BACKGROUND: The global incidence of liver diseases is rising, yet there remains a dearth of precise research models to mimic these diseases. The use of normothermic machine perfusion (NMP) to study diseased livers recovered from liver transplantation (LT) recipients presents a promising avenue. Accordingly, we have developed a machine perfusion system tailored specifically for the human whole diseased livers and present our experience from the NMP of diseased livers. METHODS: Six diseased livers recovered from LT recipients with different diagnoses were collected. The diseased livers were connected to the machine perfusion system that circulated tailored perfusate, providing oxygen and nutrients. The pressure and flow of the system were recorded, and blood gas analysis and laboratory tests of perfusate and bile were examined to analyze the function of the diseased livers. Liver tissues before and after NMP were collected for histological analysis. RESULTS: Experiments showed that the system maintained the diseased livers in a physiological state, ensuring stable hemodynamics and a suitable partial pressure of oxygen and carbon dioxide. The results of blood gas analysis and laboratory tests demonstrated a restoration and sustenance of metabolism with minimal damage. Notably, a majority of the diseased livers exhibited bile production continuously, signifying their vivid functional integrity. The pathological characteristics remained stable before and after NMP. CONCLUSION: We successfully established the machine perfusion system tailored specifically for diseased human whole livers. Through the application of this system, we have developed a novel in vitro model that faithfully recapitulates the main features of human liver disease. This model holds immense promise as an advanced disease modeling platform, offering profound insights into liver diseases and potential implications for research and therapeutic development.

BACKGROUND: Trichothiodystrophy (TTD) is a rare, autosomal recessive, multisystem disorder most commonly caused by variants in ERCC2. CASE PRESENTATION: Here, we describe the first Chinese patient with a novel variant in ERCC2. A male infant, who was born to a healthy non-consanguineous couple, exhibited brittle hair, hair loss ichthyosis, eczema, retinal pigmentation and hypospadias. He carried a novel heterozygous ERCC2 variant. The maternal variant (c.2191-18_2213del) is a previous described genomic deletion that affects the splicing of intron 22. The paternal variant (c.1666-1G > A), that occurs in the splice site of intron 17 and likely alters ERCC2 gene function through aberrant splicing, has not been reported previously. CONCLUSIONS: Our case reported a novel pathogenic variant in ERCC2, which expanded the known genetic variants associated with TTD.

BACKGROUND: Group B streptococcus (GBS)-induced invasive disease is a major cause of illness and death among infants aged under 90 days in China; however, invasive GBS infection remains unknown in China. We aimed to describe the serotype and genotype distributions of early-onset disease (EOD) and late-onset disease (LOD), and to show the clinical correlations among various GBS serotypes and genotypes obtained from infants with invasive GBS infections. METHODS: Between June 1, 2016 and June 1, 2018, 84 GBS strains were collected from patients younger than 90 days at seven Chinese hospitals. Clinical data were retrospectively reviewed. GBS serotyping was conducted and multi-locus sequence typing was performed. RESULTS: Serotypes Ia, Ib, II, III, and V were detected. Serotype III (60.71%) was the most common, followed by Ia (16.67%) and Ib (14.29%). Intrapartum temperature ≥ 37.5 °C, chorioamnionitis, and mortality were noted in 28.57, 42.86, and 28.57% of patients with serotype Ia, respectively, and these rates were higher than those in patients with serotypes Ib and III (P = 0.041, P = 0.031, and P = 0.023, respectively). The incidence of respiratory distress was lower (P = 0.039) while that of purulent meningitis was higher (P = 0.026) in the serotype III group. Eighteen sequence types were detected among isolates, and ST17 [42.86% (36/84)] was the most prevalent. CONCLUSIONS: GBS isolates belonging to serotypes Ia, Ib, and III are common in southern mainland China, and ST17 is highly prevalent. Differences were found in the clinical manifestations of invasive GBS disease induced by serotypes Ia and III.

A tunable dual-wavelength filter based on degenerate modes with a ring dielectric rod inside the single point defect cavity is proposed. The band structures and mode profiles are computed by the plane-wave expansion method. The normalized transmission spectra for this structure are investigated by using the two-dimensional finite-difference time-domain method. The two orthogonal output modes of the filter can be regarded as the combination of the original degenerate cavity modes and both excited due to the introduced perturbation rod. The influences of the perturbation rod on the localized modes, band separations, and the tuning ranges are all investigated and analyzed based on the perturbation theory. With the ring dielectric rod being introduced into the defect, a narrower wavelength bandwidth and a wider wavelength tuning range can be obtained, which are superior to that of the filter based on a cavity with a larger radius rod. Besides, the perturbation rod can also be used for splitting degenerate modes for multiwavelength filters. The proposed filter has a simple structure and may be potentially applied in various integrated circuits, such as multiwavelength filters.

Road-deposited sediments (RDS) on urban impervious surfaces are important carriers of heavy metals (HMs), and can contribute to urban runoff pollution. With the outbreak of COVID-19, chlorinated disinfectants (CDs) have been extensively sprayed on these surfaces. This practice may have a superposed or priming effect on HMs contaminants in RDS, yet this remains unknown. This study examined the effects of seven CDs concentration gradients (0, 250, 500, 1000, 2000, 5000, 60,000 mg/L) on the leaching and chemical forms of HMs (Cd, Cr, Ni, Pb, and Zn) in seven particle size fractions (<44, 44-63, 63-105, 105-149, 149-250, 250-450, 450-1000 µm). The results showed that CDs can promote the leaching of HMs in RDS, at the recommended CDs dose (2000 mg/L), except for Pb, the leaching amounts increased by 21.8%-237.2% compared with the untreated RDS. The alteration in the leaching were primarily attributed to the redistribution of chemical forms of HMs in RDS, specifically, the acid-extractable fractions percentage increased by 0.23%-24.39%, and the reducible fractions percentages decreased by 3.21%-38.35%. The lower oxidation-reduction potential (ORP) and alkalinity of CDs as strong oxidants were responsible for the redistribution of forms. The leaching and chemical forms of HMs vary among different particle sizes, but in any case, finer particle sizes (< 105 µm) still dominate their contribution. The current control measure of street sweeping is ineffective in removing these particles. These findings will facilitate the development of strategies for controlling urban diffuse pollution from RDS during the pandemic. Finally, this study suggests potential directions for future research.

The cascade hydropower development in the Lancang River has significantly modified the hydrologic regime and is consequently responsible for many local environmental changes. The influence of the altered hydrological regime on heavy metals accumulation in the soils of the riparian zone was evaluated for the Xiaowan Reservoir (XWR). Specifically, this study focused on investigating the trace metals As, Cd, Cr, Cu, Hg, Ni, Pb, and Zn and their concentrations in the riparian soils. Furthermore, this research aimed to examine the contamination levels of heavy metals by employing the geoaccumulation index (Igeo) and the ecological risk index (RI), respectively. Additionally, the relationship between heavy metals and water level fluctuations as caused by the dam operation was explored. The results showed that heavy metals deposits occurred in relatively low levels in the riparian soils of XWR, even though several of these metals were revealed to occur in higher concentrations than the local background value. The Igeo assessment indicated that the riparian soils exhibited slight pollution by Hg at the Zhujie wharf (ZJW) and Cr at the transect of the Heihui River (HHR), and moderate contamination by As at ZJW. Moreover, the RI revealed that As in riparian soils is moderately hazardous while Hg poses a high risk at ZJW. The polluted water and sediments from upstream and upland of the riparian zone may be contributing to the changed concentrations of heavy metal in the riparian soils. The present study inferred that the WLFs due to reservoir impoundment play a vital role in the accumulation of trace metals in the riparian zone. However, more exhaustive investigations are necessary for verification.

Impoundment of the Three Gorges Reservoir has created an artificial riparian zone with a vertical height of 30 m and a total area of 349 km(2), which has been subjected to seasonal inundation and exposure due to regular reservoir impoundment and the occurrence of natural floods. The significant alteration of hydrologic regime has caused numerous environmental changes. The present study investigated the magnitude and spatial pattern of sedimentation and metal enrichment in a typical section of the riparian zone, composed of bench terraces with previous agricultural land uses, and explored their links to the changed hydrologic regime. In particular, we measured the total sediment depths and collected surface riparian sediments and down-profile sectioned riparian soils (at 5 cm intervals) for trace metal determination. Our analysis showed that the annual average sedimentation rates varied from 0.5 to 10 cm·yr(-1) and they decreased significantly with increasing elevation. This lateral distribution was principally attributed to seasonal variations in water levels and suspended sediment concentrations. Enriched concentrations of trace metals were found both in the riparian sediments and soils, but they were generally higher in the riparian sediments than in riparian soils and followed a similar lateral decreasing trend. Metal contamination assessment showed that the riparian sediments were slightly contaminated by Ni, Zn, and Pb, moderately contaminated by Cu, and moderately to strongly contaminated by Cd; while riparian soils were slightly contaminated by As, and moderately contaminated by Cd. Trace metal enrichment in the riparian sediments may be attributed to external input of contaminated sediments produced from upstream anthropogenic sources and chemical adsorption from dissolved fractions during pure sediment mobilization and after sink for a prolonged flooding period due to reservoir impoundment.
