RESUMO

We reported an outbreak of wild rabies in Potus flavus (choznas), a carnivorous mammal associated with insectivorous bats in Kosñipata, Cusco, Peru, from February 2021 to October 2022. Eight human exposures were reported, with aggressive attacks resulting in injuries. The outbreak prompted a public health response and raised concerns about zoonotic diseases due to habitat fragmentation and urbanization. The study involved antigenic and genetic characterization of the virus in the affected animals, revealing similarities with variants in non-hematophagous bats. This outbreak marked the first time rabies had been reported in choznas in the region, highlighting the need for early intervention programs and wildlife protection. Pre-exposure rabies vaccinations were administered to residents, and community surveillance was established to control the outbreak. The study emphasizes the importance of continuous surveillance and education to prevent rabies transmission from choznas to humans and other animals, as their role as potential secondary transmitters of the virus warrants further investigation.

Rabies in pregnancy is rare with a high mortality. We report a pregnant woman who developed urban rabies from the bite of an infected dog. An emergency Cesarean section was performed at 31 weeks of gestation, and a premature baby was delivered who survived and did not develop rabies. The mother, however, suffered a rapid clinical deterioration and was admitted to intensive care where a Milwaukee protocol was followed. The patient survived 19 months but developed severe neurological sequelae secondary to rabies encephalitis and finally died of respiratory failure.

OBJECTIVE: . To characterize the nucleoprotein (N) and establish the origin of the rabies virus in dogs coming from Arequipa. MATERIALS AND METHODS.: Thirty samples of nervous tissue from the departments of Arequipa and Puno were analyzed. Total RNA was extracted from the samples and cDNA was synthesized to amplify the nucleoprotein gene, sequence it, and perform bioinformatics analysis. RESULTS: . A defined group was formed with respect to the external group (European bat lyssavirus). This group was classified into two subgroups, one constituted by samples coming from Puno and Arequipa (subgroup A), and another one by samples from Puno (subgroup B), exhibiting a nucleotide identity of 99.9% in subgroup A. This group was classified in two subgroups, one constituted by samples coming from Puno and Arequipa (subgroup A), and another one by samples from Puno (subgroup B), observing a nucleotide identity of 99.9% in subgroup A. CONCLUSIONS.: The groupings of viral sequences show that the cases of canine rabies reported in Arequipa are the result of the expansion of canine rabies from the endemic region of Puno.

Este grupo fue clasificado en dos subgrupos, uno constituido por muestras procedentes de Puno y Arequipa (subgrupo A), y otro por muestras de Puno (subgrupo B), observándose una identidad nucleotídica de 99,9% en el subgrupo A. CONCLUSIONES.: Los agrupamientos de las secuencias virales muestran que los casos de rabia canina notificados en Arequipa son el resultado de la expansión de rabia canina procedente de la región endémica de Puno.

The Potos flavus is a nocturnal mammal that lives in neotropical forests from Central America to South America. A study of four cases of rabies in Potos flavus was conducted; these occurred in April 2012 in the Madre de Dios region in Peru and were collected as part of epidemiological surveillance. The analysis performed in the regional reference laboratory of Madre de Dios determined the presence of the rabies virus antigen in three of the brain tissue samples. Results were verified in the Laboratory of Viral Zoonoses of the Peruvian National Institute of Health by direct immunofluorescence. The typification did not identify any of the known variants in bats or dogs. The occurrence of four cases of rabies in Potos flavus adds evidence of the emergence of a new reservoir of the rabies virus previously reported in the same region in 2007.

El Potos flavus es un mamífero nocturno que habita en bosques neotropicales desde Centroamérica hasta Sudamérica. Se realizó un estudio de cuatro casos de rabia en Potos flavus ocurridos desde abril de 2012 en el departamento de Madre de Dios en Perú, captados como parte de la vigilancia epidemiológica. Los análisis realizados en el laboratorio de referencia regional de Madre de Dios determinaron presencia de antígeno del virus de la rabia en tres de las muestras de tejido encefálico, dichos resultados fueron corroborados en el Laboratorio de Zoonosis Virales del Instituto Nacional de Salud del Perú mediante inmunofluorescencia directa, la tipificación no identificó ninguna de las variantes conocidas en murciélagos o en perros. La ocurrencia de cuatro casos de rabia en Potos flavus suma evidencias de la emergencia de un nuevo reservorio del virus de la rabia y que ha sido reportada previamente en el mismo departamento el año 2007.
