RESUMO

BACKGROUND: Halo nevi are often considered benign, and the possibility of malignancy is not always clear to practitioners. We present two case reports suggesting that a halo nevus appearance can be seen in melanoma, even in young adults. A literature search for halo nevi revealing melanoma shows that this is a very rare condition. Case presentation. This report of two young patients indicates the importance of obtaining a detailed history to detect warning signs such as itching, pain, spontaneous bleeding, and previous alterations according to the patient, including a previously totally black colour in an already fully regressed melanoma. CONCLUSIONS: The risk of a halo nevus being malignant is higher if there is only one unique halo nevus and no personal or familial history of vitiligo. We postulate that a regressing atypical nevus or a regressing melanoma may be induced by an immunologic reaction as halo nevus type of clinical picture.

RESUMO

This is a case report of a Pott's puffy tumor, characterized by a subperiosteal abscess associated with frontal bone osteomyelitis, as a consequence of a frontal sinusitis, in a 15-year-old boy. Pott's puffy tumor is a rare condition usually seen as a complication of frontal sinusitis and more commonly described in children. Given that, superficial temporal artery pseudoaneurysms might be interpreted as a cyst or lipoma, it is imperative that physicians be aware of their presentation.

It is widely accepted that chronic burn wounds may lead to the development of various malignant skin tumors. Deep stage 3 burned areas may facilitate deeper carcinogenesis. Deep tissues are probably less subject to severe insult than is the epithelial layer during physical insult, suggesting that soft tissues transform to a lesser extent during the late stages of tumoral development as in an immunocompromised district with altered local immune defense with both cellular and humoral defense affected. Most authors claim that tumors are almost squamous cell carcinomas, although other types of malignancies such as basal cell carcinoma and, to a lesser extent, melanoma can also be seen. However, malignant transformation of cutaneous soft tissue in a burn insult area has rarely been described. Similarly, burn-induced tumors of histiocytic origin have been reported in few cases and osteosarcoma only in two case reports. Here, we report a patient case suffering from severe large stage 3 burn after-effects on the leg. Fifty-five years after the injury, this patient developed a large extraosseous osteosarcoma on the scar.

Belgium has been confronted to cases of paedophilia for years. It seems adequate to suggest some guidelines in cases of a child or a teenager diagnosed with ano-genital warts during a consultation. They are not always the consequence of sexual abuse. Three contaminations are possible: ante natal and peri natal (in utero or during delivery), by direct contact (auto or hetero inoculation), through sexual contact. This trusted practitioners to handle this unusual situation from the child protection, to multidisciplinary management and judicial prosecution against suspect. When using some different way to know what kind of HPV type; it's possible to give some information about transmission and it's possible to do confrontations with the results of the lesions' suspect. It is also interesting to identify very highs oncogenic potential HPV for the survey's patient. Finally the most important is to know that the most recurrences result into reactivations from infra-clinically infections and are not new contaminations. It is important do not drop a new suspicion through hypothetic contaminator.

Arsenic is an ubiquitary element which has been widely used for centuries in different fields such as medicine, agriculture or industry. Acute or chronic exposure to As can lead to various dermatological and systemic disorders with a possibe latency over decades. The dermatological signs of As intoxication are important to detect since one of the potential complications is carcinoma.

RESUMO

Onychomatricoma is a rare and benign tumour of the nail matrix but originates rarely from the ventral portion of the proximal nail fold. This tumour is characterised by fingerlike projections that invade the nail plate. This lesion, of unknown aetiology, is typically asymptomatic with slow progression. Localisation on the finger is the most frequently described. We report the case of a 68-year-old woman who has an onychomatricoma in an unusual location, the fifth toe of the left foot. Due to its clinical appearance, the tumour can be confused with and treated as onychomycosis. However, if it is resistant to an oral antifungal well behaved treatment, one must consider onychomatricoma diagnosis.

RESUMO

Cocaine has been used for centuries for its stimulating and euphoric effects, but it also possesses vasoconstrictive and thrombotic properties. The cocaine-induced lesions depend mainly on the method of administration. In the present case, in which cocaine was nasally inhaled, the perforation of the cartilaginous nasal septum propably result from a combination of factors, including the topical effects and the complications linked to the use of the drug. This clinical observation corresponds to a high level of the progressive nasal pathology associated with cocaine abuse.

RESUMO

Vulvar lichen sclerosus is a frequent mucocutaneous disease especially affecting 50 to 60 year-old women but with a possible onset at very young age. Symptoms are most disabling including pruritus and dyspareunia. Vulvar mucosa gradually becomes more white and atrophied. Degeneration into epidermoid carcinoma is possible. Treatment only consists of topical corticosteroids.

Pyoderma gangrenosum (PG) is a rare chronic inflammatory skin disease characterized by the recurring development of necrotizing and painful ulcers. The skin lesions appear spontaneously or after minor traumatic injuries. Sites of predilection include the lower limbs and the trunk but any part of the body may be affected. PG is not an infectious disease; although the etiology is not completely understood, an immune disturbance is certainly involved. An underlying systemic disorder is associated in up to 50% of the cases, specially inflammatory bowel diseases, arthritis, paraproteinemias and hematologic malignancies. Chronic venous or arterial ulcers as well as bacterial gangrene are the most frequent false diagnoses. A right diagnosis, based upon the distinctive clinical features and a compatible histology, is essential to avoid surgical procedure that often tends to exacerbed the process. Because of its persistent and recurrent nature, systemic long-term therapy based upon corticosteroids associated with sulfones or immunosuppressive agents is required.

RESUMO

We report a bilateral metastasis of the fifth nail unit of the hand in a man with primary squamous-cell carcinoma of the lung. The presence of fingertip metastasis may be the first manifestation of an internal neoplasm and is related to a poor prognosis, with a survival rate of less than 50% after 6 months. Therefore, physicians should be aware of this condition when examining patients with similar unusual lesions.
