The estimated glomerular filtration rate (eGFR) using serum creatinine is widely utilized for assessing renal function. Its decrease with age and in the presence of chronic diseases such as diabetes, hypertension, and obesity is well-known. However, there are no representative data for the Chilean population. AIM: To estimate the decline in eGFR with age according to gender and the presence of chronic diseases in the adult Chilean population. METHODS: This cross-sectional study involved 5,638 participants aged ≥18 years from the National Health Survey 2009 and 2017. The eGFR was estimated using the CKD-EPI formula based on serum creatinine. The decline in eGFR was compared by gender and the presence of chronic diseases (diabetes, hypertension, dyslipidemia, and/or obesity). RESULTS: eGFR declined with age in both genders, with a steeper decrease in women (-0.88 vs. -0.78 mL/min/1.73 m2/year, p<0.01). The decline in eGFR started early and uniformly from the age of 18. In the presence of chronic diseases, the slope was significantly steeper (-0.94 vs. -0.83 mL/min/1.73 m2/ year, p<0.001), with women with chronic diseases experiencing the greatest decline (-1.00 mL/min/1.73 m2/year). CONCLUSION: eGFR progressively decreased with age in the Chilean population, showing an early decline starting from 18 years, more pronounced in women, and in the presence of chronic diseases. Our findings provide relevant population-based information for interpreting eGFR across different age groups and risk categories.

Hypertension (arterial blood pressure ≥ 140/90 mmHg) is a risk factor for cardiovascular diseases, with the greatest burden of attributable deaths in Chile, having a national prevalence of 27.6%. In 2018, the implementation of HEARTS begun in primary health care centers of the Public Health System, with the aim of achieving increase in control rates, by raising the proportion of hypertensive individuals who meet blood pressure goals (< 140/90 mmHg for individuals 15-79 years old and of 150/90 mmHg for individuals 80 years and older), and thus contributing to reduce cardiovascular morbidity and mortality associated with this condition. This is a descriptive study that follows average treatment and control rates from the Public Health System between 2017-2021 obtained from health centers statistics reports during HEARTS implementation. Treatment and control rates remained at 57% and 39% respectively between 2017-2019. Between 2020 and 2021, in the context of the SARS-CoV-2 pandemic, treatment and control rates decreased very significantly, reaching 46% and 26%, respectively, in December 2021, even though the number of centers reporting the implementation of HEARTS increased from 227 to 387 in this same period. Prior to the pandemic, during the last quarter of 2019, a decrease in cardiovascular health controls was already observed as a result of social protests. In light of the results, the technical pillars of the HEARTS Initiative have an important role in helping to recover the population control rates reached in 2019 and increasing the speed to achieve better hypertension control rates.
La hipertensión arterial (presión arterial ≥ 140/90 mmHg) es un factor de riesgo para las enfermedades cardiovasculares, que tienen la mayor carga de muertes atribuibles en Chile. En este país, la hipertensión tiene una prevalencia nacional del 27,6%. En el 2018 se inició la aplicación del paquete técnico HEARTS en los centros de atención primaria de salud del sistema público de salud de Chile, con el objetivo de lograr un aumento de las tasas de control, al elevar la proporción de personas hipertensas que cumplen con las metas de presión arterial (< 140/90 mmHg para personas de 15 a 79 años y de 150/90 mmHg para personas de 80 años o más) y así contribuir a reducir la morbilidad y la mortalidad cardiovascular asociadas a esta enfermedad. En este estudio descriptivo se realiza un seguimiento de las tasas promedio de tratamiento y control del sistema público de salud entre el 2017 y el 2021 obtenidas de los informes estadísticos de los centros de salud durante la aplicación de la iniciativa HEARTS. Las tasas de tratamiento y control se mantuvieron en 57% y 39%, respectivamente, en el período entre el 2017 y el 2019. Entre el 2020 y el 2021, en el contexto de la pandemia de SARS-CoV-2, las tasas de tratamiento y control disminuyeron de manera muy significativa, y se ubicaron en 46% y 26%, respectivamente, en diciembre del 2021, a pesar de que el número de centros que notificaron la aplicación de HEARTS aumentó de 227 a 387 en el mismo período. Antes de la pandemia, en el último trimestre del 2019, ya se había observado una disminución en los controles de salud cardiovascular debido a las protestas sociales. En vista de estos resultados, los pilares técnicos de la iniciativa HEARTS desempeñan un papel importante para ayudar a recuperar las tasas de control que se habían alcanzado en el 2019 y acelerar la consecución de mejores tasas de control de la hipertensión.
A hipertensão (pressão arterial ≥ 140/90 mmHg) é um fator de risco para doenças cardiovasculares, com a maior carga de mortes atribuíveis no Chile, onde a prevalência nacional é de 27,6%. Em 2018, teve início a implementação da iniciativa HEARTS em centros de atenção primária à saúde do sistema de saúde pública, com o objetivo de elevar as taxas de controle, pelo aumento da proporção de indivíduos hipertensos que alcançam as metas de pressão arterial (< 140/90 mmHg para pessoas de 15-79 anos e de 150/90 mmHg para pessoas a partir de 80 anos), e, assim, contribuir para a redução da morbimortalidade cardiovascular associada a essa condição. Este é um estudo descritivo que acompanha as taxas médias de tratamento e controle no sistema de saúde pública entre 2017 e 2021, obtidas de relatórios estatísticos dos centros de saúde durante a implementação da iniciativa HEARTS. Entre 2017 e 2019, as taxas de tratamento e de controle foram, respectivamente, de 57% e 39%. Entre 2020 e 2021, no contexto da pandemia causada pelo SARS-CoV-2, houve uma diminuição muito significativa das taxas de tratamento e de controle, que chegaram, respectivamente, a 46% e 26% em dezembro de 2021, embora o número de centros que informaram ter implementado o pacote HEARTS tenha aumentado de 227 para 387 no mesmo período. Antes da pandemia, durante o último trimestre de 2019, já se observava uma diminuição dos controles da saúde cardiovascular em consequência de uma onda de protestos sociais. Os resultados mostram que os pilares técnicos da iniciativa HEARTS são importantes para ajudar a recuperar as taxas de controle na população alcançadas em 2019 e aumentar a velocidade para atingir melhores taxas de controle da hipertensão.

Objective: To evaluate the effectiveness of a standardized and simplified protocol based on the technical pillars of the HEARTS Initiative for the control of hypertensive patients in the Cardiovascular Health Program at the first level of care in Chile. Methods: Longitudinal observational study (historical cohort) in two family health centers at the first level of care in Santiago. The control of blood pressure in hypertensive adults using a standardized and simplified protocol was compared to the usual protocol based on national guidelines. Innovations in the standardized protocol included changes in how the health team is coordinated, initiation of pharmacological treatment immediately after confirmed diagnosis, standardized pharmacological treatment with a combination of at least two or three antihypertensive drugs taken daily in a single tablet. Follow-up was conducted after one year to assess the percentage of adherence to treatment and achievement of blood pressure control targets (< 140/90 mmHg). Results: A total of 1490 patients were evaluated: 562 who followed the standardized and simplified protocol, and 928 who were treated with the usual protocol (family health centers: 650; family health centers: 278). After one year, patients in the standardized and simplified protocol group had a higher proportion of adherence to blood pressure control targets (65% versus 37% and 41%, p<0.001) and higher adherence to treatment compared to those following the usual protocol (71% versus 18% and 23%, p<0.001). Conclusions: The results show that the standardized and simplified protocol is more effective than the usual protocol in controlling arterial hypertension in patients undergoing treatment at the first level of care in Chile. Its implementation at the national level could contribute to a decrease in major cardiovascular events.
Objetivo: Avaliar a eficácia de um protocolo padronizado e simplificado, com base nos pilares técnicos da iniciativa HEARTS, para o controle de pacientes com hipertensão arterial do Programa de Saúde Cardiovascular na atenção primária à saúde do Chile. Métodos: Estudo observacional longitudinal (coorte histórica) em 2 centros de atenção primária de saúde da família em Santiago, que comparou o controle da pressão arterial em adultos com hipertensão, atingido com o protocolo padronizado e simplificado, versus o protocolo habitual, de acordo com as diretrizes nacionais. As inovações do protocolo padronizado incluíram mudanças na coordenação da equipe de saúde, início do tratamento farmacológico imediatamente após a confirmação do diagnóstico e tratamento farmacológico padronizado com associação de pelo menos 2 ou 3 anti-hipertensivos em um único comprimido, tomados uma vez ao dia. O acompanhamento foi realizado por 1 ano para avaliar o percentual de adesão ao tratamento e o cumprimento das metas de controle da pressão arterial (menor que 140/90 mmHg). Resultados: Foram avaliados 1.490 pacientes: 562 que utilizaram o protocolo padronizado e simplificado e 928 que foram tratados com o protocolo habitual (unidade de saúde da família 1: 650, unidade de saúde da família 2: 278). Em 1 ano de seguimento, os pacientes do grupo do protocolo padronizado e simplificado apresentaram maior proporção de cumprimento das metas de controle da pressão arterial (65% versus 37% e 41%, p<0,001) e maior percentual de adesão ao tratamento, em comparação com aqueles que utilizaram o protocolo habitual (71% versus 18% e 23%, p<0,001). Conclusões: Os resultados mostram que o protocolo padronizado e simplificado é mais eficaz que o protocolo habitual no controle da hipertensão arterial em pacientes que estão em tratamento na atenção primária do Chile. Sua implementação no nível nacional poderia contribuir para a redução de eventos cardiovasculares maiores.

Objective: To describe the types of blood pressure measurement devices marketed through the Mercado Público platform between 2018 and 2020. Methods: This is a descriptive study based on transactions on the Mercado Público platform for the purchase of blood pressure measurement devices in the 2018-2020 period. A search was made for any validation granted by international organizations for automatic devices. The results are expressed as absolute and relative frequencies. Results: Between 2018 and 2020, purchases of automatic devices increased from 15% to 75%, while purchases of mercury-based devices dropped from 40.9% to 7.5%, and aneroid devices fell from 38.7% to 15.8%. Of the 772 automatic devices, 52.6% were validated for accuracy. Conclusions: Given that Chile is committed to the HEARTS Initiative in the Americas, and that prevention and control of hypertension is critical to reducing mortality from noncommunicable diseases, it is crucial to ensure accurate blood pressure measurement. This document provides relevant information on transactions and the purchase profile of blood pressure measurement devices on the Mercado Público platform. A growing trend was observed in the purchase of validated automatic equipment during the evaluation period.
Objetivo: Descrever os tipos de dispositivos de aferição da pressão arterial comercializados por meio da plataforma Mercado Público, entre 2018 e 2020. Métodos: Estudo descritivo das transações realizadas por meio do Mercado Público, no período de 2018 a 2020, para a compra de dispositivos de aferição da pressão arterial. No caso de dispositivos automáticos, foi verificado se possuíam validação concedida por organizações internacionais. Os resultados são apresentados na forma de frequências absolutas e relativas. Resultados: Entre 2018 e 2020, a aquisição de dispositivos automáticos aumentou de 15% para 75%. Houve uma redução de 40,9% para 7,5% na compra de esfigmomanômetros de mercúrio e de 38,7% para 15,8% na compra de esfigmomanômetros aneroides. Dos 772 dispositivos automáticos, 52,6% tinham exatidão validada. Conclusões: Considerando que o Chile está comprometido com a Iniciativa HEARTS nas Américas e que a prevenção e o controle da hipertensão arterial sistêmica é fundamental para reduzir a mortalidade por doenças não transmissíveis, é essencial garantir a aferição exata da pressão arterial. Este documento fornece informações relevantes sobre as transações realizadas e o perfil de compra de dispositivos de aferição da pressão arterial por meio da plataforma Mercado Público, mostrando uma tendência crescente na aquisição de dispositivos automáticos validados durante o período de avaliação.

Hypertension is a major preventable cause of death worldwide. May Measurement Month (MMM) is a global initiative aimed at raising awareness of high blood pressure (BP) and identifying individuals with increased BP. A cross-sectional survey of volunteers aged ≥18 years old was carried out in May 2019 in Chile. Participating sites were distributed across the country, most of them from the Public Health System outpatient clinics. In addition, clinical research sites, universities, and private clinics participated. Blood pressure measurement protocol, hypertension categories, and statistical analysis followed the MMM protocol. Hypertension was diagnosed as mean systolic BP ≥140 mmHg and/or diastolic BP ≥90 mmHg or receiving antihypertensive medication. Overall, 6876 individuals were screened. After multiple imputations, hypertension prevalence was 35.4%, of which 65.9% were aware of their condition. While 60.1% were on antihypertensive medication and 34.4% of the total number of hypertensives had their BP controlled. Of participants not receiving antihypertensive treatment, 17.9% were identified as hypertensive. The MMM 2019 survey demonstrated a high proportion of participants with hypertension, with only one-third of these individuals having controlled BP (systolic BP <140 mmHg and diastolic BP <90 mmHg). The high percentage of participants with hypertension who were either untreated (39.9%) or were treated but uncontrolled (57.2%) suggesting that such opportunistic screening programmes may be a useful tool to improve hypertension control in Chile.

OBJECTIVES: As part of the HEARTS in the Americas initiative, Chilean primary healthcare centers have implemented novel hypertension management strategies, including new diagnostic approaches. This study evaluated the concordance between attended automated office blood pressure (AOBP) measurements with an oscillometric device and ambulatory blood pressure monitoring (ABPM). METHODS: This was an observational cohort study to evaluate and compare attended AOBP and ABPM for the diagnosis of hypertension in adults in a primary healthcare setting. RESULTS: The study evaluated 309 participants (54.2 ± 15.7 years; 50.5% male) from four primary healthcare centers in Santiago, Chile. Attended AOBP measurements were obtained at the clinic on two separate days, followed by ABPM. AOBP values indicated that 69.6% of patients had a systolic blood pressure (SBP) of ≥140 mm Hg and 34.6% had a diastolic blood pressure (DBP) of ≥90 mm Hg. A total of 83.5% had hypertension, 45.3% had high SBP, and 56.0% had high DBP. ABPM values indicated that 65.0% of patients had hypertension. The combined AOBP and ABPM analysis showed that 57.0% of patients had sustained hypertension, 26.5% had white coat hypertension, 8.1% had masked hypertension, and 8.4% were normotensive. The concordance between AOBP and ABPM (κ coefficient) was low (κ = 0.133; 95% confidence interval 0.028-0.237). The comparison of AOBP and ABPM measurements (Bland-Altman plots and bias calculations) showed an important bias in BP as measured using the AOBP method, especially for SBP (13.7 ± 11.6, 95% confidence interval -9.1 to 36.5). CONCLUSIONS: Attended AOBP alone may not be sufficient for adequate classification, diagnosis, and management of hypertension in Chile or other countries with similar demographics.

Hypertension is highly prevalent in Chile. The National Health Survey 2016-17 reported a 27.6% prevalence, 68.7% awareness, and 33.3% of hypertensives had controlled blood pressure (BP). May Measurement Month (MMM) is a global initiative aimed at raising awareness of high BP and to act as a temporary solution to the lack of screening programmes worldwide. A cross-sectional survey of volunteers aged ≥18 years old was carried out in May 2018. Blood pressure measurement, the definition of hypertension, and statistical analysis followed the MMM protocol. Ninety-four sites participated, most of them from the Public Health System outpatient clinics distributed along the country. In addition, universities, clinical research sites, and private clinics participated. Hypertension was diagnosed as mean systolic BP ≥140 mmHg and/or diastolic BP ≥90 mmHg or receiving antihypertensive medication. Overall 9344 individuals were screened. After multiple imputation, hypertension prevalence was 29.2%, of which 64.0% were aware of their condition. Of those aware of their hypertension diagnosis, 87.7% were receiving antihypertensive medication and 56.1% of the total number of hypertensives were on antihypertensive medication. Moreover, 15.3% of the participants who were not receiving treatment were considered potential hypertensives identified in the MMM18. MMM18 was one of the largest BP screening campaigns performed in Chile. It demonstrated a high prevalence of hypertension with one-third of these individuals having controlled BP. The high percentage of persons untreated or with uncontrolled hypertension while on pharmacologic treatment suggests that systematic screening programmes may be a useful tool to improve hypertension control in Chile.

Elevated blood pressure (BP) is a growing burden worldwide, leading to over 10 million deaths each year. May Measurement Month (MMM) is a global initiative aimed at raising awareness of high BP and to act as a temporary solution to the lack of screening programmes worldwide. National Health Surveys, PURE and CESCAS Chilean cohorts have shown a high prevalence of hypertension, with a significant proportion of people unaware of being hypertensive or under treatment but with uncontrolled BP. An opportunistic cross-sectional survey of volunteers aged ≥18 was carried out in May 2017. BP measurement, the definition of hypertension and statistical analysis followed the MMM protocol. Sixty-seven sites participated. Most screening sites were National Heath Public System outpatient clinics, mainly in the 9th region of the country. In addition, clinical research sites and private clinics participated. Overall, 4754 individuals were screened during MMM17. After multiple imputations, 1153 (24.2%) had hypertension. Of individuals not receiving anti-hypertensive medication, 653 (15.3%) were hypertensive. Of the 500 individuals receiving anti-hypertensive medication, 162 (32.5%) had uncontrolled BP. MMM17 was one of the largest BP screening campaigns performed in Chile. It demonstrated a high proportion of hypertension among screenees with a low proportion the individuals with controlled hypertension in the community. The high percentage of persons untreated or with uncontrolled hypertension whilst on pharmacologic treatment suggests that systematic screening programmes may be a useful tool to improve hypertension control in Chile.
RESUMO
The aim of the HEARTS in the Americas initiative is to promote the adoption of global best practices in the prevention and control of cardiovascular diseases, and improve the control of hypertension. HEARTS is being implemented in 21 countries and a diverse set of actions and measures are in progress to improve exclusive access in primary health care facilities to automated blood pressure measuring devices that have been validated for accuracy. The purpose of this manuscript is to illustrate these efforts, mainly in the regulatory and public procurement arena, and to present information on common challenges and solutions identified. Examples from six countries confirm the need for not only a robust regulatory framework to increase availability of validated automated blood pressure measuring devices but also a comprehensive strategic approach that involves relevant stakeholders, includes a multi-pronged approach and is associated with a national program to prevent and control non communicable diseases.

OBJECTIVES: This study estimates the prevalence of chronic kidney disease (CKD) among Chilean adults and examines its associations with sociodemographic characteristics, health behaviours and comorbidities. DESIGN: Analysis of cross-sectional data from the two most recent large nationally representative Chilean Health Surveys (Encuesta Nacional de Salud, ENS) 2009-2010 and 2016-2017. PARTICIPANTS: Adults aged 18+ years with serum creatine data (ENS 2009-2010: n=4583; ENS 2016-2017: n=5084). PRIMARY AND SECONDARY OUTCOME MEASURES: Reduced kidney function (CKD stages 3a-5) based on the estimated glomerular filtration rate (eGFR <60 mL/min/1.73 m2) was the primary outcome measure. Using the urine albumin-to-creatinine ratio (ACR ≥30 mg/g), increased albuminuria was ascertained among adults aged 40+ years with diabetes and/or hypertension. Both outcomes were analysed using logistic regression with results summarised using OR. CKD prevalence (stages 1-5) among adults aged 40+ years was estimated including participants with an eGFR of >60 mL/min/1.73 m2 but with increased albuminuria (stages 1-2). RESULTS: Overall, 3.2% (95% CI: 2.4% to 3.8%) of adults aged 18+ in ENS 2016-2017 had reduced kidney function. After full adjustment, participants with hypertension (OR: 2.37; 95% CI: 1.19 to 4.74) and those with diabetes (OR: 1.66; 95% CI: 1.03 to 2.66) had significantly higher odds of reduced kidney function. In ENS 2016-2017, 15.5% (13.5% to 17.8%) of adults aged 40+ years with diabetes and/or hypertension had increased albuminuria. Being obese versus normal-weight (OR: 1.66; 95% CI: 1.08 to 2.54) and having both diabetes and hypertension versus having diabetes alone (OR: 2.30; 95% CI: 1.34 to 3.95) were significantly associated with higher odds of increased albuminuria in fully-adjusted analyses. At least 15.4% of adults aged 40+ years in ENS 2016-2017 had CKD (stages 1-5), including the 9.6% of adults at CKD stages 1-2. CONCLUSIONS: Prevention strategies and Chilean guidelines should consider the high percentage of adults aged 40 years and older at CKD stages 1-2.

The Pan American Health Organization (PAHO)-World Hypertension League (WHL) Hypertension Monitoring and Evaluation Framework is summarized. Standardized indicators are provided for monitoring and evaluating national or subnational hypertension control programs. Five core indicators from the World Health Organization hearts initiative and a single PAHO-WHL core indicator are recommended to be used in all hypertension control programs. In addition, hypertension control programs are encouraged to select from 14 optional qualitative and 33 quantitative indicators to facilitate progress towards enhanced hypertension control. The intention is for hypertension programs to select quantitative indicators based on the current surveillance mechanisms that are available and what is feasible and to use the framework process indicators as a guide to program management. Programs may wish to increase or refine the number of indicators they use over time. With adaption the indicators can also be implemented at a community or clinic level. The standardized indicators are being pilot tested in Cuba, Colombia, Chile, and Barbados.

RESUMEN Objetivo. Evaluar la eficacia de un protocolo estandarizado y simplificado basado en los pilares técnicos de la Iniciativa HEARTS para el control de pacientes hipertensos del Programa de Salud Cardiovascular en el primer nivel de atención de salud en Chile. Métodos. Estudio observacional longitudinal (cohorte histórica), en 2 centros de salud familiar del primer nivel de atención en Santiago, que comparó el control de presión arterial en adultos hipertensos logrado con el protocolo estandarizado y simplificado, frente al protocolo habitual, según las guías nacionales. Las innovaciones del protocolo estandarizado incluyeron cambios en la coordinación del equipo de salud, inicio de tratamiento farmacológico inmediatamente después de confirmación diagnóstica, tratamiento farmacológico estandarizado con combinación de al menos 2 o 3 fármacos antihipertensivos en un sólo comprimido, en una toma diaria. Se realizó seguimiento por 1 año para evaluar el porcentaje de adherencia al tratamiento y cumplimiento de metas de control de presión arterial (menor a 140/90 mmHg). Resultados. Se evaluaron 1 490 pacientes: 562 que utilizaron el protocolo estandarizado y simplificado y 928 tratados con el protocolo habitual (centros de salud familiar-1: 650, centros de salud familiar -2: 278). A 1 año de seguimiento, los pacientes del grupo del protocolo estandarizado y simplificado tuvieron mayor proporción de cumplimiento de metas de control de presión arterial (65% versus 37% y 41%, p<0,001) y mayor porcentaje de adherencia al tratamiento en comparación con aquellos con el protocolo habitual (71% versus 18% y 23%, p<0,001). Conclusiones. Los resultados muestran que el protocolo estandarizado y simplificado es más efectivo que el protocolo habitual en el control de hipertensión arterial en pacientes en tratamiento en el primer nivel de atención en Chile. Su implementación a nivel nacional podría contribuir a la disminución de eventos cardiovasculares mayores.
ABSTRACT Objective. To evaluate the effectiveness of a standardized and simplified protocol based on the technical pillars of the HEARTS Initiative for the control of hypertensive patients in the Cardiovascular Health Program at the first level of care in Chile. Methods. Longitudinal observational study (historical cohort) in two family health centers at the first level of care in Santiago. The control of blood pressure in hypertensive adults using a standardized and simplified protocol was compared to the usual protocol based on national guidelines. Innovations in the standardized protocol included changes in how the health team is coordinated, initiation of pharmacological treatment immediately after confirmed diagnosis, standardized pharmacological treatment with a combination of at least two or three antihypertensive drugs taken daily in a single tablet. Follow-up was conducted after one year to assess the percentage of adherence to treatment and achievement of blood pressure control targets (< 140/90 mmHg). Results. A total of 1490 patients were evaluated: 562 who followed the standardized and simplified protocol, and 928 who were treated with the usual protocol (family health centers: 650; family health centers: 278). After one year, patients in the standardized and simplified protocol group had a higher proportion of adherence to blood pressure control targets (65% versus 37% and 41%, p<0.001) and higher adherence to treatment compared to those following the usual protocol (71% versus 18% and 23%, p<0.001). Conclusions. The results show that the standardized and simplified protocol is more effective than the usual protocol in controlling arterial hypertension in patients undergoing treatment at the first level of care in Chile. Its implementation at the national level could contribute to a decrease in major cardiovascular events.
RESUMO Objetivo. Avaliar a eficácia de um protocolo padronizado e simplificado, com base nos pilares técnicos da iniciativa HEARTS, para o controle de pacientes com hipertensão arterial do Programa de Saúde Cardiovascular na atenção primária à saúde do Chile. Métodos. Estudo observacional longitudinal (coorte histórica) em 2 centros de atenção primária de saúde da família em Santiago, que comparou o controle da pressão arterial em adultos com hipertensão, atingido com o protocolo padronizado e simplificado, versus o protocolo habitual, de acordo com as diretrizes nacionais. As inovações do protocolo padronizado incluíram mudanças na coordenação da equipe de saúde, início do tratamento farmacológico imediatamente após a confirmação do diagnóstico e tratamento farmacológico padronizado com associação de pelo menos 2 ou 3 anti-hipertensivos em um único comprimido, tomados uma vez ao dia. O acompanhamento foi realizado por 1 ano para avaliar o percentual de adesão ao tratamento e o cumprimento das metas de controle da pressão arterial (menor que 140/90 mmHg). Resultados. Foram avaliados 1.490 pacientes: 562 que utilizaram o protocolo padronizado e simplificado e 928 que foram tratados com o protocolo habitual (unidade de saúde da família 1: 650, unidade de saúde da família 2: 278). Em 1 ano de seguimento, os pacientes do grupo do protocolo padronizado e simplificado apresentaram maior proporção de cumprimento das metas de controle da pressão arterial (65% versus 37% e 41%, p<0,001) e maior percentual de adesão ao tratamento, em comparação com aqueles que utilizaram o protocolo habitual (71% versus 18% e 23%, p<0,001). Conclusões. Os resultados mostram que o protocolo padronizado e simplificado é mais eficaz que o protocolo habitual no controle da hipertensão arterial em pacientes que estão em tratamento na atenção primária do Chile. Sua implementação no nível nacional poderia contribuir para a redução de eventos cardiovasculares maiores.

RESUMEN Objetivo. Describir los tipos de dispositivos de medición de presión arterial comercializados a través de la plataforma Mercado Público entre los años 2018 y 2020. Métodos. Estudio de tipo descriptivo procedente de las transacciones realizadas a través de Mercado Público en el periodo 2018 y 2020, para la adquisición de dispositivos de medición de presión arterial. Para los equipos automáticos, se buscó el estado de validación otorgado por organismos internacionales. Los resultados se presentan en frecuencias absolutas y relativas. Resultados. Entre los años 2018-2020, aumentó la adquisición de dispositivos automáticos de un 15% a un 75%, y se presentó una reducción del 40,9% al 7,5% en equipos mercuriales y de 38,7% al 15,8% en equipos aneroides. De los 772 dispositivos automáticos, el 52,6% contó con validación de precisión. Conclusiones. Considerando que Chile está comprometido con la Iniciativa HEARTS en las Américas, y que la prevención y control de la HTA es fundamental para reducir la mortalidad por enfermedades no trasmisibles, resulta crucial garantizar una medición de presión arterial precisa. El presente documento aporta información relevante respecto de las transacciones realizadas y el perfil de compra de dispositivos de medición de presión arterial a través de la plataforma Mercado Público, observándose una tendencia creciente en la obtención de equipos automáticos validados durante el periodo evaluado.
ABSTRACT Objective. To describe the types of blood pressure measurement devices marketed through the Mercado Público platform between 2018 and 2020. Methods. This is a descriptive study based on transactions on the Mercado Público platform for the purchase of blood pressure measurement devices in the 2018-2020 period. A search was made for any validation granted by international organizations for automatic devices. The results are expressed as absolute and relative frequencies. Results. Between 2018 and 2020, purchases of automatic devices increased from 15% to 75%, while purchases of mercury-based devices dropped from 40.9% to 7.5%, and aneroid devices fell from 38.7% to 15.8%. Of the 772 automatic devices, 52.6% were validated for accuracy. Conclusions. Given that Chile is committed to the HEARTS Initiative in the Americas, and that prevention and control of hypertension is critical to reducing mortality from noncommunicable diseases, it is crucial to ensure accurate blood pressure measurement. This document provides relevant information on transactions and the purchase profile of blood pressure measurement devices on the Mercado Público platform. A growing trend was observed in the purchase of validated automatic equipment during the evaluation period.
RESUMO Objetivo. Descrever os tipos de dispositivos de aferição da pressão arterial comercializados por meio da plataforma Mercado Público, entre 2018 e 2020. Métodos. Estudo descritivo das transações realizadas por meio do Mercado Público, no período de 2018 a 2020, para a compra de dispositivos de aferição da pressão arterial. No caso de dispositivos automáticos, foi verificado se possuíam validação concedida por organizações internacionais. Os resultados são apresentados na forma de frequências absolutas e relativas. Resultados. Entre 2018 e 2020, a aquisição de dispositivos automáticos aumentou de 15% para 75%. Houve uma redução de 40,9% para 7,5% na compra de esfigmomanômetros de mercúrio e de 38,7% para 15,8% na compra de esfigmomanômetros aneroides. Dos 772 dispositivos automáticos, 52,6% tinham exatidão validada. Conclusões. Considerando que o Chile está comprometido com a Iniciativa HEARTS nas Américas e que a prevenção e o controle da hipertensão arterial sistêmica é fundamental para reduzir a mortalidade por doenças não transmissíveis, é essencial garantir a aferição exata da pressão arterial. Este documento fornece informações relevantes sobre as transações realizadas e o perfil de compra de dispositivos de aferição da pressão arterial por meio da plataforma Mercado Público, mostrando uma tendência crescente na aquisição de dispositivos automáticos validados durante o período de avaliação.

ABSTRACT Hypertension (arterial blood pressure ≥ 140/90 mmHg) is a risk factor for cardiovascular diseases, with the greatest burden of attributable deaths in Chile, having a national prevalence of 27.6%. In 2018, the implementation of HEARTS begun in primary health care centers of the Public Health System, with the aim of achieving increase in control rates, by raising the proportion of hypertensive individuals who meet blood pressure goals (< 140/90 mmHg for individuals 15-79 years old and of 150/90 mmHg for individuals 80 years and older), and thus contributing to reduce cardiovascular morbidity and mortality associated with this condition. This is a descriptive study that follows average treatment and control rates from the Public Health System between 2017-2021 obtained from health centers statistics reports during HEARTS implementation. Treatment and control rates remained at 57% and 39% respectively between 2017-2019. Between 2020 and 2021, in the context of the SARS-CoV-2 pandemic, treatment and control rates decreased very significantly, reaching 46% and 26%, respectively, in December 2021, even though the number of centers reporting the implementation of HEARTS increased from 227 to 387 in this same period. Prior to the pandemic, during the last quarter of 2019, a decrease in cardiovascular health controls was already observed as a result of social protests. In light of the results, the technical pillars of the HEARTS Initiative have an important role in helping to recover the population control rates reached in 2019 and increasing the speed to achieve better hypertension control rates.
RESUMEN La hipertensión arterial (presión arterial ≥ 140/90 mmHg) es un factor de riesgo para las enfermedades cardiovasculares, que tienen la mayor carga de muertes atribuibles en Chile. En este país, la hipertensión tiene una prevalencia nacional del 27,6%. En el 2018 se inició la aplicación del paquete técnico HEARTS en los centros de atención primaria de salud del sistema público de salud de Chile, con el objetivo de lograr un aumento de las tasas de control, al elevar la proporción de personas hipertensas que cumplen con las metas de presión arterial (< 140/90 mmHg para personas de 15 a 79 años y de 150/90 mmHg para personas de 80 años o más) y así contribuir a reducir la morbilidad y la mortalidad cardiovascular asociadas a esta enfermedad. En este estudio descriptivo se realiza un seguimiento de las tasas promedio de tratamiento y control del sistema público de salud entre el 2017 y el 2021 obtenidas de los informes estadísticos de los centros de salud durante la aplicación de la iniciativa HEARTS. Las tasas de tratamiento y control se mantuvieron en 57% y 39%, respectivamente, en el período entre el 2017 y el 2019. Entre el 2020 y el 2021, en el contexto de la pandemia de SARS-CoV-2, las tasas de tratamiento y control disminuyeron de manera muy significativa, y se ubicaron en 46% y 26%, respectivamente, en diciembre del 2021, a pesar de que el número de centros que notificaron la aplicación de HEARTS aumentó de 227 a 387 en el mismo período. Antes de la pandemia, en el último trimestre del 2019, ya se había observado una disminución en los controles de salud cardiovascular debido a las protestas sociales. En vista de estos resultados, los pilares técnicos de la iniciativa HEARTS desempeñan un papel importante para ayudar a recuperar las tasas de control que se habían alcanzado en el 2019 y acelerar la consecución de mejores tasas de control de la hipertensión.
RESUMO A hipertensão (pressão arterial ≥ 140/90 mmHg) é um fator de risco para doenças cardiovasculares, com a maior carga de mortes atribuíveis no Chile, onde a prevalência nacional é de 27,6%. Em 2018, teve início a implementação da iniciativa HEARTS em centros de atenção primária à saúde do sistema de saúde pública, com o objetivo de elevar as taxas de controle, pelo aumento da proporção de indivíduos hipertensos que alcançam as metas de pressão arterial (< 140/90 mmHg para pessoas de 15-79 anos e de 150/90 mmHg para pessoas a partir de 80 anos), e, assim, contribuir para a redução da morbimortalidade cardiovascular associada a essa condição. Este é um estudo descritivo que acompanha as taxas médias de tratamento e controle no sistema de saúde pública entre 2017 e 2021, obtidas de relatórios estatísticos dos centros de saúde durante a implementação da iniciativa HEARTS. Entre 2017 e 2019, as taxas de tratamento e de controle foram, respectivamente, de 57% e 39%. Entre 2020 e 2021, no contexto da pandemia causada pelo SARS-CoV-2, houve uma diminuição muito significativa das taxas de tratamento e de controle, que chegaram, respectivamente, a 46% e 26% em dezembro de 2021, embora o número de centros que informaram ter implementado o pacote HEARTS tenha aumentado de 227 para 387 no mesmo período. Antes da pandemia, durante o último trimestre de 2019, já se observava uma diminuição dos controles da saúde cardiovascular em consequência de uma onda de protestos sociais. Os resultados mostram que os pilares técnicos da iniciativa HEARTS são importantes para ajudar a recuperar as taxas de controle na população alcançadas em 2019 e aumentar a velocidade para atingir melhores taxas de controle da hipertensão.
