RESUMO

INTRODUCTION: Body composition comprising low-skeletal muscle mass (LSMM) and subcutaneous and visceral adipose tissue (SAT and VAT) can be assessed by using cross-sectional imaging modalities. Previous analyses suggest that these parameters harbor prognostic relevance in various diseases. Aim of this study was to analyze possible associations of body composition parameters on mortality in patients with clinically suspected acute mesenteric ischemia (AMI). METHODS: All patients with clinically suspected AMI were retrospectively assessed between 2016 and 2020. Overall, 137 patients (52 female patients, 37.9%) with a median age of 71 years were included in the present analysis. For all patients, the preoperative abdominal computed tomography (CT) was used to calculate LSMM, VAT, and SAT. RESULTS: Overall, 94 patients (68.6%) of the patient cohort died within 30 days within a median of 2 days, range 1-39 days. Of these, 27 patients (19.7%) died within 24 h. According to the CT, 101 patients (73.7%) were classified as being visceral obese, 102 patients (74.5%) as being sarcopenic, and 69 patients (50.4%) as being sarcopenic obese. Skeletal muscle index (SMI) was lower in non-survivors compared to survivors (37.5 ± 12.4 cm2/m2 vs. 44.1 ± 13.9 cm2/m2, p = 0.01). There were no associations between body composition parameters with mortality in days (SMI r = 0.07, p = 0.48, SAT r = -0.03, p = 0.77, and VAT r = 0.04, p = 0.68, respectively). In Cox regression analysis, a nonsignificant trend for visceral obesity was observed (HR: 0.62, 95% CI: 0.36-1.05, p = 0.07). CONCLUSION: SMI might be a valuable CT-based parameter, which could help discriminate between survivors and non-survivors. Further studies are needed to elucidate the associations between body composition and survival in patients with AMI.

INTRODUCTION: Dislocations of hip hemiarthroplasty (HHA) are serious complications. The aim of the study was to identify clinical aspects and radiographic parameters of the hip that put patients at risk for dislocation after HHA for femoral neck fractures. METHODS: This retrospective analysis included elderly patients with a femoral neck fracture treated with a HHA. A lateral (90.7%) and an anterolateral (9.3%) approach was applied. On pelvic radiographs, a nested-controlled analysis was performed. Two control patients were matched to one patient suffering a dislocation with respect to age, sex, and body-mass-index (BMI). RESULTS: In 527 HHA, 10 dislocations (1.9%) were identified. In the dislocation group (DG), all patients were female (100% vs. 73.5%, p = 0.071). No significant differences between the DG and the control group (CG) were found with respect to age, body-mass-index (BMI), ASA Score, routine laboratory parameters, and comorbidity. Radiographic analysis revealed a smaller center edge angle (CEA, 39.0, IQR 33.0-42.5 vs. 43.0, IQR 41.0-46.0, p = 0.013), a more varus neck-shaft angle (NSA, 130.0, IQR 125.8-133.5 vs. 135.0, IQR 134.0-137.0, p = 0.011) of the contralateral side and a higher femoral head extrusion index (FHEI) in the DG (FHEI, 11.5, IQR 9.8-16.3 vs. 2.0 IQR 0.0-9.0, p = 0.003). In addition, a greater trochanteric fracture was associated with an increased likelihood for HHA dislocations (30.0% vs 6.0%, p = 0.022). CONCLUSION: A smaller radiographic center edge angle, a more varus neck-shaft angle of the contralateral side, a higher femoral head extrusion index and intraoperative fractures of the greater trochanter are associated with an increased risk of HHA dislocation.

OBJECTIVE: A comprehensive CT algorithm in suspected ischemic stroke consists of a native CT scan, CT perfusion imaging of the brain and a CT angiography from the aortic arch to the vertex. Besides core findings of the brain and targeted vessels, various other findings may be identified. We analyzed a large patient sample and estimated the frequency of incidental findings (IFs) and whether these findings were correctly mentioned in the radiological reports. PATIENTS AND METHODS: In the time period between 01/16 and 02/19 the radiological database of one hospital containing 1625 patients admitted for stroke suspicion were retrospectively evaluated. In total 1175 patients with ischemic stroke CTs were included. IFs were classified according to Lumbreras et al. for their clinical relevance. The primary radiological report was used to classify, whether the finding was reported initially or not. RESULTS: Overall, 1988 IFs were identified in 1175 patients (mean 1.7 findings per patient). The most frequent finding were thyroid incidentalomas with n = 461 (23.2 % of all findings), followed by signs of mucosal swelling and/or opacification of the nasal sinus (n = 391, 19.7 %). Regarding clinical relevance, 181 findings were of major relevance (9.1 % of all findings), 902 were of moderate relevance (45.4 %) and 905 were of minor relevance (45.5 %). Overall, 772 findings (38.8 %) were not reported and 1216 (61.2 %) were sufficiently reported by the radiologist. CONCLUSION: The present study corroborates that incidental findings are frequent in patients undergoing ischemic stroke CT, which are about half of moderate and major relevance. About 40 % of these findings were not mentioned in the primary radiology report, including findings of high clinical relevance. The radiologist should be aware of these findings.
