In this study, we report the case of a Chinese patient with systemic lupus erythematosus (SLE) who developed neutropenia after treatment by olanzapine for the SLE-related psychiatric symptoms. The relationship between agranulocytosis, SLE and olanzapine is still unknown. Fcγ receptor IIIb (FcγRIIIb) is a low-affinity receptor, constitutively expressed only by neutrophils; NA1 and NA2 have been identified as representing polymorphisms of FcγRIIIb. NA1 is associated with the incidence of autoimmune neutropenia and is particularly frequent in Asiatic ethnic groups. The Chinese patient resulted to be homozygous for NA1. We suggest that the presence of NA1 allele may be a predisposing factor to olanzapine-induced agranulocytosis in patients with SLE. Hence, the analysis of FcγRIIIb polymorphism should be investigated in other cases of antipsychotic-induced agranulocytosis.

OBJECTIVE: The aim of this study is to develop a diagnostic-therapeutic algorithm for those suffering from tinnitus who seek emergency aid. MATERIALS AND METHODS: A literature review has been performed on articles from the last 30 years. RESULTS: It is important to activate medical or surgical diagnostic and therapeutic strategies, in order to safeguard and rehabilitate the various functions affected. Psychiatric comorbidity is the most frequent pathological condition of those with serious or catastrophic tinnitus. In these cases, mortality risk is linked to suicide, morbidity to tinnitus-correlated distress. CONCLUSIONS: Tinnitus, mainly linked to loss of hearing, is a frequent symptom among the population at large. About 7% of those affected by tinnitus turn to their doctor to solve their problem, while between 0.5 and 2% request urgent medical assistance. Their cry for help may be the result of an acute onset of tinnitus or the rapid impairment of an already chronic condition. Tinnitus is not considered an urgent ear, nose and throat (ENT) condition by the Associazione Otorinolaringologi Ospedalieri Italiani (AOOI) [Italian Association of Hospital ENT], even though there are many pathological conditions, sometimes serious, associated with tinnitus and emergency action is necessary to reduce the risk of morbidity and mortality.

OBJECTIVE: Dissociative symptoms are frequent among psychiatric patients and may considerably affect patients' psychopathological condition and treatment outcomes. The objectives of the study are to assess the presence of dissociative symptoms in female patients with mood and anxiety disorders, to investigate their correlation with the clinical severity of the disorders and to investigate those personality traits that are more frequent in patients with high levels of dissociation. PATIENTS AND METHODS: 50 Caucasian females were enrolled in the study. Patients were assessed through the Self-Report Symptom Check-List, the Dissociative Experiences Scale (DES) and rating scales for Depression and Anxiety. RESULTS: The mean DES score in the overall sample was 16.6. 32% of patients had a DES score > 20. Depressive symptoms positively correlated with the DES total scores. Dissociator patients presented some significantly different temperamental characteristics in comparison with non dissociator patients. CONCLUSIONS: Dissociative symptoms are highly present in patients with mood and anxiety disorders and correlate with the severity of depressive symptoms. Specific personality traits more frequently observed in dissociator people may represent predisposing factors; their early identification could be clinically relevant.

In PTSD, sleep disorders represent an important symptoms dimension which is associated with more severe PTSD and increased risk of relapse. The basic treatment for PTSD is not always associated to an improvement of sleep disturbances and nightmares. Alpha-blockers, and more specifically Prazosin, have shown a specific action on sleep disorders in PTSD. We report the clinical case of a young women with PTSD, who was suffering from severe sleep disorder and distressing nightmare. The patient was treated with Terazosin, a conger of Prazosin, and has shown symptom remission. Further studies on the use of alpha-blokers might reveal new therapeutic options in PTSD.

RESUMO

The strong comorbidity between metabolic diseases and severe psychiatric disorders, although object of extensive scientific literature in recent years, is still a challenge for researchers, in order to clarify the pathogenic mechanisms that underlie this relationship, and for clinicians, in order to optimize strategies for prevention and early interventions. Particular attention is linked to insulin resistance, which can represent a primum movens able to adversely affect the course of the dysmetabolic and psychiatric frameworks. Through extensive review of the scientific literature on PubMed, we analyze the latest scientific findings about the bidirectional nature of this comorbidity. Our aim is also to identify an instrument applicable on a large scale in psychiatric populations, able to quickly recognize the insulin resistance. In particular, the HOMA (Homeostasis Model Assessment) index for its features of high specificity, sensitivity, low-cost and easy execution of the method, can be a useful tool, predictive and diagnostic at the same time, which can significantly improve the quality of life and the course of disease for these patients.

Deep transcranial magnetic stimulation (TMS) is a technique of neuromodulation and neurostimulation based on the principle of electromagnetic induction of an electric field in the brain. The coil (H-coil) used in deep TMS is able to modulate cortical excitability up to a maximum depth of 6 cm and is therefore able not only to modulate the activity of the cerebral cortex but also the activity of deeper neural circuits. Deep TMS is largely used for the treatment of drug-resistant major depressive disorder (MDD) and is being tested to treat a very wide range of neurological, psychiatric and medical conditions. The aim of this review is to illustrate the biophysical principles of deep TMS, to explain the pathophysiological basis for its utilization in each psychiatric disorder (major depression, autism, bipolar depression, auditory hallucinations, negative symptoms of schizophrenia), to summarize the results presented thus far in the international scientific literature regarding the use of deep TMS in psychiatry, its side effects and its effects on cognitive functions.

The objective of this study was to assess the feasibility of recording pulmonary venous blood flow in the human fetus, and to evaluate its relationship with gestational age. We studied 152 singleton pregnancies between the 19th and 40th weeks of gestation. One hundred and one cases were studied cross-sectionally to gather data on color flow visualization, and 51 were studied longitudinally. The upper right pulmonary vein was interrogated by pulsed wave Doppler to assess pulmonary venous blood flow. On color Doppler, the upper right pulmonary vein could be imaged in 89.6% of cases in the peri-atrial tract and in 75% of cases within the lung. The upper left pulmonary vein could only be imaged in 8% of cases close to the atrium and in 41% of cases within the lung. Reliable velocity waveforms were obtained in 91% of cases. The mean systolic peak velocity was 22.19 +/- 6.39 cm/s and the mean diastolic peak velocity was 22.1 +/- 6.35 cm/s. Both increased significantly with gestational age. Reversed end-diastolic blood flow was present in 18% of cases, regardless of gestational age and fetal heart rate. Expressed as a percentage of the forward flow velocity time integral, its value was 7.65 +/- 5.2%. Our data seem to confirm the presence of pulsatile pulmonary venous blood flow in the second- and third-trimester fetus. Normative data have been established for the second and third trimesters of pregnancy.

RESUMO

Congenital inguinal hernias, relatively common among infants, have rarely been reported in utero. In this brief report, the prenatal diagnosis of this condition based on movement of the intestinal content within the ileal loops is described. The differential diagnosis with other masses protruding from the abdominal wall (omphalocele) or the perineal region (sacrococcygeal teratoma, hydrocele) is discussed.
