of 3.5, 17-21 days after Tx followed by a slight decrease. The inflammatory response was mainly due to non-activated lymphocytes. In the rejecting grafts the TCI (median) increased from three days before ACR to a maximum of 7 on the day of ACR, followed by a decrease when OKT3 treatment was started. The infiltrating cells were activated lymphocytes, other lymphocytes and, in smaller amounts, macrophages and monocytes.

RESUMO

Parathyroid carcinoma is a slow growing tumor, and the patients most often die from complications to the hypercalcemia. Therefore, any attempt should be made to remove local recurrence and metastasis surgically, as medical treatment is disappointing. A case treated with extensive vascular surgery to the neck is reported.

RESUMO

Aortocaval fistula is an unusual complication of ruptured abdominal aortic aneurysm. A case of coincident rupture of an abdominal aortic aneurysm into both the retroperitoneum and the inferior vena cava is presented. The symptoms and the treatment are discussed.

RESUMO

Lymphocele is a rare complication after renal transplantation. The symptomatology, diagnosis and treatment have been reported by several authors [1-9]. The symptoms are often insidious, variable and may result in difficult diagnostic problems. In this paper we present three cases of lymphocele as a surgical complication observed among 190 cases of renal transplantation.

RESUMO

The course of blood pressure and antihypertensive treatment (AHT) was studied in 27 renal transplant recipients immunosuppressed with cyclosporine-A (CyA) as monotherapy. At the time of transplantation 13 patients received AHT and 14 were normotensive. The final outcome of transplantation and CyA immunosuppression was 19 patients without AHT, 4 with reduced AHT and 3 with unchanged doses of AHT, while in 1 case a de novo mild hypertension necessitated diuretic treatment.

RESUMO

A case of vascular graft infection secondary to Salmonella septicaemia is described. The patient was a 69 year old man with an aortic bifurcation graft and a femoropopliteal bypass. Despite antibiotics treatment, operation was necessary. An especially long extra-anatomical bypass was used.

RESUMO

In cases of critical limb ischemia caused by aorto-iliac arteriosclerosis the most common operation is aortobifemoral bypass. This operation has a high patency. We describe two cases in which a thoracic aortobifemoral bypass was chosen as an alternative bypass operation because of a hostile abdomen. Both operations were performed without complications and resulted in normally perfused extremities.

RESUMO

The standard management of subclavian artery aneurysm has until now been surgical repair. The treatment of these aneurysms with transluminal placement of a prosthetic graft stent device is a new approach and seems to be feasible and safe. One case is presented. This new technique warrants further clinical assessment and trial.

RESUMO

Analysis of 156 first cadaver-kidney transplants performed in one transplantation center revealed a better graft survival in the group of transfused patients compared to the non-transfused patients. The degree of statistical significance between the groups improved with increasing number of pre-transplant blood transfusions. Pregnancy had no influence on graft survival. As could be expected the frequency of lymphocytotoxic antibodies before transplantation increased with increasing number of blood transfusions. However, lymphocytotoxic antibodies seemed not to influence graft survival.
