RESUMO

Materials with tunable negative electromagnetic performance, i.e., where dielectric permittivity becomes negative, have long been pursued in materials research due to their peculiar electromagnetic (EM) characteristics. Here, this promising feature is reported in materials on the case of plasma-synthesized nitrogen-doped graphene sheets with tunable permittivity over a wide (1-40 GHz) frequency range. Selectively incorporated nitrogen atoms in a graphene scaffold tailor the electronic structure in a way that provides an ultra-low energy (0.5-2 eV) 2D surface plasmon excitation, leading to subunitary and negative dielectric constant values in the Ka-band, from 30 up to 40 GHz. By allowing the tailoring of structures at atomic scale, this novel plasma-based approach creates a new paradigm for designing 2D nanomaterials like nanocarbons with controllable and tunable permittivity, opening a path to the next generation of 2D metamaterials.

The genus Mimosa L. (Leguminosae; Caesalpinioideae; mimosoid clade), comprising more than 500 species, is an intriguing genus because, like other members of the mimosoid clade, it presents an enormous variation in floral characteristics and high merism lability. Thus, this study aimed to elucidate the floral development and identify which ontogenetic pathways give rise to merism variation and andromonoecy in Mimosa caesalpiniifolia, M. pudica, M. bimucronata, and M. candollei. Floral buds at various stages of development and flowers were collected, fixed, and processed for surface analysis (SEM). The development of the buds is synchronous in the inflorescences. Sepals appear simultaneously as individualized primordia in M. caesalpiniifolia and in reversed unidirectional order in M. bimucronata, with union and formation of an early ring-like calyx. Petal primordia appear in unidirectional order, with a noticeably elliptical shape in M. caesalpiniifolia. The wide merism variation in Mimosa results from the absence of organs from inception in the perianth and androecium whorls: in dimerous, trimerous, or tetramerous flowers, the additional organs primordia to compose the expected pentamerous flowers are not initiated. The haplostemonous androecium of M. pudica results from the absence of antepetalous stamens from inception. In the case of intraspecific variations (instabilities), there is no initiation and subsequent abortion of organs in the events of reduction in merosity. In addition, extra primordia are initiated in supernumerary cases. On the other hand, staminate flowers originate from the abortion of the carpel. Mimosa proved to be an excellent model for studying merism variation. The lability is associated with actinomorphic and rather congested flowers in the inflorescences. Our data, in association with others of previous studies, suggest that the high lability in merism appeared in clades that diverged later in the mimosoid clade. Thus, phylogenetic reconstruction studies are needed for more robust evolutionary inferences. The present investigation of ontogenetic processes was relevant to expand our understanding of floral evolution in the genus Mimosa and shed light on the unstable merism in the mimosoid clade.

This study aimed to explore how positional performance varies across different youth age groups and during matches in football competitions. The study encompassed 160 male outfield youth football players (n = 80, under-13, U13; n = 80, under-15, U15) who belonged to the starting line-up and played the entire first half of each match. The players' positional data were gathered through the global positional system for each of the eight matches performed by each age group. The frequency of near-in-phase synchronization based on speed displacements, spatial exploration index, and the distance to the nearest teammate and opponent were used as variables. Additionally, each match half was segmented into three equal parts to assess changes over time and used as a period factor along with age group. The results indicated that U13 players showed a significant decrease (from small to large ES) in synchronization speed and spatial exploration index throughout the first half of the match, along with a decrease in the distance to the nearest opponent. In contrast, U15 players exhibited most changes during the third segment of the half, with a decrease in speed synchronization and spatial exploration, but an increase in the distance and regularity to the nearest opponent. Comparing both age groups revealed significant differences in speed synchronization across the entire half of the match and within each segmented period (from small to large ES), with U13 consistently showing higher values. The study highlights that long durations in 11 vs. 11 matches might not provide an appropriate learning environment in the U13 age group. Conversely, the U15 group displayed better capacity for tactical adjustments over time, suggesting a higher level of tactical maturity. Overall, these findings emphasize the importance of adapting youth football training and competition structures to the developmental needs and capabilities of different age groups to optimize learning and performance outcomes.

RESUMO

Intraductal radiofrequency ablation (RFA) has been used in the management of malignant biliary obstruction and ampullary neoplasms. Some small studies refer to its role in managing benign biliary strictures with some promising results. The complications are not neglectable, namely cholangitis, pancreatitis, bleeding, and perforation, although most of them can be managed conservatively. There are two catheters available. Only the ERLA (EndoLumunal Radiofrequency Ablation, Taewoong Medical) catheter can control temperature and impedance, allowing it to reduce the risk of complications.

BACKGROUND: Aneurysmal subarachnoid hemorrhage (SAH) is associated with high mortality and long-term functional impairment. Data on clinical management and functional outcomes from developing countries are scarce. We aimed to define patient profiles and clinical practices and evaluate long-term outcomes after SAH in a middle-income country. METHODS: This was a prospective study including consecutive adult patients admitted with SAH to two reference centers in Brazil from January 2016 to February 2020. The primary outcome was functional status at 6 months using the modified Rankin Scale. Mixed multivariable analysis was performed to determine the relationship between clinical variables and functional outcomes. RESULTS: From 471patients analyzed, the median time from symptom onset to arrival at a study center was 4 days (interquartile range 0-9). Median age was 55 years (interquartile range 46-62) and 353 (75%) patients were women. A total of 426 patients (90%) were transferred from nonspecialized general hospitals, initial computed tomography revealed thick hemorrhage in 73% of patients (modified Fisher score of 3 or 4), and 136 (29%) had poor clinical grade (World Federation of Neurological Surgeons score of 4 or 5). A total of 312 (66%) patients underwent surgical clipping, and 119 (25%) underwent endovascular coiling. Only 34 patients (7%) underwent withdrawal or withholding of life-sustaining therapy during their hospital stay, and in-hospital mortality was 24%. A total of 187 (40%) patients had an unfavorable long-term functional outcome (modified Rankin Scale score of 4 to 6). Factors associated with unfavorable outcome were age (adjusted odds ratio [OR] 1.05, 95% confidence interval [CI] 1.03-1.08), hypertension (adjusted OR 1.81, 95% CI 1.04-3.16), poor clinical grade (adjusted OR 4.92, 95% CI 2.85-8.48), external ventricular drain (adjusted OR 3.8, 95% CI 2.31-6.24), postoperative deterioration (adjusted OR 2.33, 95% CI 1.32-4.13), cerebral infarction (adjusted OR 3.16, 95% CI 1.81-5.52), rebleeding (adjusted OR 2.95, 95% CI 1.13-7.69), and sepsis (adjusted OR 2.68, 95% CI 1.42-5.05). CONCLUSIONS: Our study demonstrated that SAH management in a middle-income country diverges significantly from published cohorts and current guidelines, despite comparable clinical profiles on presentation and admission to high-volume referral centers. Earlier aneurysm occlusion and increased use of endovascular therapy could potentially reduce modifiable in-hospital complications and improve functional outcomes in Brazil.

BACKGROUND: Delayed cerebral ischemia (DCI) occurs in around 30% of patients suffering from nontraumatic subarachnoid hemorrhage (SAH) and is associated with poor neurological outcome. Whether the Neurological Pupil index (NPi) derived from the automated pupillometry could help to diagnose the occurrence of DCI remains unknown. The aim of this study was to investigate the association of NPi with the occurrence of DCI in patients with SAH. METHODS: This was a multicenter, retrospective cohort study of consecutive patients with SAH admitted to the intensive care units of five hospitals between January 2018 and December 2020 who underwent daily NPi recordings (every 8 h) during the first 10 days of admission. DCI was diagnosed according to standard definitions (in awake patients) or based on neuroimaging and neuromonitoring (in sedated or unconscious patients). An NPi < 3 was defined as abnormal. The primary outcome of the study was to assess the time course of daily NPi between patients with DCI and patients without DCI. Secondary outcome included the number of patients who had an NPi < 3 before DCI. RESULTS: A total of 210 patients were eligible for the final analysis; DCI occurred in 85 (41%) patients. Patients who developed DCI had similar values of mean and worst daily NPi over time when compared with patients without DCI. Patients with DCI had a higher proportion of at least one NPi < 3 at any moment before DCI when compared with others (39/85, 46% vs. 35/125, 38%, p = 0.009). Similarly, the worst NPi before DCI diagnosis was lower in the DCI group when compared with others (3.1 [2.5-3.8] vs. 3.7 [2.7-4.1], p = 0.05). In the multivariable logistic regression analysis, the presence of NPi < 3 was not independently associated with the development of DCI (odds ratio 1.52 [95% confidence interval 0.80-2.88]). CONCLUSIONS: In this study, NPi measured three times a day and derived from the automated pupillometry had a limited value for the diagnosis of DCI in patients with SAH.

BACKGROUND: Rebleeding from a ruptured aneurysm increases the risk of unfavorable outcomes after subarachnoid hemorrhage (SAH) and is prevented by early aneurysm occlusion. The role of antifibrinolytics before aneurysm obliteration remains controversial. We investigated the effects of tranexamic acid on long-term functional outcomes of patients with aneurysmal SAH (aSAH). METHODS: This was a single-center, prospective, observational study conducted in a high-volume tertiary hospital in a middle-income country from December 2016 to February 2020. We included all consecutive patients with aSAH who either received or did not receive tranexamic acid (TXA) treatment. Multivariate logistic regression analysis using propensity score was used to evaluate the association of TXA use with long-term functional outcomes, measured by the modified Rankin Scale (mRS) at 6 months. RESULTS: A total of 230 patients with aSAH were analyzed. The median (interquartile range) age was 55 (46-63) years, 72% were women, 75% presented with good clinical grade (World Federation of Neurological Surgeons grade 1-3), and 83% had a Fisher scale of 3 or 4. Around 80% of patients were admitted up to 72 h from ictus. The aneurysm occlusion method was surgical clipping in 80% of the patients. A total of 129 patients (56%) received TXA. In multivariable logistic regression using inverse probability treatment weighting, the long-term rate of unfavorable outcomes (modified Rankin scale 4-6) was the same in the TXA and non-TXA groups (61 [48%] in TXA group vs. 33 [33%] in non-TXA group; odds ratio [OR] 1.39, 95% confidence interval [CI] 0.67-2.92; p = 0.377). The TXA group had higher in-hospital mortality (33 vs. 11% in non-TXA group; OR 4.13, 95% CI 1.55-12.53, p = 0.007). There were no differences between the groups concerning intensive care unit length of stay (16 ± 11.22 days in TXA group vs. 14 ± 9.24 days in non-TXA group; p = 0.2) or hospital (23 ± 13.35 days in TXA group vs. 22 ± 13.36 days in non-TXA group; p = 0.9). There was no difference in the rates of rebleeding (7.8% in TXA group vs. 8.9% in non-TXA group; p = 0.31) or delayed cerebral ischemia (27% in TXA group vs. 19% in non-TXA group; p = 0.14). For the propensity-matched analysis, 128 individuals were selected (64 in TXA group and 64 in non-TXA group), and the rates of unfavorable outcomes at 6 months were also similar between groups (45% in TXA group and 36% in non-TXA group; OR 1.22, 95% CI 0.51-2.89; p = 0.655). CONCLUSIONS: Our findings in a cohort with delayed aneurysm treatment reinforce previous data that TXA use before aneurysm occlusion does not improve functional outcomes in aSAH.

The European Shock Tube for High-Enthalpy Research is a new state-of-the-art facility, tailored for the reproduction of spacecraft planetary entries in support of future European exploration missions, developed by an international consortium led by Instituto de Plasmas e Fusão Nuclear and funded by the European Space Agency. Deployed state-of-the-art diagnostics include vacuum-ultraviolet to ultraviolet, visible, and mid-infrared optical spectroscopy setups, and a microwave interferometry setup. This work examines the specifications and requirements for high-speed flow measurements, and discusses the design choices for the main diagnostics. The spectroscopy setup covers a spectral window between 120 and 5000 nm, and the microwave interferometer can measure electron densities up to 1.5 × 1020 electrons/m3. The main design drivers and technological choices derived from the requirements are discussed in detail herein.

One of the main challenges in the development of a plasma diagnostic and control system for DEMO is the need to cope with unprecedented radiation levels in a tokamak during long operation periods. A list of diagnostics required for plasma control has been developed during the pre-conceptual design phase. Different approaches are proposed for the integration of these diagnostics in DEMO: in equatorial and upper ports, in the divertor cassette, on the inner and outer surfaces of the vacuum vessel and in diagnostic slim cassettes, a modular approach developed for diagnostics requiring access to the plasma from several poloidal positions. According to each integration approach, diagnostics will be exposed to different radiation levels, with a considerable impact on their design. This paper provides a broad overview of the radiation environment that diagnostics in DEMO are expected to face. Using the water-cooled lithium lead blanket configuration as a reference, neutronics simulations were performed for pre-conceptual designs of in-vessel, ex-vessel and equatorial port diagnostics representative of each integration approach. Flux and nuclear load calculations are provided for several sub-systems, along with estimations of radiation streaming to the ex-vessel for alternative design configurations. The results can be used as a reference by diagnostic designers.

Providing energy from fusion and finding ways to scale up the fusion process to commercial proportions in an efficient, economical, and environmentally benign way is one of the grand challenges for engineering. Controlling the burning plasma in real-time is one of the critical issues that need to be addressed. Plasma Position Reflectometry (PPR) is expected to have an important role in next-generation fusion machines, such as DEMO, as a diagnostic to monitor the position and shape of the plasma continuously, complementing magnetic diagnostics. The reflectometry diagnostic uses radar science methods in the microwave and millimetre wave frequency ranges and is envisaged to measure the radial edge density profile at several poloidal angles providing data for the feedback control of the plasma position and shape. While significant steps have already been given to accomplish that goal, with proof of concept tested first in ASDEX-Upgrade and afterward in COMPASS, important, ground-breaking work is still ongoing. The Divertor Test Tokamak (DTT) facility presents itself as the appropriate future fusion device to implement, develop, and test a PPR system, thus contributing to building a knowledge database in plasma position reflectometry required for its application in DEMO. At DEMO, the PPR diagnostic's in-vessel antennas and waveguides, as well as the magnetic diagnostics, may be exposed to neutron irradiation fluences 5 to 50 times greater than those experienced by ITER. In the event of failure of either the magnetic or microwave diagnostics, the equilibrium control of the DEMO plasma may be jeopardized. It is, therefore, imperative to ensure that these systems are designed in such a way that they can be replaced if necessary. To perform reflectometry measurements at the 16 envisaged poloidal locations in DEMO, plasma-facing antennas and waveguides are needed to route the microwaves between the plasma through the DEMO upper ports (UPs) to the diagnostic hall. The main integration approach for this diagnostic is to incorporate these groups of antennas and waveguides into a diagnostics slim cassette (DSC), which is a dedicated complete poloidal segment specifically designed to be integrated with the water-cooled lithium lead (WCLL) breeding blanket system. This contribution presents the multiple engineering and physics challenges addressed while designing reflectometry diagnostics using radio science techniques. Namely, short-range dedicated radars for plasma position and shape control in future fusion experiments, the advances enabled by the designs for ITER and DEMO, and the future perspectives. One key development is in electronics, aiming at an advanced compact coherent fast frequency sweeping RF back-end [23-100 GHz in few µs] that is being developed at IPFN-IST using commercial Monolithic Microwave Integrated Circuits (MMIC). The compactness of this back-end design is crucial for the successful integration of many measurement channels in the reduced space available in future fusion machines. Prototype tests of these devices are foreseen to be performed in current nuclear fusion machines.

J Strength Cond Res 37(9): 1821-1827, 2023-Postactivation performance enhancement activities may be relevant warm-up strategies aiming to improve subsequent physical performance. The purpose of the current study was to investigate the effects of adding barbell deadlift or hex-bar deadlift exercises to current warm-up routines on running and jumping performances in football players. Ten highly trained male football players participated in the study during the competitive phase of the season. All players performed 3 protocols in the same week: a standard warm-up that included players' regular routines and 2 other protocols with the addition of barbell or hex-bar deadlift, after the end of the warm-up (3 sets of 3 reps, progressing set by set from 60% to 85% repetition maximum). All protocols had the same time interval between pretest (immediately after the warm-up) and posttest (15 minutes after the warm-up). Vertical jumping (countermovement jump [CMJ]; Abalakov jump [AJ]) and running performances (505 test) were impaired 15 minutes after the standard warm-up (CMJ: -6.7 ± 4.2%; AJ: -8.1 ± 8.4%; and 505 time: 1.4 ± 2.5%). For warm-up with the addition of barbell deadlift, vertical jump increased by 4.3 ± 5.6% (Cohen's dunb : 0.23 [0.02-0.47]) and 505 time decreased by -5.9 ± 3.6% (Cohen's dunb : 0.97 [-1.68 to -0.43]). The warm-up with hex-bar deadlift led to trivial changes for CMJ and AJ, but 505 time decrease by -2.7 ± 2.6% (Cohen's dunb : -0.53 [-1.01 to -0.13]). The deadlift exercise can be added to warm-up routines to maintain or even enhance acute physical performance. However, coaches and practitioners should be aware that performance enhancements resultant from deadlift may vary according to individual physical profiles.

An 87-year-old man presented with malaise, anorexia and postprandial vomiting for 3 days. At presentation the patient was hypotensive and tachycardic, without any other relevant findings. Laboratory workout showed a normocytic normochromic anemia of 4.9g/dl. No hematic losses from the gastrointestinal tract were described or seen. The patient underwent a colonoscopy to study the anemia and an 18mm semi-pedunculated polyp in the descendent colon was found and excised. A whole-body tomography performed during hospitalization showed multiple nodular lesions in the lungs, pleura, peritoneum and left gluteal muscle compatible with metastatic disease. The polyp histological examination was finally compatible with a high-grade undifferentiated sarcoma. Patient died during hospitalization due to disease progression and multiple complications. Colorectal sarcomas are a very rare group of mesenchymal tumours, accounting for 0,1% of all colonic cancers [1]. They usually present as a high grade and aggressive tumour and are often located in the colon (70%) comparing with the rectum. Metastization is common and more often to the liver, peritoneum and lung. Curative treatment depends on surgical resection without need of extensive lymphadenectomy due to its rare lymphatic dissemination, but prognosis is usually poor [2]. Even so, the polyp described probably reflects a metastatic lesion of an extra-colonic sarcoma and not the primary sarcoma itself. Extrapulmonary metastases, namely intra-abdominal metastases, of musculoskeletal sarcomas are rare and are associated with even poorer prognosis [3]. There are few published papers describing the presence of colonic metastases since their implantation is usually subepithelial.

RESUMO

A 62-year-old woman was referred to Gastroenterology appointment due to severe iron deficiency anemia (5.9 g/dL), complaining of asthenia and requiring blood transfusion. The patient denied blood loss. Initial blood test showed a severe iron deficiency, with ferritin of 5ng/mL and transferrin saturation of 2.7%. Folic acid and vitamin B12 were normal. Upper gastroscopy and colonoscopy didn't show any lesions. Abdominopelvic CT and capsule endoscopy were, also, normal.

A 50-year-old male was referred to a Gastroenterology appointment after a screening colonoscopy with a 25mm exophytic lesion, with a depressed central area, on the transverse colon. Histologic examination of the biopsy specimen showed low-grade dysplasia. The patient was submitted to a new colonoscopy and what was seen was a flat lesion with central depression, with no lift-sign (Figure 1a and 1b) and, therefore, endoscopic resection was not performed. New endoscopic biopsies were taken and showed no dysplasia.

This study aimed to identify the effects of playing with additional individual (IND), collective (COL) or individual-collective (MIX) variability on youth football players' performance during small-sided games. Twelve youth football players (U17, age = 16.1 ± 0.9 years) played a goalkeeper (Gk) + 6 outfield players a-side (Gk+6vs6+Gk) under four conditions: (i) playing in the 1:2:3:1 formation without any other rule (control condition, CTR); (ii) 1:2:3:1 formation with additional body restrictions changing each minute (individual condition, IND); (iii) using different tactical formations modified each minute (collective condition, COL; (iv) using different tactical formations and body restrictions varied each minute (individual-collective condition, MIX). Generally, there were similar behaviours across conditions, especially for the CTR and the MIX. Nevertheless, the CTR condition presented moderate higher values in the lateral direction (p ≤ .05), while also higher longitudinal synchronization compared to the IND (p ≤ .05). The COL condition presented higher spatial exploration (p ≤ .05), which may justify the higher values for distance covered while running and sprinting (p ≤ .05). Overall, coaches may use the IND condition to refine players' technical actions, while the COL condition to develop players' ability to perform in different playing positions and team structures.

The demand for new biomass-derived fine and commodity chemicals propels the discovery of new methodologies and synthons. Whereas furfural and 5-hydroxymethylfurfural are cornerstones of sustainable chemistry, 3-acetamido-5-acetyl furan (3A5AF), an N-rich furan obtained from chitin biomass, remains unexplored, due to the poor reactivity of the acetyl group relative to previous furanic aldehydes. Here we developed a reactive 3-acetamido-5-furfuryl aldehyde (3A5F) and demonstrated the utility of this synthon as a source of bio-derived nitrogen-rich heteroaromatics, carbocycles, and as a bioconjugation reagent.

Momordica balsamina L. (Cucurbitaceae), frequently named balsam apple, southern balsam pear or African pumpkin, is a vegetable with high nutritional value, being mostly used as food in sub-Saharan Africa. It has also been largely used in traditional medicine to treat several diseases, such as malaria fevers and diabetes. As a member of the Cucurbitaceae family, the main constituents are cucurbitane-type triterpenoids, with different oxidation patterns, named cucurbitacins. This review aims at summarizing our contribution to the phytochemical study of M. balsamina and the evaluation of the isolated cucurbitacins and derivatives as multidrug resistance reversers in cancer cells and bacteria. In this way, the selective antiproliferative activity against multidrug resistant cancer cells of cucurbitacins obtained from M. balsamina, their ability as P-glycoprotein inhibitors in cancer cells overexpressing this ABC transporter, as well as efflux pump inhibitors in resistant bacteria strains are reviewed. Moreover, the in vitro antimalarial activity of cucurbitacins and acyl derivatives against the blood and liver-stages of Plasmodium strains, and the in vivo activity of selected compounds is also reviewed. Besides our work, edible and medicinal uses, and other studies mainly reporting the biological activities of M. balsamina extracts, such as antidiabetic, antibacterial, anti-inflammatory, and antioxidant properties are also addressed.

Studies on the relationship between personality and depressive disorders (DD) or substance use disorders (SUD) often refer to the normal personality model and focus mainly on the "big" factors. Domain level results with the Personality Inventory for DSM-5 (PID-5) tend to be consistent with NEO-PI-R or NEO-FFI results, however facet level results in the specific characteristics of these disorders are scarce. The main objective of this study was to characterize DD and SUD's maladaptive personality traits through the PID-5. A sample of DD was compared with a sample of SUD, with a sample of other disorders, and with a community sample. A sample of the general Portuguese population (N = 693) and a heterogeneous clinical sample (N = 310) were studied. Participants responded to the PID-5 and to the Brief Symptom Inventory (BSI). Depressivity, Irresponsibility and Anhedonia were the main characteristics of DD. However, high values in Depressivity and in depressive symptomatology were present in all the clinical subsamples. Irresponsibility, Deceitfulness and Callousness were the main characteristics of SUD. The occurrence of DD, SUD or Other disorders could be predicted using multinonomial logisitic regression analysis, and PID-5 facets as independent variables. These results are in line with comorbidity data and tend to confirm the PID-5 validity.

RESUMO

A 21-year-old man, with personal history of asthma and no usual medication, was referred to gastroenterology appointment due to dysphagia for solids and previous episodes of food impaction in the last 5 months. He also reported nocturnal heartburn without any other warning signs. Upper gastroscopy revealed a peptic stenosis in the distal esophagus. Histopathologic examination showed hyperplasia and numerous intraepithelial eosinophils, without dysplasia or malignancy. Therapy with a double-dose proton pump inhibitor (PPI) was started.
