BACKGROUND: The evolving COVID-19 pandemic has and continues to present a threat to health system capacity. Rapidly expanding an existing acute care physician workforce is critical to pandemic response planning in large urban academic health systems. INTERVENTION: The Medical Emergency-Pandemic Operations Command (MEOC)-a multi-specialty team of physicians, operational leaders, and support staff within an academic Department of Medicine in Calgary, Canada-partnered with its provincial health system to rapidly develop a comprehensive, scalable pandemic physician workforce plan for non-ventilated inpatients with COVID-19 across multiple hospitals. The MEOC Pandemic Plan comprised seven components, each with unique structure and processes. METHODS: In this manuscript, we describe MEOC's Pandemic Plan that was designed and implemented from March to May 2020 and re-escalated in October 2020. We report on the plan's structure and process, early implementation outcomes, and unforeseen challenges. Data sources included MEOC documents, health system, public health, and physician engagement implementation data. KEY RESULTS: From March 5 to October 26, 2020, 427 patients were admitted to COVID-19 units in Calgary hospitals. In the initial implementation period (March-May 2020), MEOC communications reached over 2500 physicians, leading to 1446 physicians volunteering to provide care on COVID-19 units. Of these, 234 physicians signed up for hospital shifts, and 227 physicians received in-person personal protective equipment simulation training. Ninety-three physicians were deployed on COVID-19 units at four large acute care hospitals. The resurgence of cases in September 2020 has prompted re-escalation including re-activation of COVID-19 units. CONCLUSIONS: MEOC leveraged an academic health system partnership to rapidly design, implement, and refine a comprehensive, scalable COVID-19 acute care physician workforce plan whose components are readily applicable across jurisdictions or healthcare crises. This description may guide other institutions responding to COVID-19 and future health emergencies.

Background: Females who experience hypertensive disorders of pregnancy (HDP) have an increased lifelong risk of cardiovascular disease. Thus, Canadian clinical practice guidelines recommend cardiovascular risk reduction follow-up after a patient has HDP. This study examined the experiences of patients with HDP who attended a specialized, longitudinal general internal medicine postpartum cardiovascular risk reduction clinic called PreVASC. PreVASC focuses on comprehensive cardiovascular risk reduction through cardiovascular risk factor screening and management tailored specifically for female patients after they have HDP. Methods: This multimethod study examined the experiences of female patients with HDP via the following: (i) a quantitative survey (summarized with descriptive statistics); (ii) semistructured qualitative patient phone interviews (results grouped thematically); and (iii) triangulation of qualitative themes with quantitative survey results. Results: Overall, 37% of eligible clinic patients (42 of 115) participated; 79% of participants (n = 33) reported being "very satisfied" with the PreVASC clinic's specialized longitudinal model of care, and 95% (n = 40) reported making at least one preventive health behaviour change after receiving individualized counselling on cardiovascular risk reduction. Qualitative results found improvements in patient-reported cardiovascular health knowledge, health behaviours, and health-related anxiety. A preference for in-person vs phone clinic visits was reported by participants. Conclusions: An in-person, general internal medicine specialist-led, longitudinal model of cardiovascular disease preventive care focused specifically on cardiovascular risk reduction after HDP had positive impacts on patient experience, health knowledge, and preventive health behaviours. This novel knowledge on patient preferences for a longitudinal, specialized model of care advances cardiovascular risk reduction tailored specifically for high-risk people after HDP.


Contexte: Les femmes qui sont atteintes de troubles hypertensifs de la grossesse (THG) présentent un risque accru de maladie cardiovasculaire durant leur vie. Par conséquent, les lignes directrices canadiennes de pratique clinique recommandent un suivi pour la réduction du risque cardiovasculaire après la survenue d'un THG. Cette étude visait à examiner l'expérience des patientes qui ont été atteintes de THG et qui ont fréquenté l'une des cliniques de médecine interne spécialisées dans la réduction du risque cardiovasculaire post-partum et offrant une prise en charge longitudinale (PreVASC). Les cliniques PreVASC se concentrent sur la réduction des risques cardiovasculaires globaux par la détection des facteurs de risque cardiovasculaire et une prise en charge spécialement adaptée pour les femmes qui ont subi un THG. Méthodologie: Cette étude visait à examiner l'expérience des femmes atteintes d'un THG en faisant appel à diverses méthodes : i) sondage quantitatif (résumé par des statistiques descriptives); ii) entrevues téléphoniques semi-structurées de nature qualitative avec des patientes (résultats regroupés par thèmes); et iii) triangulation des thèmes qualitatifs et des résultats du sondage quantitatif. Résultats: Globalement, 37 % des patientes admissibles (42 sur 115) ont participé à l'étude; 79 % des participantes (n = 33) ont déclaré être « très satisfaites ¼ du modèle de soins longitudinal spécialisé des cliniques PreVASC, et 95 % (n = 40) ont déclaré avoir adopté au moins un comportement préventif pour leur santé après avoir reçu des conseils personnalisés sur la réduction du risque cardiovasculaire. Les résultats qualitatifs obtenus auprès des patientes font état d'une amélioration des connaissances sur la santé cardiovasculaire, les comportements sains et l'anxiété liée à la santé. Les participantes ont dit préférer les visites cliniques en personne aux consultations par téléphone. Conclusions: L'adoption d'un modèle longitudinal de médecine interne comprenant des rencontres avec des spécialistes pour prévenir les maladies cardiovasculaires, en particulier réduire le risque cardiovasculaire après un THG a eu des effets positifs chez les patientes en ce qui concerne l'expérience, les connaissances en matière de santé et les comportements à adopter pour prévenir les problèmes de santé. Ces nouvelles connaissances sur les préférences des patientes à l'égard de soins longitudinaux spécialisés représentent un pas en avant dans la mise en place d'une approche personnalisée de réduction du risque cardiovasculaire pour les personnes présentant un risque élevé après un THG.

BACKGROUND: Information in health administrative databases increasingly guides renal care and policy. STUDY DESIGN: Systematic review of observational studies. SETTING & POPULATION: Studies describing the validity of codes for acute kidney injury (AKI) and chronic kidney disease (CKD) in administrative databases operating in any jurisdiction. SELECTION CRITERIA: After searching 13 medical databases, we included observational studies published from database inception though June 2009 that validated renal diagnostic and procedural codes for AKI or CKD against a reference standard. INDEX TESTS: Renal diagnostic or procedural administrative data codes. REFERENCE TESTS: Patient chart review, laboratory values, or data from a high-quality patient registry. RESULTS: 25 studies of 13 databases in 4 countries were included. Validation of diagnostic and procedural codes for AKI was present in 9 studies, and validation for CKD was present in 19 studies. Sensitivity varied across studies and generally was poor (AKI median, 29%; range, 15%-81%; CKD median, 41%; range, 3%-88%). Positive predictive values often were reasonable, but results also were variable (AKI median, 67%; range, 15%-96%; CKD median, 78%; range, 29%-100%). Defining AKI and CKD by only the use of dialysis generally resulted in better code validity. The study characteristic associated with sensitivity in multivariable meta-regression was whether the reference standard used laboratory values (P < 0.001); sensitivity was 39% lower when laboratory values were used (95% CI, 23%-56%). LIMITATIONS: Missing data in primary studies limited some of the analyses that could be done. CONCLUSIONS: Administrative database analyses have utility, but must be conducted and interpreted judiciously to avoid bias arising from poor code validity.

BACKGROUND: Simulation-based training's impact on learning outcomes may be related to cognitive load or emotions during training. We evaluated the association of validated measures of cognitive load and emotion with learning outcomes in simulation-based obstetric internal medicine cases. METHODS: All internal medicine learners (n = 15) who completed the knowledge test pre-training, post-training (knowledge acquisition), and at 3-6 months (knowledge retention) for all three simulation cases were included. RESULTS: Mean knowledge scores differed over time in all three cases (p < 0.0001 for all). Knowledge retention scores were significantly higher only for cases 1 and 3. Cognitive load associated with frustration was positively associated with knowledge acquisition for case 2 (beta = 5.18, P = 0.007), while excitement was negatively associated with knowledge retention in case 1 (beta = -33.07, p = 0.04). CONCLUSION: Simulation-based education for obstetric internal medicine can be effective in select cases. Attention to cognitive load and emotion may optimize learning outcomes.

RESUMO

Tofacitinib, a potentially teratogenic nonselective Janus Kinase inhibitor was used as salvage therapy for ulcerative colitis during pregnancy with corticosteroids, maintenance ustekinumab, and rectal 5-ASA therapy. Corticosteroid-free remission ensued, resulting in term delivery without congenital malformations and avoidance of colectomy.

BACKGROUND: Knowledge of any harm associated with living kidney donation guides informed consent and living donor follow-up. Risk estimates in the literature are variable, and most studies did not use a healthy control group to assess outcomes attributable to donation. METHODS: We observed a retrospective cohort using health administrative data for donations which occurred in Ontario, Canada between the years 1993 and 2005. There were a total of 1278 living donors and 6359 healthy adults who acted as a control group. Individuals were followed for a mean of 6.2 years (range, 1-13 years) after donation. The primary outcome was a composite of time to death or first cardiovascular event (myocardial infarction, stroke, angioplasty, and bypass surgery). The secondary outcome was time to a diagnosis of hypertension. RESULTS: There was no significant difference in death or cardiovascular events between donors and controls (1.3% vs. 1.7%; hazard ratio 0.7, 95% confidence interval 0.4-1.2). Donors were more frequently diagnosed with hypertension than controls (16.3% vs. 11.9%, hazard ratio 1.4, 95% confidence interval 1.2-1.7) but were also seen more often by their primary care physicians (median [interquartile range] 3.6 [1.9-6.1] vs. 2.6 [1.4-4.3] visits per person year, P<0.001). CONCLUSIONS: Based on administrative data, the risk of cardiovascular disease was unchanged in the first decade after kidney donation. The observed increase in diagnosed hypertension may be due to nephrectomy or more blood pressure measurements received by donors in follow-up and requires prospective study.

BACKGROUND: The conventional calcium correction formula (corrected total calcium (mmol/L) = TCa (mmol/L) + 0.02 [40 (g/L) - albumin (g/L)]) is broadly applied for the estimation of serum calcium in haemodialysis (HD) patients, despite the fact that it was not derived or validated in a HD population. A novel formula was derived and validated for corrected serum calcium in HD patients. METHODS: Total calcium (TCa), ionized calcium (iCa(2+)), magnesium, phosphate, albumin and bicarbonate were collected from 60 HD patients to derive the formula. A validation set of 237 stable HD patients was then examined, and subjects were classified as hyper-, hypo- and normocalcaemic based on the iCa(2+). Agreement of the new formula was calculated with iCa(2+) as the gold standard, using the intraclass correlation coefficient (ICC). This was compared to the agreement between iCa(2+) and the following: uncorrected total serum calcium (TCa), the conventional correction formula, the Orrell formula and the Clase formula. RESULTS: Using multiple linear regression the following formula was derived: corrected total calcium (mmol/L) = TCa (mmol/L) + 0.01 [30 (g/L) - albumin (g/L)]. The new formula had superior agreement compared to all of the other formulae. There was a statistically significant greater agreement between the new formula and the iCa(2+) as compared to the conventional formula (P < 0.01). However, the new formula did not significantly outperform the Orrell formula, the Clase formula or Total calcium. CONCLUSIONS: The use of our simple new formula should enable more appropriate decision making compared to the conventional formula in the highly complex HD population.

The frequency at which housestaff need to assess volume status on medical inpatients is unknown. In this brief report, we invited 39 housestaff, over 13 randomly selected dates, to complete a 25-item survey. Participants (n = 31, 79%) logged a total of 455 hours, reporting 197 pages or telephone requests received regarding medical inpatients. Of these, 41 pages (21%) required a volume status assessment. Participants reported their volume status assessment competency to be moderate (median score = 3, interquartile range = 3 to 4, where 1 = not competent to perform independently and 6 = above average competence). In 9 of the 41 assessments (22%), at least 1 barrier was reported in determining volume status. The most commonly reported barriers were conflicting physical examination findings (n = 8, 20%) and suboptimal patient examination (n = 5, 12%). Over 20% of pages regarding admitted medical patients required volume status assessments by medical housestaff. Despite moderate self-reported competence in the ability to assess volume status, barriers such as conflicting physical examination findings and suboptimal patient examinations were present in up to 20% of assessments. Therefore, we urge educators to consider incorporating bedside ultrasound training for volume status into the internal medicine curriculum.
