RESUMO

Transient osteoporosis or transient bone marrow oedema is a rare cause of acute hip pain that predominantly affects adults of middle and younger age. We report on the MR image in 8 patients with transient bone marrow oedema of the hip and in one patient with affection of the knee joint. In three of these, sympathetic nerve blockade has been performed. The MR image after sympathicolysis is discussed.

The present ophthalmologic study is the third part of a major epidemiologic health investigation on work with a video display terminal (VDT). An initial study showed that VDT operators replying to a questionnaire reported more eye discomfort than a reference group not employed in VDT work and that women reported more eye discomfort, musculoskeletal discomfort, headache, and skin disorders than men, irrespective of whether or not they were employed in VDT work. In the present study the ophthalmologic history of eye diseases and eye discomfort yielded a much lower percentage response for symptoms and discomfort than the questionnaire, and, just as with visual acuity and refraction, there was no difference between the exposed and reference groups or between the men and women. The exposed subjects were found to be overcorrected in terms of presbyopia addition in relation to work distance. As regards ocular examination findings, low frequency rates were noted for pathological lens opacities. Opacities of this kind were slightly more frequent among the VDT operators than among the referents, but the difference was not statistically significant. There were no other differences in the ocular findings of the exposed and reference groups.

RESUMO

A case of a malignant triton tumor, a malignant schwannoma with a rhabdomyoblastic component is described. The tumor appeared in the right thyroid gland with no relationship to a peripheral nerve. Beneath pulmonary metastases the tumor produced bone metastases and showed responsiveness to radiotherapy and treatment with ifosfamide. These findings are uncommon in malignant triton tumors.

While the mortality rate in patients with acute vascular mesenteric insufficiency is extremely high when angiography is not performed, it can be lowered by consistent angiographic examination. In a period of 16 years, 62 patients were investigated by angiography. A primarily vascular mechanism had caused the condition in 59 patients. In 25 cases arterial embolism was found, in 11 cases arterial thrombosis, in 3, venous thrombosis, and in 20 cases non-occlusive mesenteric ischemia was present. The mortality was 53%. Thus angiography is indicated whenever there is any suspicion of a mesenteric vascular disorder.

The importance of diagnostic imaging in malignant gastrointestinal tumors has moved from the primary diagnosis towards staging and follow-up studies. An accurate staging by diagnostic radiology is the cornerstone in planning radiation therapy. Especially in estimating the T- and N-stage as well as the primary diagnosis in esophagus-, gastric and rectal cancer endoscopy with endoscopic ultrasound is the method of choice. When planning radiation therapy barium studies of the esophagus and contrast enema of the colon are important. Only in the case of gastric lymphoma an upper gastrointestinal series is necessary. In the case of suspected recurrent rectal cancer CT-guided biopsy distinguishes between tumor recurrence and reactive fibrosis.

A cell surface glycoprotein (designated T100) of apparent m.w. 100,000 by SDS-PAGE under reducing and nonreducing conditions was precipitated from NP-40 extracts of surface radiolabeled thymocytes from a variety of inbred strains of mice by the standard noncongenic Lyt-2.1-typing serum. The inbred stain distribution, trypsin sensitivity on intact cells, and apparent m.w. of T100 suggest that it is different from Lyt-2.1. Inheritance and expression of T100 suggest that it is determined by an allele at a single locus, and testing of CXB recombinant inbred strains and B6.C minor histocompatibility congenic strains suggest that this locus is linked to H-25. Antiserum absorption experiments, two-stage cytotoxicity assays, and results of immunoprecipitations performed after prebinding antibody to radiolabeled thymocytes suggest that some T100 is accessible to antibody on the intact cell surface. However, for unknown reasons the number of cells required to absorb anti-T100 precipitating activity from antiserum was much higher than for removal of anti-Lyt-2.1 activity. A molecule with properties of T100 was also detected on lymph node cells and on the AKTB-1 lymphoma.

In vitro studies have shown that rifampicin is an effective antibiotic for mycobacteria infections. Two species of tropical fish, the Firemouth Cichild Cichlasoma meeki and the Congo Tetra Phenacogrammus interruptus, were used to determine whether oral application of rifampicin might serve as an effective treatment for mycobacteriosis in tropical fish. Fish of the two species were infected with M. marinum under controlled conditions. Six or twelve weeks after infection, treatment was begun with medicated fish food containing rifampicin in combination with tetracyclin. Histological examination of epithelial cell granuloma in the anterior and posterior kidneys, as well as in liver and spleen, showed that antibiotic treatment could somewhat reduce the intensity of, but could not successfully eliminate infection. In addition, after treatment, acid-resistant rods could still be isolated from histological samples and M. marinum could be cultured from organ samples.

PURPOSE: To evaluate the use of the transjugular intrahepatic portosystemic shunt (TIPS) and local, low-dose thrombolysis in the treatment of complete, noncavernomatous portal vein occlusion. MATERIALS AND METHODS: TIPS implantation and portal vein recanalization was attempted in seven patients with noncavernomatous portal vein obstruction and recurrent variceal bleeding. TIPS placement was followed by thrombolytic therapy to restore portal venous blood flow. Hemodynamic effects and clinical characteristics after the procedure and during follow-up were assessed. RESULTS: The implantation of TIPS and the recanalization of the portal vein trunk were successful in all patients. The treatment reduced the portal venous pressure gradient and restored portal blood flow. No bleeding complications were observed. CONCLUSION: TIPS placement and recanalization of the main portal vein is a safe and effective treatment option for patients with liver cirrhosis and noncavernomatous portal vein occlusion.

RESUMO

An unusual case of lipid embolization to brain and kidney after lymphography in a patient with non-Hodgkin lymphoma of the upper anterior mediastinum is reported. Contrast material-enhanced echocardiography demonstrated a right-to-left shunt to the left atrium without evidence of a patent foramen ovale. Echo contrast particles were transiently present within the tumor surrounding the great vessels.

BACKGROUND: The clinical presentation of patients with pancreatic cancer may resemble the clinical picture of chronic pancreatitis. A definitive preoperative diagnosis is not always obtained in patients with a history of chronic pancreatitis despite the use of modern imaging techniques. Operative strategy therefore remains unclear before operation in these patients. METHODS: Positron emission tomography (PET) with 2-[18F]fluoro-2-deoxy-D-glucose (FDG) was introduced recently into clinical oncology because of its ability to demonstrate metabolic changes associated with various disease processes. The impact of FDG-PET on the differentiation of chronic pancreatitis and pancreatic cancer was investigated. FDG-PET was performed in 48 patients with chronic pancreatitis (n = 12), acute pancreatitis (n = 3) and pancreatic cancer (n = 27), and in controls (n = 6). Histological examination was undertaken in all cases except controls. The FDG-PET results were obtained without knowledge of results of other imaging procedures. The results were then compared with those of computed tomography, ultrasonography, endoscopic retrograde cholangiopancreaticography, operative findings and histology. PET images were analysed semiquantitatively by calculating a standard uptake value (SUV) 90-120 min after application of the tracer. RESULTS: Cut-off values were validated as follows: SUV greater than 4.0 for pancreatic cancer, SUV of 3.0-4.0 for chronic pancreatitis, and SUV of less than 3.0 for controls. Sensitivity and specificity of PET imaging were 0.96 and 1.0 for pancreatic cancer, and 1.0 and 0.97 for chronic pancreatitis. In five cases only FDG-PET led to the correct preoperative diagnosis. CONCLUSION: The results give further evidence that FDG-PET is an important non-invasive method for the differentiation of chronic pancreatitis and pancreatic cancer. Delayed image acquisition in the glycolysis plateau phase permits improved diagnostic performance. This imaging technique is extremely helpful before operation in patients with an otherwise unclear pancreatic mass, despite its costs.
