RESUMO

A 65-year-old woman with spontaneous osteonecrosis of the medial femoral condyle of the left knee was treated by a fresh cadaveric osteochondral bone graft. The latest follow-up examination at 2.5 years demonstrated good functional results, as well as radiographic evidence of satisfactory consolidation with no aseptic necrosis or osteoarthritis.

RESUMO

Four elderly patients with longstanding unrecognized dislocation of the shoulder were treated by physiotherapy only. Good functional results, associated with a greatly increased range of pain-free motion, demonstrate the usefulness of conservative treatment in cases of persistent dislocation of the shoulder in the elderly.

RESUMO

Villonodular synovitis of the shoulder developed in two elderly patients. Huge soft tissue mass, marked joint effusion, and radiographs demonstrating severe osteoarthritic changes characterized these patients. Localized shoulder muscle atrophy accounts for residual painfree limitation of active shoulder motion following synovectomy of the shoulder joint.

Fourteen patients with 20 total hip joint replacements were studied for 14 painful prosthetic hips. Clinical examination, plain film radiographs and 99Tcm-MDP bone scans failed to differentiate between infection and mechanical loosening of a prosthesis. Sequential use of 99Tcm-MDP and 67Ga-citrate bone scans were performed in an attempt to discover underlying infectious process. Increased focal uptake of both radiopharmaceuticals over the same hip indicated an infectious process responsible for prosthetic loosening. There were no false positive gallium examinations. Sequential use of 99Tcm-phosphate compounds and 67Ga-citrate is recommended for differentiation between mechanical loosening of a prosthesis and loosening of a prosthesis secondary to an infectious process.

RESUMO

A 5-year-old boy presented with a limp, fever, and right hip pain of 72 hours' duration. An intensive workup of right hip pain synovitis failed to diagnose any local pathology. Delayed diagnosis of psoas abscess was made on the 12th day of hospitalization. A rapid recovery with no further complications followed surgical evacuation of the abscess.

A known severe classical hemophiliac patient presented with a large hemorrhage into his left shin and complained of paresthesia in his toes. Large doses of factor VIII had failed to halt the expanding shin hematoma. Laboratory investigations revealed an antibody titer of 5 Bethesda units. In vitro and in vivo testing showed swift neutralization of factor VIII to a level of 20% and thereafter degradation of factor VIII in accordance to the half-life expectancy. The patient was successfully treated with a continuous infusion of small doses of cryoprecipitate. The Bethesda method of antibody measurement does not distinguish between low and high affinity antibodies and thus in vitro testing should be carried out before a decision is made on the use of expensive activated prothrombin complex. The initial approach of the physical therapist is to aid in attempting to limit the swelling and enhance hematoma reabsorption. Once the bleeding has been controlled, a long-term physical therapy program is necessary to regain dorsiflexion of the ankle joint.J Orthop Sports Phys Ther 1988;10(4):138-141.

BACKGROUND: When encountering complaints of pain in the area of the Achilles tendon, the clinician seldom reaches a correct and precise diagnosis based solely on the grounds of physical examination and standard X-rays. OBJECTIVES: To assess the usefulness of ultrasound in diagnosing pathologies of the Achilles tendon. METHODS: We conducted a retrospective review of patients presenting at our orthopedic clinics. RESULTS: Sonography was used to evaluate 41 patients with achillodynia. This modality enabled the diagnoses of 19 abnormal tendons (46%), peritendinous and other lesions; a complete rupture in two patients (5%); a partial rupture of the Achilles tendon in 3 (7%); various degrees of calcification of the tendon in 7 (17%); and peritendinous lesions discerned by the tendon's hypoechoic regions with disorganized arrangement of collagen fibrils in 4 patients (10%). Other lesions included tendonitis (3 patients, 7%), retrocalcaneal bursitis (3 patients, 7%), lipoma (1 patient, 2%), and foreign bodies (2 patients, 5%). The mean diameter of the pathological tendons was 10.4 +/- 2.7 mm, while normal tendons measured 5.2 +/- 0.8 mm (P < 0.001). CONCLUSION: As in many other soft tissue lesions, ultrasonography is a useful tool in the evaluation of the underlying pathology in patients presenting with achillodynia.

RESUMO

The recommended treatment for macrodactyly of the foot will often include epiphysiodesis of the proximal phalanx in an attempt to halt further longitudinal growth of the toe. Nine patients who underwent open epiphysiodesis and debulking of the excess soft tissues involving 11 toes were reviewed to evaluate the effectiveness of this procedure. In 9 of 11 toes, overall length of the proximal phalanx did not change after surgery. Two toes demonstrated continued growth; one of these toes underwent a repeat epiphysiodesis of the phalanx, and the other foot underwent epiphysiodesis of the affected metatarsal. Overall, this surgical approach led to radiographic results that satisfied the surgical goals.

Body image disturbance was studied in 6 achondroplastic children before and after surgical leg elongation (Wagner's procedure) using Fisher's Draw-a-Person Test and the Body Cathexis Test, as well as on another group of 6 achondroplastic children who had undergone the same procedure several years earlier. These groups were compared with 12 healthy control children. Gross disturbance of body image was present before the operation with a substantial improvement after the procedure. However, even years after the surgical procedure there still exists a difference in body image between the control-normal group and achondroplastic adolescents.

32 patients (age range 1.5-22 years) were treated for leg-length discrepancy. 39 lengthening procedures were performed of which 36 involved gradual distraction, in 2 cases chondrodiasthesis was used, and 1 had 1-step elongation. Gradual distraction was done according to Ilizarov, including corticotomy and a 1-week delay before the initiation of distraction. In 33 cases Wagner's external fixator was used, in 2 Ilizarov frame and in 1 an orthofix fixator. There was complete clinical and radiologic union in all but 1 case. Significant correction of leg-length discrepancy and equalization of leg-length was achieved in most patients. In some growing patients over-correction was successfully achieved. The most prevalent complications were pin-site problems, axis deviation, joint subluxation and joint contractures. At latest follow-up most of these complications had disappeared. 91% of the patients either functioned normal or were only mildly limited.

Torticollis results from various pathological mechanisms, and its elucidation depends on identifying diseases of musculoskeletal, neural and ocular tissues. This study characterized the underlying diseases of children hospitalized with torticollis, excluding congenital torticollis. Records of 36 children with torticollis seen during 4 years were reviewed and categorized according to presumed etiology. Most could be classified into 2 categories: in 39% it was due to trauma and in 36% to upper respiratory tract infection. Most girls were in the first group and most boys in the second group. There were 3 cases of ocular torticollis due to superior-oblique muscle palsy, 1 with a post-burn eschar, 2 with neurological disorders (intramedullary cervical astrocytoma and leukodystrophy with macrencephaly), and in 3 no associated cause was found. There was a clear seasonal trend with 58% of cases presenting from November through February, 33% from April through July, and the rest, of neurological or ocular origin, during the rest of the year. In cases of post-traumatic torticollis 21% had neurological symptoms such as weakness of the limbs, headaches or incontinence. Only a few had prior upper respiratory tract infection. All children whose torticollis was assigned to infection had had fever. Only 8% had had neurological complaints or vomiting, half of whom presented with fever exceeding 37.5 degrees C. 46% had restriction of movement and 38% had tenderness. In over 60% of those in this group there were signs of an upper respiratory tract infection, such as lymphadenopathy or a white blood cell count exceeding 15,000/microliter, 3 patients with recurrent torticollis were diagnosed as having severe neurological diseases. Mean hospitalization time was 4 days (range 1-28). Hospitalization periods were similar for all kinds of patients and treatment by traction or fixation did not affect this period.

RESUMO

Destructive bone lesions in long bones associated with onion like periosteal reaction was found in two girls. Diagnosis of Ewing sarcoma was followed by bone biopsy which indicated eosinophilic granuloma. The radiological appearances of osteomyelitis, tuberculosis, congenital syphilis, Caffey's disease and metastases of neuroblastoma are similar. Bone biopsy is mandatory for correct diagnosis.

A retrospective review of 19 elderly patients (65 years of age or older) with operatively managed tibial plateau fractures demonstrated that most patients had acceptable functional results, based on Rasmussen's criteria. The follow-up period was of 8-14 years (average 11 years). Seventy-nine percent of the patients (15/19) had excellent or good results. They regained their preinjury walking capacity and suffered only occasional pain. Four patients had fair results with limitation of walking capacity, pain on certain positions, and/or pain after activity. One of these 4 patients had significant medial instability. In this age group, internal fixation of tibial plateau fractures and early mobilization appeared to be beneficial in achieving good functional results.
