Pulmonary artery aneurysm is a rare pathology and the optimal treatment approach remains unclear in the absence of clear guidelines. We report a case of pulmonary aneurysm and discuss the treatment options.

OBJECTIVE: The purpose of our study is to describe the use of recombinant factor VIIa (rFVIIa) in patients on central veno-arterial ECMO with a particular attention on associated thrombotic complications. STUDY DESIGN: Monocentric retrospective study. PATIENTS AND METHODS: We examined 91 files of patients on ECMO between 2005 and 2010. During this period, eight patients presented refractory bleeding and benefited from rFVIIa treatment. RESULTS: In six of the eight patients, the bleeding stopped. A decrease of the bleeding was noticed after the treatment of rFVIIa (before rFVIIa: 40.1±33.1mL/kg per 3 hours after rFVIIa: 5.4±3.2mL/kg per 3 hours (P=0.01). The transfusional needs were decreased after administration of rFVIIa. No thrombotic event was detected. Fibrinogen, d-dimers, platelet count and lactate were not modified by the treatment. Two patients were weaned from ECMO. One patient died 17 days after the weaning. The other patient survived without neurological damages. CONCLUSION: The rFVIIa is a treatment of exception for patients on central veno-arterial ECMO and could be a last-resort treatment in the presence of a not curable massive bleeding.

Resumen: El estudio del control postural humano mediante técnicas mecano-estadísticas, permite identificar comportamientos dinámicos con características de persistencia y anti-persistencia. Tales comportamientos, pueden ser interpretados como variabilidad tiempo dependiente en los sistemas de control y algunos indicadores que dan cuenta de éstos, son el coeficiente de difusión y el exponente de ajuste obtenidos con la técnica de Análisis de Difusión del Estabilograma (ADE). Se utilizó una plataforma de fuerza para los registros estabilométricos y se aplicó el ADE con el objeto de discriminar el comportamiento temporal de la variabilidad del centro de presión, en posición bípeda cuasi-estática con y sin restricción visual (Ojos cerrados y Ojos abiertos) en un grupo de 22 escolares en edad crítica (11,2±1,5 años) de maduración del proceso de control. Los resultados obtenidos en los coeficiente de difusión (Ds, DƖ) y exponentes de ajuste (Hs, HƖ ) en ambas condiciones visuales, caracterizan claramente comportamientos diferenciados para cada fases de la gráfica del ADE, y se establece diferencias significativas en los indicadores Ds y Hs, al comparar la excursión del COP con y sin restricción visual. El ADE nos permite identificar un comportamiento de sistema de control postural similar al de los adultos, modelado como un movimiento browniano fraccionario de dos fases.
Abstract: The human postural control study through statistical mechanic techniques identifies dynamic behavior with characteristics of persistence and anti-persistence. Such behaviors are interpreted like time-dependent variability of the control systems, and indicators that reflect these ones, are diffusion coefficient and scaling exponent obtained with stabilogram diffusion analysis (SDA) technique. A force platform was used to obtain stabilometry records and SDA was applied in order to discriminate the temporal behavior of the variability in quasi-static bipedal position in a group of 22 students in critical age (11,2 ± 1,5 years old) for postural control maturity, between eyes-closed and eyes-open conditions. The results of diffusion coefficients (Ds, DƖ ) and scaling exponents (Hs, HƖ ) in both visual condition have two distinct phases from ADE graphic and establish statistically significant differences for D s and H s, when comparing the displacement of COP between open eyes and closed eyes. The SDA allows us to identify a behavior of postural control system similar to adults, modeled like a two-phase fractional Brownian motion.

OBJECTIVES: The present study was aimed at evaluating telomere length in blood and in different vascular tissues with or without atheroma, in 3 groups of subjects: a group of atherosclerotic subjects who underwent surgery (Atherosclerosis-Surgery), a second group of subjects with asymptomatic atherosclerotic carotid plaques but who did not undergo cardiovascular surgery (Atherosclerosis-No surgery), and a third group of subjects without atherosclerotic disease (Controls). The main objective was to determine if there is in vivo regulation of telomere length in situ by atherosclerotic lesions. METHODS: A total of 84 subjects (mean age 69 ± 8 years) were studied. Blood and arterial tissue telomere lengths were determined by Southern blotting. Personal medical history (diabetes, hypertension, cardiovascular disease, dyslipidemia), family medical history, drug intake, and lifestyle were evaluated in the entire population through the use of a questionnaire. RESULTS AND CONCLUSION: Arterial segments which did not develop atherosclerosis such as the saphenous vein and internal mammary artery, had longer telomere length than aortic segments. On the other hand, telomere length was shorter in aortic tissues which presented atherosclerotic lesions compared to corresponding tissues without atherosclerotic lesions. These results also suggest tissue regulation of telomere size by local factors likely related to oxidative stress responses.

El propósito del presente caso clínico es proponer una estrategia diagnóstica desde el paradigma de la función, se describe el caso de una niña portadora del síndrome de Prader - Willi que en el transcurso de su hospitalización se deriva al servicio de kinesiterapia para su control respiratorio, agregadamente se realiza la aplicación de pruebas funcionales, con el fin de conocer su perfil físico lo cual a juicio de los autores demuestra la necesidad de implementar una conducta.
This article describe the Prader - Willi syndrome in a girl in her hospitalization, her evaluations include physiotherapist. Agreguement the experience functionals test are not applied in the traditionally way in the utility diagnosis of physical therapy.
