RESUMO

PURPOSE: A comprehensive review of the literature on the time between the onset of symptoms and the first episode of care and its effects on important worker outcomes in compensated musculoskeletal conditions is currently lacking. This scoping review aimed to summarize the factors associated with time to service and describe outcomes in workers with workers' compensation accepted claims for musculoskeletal conditions. METHODS: We used the JBI guidelines for scoping reviews and reported following the PRISMA-ScR protocol. We included peer-reviewed articles published in English that measured the timing of health service initiation. We conducted searches in six databases, including Medline (Ovid), Embase (Ovid), PsycINFO, Cinahl Plus (EBSCOhost), Scopus, and the Web of Science. Peer-reviewed articles published up to November 01, 2022 were included. The evidence was summarized using a narrative synthesis. RESULTS: Out of the 3502 studies identified, 31 were included. Eight studies reported the factors associated with time to service. Male workers, availability of return to work programmes, physically demanding occupations, and greater injury severity were associated with a shorter time to service, whereas female workers, a high number of employees in the workplace, and having legal representation were associated with a longer time to service. The relationship between time service and worker outcomes was observed in 25 studies, with early access to physical therapy and biopsychosocial interventions indicating favourable outcomes. Conversely, early opioids, and MRI in the absence of severe underlying conditions were associated with a longer duration of disability, higher claim costs, and increased healthcare utilization. CONCLUSION: Existing evidence suggests that the time to service for individuals with compensated musculoskeletal conditions was found to be associated with several characteristics. The relationship between time to service and worker outcomes was consistently indicated in the majority of the studies. This review highlights the need to consider patient-centred treatments and develop strategies to decrease early services with negative effects and increase access to early services with better outcomes.

INTRODUCTION: Low back pain (LBP) is a key social, economic, and public health problem in the world. The impact of LBP is given less priority and is empirically unrepresented in low- and middle-income countries as a result of the focus on more pressing and life-threatening health issues, including infectious diseases. In Africa, the prevalence of LBP is irregular and increasing among schoolteachers on account of teaching activities performed under suboptimal working conditions. Therefore, the objective of this review was to estimate the pooled prevalence and associated factors of LBP among school teachers in Africa. METHODS: This systematic review and meta-analysis was designed based on the PRISMA guidelines. A comprehensive systematic literature search focused on LBP in African school teachers was conducted using the PubMed/MEDLINE, CINAHL, and CABI databases, regardless of publication timelines, from October 20 to December 3, 2022. In addition, gray literature was searched using Google Scholar and Google Search. Data were extracted in Microsoft Excel by using the JBI data extraction checklist. The overall effect of LBP was estimated using a random effect model via DerSimonian-Laird weights. The pooled prevalence and odds ratio of associated factors with 95% CI were computed using STATA 14/SE software. The I2 test and Egger's regression test were used to assess heterogeneity and publication bias, respectively. RESULTS: A total of 585 articles were retrieved, and 11 eligible studies involving a total of 5,805 school teachers were included in this systematic review and meta-analysis. The overall estimated pooled prevalence of LBP in African school teachers was found to be 59.0% (95% CI: 52.0%-65.0%). Being female [POR: 1.53; 95% CI (1.19, 1.98)], being older [POR: 1.58; 95% CI (1.04, 2.40)], being physically inactive [POR: 1.92; 95% CI (1.04, 3.52)], having sleep problems [POR: 2.03; 95% CI (1.19, 3.44)] and having a history of injury [POR: 1.92; 95% CI (1.67, 2.21)] were factors significantly associated with LBP. CONCLUSIONS: The pooled prevalence of LBP was high among school teachers in Africa compared to developed nations. Sex (female), older age, physical inactivity, sleep problems, and a history of previous injury were predictors of LBP. It is suggested that policymakers and administrators ought to gain awareness of LBP and its risk factors to put existing LBP preventive and control measures into action. Prophylactic management and therapeutic strategies for people with LBP should also be endorsed.

BACKGROUND: Over the last decade, flower farms have been rapidly growing in Ethiopia. Following the advent and development of the sector, various work-related chemical, biological, physical, psychosocial, and ergonomic hazards have been emerging unacceptably, with increased risks of exposures for workers and local communities. However, evidence that describes knowledge and prevention practice of occupational hazards among flower farm workers in the country is little documented. The knowledge and safety practice of occupational hazards among flower farm workers in Ethiopia were explored in the current study. METHODS: A cross-sectional survey of 471 flower farm workers was implemented from March to April 2017. A stratified random sampling technique was used to select the eligible participants. An interviewer-administered questionnaire was used to collect data, and the data were entered in to Epi Info program version 7 and analyzed by SPSS program version 20. Bivariate and multivariate linear regression analyses were performed to evaluate significance of associations at < 0.05 p-values. RESULTS: A total of 451 flower farm workers were interviewed with a response rate of 95.7%. The majority, 72.1% (N = 325) were females. Mean age was 24.1 (SD + 6.5) years. About 39.2% (N = 177) of the participants had good knowledge on occupational hazards. The level of safety practice was 26.6% (N = 120). The level of knowledge on occupational hazards was affected by level of education [AOR: 20.03;95% CI (16.30,23.75)], work experience [AOR: 5.97; 95% CI (4.22,7.72)], and type of employment [AOR: 5.35; 95% CI (2.50,8.19)], whereas the level of safety practice was influenced by regular use of personal protective equipment (PPE) [AOR:17.53;95% CI (13.36,21.71)], level of knowledge [AOR: 7.29; 95% CI (3.87,10.73)], and provision of appropriate PPE [AOR: 4.59; 95% CI (2.34,8.86)]. CONCLUSION: This study revealed the levels of knowledge and safety practice towards occupational hazards were low. The knowledge on occupational hazards was significantly affected by the level of education and duration of employment. Moreover, the use of PPE and level of knowledge considerably influenced safety practice. Therefore, we recommend employers to ensure that workplace health and safety programs account for workers' level of education and work experience. It is also pivotal to provide workers witha suitable PPE and instructions on its use, and to arrange safety communication in the local languages at the relevant workplaces.

BACKGROUND: International Labour Organization (ILO) report indicates more than 2.4 million workers die from work-related diseases and accidents each year. Work-related respiratory ailments related to airborne particulate matter such as flour dust are responsible for about 386,000 deaths and 6.6 million illness-adjusted life years. Even though exposure to flour dust together with the extreme expansions of flour mill sectors is a priority health concern, extent of the problem is little investigated in Ethiopia. The aim of this study was to evaluate the magnitude and risk factors of work-related respiratory symptoms among flour mill workers in Bahir Dar City, Ethiopia. METHODS: This study employed a comparative cross-sectional survey of 560 samples (280 exposed group from flour mill workers and 280 unexposed group from office workers) with a stratified random sampling technique. The study was conducted from March to April 2019 in Bahir Dar City, Northwest Ethiopia. We used the British Medical Research Council (BMRC) questionnaire to assess work-related respiratory symptoms. The questionnaire was pretested and interview administered to collect data. Binary logistic regression analysis was fitted to evaluate significant factors of respiratory symptoms at a < 0.05 p value. Adjusted odds ratio (AOR) with a confidence interval (CI) of 95% was calculated to determine a strength of association. RESULTS: All the sampled participants had fully responded to the interview. The median age of exposed and unexposed groups was 28.5 interquartile range (IQR, 20) and 31 (IQR, 15) years, respectively. The prevalence of work-related respiratory symptoms among flour mill workers was substantially higher than that of among controls, 63.9% and 20.7%, respectively (Χ2 = 107.11; p < 0.0001). Chest illness among flour mill workers was higher, 43.6% (N = 122) compared to that of among control group, 7.9% (N = 22) followed by dyspnea, 33.6% (N = 94) versus 2.5% (N = 7) among flour mill workers and control group, respectively. Age > 35 years [AOR, 2.03; 95% CI (1.34, 5.48), having no education [AOR, 1.54; 95% CI (1.28, 3.06)], work set up with inadequate ventilation [AOR, 2.05; 95% CI (1.18, 3.56)], work experience > 5 years [AOR, 1.89; 95% CI (1.23, 4.67)] and having no training in safety [AOR, 2.45; 95% CI (1.45, 4.76)] significantly affected the experience of respiratory symptoms among flour mill workers whereas age [AOR, 1.79; 95% CI (1.06, 3.04)], monthly salary [AOR, 1.98; 95% CI (1.04, 3.78)] and exposure status [AOR, 5.18; 95% CI (3.34, 8.04)] were detected to be significant factors of respiratory symptoms in the exposed and the unexposed combined model. CONCLUSION: Respiratory symptoms emanating from exposure to various flour dusts were significantly higher among flour mill workers than among the control group. Therefore, we recommend the need to effectively implement health and safety programs that account for the reduction of dust at a source, use of engineering controls (e.g., provision of adequate ventilation systems), use of administrative measures (e.g., training program and health surveillance) and provision of a suitable personal protective equipment (PPE). Furthermore, it is vital to integrate workplace health and safety programs to the wider public health policies and strategies to effectively mitigate the burden of work-related respiratory conditions. We also encourage future studies to evaluate concentration of flour dusts combined with physical examinations to establish plausible associations of respiratory symptoms with dusts of flour mill-related origin.

BACKGROUND: Musculoskeletal disorders (MSD) caused by occupational-related factors continue to place huge burdens on global workforces. Significant numbers of workers report potential adverse health outcomes related to the condition, such as physical injury, disability, and decline in quality of life. Occupational-related MSD also poses additional burdens to healthcare services and diminishes productivity at work. The condition usually worsens in informal sectors where the work environments are often poorly designed. This paper explored occupational and physical environmental factors that induce work-related neck and/or shoulder pains among self-employed tailors in Gondar city, Northwest Ethiopia. METHODS: We conducted across-sectional survey from April to May 2019 on 422 tailors selected with systematic random sampling technique. Nordic Musculoskeletal questionnaire was used to measure pains in neck and/or shoulder, and the questionnaire was pretested and administered by interviewers. Work-related factors such as working posture, rest break, training in safety and health, and the availability of adjustable chairs at workplaces were assessed. The significance of associations was set at a < 0.05 p-value and adjusted odds ratios (AOR) with a confidence interval (CI) of 95% were used to determine strength of associations. RESULTS: A total of 419 tailors participated with a response rate of 99.3%. The mean age and mean years of experience were 29.23 (SD ± 7.03) and 1.48 (SD ± 0.50) years, respectively. The study found that the prevalence of pain in either neck or shoulder or both sites in the last 12 months was 66.6% (N = 279) [95% CI (62.1, 71.1)]. Pains in shoulder and neck were observed in 72.1% (N = 302)[95% CI (67.8, 76.4)] and 68.3% (N = 286) [95% CI (64.0, 72.6)] of the interviewees, respectively. The majority, 78.1% (n = 218) of those with pains indicated they were prevented from doing normal daily activities. Work experience (AOR = 1.81), rest break (AOR = 2.13), awkward working posture (AOR = 2.60), prolonged sitting (AOR = 2.00) and inadequate light (AOR = 5.02) were significantly associated factors of neck and/ or shoulder pains. CONCLUSION: Work-related neck and/or shoulder pain induced by physical factors of the work environment among self-employed tailors is pervasive in Ethiopia. Efforts to curb the condition, therefore, need to impalement diverse approaches addressing the physical environment and occupational factors. We also promote the integration of schemes for the effective use of rest breaks into health and safety programs in the workplace.

BACKGROUND: Work-related musculoskeletal disorders (MSDs) are the major threats to public health, with a significant impact on workers, employers, and the general population. Musculoskeletal disorder related to work not only results in adverse health effects such as physical injury, disability, and a reduction in workers' quality of life, but it also places immense burdens on the use of healthcare facilities and a substantial loss of productivity. The purpose of this research was to discover the prevalence and associated factors of work-related MSDs among hairdressers in Oromia Special Zone Surrounding Finfine, Ethiopia. METHODS: A cross-sectional survey was carried out between March and April 2019. We included a total of 699 hairdressers with a systematic random sampling technique. Work-related MSDs was evaluated with the standardized Nordic Musculoskeletal survey, and the survey was administered by the interviewer. We employed SPSS version 20 software to perform a bivariate and multivariate analysis. A p-value of < 0.05 was considered a significant association. RESULTS: In total, 652 hairdressers were interviewed with a response rate of 93.2%. Participants' mean age was 33.19 (SD ± 9.639) years. The prevalence of work-related MSDs was 70.2% (N = 458) [95% CI (66.7, 73.9)] and 55.7% in the past 12-months and 7 days, respectively. The study showed the highest prevalence rate was observed in shoulder 53.7% (n = 350) followed by 53.4% (n = 348) neck and 53.2% (n = 347) low back. Of the participants, 33.4% (n = 153) perceived their pain to be severe whereas 28% (n = 128) a high disabling. Almost one-third (n = 187) of the respondents used healthcare services. Age [AOR = 2.73; 95% CI (2.55, 5.46)], work experience [AOR = 1.51; 95% CI (1.03, 2.20)], number of customers served per day [AOR = 2.35; 95% CI (1.35, 4.11)], and hours spent standing to make hair [AOR = 3.4; 95% CI (2.49, 7.77)] were significantly associated factors. CONCLUSION: This study found work-related MSDs were prevalent among hairdressers, but the use of healthcare services remains low. Age, length of employment, number of customers served per day, and number of hours spent standing per day to make hair were significantly associated. Therefore, we recommend employers need to develop health and safety programs that account for factors related to the workplaces. The findings also demonstrate that health practitioners would encourage pain management procedures.

BACKGROUND: Knowing the factors for patient satisfaction is an important and direct indicator of quality of health care which is essential for providers to fill their gaps. Although few studies have been conducted on patient satisfaction in Ethiopia; but there is limited evidence for comparing patient satisfaction and associated factors in the public and private wing of the health services. Thus, this study aimed to investigate factors of patient satisfaction in adult outpatient departments in the private wing and regular services at public hospitals of Addis Ababa, Ethiopia. METHODS: A comparative institution based cross-sectional study was conducted from March to April 2018. A total of 955 systematically selected patients were interviewed by using an interviewer-administered structured questionnaire. Binary logistic regression analysis was performed. In the multivariable logistic regression analysis p value < 0.05 and adjusted odd ratio (AOR) with 95% confidence interval (CI) were used to identify the associated factors. RESULTS: The overall patient satisfaction was 89.3% (95% CI: 87.2-91.2). At the regular and private wings of outpatient departments it was 88.3% (95% CI: 85.4-91.2) and 90.4% (95% CI: 87.6-93), respectively. At regular service OPD, patient satisfaction was affected by female sex (AOR: 7.78; 95% CI: 2.89-20.93), long waiting time (AOR: 0.22; 95% CI: 0.07-0.73), information on the prevention of recurrent illnesses (AOR: 14.16; 95% CI: 4.58-43.83), and information on drug use and side effects (AOR: 0.22; 95% CI: 0.08-0.63). In private wing, it was affected by being in the age group of 38 to 47 years (AOR: 22.1; 95% CI: 2.39-203.6), attended elementary school (AOR: 4.69; 95% CI: 1.04-21.26), availability of drugs (AOR: 0.14; 95% CI: 0.04-0.58), and the accessibility of latrines (AOR: 6.56; 95% CI: 1.16-37.11). CONCLUSIONS: Patient satisfaction at the private wing and regular adult OPDs' of public hospitals had no statistically significant difference. Female sex and information on the prevention of recurrent illnesses were factors positively affected patient satisfaction at regular services, whereas at private wing OPDs' age, attended elementary school, and accessibility of latrines were factors that positively affected patient satisfaction.

BACKGROUND: Occupational skin diseases are the second most common occupational diseases and are responsible for an estimated 25% of all lost work days. Occupational contact dermatitis (OCD) comprises 70-90% of all occupational skin diseases. In Ethiopia, information about the prevalence and factors which determine developments of contact dermatitis is not recognized. The objective of this study was to investigate prevalence and factors influencing the occurrences of occupational-related contact dermatitis among healthcare workers in Gondar town, Northwest Ethiopia. METHODS: We employed a healthcare-based cross-sectional study from March to April 2018. A stratified sampling method followed by simple random sampling method was used to select 422 participants. The standardized Nordic Occupational Skin Questionnaire was pretested and interviewer-administered for data collection. We used SPSS version 20 to conduct a binary logistic regression analysis. We set ≤ 0.05 p value to ascertain significance and 95% CI with odds ratios to evaluate the strength of associations. RESULTS: Response rate was 100%. The majority, 52.4% (N = 221), were males. The mean age was 22.6 (SD ± 6.3) years. The overall prevalence of self-report occupational contact dermatitis in the previous 12 months was 31.5% (N = 133) [95% CI (27, 36.2)]. The highest symptoms indicated was redness, 28.5% (n = 38), followed by burning, 17.3% (n = 23). The hand is the most commonly affected body sites, 22% (N = 93). Hand washing frequency [AOR 1.80, 95% CI (1.10, 3.20)], pairs of hand gloves used per day [AOR 3.22, 95% CI (2.05, 5.87)], personal history of allergy [AOR 2.37, 95% CI (1.32, 4.61)], and lack of health and safety training [AOR 2.12, 95% CI (1.12, 2.25)] were factors considerably associated with contact dermatitis. CONCLUSIONS: The prevalence of occupational-induced contact dermatitis is common among healthcare workers in Ethiopia. Therefore, our finding indicates that intervention aiming at workers' health and safety training demands urgent public health responses to tackle the ailment. The result also demonstrates that healthcare workers should be aware of when and how hands should be washed. The number of pairs of gloves used per day should also be taken into consideration while devising prevention strategies.

BACKGROUND: Road traffic injury is one of the persistent public health challenges in most regions of the world, representing substantial human and economic losses. Annually, about 1.25 million lives are lost, whereas 50 million suffer from road traffic injuries globally. It has been shown that over 60% of the reasons for traffic injuries are a risky driving behavior (RDB). Despite the problem's pervasiveness, there is a paucity of information about level and factors influencing RDB among professional car drivers in Bahirdar city, northwest Ethiopia. METHODS: An institution-based cross-sectional study was conducted from February to March 2016. A systematic random sampling technique was used to select 376 participants. A self-administered driver behavior questionnaire (DBQ) was used for data collection. We performed a binary logistic regression analysis to investigate the associations of variables. Potential confounders were controlled using a multivariable logistic regression model. We ascertained the significance at < 0.05 p value and evaluated strength of associations using crude odds ratios (COR) and adjusted odds ratios (AOR) with 95% confidence intervals (CI). RESULTS: A total of 361 drivers participated (response rate, 96%). The mean age was 34 (standard deviations ± 7.97) years. The majority, 98.9% (N = 357), were males. The level of risky driving behavior and road traffic crashes were 79.4% (95% CI 75.92, 83.97) and 16.3% (95% CI 15.91, 24.84), respectively. Average monthly salary [AOR 2.04; 95% CI (1.23, 2.74)], driving experience [AOR 2.72; 95% CI (1.07, 6.89)], distance driven per year [AOR 2.06; 95% CI (1.13, 4.10)], and previous history of involvement in traffic crashes [AOR 2.30; 95% CI (1.15, 7.35)] were significantly associated with risky driving behavior. CONCLUSIONS: The study shows that risky driving behavior is common among professional car drivers in the study setting. Therefore, it is strongly advisable for policy makers and other stake holders to devise strategies that consider working conditions, like monthly salary and driving experiences. The study also suggests that it is often advisable to reduce the distance driven per year and learn from implications of previous history of involvement in traffic crashes.

BACKGROUND: Maternal and neonatal tetanus (MNT) is still the major public health problem in about 25 countries, mainly in Africa and Asia. However, the utilization of intervention strategies, like tetanus toxoid (TT) immunization remains low in these countries. In Ethiopia, only 49% of the pregnant mothers received TT2+ in 2016. This study was designed to evaluate perceptions and factors affecting the utilization of TT immunization among reproductive-age women in Dukem town, Eastern Ethiopia, 2016. METHODS: We conducted a community-based cross-sectional study from May to October 2016. A simple random sampling method was employed to select samples of 422 women. Data were collected using a, pretested semi-structured and a face-to-face interviewer-administered questionnaire. We entered data in to Epi Info version 7 and analyzed them by SPSS version 20 software. Odds ratios and a 95% CI at 0 < 0.05 p-value were calculated to ascertain the significance of associations. RESULTS: Response rate was 98.6% (N = 416). Mean age with standard deviation was 29.25± 5.11 years, and average family size was 4.19. Our study showed the utilization of TT immunization was 39.2% (N = 163). Of the participants, 33.9% (N = 141) had never been vaccinated. ANC follow up service [AOR: 2.56, 95% CI: (1.18, 5.49)], distance from health facilities [AOR: 2.27, 95% CI: (1.27, 4.09)], knowing vaccination date [AOR: 1.98, 95% CI: (1.23, 3.18)], having a TV set in the house [AOR: 1.80, 95% CI: (1.11, 2.917)], maternal education [AOR: 1.41, 95% CI: (1.84, 2.30), and place of delivery [AOR: 1.19, 95% CI: (1.00, 1.43)] were factors significantly associated with the utilization of TT immunization. CONCLUSIONS: This study indicated the utilization of TT immunization was low. ANC service follow up, distance from health facilities, knowing vaccination date, having a TV in the house, mothers' educational status, and place of delivery were significant predictors. Our study suggests that policymakers and other stakeholders should consider the need for increasing access to maternal education, like basic adult education, ANC follow up services, providing accessible health facilities, improving varieties of communication media, promoting female occupational status, and providing appropriate vaccination cards.

Understanding the combining ability effects, heterosis, and genetic relationships between parental lines would be helpful in the maize breeding program to develop high-yielding and genetically stable maize varieties for various contrasting environments. Hence, the objective of this study was to estimate combining ability effects, heterosis, genetic distance (GD) and associations for grain yield and other agronomic traits. Forty-five F1 diallel crosses, four commercial checks, and ten inbred lines were evaluated at Bako and Jima Agricultural Research Centers, Ethiopia. Significant differences were observed among environments, genotypes and genotype by environment interaction for grain yield and almost all studied agronomic traits. Combining ability analysis showed both additive and non-additive gene effects significantly controlled grain yield and all other studied traits. The preponderance of general combining ability (GCA) effects indicates the importance of additive gene action inheriting most agronomic traits. Lines L2, L7, and L9 were the best combiners for grain yield, whereas lines L3 and L4 had desirable GCA values to improve days to flowering and plant height. Among the top ten crosses, L6 × L9, L6 × L7 and L4 × L9 were good specific combiners and had 18.8 %, 17.2 % and 16.2 % grain yield advantage over the best check, BH546. These hybrids also had high mid and better-parent heterosis compared with other crosses. The associations of GD with mean of F1 and SCA effects were positive and highly significant for grain yield and some other traits. In contrast, correlations of GD with mid and better parent heterosis were non-significant for grain yield and most other traits. The results of this study are particularly useful for breeders who envisage combining conventional and molecular methods.

INTRODUCTION: Work-related musculoskeletal disorders (WRMSDs) are the leading cause of disability worldwide. Shopkeepers are prone to developing work-related musculoskeletal disorders, but they are largely overlooked in research and policy actions, particularly in developing countries. So far, there is a lack of data on the magnitude and factors influencing work-related musculoskeletal disorders among shopkeepers in Ethiopia. Therefore, the current study aimed to explore the prevalence and risk factors of work-related musculoskeletal disorders among shopkeepers in Gondar City, Ethiopia. METHODS: A workplace-based cross-sectional study was conducted from July to August 2022, in Gondar city, Northwest Ethiopia. A multistage sampling technique was used to select 625 shopkeepers. The data were collected using an interviewer-administered standardized Nordic Musculoskeletal Questionnaire. Analysis was made using Stata version 14. Factors associated with the prevalence of work-related musculoskeletal disorders were identified using the multivariable Poisson regression model. The adjusted prevalence ratio with 95% confidence intervals (CIs) and p-value < 0.05 were applied to establish the significance of associations. RESULTS: The overall prevalence of work-related musculoskeletal disorders among shopkeepers in the past 12 months was found to be 81.1% (N = 507). The most frequently affected body part was the lower back (46.6%), followed by the upper back (43.8%) and shoulder (35.4%). Being female (p = 0.043), being in the age group of ≥40 years (p = 0.028), being overweight (p = 0.035), experiencing job stress (p = 0.006) and prolonged sitting (p = 0.045) were significant factors for the prevalence of work-related musculoskeletal disorders among shopkeepers. CONCLUSION: This study revealed that shopkeepers face an alarmingly high prevalence of work-related musculoskeletal disorders. Female, older, overweight, stressed and shopkeepers who sit in the same position for long periods of time were identified as particularly vulnerable groups. These findings call for the urgent development and implementation of preventive measures, including ergonomic adjustments, education and training programs, stress management techniques and the promotion of physical activity, to protect this vulnerable workforce from the debilitating effects of work-related musculoskeletal disorders and to ensure their long-term health and well-being.

Background: The practice of taking medication as directed by a healthcare provider is known as medication adherence. Therefore, the application of a socio-ecological model to this study identifies multilevel factors on barriers of medication adherence on chronic non-communicable disease and provides information to develop scientific health communication interventional strategies to improve medication adherence. Objective: This study aimed to explore barriers of medication adherence on non-communicable disease prevention and care among patients in North Wollo Zone public hospitals, northeast Ethiopia. Methods: A phenomenological study design was carried out between February 5 and February 30, 2023. The study participants were chosen using a heterogeneous purposive sampling technique. In-depth interviews and targeted focus groups were used to gather data. The focus group discussions and in-depth interviews were captured on audio, accurately transcribed, and translated into English. Atlas TI-7 was utilized to do the thematic analysis. Results: Four main themes, intrapersonal, interpersonal, community level, and health care related, as well as seven subthemes, financial problems, lack of family support, poor communication with healthcare providers, effects of social ceremonies, remote healthcare facility, and drug scarcity, were identified by this study. In this study participants reported that lack of knowledge about the disease and drugs were the main barrier for medication adherence. The study revealed that financial problems for medication and transportation cost were the main factor for medication adherence for non-communicable disease patients. Conclusion: This study explored that lack of knowledge, financial problem, lack of family support, poor communication with healthcare providers, social ceremony effects, remote healthcare facility, and scarcity of drugs were barriers of medication adherence among non-communicable disease patients. In order to reduce morbidity and mortality from non-communicable diseases, it is advised that all relevant bodies look for ways to reduce medication adherence barriers for patients at every level of influence.

Background: Healthcare-associated infections are a global health problem and are more prevalent in developing countries such as Ethiopia, but there is a paucity of research on the infection prevention practices of cleaning staff. Therefore, this study aimed to assess infection prevention and control practices and associated factors among cleaners working in healthcare facilities in Gondar City, Ethiopia. Methodology: A cross-sectional survey was conducted among healthcare cleaning staff from May to June 2022. A total of 428 cleaners took part in the survey. Data were collected using a semi-structured interviewer-administered questionnaire. The data were entered into EpiData version 4.6 and analyzed using Stata version 14 software. A multivariable binary logistic regression analysis was used to ascertain the significance of associations at <0.05 p-value and the adjusted odds ratio (AOR) with a 95% confidence interval (CI). Results: Among the 390 study participants included, 294 (75.1%) were female. Of the surveyed participants, 186 (47.7%) had good knowledge of infection prevention and control practices. This study revealed that out of the 390 healthcare cleaners, 204 (52.3%) had good infection prevention and control practices with 52.3% [95% CI (47.2, 56.4)]. Good knowledge of infection prevention and control [AOR: 1.56, 95% CI (1.03, 2.37)] and the availability of infection prevention and control guidelines in the workplace [AOR: 1.54, 95% CI (1.01, 2.33)] were significant factors associated with infection prevention and control practice. Conclusion: The present study found that almost half of the healthcare cleaners had poor IPC practices. The finding underlines the importance of good IPC knowledge and the accessibility of IPC guidelines to improve IPC practices among healthcare cleaning staff. The findings of this study also highlight that behavioral change interventions and paying attention, particularly to nonclinical staff such as cleaners in health care settings, are critical to reducing infection in health care settings.

Maize is one of the most important staple food crops for most low-income households in the Southern African region. Erratic and inconsistent rainfall distribution across maize-growing areas is a major threat to maize production. Late rains in recent years have forced farmers to plant later than the optimal planting dates, leading to poor maize quality being reported by industry, which raised the question of the influence of later planting dates on grain yield and quality traits of maize. Three yellow and three white maize hybrids were evaluated at three planting dates in three different production environments for three consecutive seasons using a randomized complete block design with three replications. The second and third planting dates caused a significant yield decrease of 23.37 % and 53.73 % from the first planting date across environments, respectively. Planting date three was associated with decreased grain yield, starch content, and increased protein but no significant change in fat and fiber content. Some hybrids yielded relatively well at all planting dates. In conclusion, the early planting date was the most suitable for maize grain yield and starch production in the maize-growing areas of the country. However, planting in January should be avoided at all costs, as it leads to very low yield and poor grain quality.

Iron (Fe) and zinc (Zn) nutrient enrichment of staple crops through biofortification can contribute to alleviating micronutrient deficiency in sub-Saharan Africa. A line × tester mating design was used to determine the general combining ability (GCA), specific combining ability (SCA) and heterosis for grain yield, iron, Zn and phytic concentration of six lines crossed with three testers. Lines and testers were selected for high, intermediate and low mineral content. The F1 hybrids and parental lines were evaluated under low nitrogen (N) and optimum conditions across four environments over two seasons. Under low N conditions, Fe and Zn concentration in grain, and grain yield of genotypes were reduced by 9%, 9%, and 59%, respectively. However, phytic acid concentration in grain was increased by 10% under low N conditions. Both additive and non-additive gene effects were important in controlling Fe, Zn and phytic acid concentration in grain and grain yield of maize under both N conditions. The preponderance of GCA effects indicates the importance of additive gene effects in the inheritance of grain yield. Line GCA effects were more sensitive to N conditions across the environments than the tester GCA. High and significant positive SCA effects for grain yield, Fe and Zn content under low N conditions, would be a good indicator of possible heterosis in these traits. Hybrid CBY101 LM-1600 × CBY358 LM-1857 had high and significant positive SCA for grain yield under low N conditions and is a promising candidate for production in low N environments. CBY358 LM-1857 (tester) and CBY102 LM-1601 (line) are a good general combiners for Fe, Zn and GY can be used as parents in future maize hybrid breeding programs to develop high-yielding maize genotypes with high Fe and Zn content.

Maize is the staple food crop for millions of people in sub-Saharan Africa. Iron (Fe) and zinc (Zn) deficiency is a significant health risk that mainly affects low-income populations who rely solely on maize-based diets. This problem can be alleviated by developing micronutrient-rich maize grain. The aim of this study was to determine the adaptation and performance of hybrids for Fe and Zn concentration and grain yield under low soil nitrogen (N) and optimal conditions. Eighteen hybrids derived from lines and testers with low, medium, and high Fe and Zn concentration were grown during the summer rainy seasons of 2017 and 2018 at three locations under low and optimal N conditions. There were significant genotype and environment effects for grain yield, and Fe and Zn concentration, but the genotype by environment interaction effects were the largest, accounting for between 36% and 56% of variation under low N conditions. Low N levels significantly reduced grain yield, and Fe and Zn concentration. Hybrids G1, G2, G4, G7, G10, G11, and G16 were relatively stable, with relatively high mean Fe and Zn concentrations, and low additive main effects and multiplicative interaction (AMMI) stability values and iron stability index (FSI) and zinc stability index (ZSI) under low N conditions. These genotypes can be considered for production under low N stress conditions. Two environments (E4 and E3) were identified for good discriminatory power for genotype performance in terms of Fe and Zn content, respectively. Stable and high-yielding genotypes with high Fe and Zn concentration can be used as biofortified hybrids, which can contribute to a sustainable solution to malnutrition in the region, especially under low N conditions.

Background: Work-related upper extremity disorders (WRUEDs) are aches, pains, tension, and discomfort in the neck, shoulders, arms, wrists, hands, and fingers. The situation is escalating in educational sectors due to a lousy working environment intertwined with extracurricular deeds. However, empirical evidence focusing on academicians in higher education society is negligible. The purpose of this study is to examine the prevalence and risk factors of WRUEDs among university teaching staff in Ethiopia. Materials and Methods: We conducted a cross-sectional study design from March to April 2021. A sample of 607 academicians were recruited using a stratified sampling technique, and a self-administered structured Nordic Musculoskeletal questionnaire was used to assess upper extremity disorders during the past 12 months. The collected data were entered into EpiData version 4.6 and analyzed using STATA version 14 software. The association between dependent and independent variables was computed with a binary logistic regression. The association was ascertained using an adjusted odds ratio (AOR) with a 95% confidence interval (CI) at a p value of <0.05. Results: A total of 607 participants correctly completed the questionnaire (response rate of 95.44%). Age ranges from 21 to 70 with a mean of 32.39 (SD ± 6.80)) years, and the majority (76.28%) of them were males. The prevalence of WRUED during the last 12 months was 59.14% [95% CI (55.1, 63.1)]. There is no significant difference in prevalence between males and females (45.14% versus 14%), respectively; χ 2 = 0.001; p=0.974. Working more than 8 hours per day [AOR: 2.37; 95% CI (1.40, 4.00)], not performing physical exercise [AOR: 2.34; 95% CI (1.6, 3.45)], and job dissatisfaction [AOR: 2.50; 95% CI (1.69, 3.68)] were factors significantly increased the risk of experiencing WRUEDs. Conclusion: This study divulged upper extremity disorder among university teaching staff is pervasive, with more than three-fifth of the academicians were suffering from the condition, and it also indicates that males experienced higher proportions of pain than females. The manifestation of upper extremity disorder was affected by working hours per day, physical activity, and job satisfaction. Optimizing working hours, having a group regular exercise, and proper management of workplace conditions related to job satisfaction are recommended to lessen the condition.

Introduction: Computer vision syndrome (CVS) is the leading occupational hazard of the 21st century, which causes the critical public health issues in the present era. In developing countries, including Ethiopia, data on the proportion and the causes of CVS are scarce among academic staff for policy conclusions. Accordingly, this study was conducted to determine the prevalence and factors associated with CVS among academic staff at the University of Gondar. Methods: Institution-based cross-sectional study design was conducted from March to April, 2021. A simple random sampling technique was used to select 525 study participants. Data were collected using a pre-tested self-administered structured questionnaire. The collected data were then entered into EpiData version 4.6 and analyzed using STATA version 14 software. The association between dependent and independent variables was computed with a binary logistic regression. Results: The prevalence of CVS was found to be 78.8% out of a total of 500 participants [95% CI (74.95, 82.30)], of which the majority (71.60%) of them were males. Using a computer for more than 9 years [AOR = 1.65, 95% CI (1.01, 2.71)], using visual display terminals [AOR = 2.63, 95% CI (1.60, 4.32)], and working in improper illumination levels (300 and >500 lux) [AOR = 2.47, 95% CI (1.47, 4.33)] increased the risk of CVS, whereas, Using eye droplets [AOR = 0.30, 95% CI (0.15, 0.58)] and taking rest breaks [AOR = 0.55, 95% CI (0.34-0.92)] were protective factors. Conclusion: This study disclosed that CVS was common among academicians. In the current study, the manifestation of CVS was affected by years of computer use, visual display terminals use, workplace illumination level, rest breaks, and eye droplets. Taking rest breaks in between the work, reducing exposure to display screens, use of eye drops, and optimizing workplace illumination levels are recommended to reduce the problem.

Currently, the world population is increasing, and humanity is facing food and nutritional scarcity. Climate change and variability are a major threat to global food and nutritional security, reducing crop productivity in the tropical and subtropical regions of the globe. Cowpea has the potential to make a significant contribution to global food and nutritional security. In addition, it can be part of a sustainable food system, being a genetic resource for future crop improvement, contributing to resilience and improving agricultural sustainability under climate change conditions. In malnutrition prone regions of sub-Saharan Africa (SSA) countries, cowpea has become a strategic dryland legume crop for addressing food insecurity and malnutrition. Therefore, this review aims to assess the contribution of cowpea to SSA countries as a climate-resilient crop and the existing production challenges and perspectives. Cowpea leaves and immature pods are rich in diverse nutrients, with high levels of protein, vitamins, macro and micronutrients, minerals, fiber, and carbohydrates compared to its grain. In addition, cowpea is truly a multifunctional crop for maintaining good health and for reducing non-communicable human diseases. However, as a leafy vegetable, cowpea has not been researched and promoted sufficiently because it has not been promoted as a food security crop due to its low yield potential, susceptibility to biotic and abiotic stresses, quality assurance issues, policy regulation, and cultural beliefs (it is considered a livestock feed). The development of superior cowpea as a leafy vegetable can be approached in different ways, such as conventional breeding and gene stacking, speed breeding, mutation breeding, space breeding, demand-led breeding, a pan-omics approach, and local government policies. The successful breeding of cowpea genotypes that are high-yielding with a good nutritional value as well as having resistance to biotics and tolerant to abiotic stress could also be used to address food security and malnutrition-related challenges in sub-Saharan Africa.
