RESUMO

In recent years, predictive methods for assessing the preservation of the parathyroid glands have been actively implemented. The article describes the first experience of evaluating the blood supply of the parathyroid glands by quantitative determination of the indocyanine green (ICG) accumulation index in real time in 6 patients before and after a thyroidectomy with central neck lymph node dissection for papillary thyroid cancer. Intraoperative fluorescent angiography was performed by using domestic equipment with a fluorescent module, as well as by using a domestic medication of ICG. Intraoperative values of the ICG accumulation index were compared with the levels of ionized calcium and parathyroid hormone perioperatively. No clinical manifestations of hypocalcemia were detected in the postoperative period. The obtained results showed the informativeness of the numerical assessment of the intensity of ICG fluorescence. The evaluation of the distribution (accumulation) of ICG has prospects for practical application in thyroid surgery in the formation of tactics for preserving the parathyroid glands and predicting postoperative hypoparathyreosis.

The current generally accepted structure of light-harvesting LH2 complexes from purple phototrophic bacteria conflicts with the observation of singlet-triplet carotenoid excitation fission in these complexes. In LH2 complexes from the purple bacterium Allochromatium minutissimum, a drop in the efficiency of carotenoid triplet generation is demonstrated, which correlates with the extent of selective photooxidation of bacteriochlorophylls absorbing at ~850 nm. We conclude that singlet-triplet fission of carotenoid excitation proceeds with participation of these excitonically coupled bacteriochlorophylls. In the framework of the proposed mechanism, the contradiction between LH2 structure and photophysical properties of carotenoids is eliminated. The possibility of singlet-triplet excitation fission involving a third mediator molecule was not considered earlier.

The authors have analyzed their experiences with treatment of 61 patients using the method of endoscopic clipping: 24 of them had the source of bleeding in chronic ulcers of the duodenal bulb, 6 had chronic gastric ulcers, 15 had acute gastric ulcers, 2 had acute ulcers of the duodenal bulb, Mallory-Weiss syndrome was found in 2 patients, Dieulafoy syndrome in 10 patients and one patient had peptic ulcer of the gastro-entero-anastomosis. The endoscopic clipping was made in 11 patients, in 31 patients injections of adrenaline and coagulation were added, in 9 patients--coagulation only, in 2 patients--irrigation with caproferon, in 8--adrenaline injections. In 57 out of 61 patients reliable hemostasis was achieved.

AIM: To compare clinical and hormonal status in thyrotoxicosis with- or without comorbid resistant depression. MATERIAL AND METHODS: We examined 100 patients with thyrotoxicosis, including 61 patients with comorbid resistant depression (RD) and 39 patients without RD. Mental status, somatic symptoms of disease and blood content of main thyroid hormones were studied. The Hamilton scale was used to measure depression and anxiety. RESULTS AND CONCLUSION: Depression developed in the structure of thyrotoxicosis increased the symptoms of hyperthyroidism. However, it did not change the general tendency of development of the main disease assessed by hormonal indicators.

RESUMO

A morphofunctional changes of peritoneal and mesenteric mast cell populations in rats from first minutes up to 10 day after irradiation in dose 5.5 Gr were studied. It is shown that the increased mast cell degranulation is of the first reactions of organism on irradiation. Mast cell reaction on irradiation is biphasic; the quick, short first phase is observed approximately during 0.5 h after irradiation, the gradual prolonged second phase reaches its peak about 5-12 h and continues at least during 10 days.
