RESUMO

Described in the article is a clinical case report concerning staged management of a patient presenting with a thoracoabdominal aortic aneurysm. The first stage consisted in complete lower debranching with prosthetic repair of all visceral arteries and the right renal artery. The second stage was endovascular repair of the aortic aneurysm. The chosen approach made it possible to avoid clamping of the thoracic portion of the aorta, which favourably contributed to a decrease in the traumatic nature of the operative procedure, simultaneously preserving the radicality of treatment after the two-stage intervention.

RESUMO

Presented herein is a clinical case report concerning successful surgical management of a rare variety of a primary tumour of the heart, i. e., a capillary haemangioma of the left atrium, simulating by the contours and localization a myxoma. The final diagnosis was verified only by histological examination. The authors describe difficulties of diagnosis of the disease involved, underlying the necessity of plastic correction for restoration of the normal anatomical configuration of the heart. This is followed by a review of the literature, reflecting a possible course of the disease, problems of diagnosis, and therapeutic policy.

RESUMO

The paper gives data on the sorption of influenza virus pandemic strain A/IIV-Moscow/01/2009 (H1N1)swl, avian influenza viruses with A/H5 and A/H7 hemagglutinin, poliomyelitis virus, and T4-D bacteriophage on polyaniline sorbents, carbon nanotubes, and their based nanocomposites. The sorption of viruses occurred in different solutions at 4-37 degrees C during 15 min or more. The rate of viral sorption depended on the structure of sorbents.

RESUMO

The paper gives data on the sorption of influenza A and B viruses diluted from different solutions on polyaniline sorbents synthesized with low and high molecular-weight polymer acids. Irrespective of the antigenic structure of surface proteins, the influenza A and B viruses isolated in the period of 1977 to 2008 were shown to be able to be immobilized on polyaniline sorbents in the temperature range of 4 to 37 degrees C. The immobilized viruses interacted with homologous immune sera.

RESUMO

The article is dedicated to analyzing the outcomes of an open prospective study of efficacy of Liprimai administered in the postoperative period. The study comprised a total of 30 patients presenting with atherosclerotic lesions of the arteries of the carotid bed and aorto-femoral segment, who were subjected to reconstructive vascular operations. The aim of the present study was to look into a possibility of preventing cardiac complications in patients after endured reconstructive vascular surgery. Liprimar was administered at a daily dose of 10 mg seven days prior to the operation, and for eight days thereafter.

RESUMO

We report results of our prospective study - administration of Nebilet in 30 patients in perioperative period. Aim of this study is to estimate the protective effect of Nebilet for perioperative cardiac complications (nonfatal myocardial infarction and cardiac death) in patients which underwent recostructive vascular surgery. Patients were administrated Nebilet (2.5 to 7.5 mg/per day, according to arterial pressure and heart rates) 20 days before operation and 2 months after surgery. In early postoperative period we performed the control examinations: troponin T (1, 3, 7 days), ECG (1 day), echocardiography (1-2 day) and 2 months after operation (ECG and echocardiography). During whole follow up period we controlled in patients arterial pressure and heart rates.

RESUMO

Presented herein is the first case of dislocation of the closing element of the artificial low-profile aortic valve to the abdominal aorta, which happened during replacement. Toward completion of extracorporeal circulation a 37-year-old man showed the signs of aortic insufficiency because of dislocation of the closing element. After resumption of extracorporeal circulation repeated replacement was accomplished. To detect the site of graft cusp embolism, transesophageal echocardiography and duplex scanning of the abdominal aorta were employed. On the 18th day following the first intervention the patient developed thromboembolism to the distal arterial bed. Emergency thrombectomy from the arteries of the right lower extremity was performed and a foreign body was removed from the abdominal aorta, using left-sided thoracophrenolumbotomy. The patient was discharged from the clinic in a satisfactory condition.

RESUMO

Blind randomized placebo-control research included 39 women. The following criteria were: menopause period for more then 1 year long; diabetes mellitus; arterial hypertension; hypercholesterolemia. The patients were divided into two groups. Women from the first group (N = 24) were given 30 mg lycopene (composed with "Tomatol") per day during 24 weeks. The second group (N = 15) were given placebo. It was showed that during 12-week "Tomatol" intake we state that lycopene concentration in serum increased twice (337.0 +/- 133.0 nM), total cholesterol (CH) and CH LDL decreased by 12% and 16% respectively. The lipoproteins spectrum normalization correlated to serum antioxidative activity (r = 0.30, p < 0.05), and malonic dialdehyde concentration decrease by 44.8% correlated to lycopene increase (r = -0.51, p < 0.05). Our results give us a possibility to use "Tomatol" as an additional medicine for the prevention of atherosclerosis.

From 1994 to 2001 balloon angioplasty was fulfilled for 19 patients aged from 4 to 28 (mean 11.0+/-7.0) with discrete type aortic coarctation. Early outcomes were satisfactory in all cases. Catheterization showed 10 mm Hg decrease in systolic pressure gradient (SPG). Long-term results (from 6 months to 8 years, mean 3.0+/-2.5) were evaluated with transthoracic EchoCG, Doppler ultrasound, duplex scanning of iliofemoral segment and CT-angiography with 3D reconstruction. Surgical outcomes were considered good when SPG did not exceed 20 mm Hg for Doppler ultrasound and 25 mm Hg for Doppler EchoCG. In the study group there was statistically significant decrease in arterial pressure in right upper limb and SPG (measured with both Doppler EchoCG and Doppler ultrasonography) and rise of ankle-brachial index. Beneficial hemodynamic effect was detected in 15 from 19 patients (79%). Recoarctation has developed in 4 (21%) patients, in 1 (5.3%) case it was combined with true posttraumatic aortic aneurysm. Three patients underwent open surgical intervention with uneventful postoperative period. Aneurysmal dilatation of thoracic aorta has developed in 1 patient (5.3%), non-progressive intimal dissection - in another one (5.3%), 2 patients (10.5%) experienced ipsilateral 40-60% stenosis of right external iliac and femoral arteries. Thus good long-term outcomes of balloon angioplasty in patients with discrete type aortic coarctation were achieved in 79% cases. Patients after balloon angioplasty must be assessed regularly with non-invasive diagnostic methods such as Doppler ultrasonography, Doppler EchoCG, CT-angiography.

A total of 170 catheter balloon valvuloplastic (CBV) operations for rheumatic mitral stenosis were carried out in patients aged 19 to 68, 30 of these in pregnant women, at A. V. Vishnevsky Institute of Surgery, Russian Academi of Medical Scients, from 1988 to 1994. Characteristic features of such operations are a minor surgical trauma and no necessity in deep narcosis and artificial circulation. Analysis of the immediate results and of the data of five-year follow-up brings the authors to a conclusion about the efficacy of the new method of treatment of this heart disease and about the physiological nature of forming an adequate route of blood flow into the right ventricle with a balloon. Comparing various balloon catheters for valvuloplasty and methods to carry out this procedure, the authors emphasize the advantages of the instruments and method developed by professor V. A. Silin et al. (St. Petersburg). The possibility of correction of mitral stenosis by CBV in pregnant women appears to be particularly important. Retrospective analysis of the immediate results and follow-up data helped the authors not only formulate the indications for this intervention, but permitted them to develop an original system of comprehensive clinical and instrumental assessment of the degree of involvement of the mitral valve and subvalvular structures, and of hemodynamic disorders.
