OBJECTIVES: To conduct a systematic review on the translucency of bulk-fill (BF) composite materials, evaluating the factors influencing it and its clinical implications. MATERIALS AND METHODS: A systematic search was performed on four databases (Embase, Medline/PubMed, Scopus, and Web of Science) for articles published until October 2, 2023. Search terms were: "translucency," "translucency parameter," "contrast," "contrast ratio," "bulk," "bulk-fill," "bulk fill," "composite," "resin," "resin based," "resin-based," "restoration," and "dental." In vitro studies, written in English language, investigating BF resin-based composites translucency were included. The risk of bias was performed with the RoBDEMAT tool. RESULTS: Of 590 screened articles, 13 were included in this review. They investigated 11 high-viscosity BF and 5 low viscosity ones. The limited number of the identified studies and the heterogeneity of the extracted data did not allow to conduct a meta-analysis. CONCLUSIONS: BF materials exhibit variability in translucency due to variations in experimental designs and the heterogeneous composition. CLINICAL SIGNIFICANCE: BFs may behave differently in masking the substrate or in blending capacity.

OBJECTIVES: To conduct a scoping review on color stability of bulk-fill compared to conventional resin-based composites exposed to colored beverages. MATERIALS AND METHODS: In vitro studies, investigating bulk-fill resin-based composites color stability submitted to artificial staining by colored beverages were included. Studies investigating color change induced by smoke and whitening treatments as well as papers not written in English language were excluded. A systematic search, following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses extension for Scoping Reviews (PRISMA-ScR) was performed on four databases (Embase, Medline, Scopus, Web of Science) for articles published until October 01st 2022. The study selection was then performed by two authors who screened the abstracts independently and followed the JBI approach. RESULTS: Of 717 screened abstracts, 59 were selected for full-text analysis. Finally, 19 studies were included in this review. They investigated 19 different bulk-fill composites and different artificial-staining-by-liquids-protocols including coffee, red wine, tea, coke, and others. Seven papers reported higher color change in the investigated bulk fill materials than in conventional resin-based composites, while nine studies reported the opposite. SonicFill showed the highest color stability for most of the papers. CONCLUSIONS: The available evidence suggests that bulk-fill materials show variability in color stability. This behavior can be attributed to the heterogeneity of composition and staining procedures in the selected studies. CLINICAL SIGNIFICANCE: Clinicians should keep in mind that bulk-fill materials may change color when exposed to colorant drinks or food.

OBJECTIVE: To conduct a systematic review on color stability of dental resin-based composites (RBC) exposed to conventional and electronic cigarettes. MATERIALS AND METHODS: In vitro studies reporting on the color stability of RBC exposed to conventional cigarettes or to e-cigarettes: both Tobacco Heating Systems (THS) and Electronic Nicotine Delivery Systems (ENDS). The quality of the included studies was assessed with the QUIN tool (risk-of-bias tool for assessing in vitro studies conducted in dentistry). A systematic search, following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) was performed on four (n = 4) databases (Embase, Medline, Scopus, Web of Science) for articles published until March 28th, 2022. RESULTS: Of the 365 screened articles, 13 were included in this review. All the included articles analyzed conventional cigarette smoke (CS), four analyzed Electronic Nicotine Delivery Systems (ENDS) and two Tobacco Heating Systems (THS). In terms of study design, smoke exposure time, smoke flow, type and number of cigarettes a high variability was reported. CONCLUSIONS: The available evidence suggests that CS smoke significantly affects color stability. Electronic cigarettes show less color change that seems to be easily recovered under clinical acceptability thresholds, although evidence is scarce. CLINICAL SIGNIFICANCE: Clinicians should be aware, and should therefore warn their patients, that RBCs are subjected to irreversible color change if exposed to smoke. Electronic cigarettes (both ENDS and THS) induce less color change that can be recovered with repolishing or whitening procedures.

(1) Background: In the oral environment, sound enamel and dental restorative materials are immediately covered by a pellicle layer, which enables bacteria to attach. For the development of new materials with repellent surface functions, information on the formation and maturation of salivary pellicles is crucial. Therefore, the present in situ study aimed to investigate the proteomic profile of salivary pellicles formed on different dental composites. (2) Methods: Light-cured composite and bovine enamel samples (controls) were exposed to the oral cavity for 30, 90, and 120 min. All samples were subjected to optical and mechanical profilometry, as well as SEM surface evaluation. Acquired pellicles and unstimulated whole saliva samples were analyzed by SELDI-TOF-MS. The significance was determined by the generalized estimation equation and the post-hoc bonferroni adjustment. (3) Results: SEM revealed the formation of homogeneous pellicles on all test and control surfaces. Profilometry showed that composite surfaces tend to be of higher roughness compared to enamel. SELDI-TOF-MS detected up to 102 different proteins in the saliva samples and up to 46 proteins in the pellicle. Significant differences among 14 pellicle proteins were found between the composite materials and the controls. (4) Conclusions: Pellicle formation was material- and time-dependent. Proteins differed among the composites and to the control.

OBJECTIVE: The objective of this study was to review the experimental staining procedures performed in in-vitro studies for evaluating color stability of resin-based composites used for direct restorations. OVERVIEW: A review was performed with the purpose to identify in vitro procedures carried out to establish staining and color stability of resin-based composites used for direct restorations. A literature search was performed on four online databases (PubMed, Scopus, Embase and Web of Science) for articles published from January 1st 2001 till March 20th 2021, using a combination of free words and Medical Subject Headings (MESH) terms: "Composite Resins" AND "Color Stability" OR "Staining." Two reviewers screened titles and/or abstracts of 1728 unique studies. In total, 208 studies were selected for full-text reading, from which 178 were included in the review. The assessed variables were specimen dimensions, shape, and surface finishing; time before immersion; staining media type, dwell time, temperature and renewal interval; type of composite; color and translucency assessment parameters and measuring system. One hundred and seventy-eight articles analyzed color stability with staining liquids. Coffee was the most used staining medium (n = 102), followed by tea (n = 61), and red wine (n = 55). A disc-shaped specimen was used in most studies (n = 170) with a 2 mm thickness (n = 116). Spectrophotometers were the most used color assessment devices (n = 154), followed by colorimeters (n = 17). Color differences were quantified with ΔE (n = 154), ΔE00 (n = 23). Acceptability and or perceptibility thresholds were also taken into consideration (n = 126). CONCLUSIONS: The large variety of staining procedure suggests the need for standardization. CLINICAL SIGNIFICANCE: Aesthetic failure due to discoloration is a relevant clinical problem. Evaluating benefits and drawbacks of resin based composites artificial staining procedures will improve to predict materials clinical performances.

BACKGROUND: Evaluating the effect of different surface treatment methods on the micro-tensile bond strength (µTBS) of two different resin-matrix computer-aided design/computer-aided manufacturing (CAD/CAM) ceramics (RMCs). METHODS: A standardized inlay preparations were performed on 100 intact maxillary premolars. According to the type of the restorative material, the teeth were randomly divided into two equally sized groups (n = 50): (polymer-infiltrated ceramic (Vita Enamic) and resin-based composites (Lava Ultimate)). The inlays were fabricated using CAD/CAM technology. In each group, the specimens were randomly assigned to five subgroups (n = 10) according to the surface treatment method: group 1 used was the control group (no surface treatment); group 2, was treated with air abrasion with 50 µm Al2O3 (A) and universal adhesive (UA); group 3, was treated with air abrasion with 50 µm Al2O3 (A) and silane coupling agent (S); group 4, was treated with hydrofluoric acid (HF) and universal adhesive (UA) and group 5, was treated with Hydrofluoric acid (HF) + silane coupling agent (S). The inlays were then cemented to their respective preparations using dual-cure self-adhesive resin cement (RelyX U200, 3 M ESPE) according to the manufacturer's instructions. The µTBS test was conducted in all groups, and stereomicroscope and scanning electron microscope were used to inspect the failure mode. The data were statistically analyzed using a two-way analysis of variance (ANOVA) and Tukey's post-hoc multiple comparison tests at a significance level of p < 0.05. RESULTS: Surface treatments significantly increased the µTBS of the materials compared to the control group (p < 0.05). For CAD/CAM RBCs, the µTBS value highest in group 2 whereas, for PICN, the µTBS value was highest in group 3. Cohesive failure of CAD/CAM restorative material was the most predominant mode of failure in all treated groups, whereas adhesive failure at restoration-cement interface was the most predominant failure mode in the control group. CONCLUSION: Surface treatments increase the µTBS of resin-matrix CAD/CAM ceramics to tooth structure. Air abrasion followed by universal adhesive and hydrofluoric acid followed by silane application appears to be the best strategies for optimizing the bond strength of CAD/CAM RBCs and PICN respectively.

OBJECTIVE: The present study aimed to identify and quantify the elution of monomers of five different resin-based CAD/CAM blocks (RCBs) using HPLC. METHODS: Five different RCBs were used in the study: GC Cerasmart (CS), Voco Grandio blocs (GR), 3M Lava Ultimate (LU), Shofu Block (SB), and Vita Enamic (VE). Fifteen samples from each material were prepared using a low-speed precision diamond saw (ISOMET Buehler, Lake Bluff, IL, USA) at 5 × 5 × 4 mm size. After the preparation of samples, an extraction solution was mixed with %75/%25 ethanol/water. The samples were stored in the amber-colored bottles during three different immersed periods as 1 h, 24 h, and 90 days (n = 5). After immersion, 0.5 ml solutions were taken from each bottle and analyzed using HPLC. RESULTS: A total of 16.7 µg/ml of monomers from SB, 13.4 µg/ml of monomers from GR, 13.2 µg/ml of monomers from CS, and 6.7 µg/ml of monomers from LU were found after 3-m immersion. TEGDMA after 3-m of immersion was only released from the SB group, and also BisEMA was released from the CS group. Among the specimens immersed for 1 h, UDMA was released the least from the LU group and the most from the GR group (p < 0.05). Correspondingly, 24 h and 3 m after immersion, the highest release of monomers was found in the GR (p < 0.05). CONCLUSIONS: When the monomer release from RCBs was evaluated, it was shown that these materials released methacrylate-based monomers except VE, especially if they were kept in a solvent solution for a long time such as 3 m. CLINICAL SIGNIFICANCE: The novel resin-based CAD/CAM blocks might monomer release, which may cause cytotoxic effects. But, the detected amount of monomer release is below the estimated daily limit.

OBJECTIVES: This study investigated the fluorescence properties of the most commonly used fluorescent CAD/CAM materials for monolithic dental restorations and their suitability to perform the fluorescence-aided identification technique (FIT). MATERIALS AND METHODS: A total of 175 different color shades (n = 1) from 13 CAD/CAM material brands were analyzed with a monochromator-based microplate reader. Additionally, dentin, enamel, and combined dentin-enamel specimens (respectively, n = 11) were analyzed for comparison purposes. The maximum fluorescence intensity, the corresponding excitation and emission wavelength, and the total fluorescence for the wavelength spectrum λex = 395 nm - 415 nm used for FIT were determined. RESULTS: All assessed CAD/CAM ceramics showed virtually no total fluorescence for the wavelength spectrum λex = 395 nm - 415 nm used for FIT. CERASMARTTM, KZR-CAD HD 2, and LuxaCam Composite displayed total fluorescence values similar to that of the tooth hard substances. All other resin-based CAD/CAM materials showed a significantly higher total fluorescence than the tooth hard substances. CONCLUSIONS: Apart from the mentioned exceptions, all CAD/CAM materials assessed could be suitable for the FIT, either because they are more fluorescent than hard tooth substances or because they do not fluoresce at all at the respective wavelength of λex = 395 nm - 415 nm. CLINICAL RELEVANCE: This study provides insight into the not yet well-known fluorescent properties of dental CAD/CAM materials. This knowledge is not only necessary to reproduce the fluorescence properties of natural teeth but also for the applicability of diagnostic fluorescence inducing techniques.

PURPOSE: The aim of the study was to compare the 36-month clinical performances of a micro-hybride resin based composites (RBC) and a type II resin modified glass ionomer cement (RMGIC) in the treatment of cervical caries lesions. METHODS: Thirty-three patients (23 females,  10 males) with at least two cervical caries lesions participated in this study. A total of 110 restorations were randomly placed, half using RBC (Spectrum TPH3, Dentsply Sirona) with respective adhesive system (Prime & Bond NT, Dentsply Sirona) the other half using the RMGIC (Riva Light Cure, SDI, Bayswater, Victoria, Australia). Restorations were assessed at baseline, and after 6, 12, 18, 24 and 36 months using modified USPHS criteria. The data were analyzed statistically (p < 0.05). RESULTS: Recall rate was 90.91% at 36 months. Of the RBC lesions 84.3% (n = 43) and of the RMGIC restorations 92.2% (n = 47) were fully retained at 36 months. There was no significant difference between two materials in terms of retention and marginal adaptation (p > 0.05). No relation was found between caries activity, cavity dimensions and marginal adaptation (p > 0.05) but RMGIC restorations showed significantly more discoloration than RBC restorations after 36 months (p < 0.001). CONCLUSIONS: Considering the middle-term outcomes, both materials showed clinically successful performance in the treatment of cervical caries lesions.

OBJECTIVE: To evaluate coating materials regarding color and roughness changes when applied over a composite resin. MATERIALS AND METHODS: Specimens were made using Filtek Supreme Ultra. Coating materials were applied over the specimens' surface: Control (CTRL); Adper Single Bond Plus (SB); Scotchbond Universal (US); Clearfil SE (CL); Ambar APS (APS); Bioforty (BF); Fortify (FF); PermaSeal (PS); G-CoatPlus (GC). Specimens were evaluated regarding color changes (∆E00 ) using spectrophotometer (n = 10); surface roughness (Ra), using optical profilometer (n = 10); surface topography, using scanning electron microscopy (n = 1); and thickness of material, using optical microscopy (n = 1). Analyses were made before and after 24, 72, and 168 hours of storage in wine, used as a staining solution. Data was statistically analyzed using analysis of variance, Tukey and Pearson tests (α:.05). RESULTS: None of the materials presented color stability. ∆E00 increased with increasing storage time at red wine, until 72 hours. BF, FF, PS, SB, and CTRL presented the lowest ∆E00 after 24 hours. APS presented the highest ∆E00 (32.59; P < .05). Regarding roughness, initially, surface sealants and APS presented the lowest values. After 168 hours, GC presented the lowest Ra (0.60; P < .05). CONCLUSIONS: All the materials presented color and roughness changes after storage in red wine. CLINICAL RELEVANCE: Some coating materials can be applied over the surface of RBC restorations without changing its color stability or surface roughness.

OBJECTIVE: To compare the elastic modulus, flexural strength, and hardness of an experimental resin based composite (RBC) with and without containing silver nanoparticles (AgNPs) and bioactive glass (BAG) with a commercially available RBC. METHODS: This study was conducted, during the period August 2016-May 2018, at the Department of Dental Materials, Peshawar Dental College, Peshawar (Pakistan) and Department of Chemistry, University of Montreal, Canada. Test specimens made in the commercial RBC acted as Group-1 (G1). An experimental RBC containing 70 wt % filler content was synthesized. It was first used as such to prepare test specimens to act as the experimental control group (G2). This RBC was then modified by adding various amounts of BAG (5%, 10% and 15%) and a fixed amount of 0.009% AgNPs to use the so modified RBCs for preparing the test specimens to belong to three groups (G3, G4 & G5). The AgNPs had been synthesized in situ by reduction of salt during photo-polymerization. Flexural strength (FS), elastic modulus (EM) and Vickers hardness were determined using universal testing machine and hardness tester respectively. Data were analyzed using one-way ANOVA and Tukey post-hoc test. RESULTS: Except for G3 restorations showing significantly lower mean FS value, the FS for those in the other groups were not significantly different (p>0.05). Elastic modulus of the experimental RBC restorations was though higher than those of the others but the difference was statistically insignificant (p>0.05). Reduced Vickers hardness values were documented for the restorations in the G4 and G5 compared to those in the G3 but again the difference was insignificant (p>0.05). Flexural strength and hardness values of the test specimens in the experimental RBCs were significantly lower than those made in the commercial hybrid RBC (p<0.05). CONCLUSION: BAG and AgNPs addition to the experimental RBC in the mentioned concentration adversely affected the tested mechanical properties.

PURPOSE: To evaluate the effects of three fresh detox juices, including an orange, green, and red beverage, on the color stability and surface roughness of three anterior esthetic resin-based composites (RBCs). MATERIALS AND METHODS: Disk-shaped specimens were prepared with three different esthetic RBCs (Amaris, G-aenial Anterior, Clearfil Majesty ES-2) according to the manufacturers' instructions. Forty specimens were prepared for each RBC, and all specimens were stored in artificial saliva at 37°C for 24 hours. The initial color values and surface roughness measurements of the specimens were taken using a spectrophotometer and a profilometer. The specimens were then divided into 4 subgroups (n = 10). All specimens except the control specimens were immersed in their designated fresh detox juices (green, red, or orange) for 10 minutes twice a day. Color and surface roughness measurements were taken on day 15 and day 30, and the results were analyzed by one-way ANOVA and Tukey HSD test. The association between color change and surface roughness was evaluated by Spearman's Rank Correlation analysis. RESULTS: Color changes and surface roughness increased upon exposure to fresh detox juices for 15 and 30 days for all of the RBCs. All of the G-aenial and Amaris groups displayed color changes above the threshold of acceptability, whereas Clearfil Majesty ES-2 displayed a color change above the threshold of acceptability only after exposure to the red beverage for 30 days (ΔE > 3.7). With regard to surface roughness, Clearfil Majesty ES-2 outperformed the other RBCs (p < 0.001). According to Spearman's Rank Correlation analysis, there was no correlation between color change and surface roughness (p > 0.001). CONCLUSIONS: Exposure to the fresh detox juices used in this study led to similar color changes in the RBCs used in this study.

OBJECTIVES: To determine the effects of tooth brushing on five bulk-fill resin based composites (RBCs). METHOD: Ten samples of Filtek Supreme Enamel (control), Filtek One Bulk Fill, Tetric EvoCeram Bulk Fill, SonicFill 2, SDR flow+, and Admira Fusion X-tra were light cured for 20 seconds using the Valo Grand curing light. After 24 hours storage in air at 37°C, specimens were brushed in a random order using Colgate OpticWhite dentifrice and a soft toothbrush. Surface gloss was measured prior to brushing, after 5,000, 10,000 and 15,000 back and forth brushing cycles. Surface roughness was measured after 15,000 brushing cycles using atomic force microscopy (AFM) and selected scanning electron microscope (SEM) images were taken. The data was examined using ANOVA and pair-wise comparisons using Scheffe's post-hoc multiple comparison tests (α = 0.05). RESULTS: Surface gloss decreased and the surface roughness increased after brushing. Two-way ANOVA showed that both the RBC and the number of brushing cycles had a significant negative effect on the gloss. One-way ANOVA showed that the RBC had a significant effect on the roughness after 15,000 brushing cycles. For both gloss and roughness, brushing had the least effect on the nano-filled control and nano-filled bulk-fill RBC, and the greatest negative effect on Admira Fusion X-tra. The SEM images provided visual agreement. There was an excellent linear correlation (R2 = 0.98) between the logarithm of the gloss and roughness. CONCLUSION: After brushing, the bulk-fill RBCs were all rougher than the control nano-filled RBC. The nano-filled bulk-fill RBC was the least affected by brushing. CLINICAL SIGNIFICANCE: Bulk-fill RBCs lose their gloss faster and become rougher than the nanofilled conventional RBC, Filtek Supreme Ultra. The nanofilled bulk-fill RBC was the least affected by tooth brushing.

This study aims to quantify the influence of hue, value and opacity on the variation in light transmittance of a full color palette of an ormocer-based dental composite. Samples with a thickness of 2 mm were cured in real time while the incident irradiance and light transmittance were recorded with a spectrophotometer, either in real time during the polymerization or through the polymerized composite at different exposure distances. Across the entire shade range, light attenuation was high, varying between 70.3% and 92.1%. The light transmittance during polymerization increased exponentially with exposure time in all shades. The differences between the cured and uncured composites decrease with increasing value and with increasing opacity within a value. The pattern of variation in light transmittance with increasing value is non-linear and depends on the hue, but not on the opacity within a hue. Small variations in value in lighter shades of hue B reduce the transmitted light more than in hue A, while the opposite is true for darker shades. The results strongly suggest an adequate curing of the lower increments in larger restorations, as the additional light expected from curing the upper increments is very small, regardless of hue, value or opacity. An additional unfavorable condition by increasing the exposure distance consistently contributes to a reduction in light transmission and thus further supports the above statement.

Background Microplastics are acknowledged as significant environmental contaminants. The clinical use of dental materials, particularly adhesives containing plastic polymers, can give rise to the production of plastic micro- and nanoparticles, which subsequently find their way into the environment. The aim of the study was to detect different microplastics and identify them in various orthodontic adhesives. Materials and methods Four different light cure orthodontic adhesives, including Transbond XT (3M Unitek, Monrovia, CA), Ormco Enlight (Ormco, Orange, CA), Orthofix SPA (Orthofix, Verona, Italy), and Aqualine LC (Tomy International Inc, Tokyo, Japan), were collected and placed in separate Eppendorf tubes. Microplastics present in each adhesive were identified using scanning electron microscopy. Subsequently, each specimen was suspended in hydrogen peroxide, placed within a shaking incubator, and analyzed using Fourier transform infrared spectroscopy (FTIR) to identify the type of polymer. Results The scanning electron microscope shows the surface morphology and the most predominant types of microplastics identified were fibers, fragments, and pellets. FTIR results showed the presence of several major functional groups, including hydroxyl, amine, ester, fluoro, and halo groups. Conclusion When contrasted with the quantity of microplastic waste generated by other sectors like the textile, cosmetic, and fishing industries, the microparticulate waste stemming from dental adhesives has a minimal effect on environmental deterioration. Strategies for addressing this concern should give precedence to reducing the use of these materials and adopting effective recovery methods, which could potentially involve recycling processes.

OBJECTIVE: The ability of six curing lights to photocure four resin-based composites (RBCs) in a mold simulating a cavity was compared visually. MATERIALS AND METHODS: Four RBCs were photocured using the: Woodpecker B for 2x10s, SmartLite Pro 2x10s, Valo Cordless 2x10s, Valo Cordless 2x3s Xtra power, Valo X 2x10s, Valo X 2x5s Xtra power, PowerCure 2x3s mode, Monet 1x1s and Monet 3x1s, in a mold representing a molar Class II restoration. Immediately after photocuring, the RBC specimens were immersed in a solvent to remove the uncured RBC, after which they were photographed and de-identified. Using a REDCap survey, these images were compared visually to compare the ability of the LCUs to photocure the restorations. RESULTS: There were significant differences in how well the LCUs had photocured the RBCs. The SmartLite Pro and Valo X used for two 10s exposures produced restorations rated as the best cured, and the Monet used for 1 s was rated the worst. CONCLUSIONS: There were visually apparent differences in how well the LCUs could photocure the RBCs. The Monet used for 1 second produced the worst results for all four RBCs.

OBJECTIVES: Aim of this prospective study was to assess full mouth rehabilitation of severe tooth wear patients using minimally invasive CAD/CAM resin-based composite (RBC) restorations and direct veneers by evaluating restoration survival up to 5.5-years. METHODS: Twenty-two patients with generalized severe tooth wear with functional and/or esthetic problems were included. Following minimally invasive preparation, CAD/CAM RBC restorations (LAVA Ultimate,3M) were adhesively luted, direct RBC veneers (Filtek Supreme XTE, 3M) were applied in the aesthetic region. Patients were recalled after 1m,1y,3y,5y and seen in between recalls by their general dentists or at the clinical study center if complaints occurred. Failures were categorized as F1 (severe deficiencies requiring replacement/extraction), F2 (localized deficiencies requiring re-cementation/repair) and F3 (small chippings requiring refurbishment/monitoring). Survival of indirect restorations was evaluated using lifetables and Kaplan-Meier-graphs, distinguishing between failure categories and tooth type (front teeth=FT, premolars=PM, molars=M). F1 + F2 and F1 + F2 + F3 failures were analyzed using Cox regression on the variables tooth type/ location, age, gender and VDO increase (p < 0.05). RESULTS: 568 indirect restorations and 200 direct veneers in 21 patients evaluated for up to 5.5-years. For indirect restorations, 96 failures were recorded (F1:6;F2:41;F3:49) and annual failure rates were 0.29%(FT), 1.56%(PM), 2.93%(M) for F1 +F2 and 0.53%(FT), 2.42%(PM), 6.11%(M) for F1 + F2 + F3. Reasons for failure were chipping fracture (48), adhesive fracture (32), complete debonding (7), caries (4), endodontic treatment (1) and reasons unknown (documentation general dentists, 4). Molar tooth type had a statistically significantly increased probability of failure compared with front teeth and premolars for F1 + F2 + F3 (p < 0.006). Direct veneer restorations showed 18 failures (F1:2;F2:9;F3:7). SIGNIFICANCE: Minimally invasive CAD/CAM RBC restorations combined with direct RBC veneers showed an acceptable clinical mid-term survival for restorative rehabilitation of severely worn dentitions.

Harnessing the power of light and its photonic energy is a powerful tool in biomedical applications. Its use ranges from biomaterials processing and fabrication of polymers to diagnostics and therapeutics. Dental light curable materials have evolved over several decades and now offer very fast (≤ 10 s) and reliable polymerization through depth (4-6 mm thick). This has been achieved by developments on two fronts: (1) chemistries with more efficient light absorption characteristics (camphorquinone [CQ], ~30 L mol-1 cm1 [ʎmax 470 nm]; monoacylphosphine oxides [MAPO], ~800 L mol-1 cm-1 [ʎmax 385 nm]; bisacylphosphine oxide [BAPO], ~1,000 L mol-1 cm-1 [ʎmax 385 nm]) as well mechanistically efficient and prolonged radical generation processes during and after light irradiation, and; (2) introducing light curing technologies (light emitting diodes [LEDs] and less common lasers) with higher powers (≤ 2 W), better spectral range using multiple diodes (short: 390-405 nm; intermediate: 410-450 nm; and long: 450-480 nm), and better spatial power distribution (i.e. homogenous irradiance). However, adequate cure of materials falls short for several reasons, including improper selection of materials and lights, limitations in the chemistry of the materials, and limitations in delivering light through depth. Photonic energy has further applications in dentistry which include transillumination for diagnostics, and therapeutic applications that include photodynamic therapy, photobiomodulation, and photodisinfection. Light interactions with materials and biological tissues are complex and it is important to understand the advantages and limitations of these interactions for successful treatment outcomes. This article highlights the advent of photonic technologies in dentistry, its applications, the advantages and limitations, and possible future developments.

OBJECTIVES: The present study aimed to assess the applicability of Orr's equation on light-cured resin-based composites (RBCs) tested in 3- and 4-point bending tests, as well as assess the operator reliability of this method. METHODS: Fracture mirrors of 320 specimens (n = 320) made of four RBCs that had failed in the two tests were analyzed microscopically and consequently two radii of each specimen were measured by two operators, each measuring twice. The mirror constant A was calculated using Orr's equation. The results of the two operators were compared using a t-Test and further assessment of the data was performed using a MANOVA, Tukey's post hoc test and regression analysis. RESULTS: With the exception of 11 specimens, the measurement of the fracture mirror radii was carried out successfully. The calculation of the mirror constant proved successful with a high coefficient of determination (R2 > 0.98). The calculated mirror constants were operator-independent if the results of more than one measurement was taken into consideration. The ratio of the mirror constant A to the fracture toughness KIc lay between 2.1 and 2.4 for three of the four RBC. The fourth RBC had a ratio of 1.6 and was the only one where different mirror constants were observed as a function of the testing method. SIGNIFICANCE: The present study proves the applicability of brittle fracture mechanics on RBCs despite them often being regarded as not brittle due to their also plastic behavior.

Recent studies suggest that the incorporation of graphene in resin-based dental materials might enhance their mechanical properties and even decrease their degree of contraction during polymerization. The present study aimed at comparing the three-point flexural strength (FS), the compressive strength (CS), and the Vickers hardness (VH) of a CAD/CAM poly-methylmethacrylate (PMMA)-based resin, a recently introduced graphene-reinforced CAD/CAM PMMA-based resin (G-PMMA), and a conventional dental bis-acryl composite resin (BACR). No significant differences (p > 0.05) were detected among the materials in terms of flexural strength. On the other hand, a mean flexural modulus value of 9920.1 MPa was recorded in BACR group, significantly higher compared to the flexural modulus detected for G-PMMA (2670.2 MPa) and for conventional PMMA (2505.3) (p < 0.05). In terms of compressive modulus (MPa) and compressive strength (MPa), BACR was significantly stiffer than PMMA and G-PMMA. Concerning VH measurements, a significantly increased hardness emerged comparing the BACR group (VH 98.19) to both PMMA and G-PMMA groups (VH 34.16 and 34.26, respectively). Based on the finding of the present study, the graphene-reinforced (PMMA)-based polymer herein tested was not superior to the conventional PMMA and seemed not able to be considered as an alternative material for permanent restorations, at least in terms of hardness and mechanical response to compressive stress. More research on the mechanical/biological properties of G-PMMAs (and on graphene as a filler) seems still necessary to better clarify their potential as dental restorative materials.
