RESUMO

The authors analyze the results of 50 computed angiographies made for osteosarcoma and Ewing's sarcoma. Better tolerated than conventional angiography computed angiography is also more sensitive in preoperative evaluation of effectiveness of neoadjuvant chemotherapy. Automatic subtraction, direct measuring of tumor size and vascularization density give to angiographic changes after chemotherapy the most predictive value. After conservative surgery or radiotherapy digital angiography provides most reliable local follow up.

RESUMO

Indications for arteriography in obliterative arterial disease of the lower limbs essentially involve assessments before dilatation or operation in the presence of disabling intermittent claudication or rest pain or gangrene or moderate symptoms when Doppler findings indicate the possibility of dilatation. In order to ensure the best selection of potential surgical patients and select the appropriate technique, arteriograms must show the entire arterial system from the coeliac aorta to the most distal point. Previously this could be obtained only by conventional arteriography using a large film cassette. In view of the advantages of digitalized arteriography via an arterial approach, the possibility of obtaining equally accurate and continuous images with this technique were evaluated. The method involved 12 to 15 successive injections, with a mean total dose of 100 ml of contrast medium, precise identification of X-ray fields and end to end mounting of films. Not only were the images of identical quality, but this quality appeared to be more constant and it was often possible to visualize distal arteries with a low flow rate. Complications were exceptional in view of the absence of general anesthesia, use of a finer catheter and fractionnation of injections of contrast medium. The majority of these investigations were carried out on an ambulatory basis, whilst the majority of standard arteriograms require hospitalization. Patient comfort was increased and cost decreased. In conclusion, digitalized arteriography by an arterial approach offers a complete image of the arterial system from the renal arteries to the pedal arches provided X-ray fields are carefully identified and films are mounted end to end.(ABSTRACT TRUNCATED AT 250 WORDS)

RESUMO

The line between the summit of the tibial tuberosity and the bottom of the trochlea groove as seen on a radiograph taken with the knee in 30 degrees of flexion and neutral rotation is indicative of the valgus position of the quadriceps mechanism at the moment of engatement of the patella in the groove. The authors stress the technical requirements to obtain the radiograph which permits this measurement. Measurement of a group of normal knees and of two groups of knees presenting with known patello-femoral pathology (lateral patello-femoral arthrosis and current dislocation of the patella) are given. In the study of distrubance of the patello-femoral articulation, this measurement provides information of theoretical and therapeutic interest.

A patient with severe acute pyonephritis and septicaemia had persistent symptoms of infection despite appropriate chemotherapy. Although no obstruction was seen on ascending ureteropyelography, exploratory lumbar incision was considered. However, computerized tomography (CT scan) failed to show any obstacle or collection of pus but indicated diffuse infection of kidney. Medical treatment was continued and the patient recovered. This case and a review of the literature has prompted the authors to compare CT scan with conventional contrast-radiography methods in septicaemias of renal origin. It appeared that intravenous and ascending uretero-pyelography are often at fault, and the latter is known to carry iatrogenic risks. CT scan is of considerable help when looking for foci of infection and is proposed as a deciding examination in septicaemias with or without signs suggestive of renal lesions.
